Immunotherapy in gram-negative bacterial infections.

Endotoxins are responsible for initiation of septic shock which increases the number of fatalities in Gram-negative bacteremia among hospital patients. The mortality from septic shock is still high despite recent developments in antibiotic therapy because antibiotics are unable to decrease the level of free lipopolysaccharide in the blood stream. Another approach to the treatment and prevention of septicaemia involves stimulation of an immune response against LPS. It was found that immunization with the core structures of endotoxin conjugated with proteins protected animals against infections

Multidrug-resistant Gram-negative bacterial infections.

Multidrug-resistant Gram-negative bacterial infections cause significant morbidity and mortality globally. These pathogens easily acquire antimicrobial resistance (AMR), further highlighting their clinical significance. Third-generation cephalosporin-resistant and carbapenem-resistant Enterobacterales (eg, Escherichia coli and Klebsiella spp), multidrug-resistant Pseudomonas aeruginosa, and carbapenem-resistant Acinetobacter baumannii are the most problematic and have been identified as priority pathogens. In response, several new diagnostic technologies aimed at rapidly detecting AMR have bee

Combating multidrug-resistant Gram-negative bacterial infections.

INTRODUCTION: Multidrug-resistant (MDR) bacterial infections, especially those caused by Gram-negative pathogens, have emerged as one of the world's greatest health threats. The development of novel antibiotics to treat MDR Gram-negative bacteria has, however, stagnated over the last half century. AREAS COVERED: This review provides an overview of recent R&D activities in the search for novel antibiotics against MDR Gram-negatives. It provides emphasis in three key areas. First, the article looks at new analogs of existing antibiotic molecules such as β-lactams, tetracyclines, and aminoglycos

Carbapenem-Resistant Gram-Negative Bacterial Infections in Children.

Carbapenem-resistant organisms (CRO) are a major global public health threat. Enterobacterales hydrolyze almost all β-lactams through carbapenemase production. Infections caused by CRO are challenging to treat due to the limited number of antimicrobial options. This leads to significant morbidity and mortality. Over the last few years, several new antibiotics effective against CRO have been approved. Some of them (e.g., plazomicin or imipenem-cilastatin-relebactam) are currently approved for use only by adults; others (e.g., ceftazidime-avibactam) have recently been approved for use by childre

The evolution of knowledge for treating Gram-negative bacterial infections.

PURPOSE OF REVIEW: Infections caused by nonprimarily pathogenic Gram-negative bacilli (GNB) have been increasingly reported from the second half of the 20th century to the present. This phenomenon has expanded during the antibiotic era and in the presence of immunodeficiency. Before the discovery of sulphonamides and penicillin G, infections caused by GNB were rare compared to Gram-positive infections. The advent of anticancer therapy, the expansion of surgical procedures, the use of corticosteroids, and the implantation of prosthetic materials, along with better control of Gram-positive infect

Developments on antibiotics for multidrug resistant bacterial Gram-negative infections.

Introduction: The constantly increasing spread of severe infections due to multidrug-resistant (MDR) Gram-negative bacteria (GNB) is a critical threat to the global medical community. After a long period of antibiotic pipeline pause, new antibiotic compounds are commercially available or are at late stages of clinical evaluation, promising to augment the therapeutic armamentarium of clinicians against deadly pathogens. Areas covered: This review summarizes available data regarding agents with potent activity against critical MDR Gram-negative pathogens, which urgently require new efficient ant

Clinical evaluation of aztreonam for serious gram-negative bacterial infections.

In an open study aztreonam was evaluated for efficacy and safety in 20 hospital in-patients presenting with presumptive or confirmed hospital acquired aerobic gram-negative infections. The overall clinical cure rate was 100% (19/19) with a corresponding microbiological cure rate of 100% in those patients in whom an organism was isolated initially (17/17). Reinfection with enterococcus occurred in 2 patients with UTI within a week of stopping aztreonam and one patient developed superinfection during treatment, but none were given any additional therapy. No evidence of drug-associated toxicity w

[Multiresistant Gram-negative bacterial infections: Enterobacteria, Pseudomonas aeruginosa, Acinetobacter baumannii and other non-fermenting Gram-negative bacilli].

Multiresistant Gram-negative bacteria represent a major health problem worldwide. This is related to the severity of the infections they cause, the difficulties for empiric (even directed) treatment, the ease of multiresistance spread, and the absence of new antimicrobial agents active against this group of pathogens. Accordingly, antimicrobial therapy should be based on the results of susceptibility testing, and may require using antimicrobial combinations. The production of extended-spectrum beta-lactamases represents the most important current problem of resistance among enterobacteria; the

Host-directed antimicrobial peptide biomaterials for combating Gram-negative bacterial infections.

Antimicrobial resistance (AMR) represents a defining crisis in modern infectious disease medicine. Gram-negative microbes such as carbapenem-resistant Acinetobacter baumannii, Pseudomonas aeruginosa, and Klebsiella pneumoniae are becoming more resistant than ever. Traditional methods of treatment using antibiotics are ineffective because of the presence of several envelope layers, active efflux pumps, and biofilm production. In contrast, bacteriolytic therapy can exacerbate host immunopathology owing to systemic endotoxin release. Host-defense peptides are thus being explored for their ability

Therapeutic Bacteriophages for Gram-Negative Bacterial Infections in Animals and Humans.

Drug-resistant Gram-negative bacterial pathogens are an increasingly serious health threat causing worldwide nosocomial infections with high morbidity and mortality. Of these, the most prevalent and severe are Pseudomonas aeruginosa, Klebsiella pneumoniae, Escherichia coli, Acinetobacter baumannii, and Salmonella typhimurium. The extended use of antibiotics has led to the emergence of multidrug resistance in these bacteria. Drug-inactivating enzymes produced by these bacteria, as well as other resistance mechanisms, render drugs ineffective and make treatment of such infections more difficult

Role of Gram-Negative Bacterial Infections in Acute-On-Chronic Liver Failure.

Acute-on-chronic liver failure (ACLF) is a systemic disease characterised by an acute clinical deterioration of pre-existing liver cirrhosis leading to multiple organ failure due to systemic inflammation. It may be triggered by the translocation of bacteria and bacterial compounds across the gut barrier. The transmigration of Gram-negative bacteria from the gut to the liver, along with the development of bacterial multidrug resistance, drives the progression of chronic liver disease from a compensated or stable decompensated state to acute organ failure associated with high mortality rates. Treatment of Carbapenem-Resistant Gram-Negative Bacterial Infections with Polymyxins: A Review.

Antimicrobial resistance poses a serious threat to human health. Polymyxins, as cyclic polypeptide antibiotics, include polymyxin B (PMB) and polymyxin E. Although they have different metabolic pathways, their antibacterial activities are similar. Polymyxins exert their effects through mechanisms such as disrupting bacterial cell membranes, neutralizing endotoxins, and impairing the respiratory chain. In clinical practice, polymyxins are often used in combination with other drugs to treat infections caused by carbapenem-resistant gram-negative bacteria (CRGNB). No difference in efficacy has be

Advances in novel antibiotics to treat multidrug-resistant gram-negative bacterial infections.

Antimicrobial resistance is a growing threat to public health and an increasingly common problem for acute care physicians to confront. Several novel antibiotics have been approved in the past decade to combat these infections; however, physicians may be unfamiliar with how to appropriately utilize them. The purpose of this review is to evaluate novel antibiotics active against resistant gram-negative bacteria and highlight clinical information regarding their use in the acute care setting. This review focuses on novel antibiotics useful in the treatment of infections caused by resistant gram-

Multidrug-resistant Gram-negative bacterial infections: are you ready for the challenge?

Paralleling the developments in Gram-positive bacteria, infections caused by multidrug-resistant (MDR) Gramnegative bacilli have become a growing challenge. The most important resistance problems are encountered in Enterobacteriaceae, Pseudomonas aeruginosa and Acinetobacter spp., with increasing trends observed for all major anti- Gram-negative agents (beta-lactams, fluoroquinolones and aminoglycosides). A matter of major concern is the emergence of new beta-lactamases capable of degrading the expanded-spectrum cephalosporins and/or carbapenems, such as the extended-spectrum beta-lactamases (

Unrealized targets in the discovery of antibiotics for Gram-negative bacterial infections.

Advances in areas that include genomics, systems biology, protein structure determination and artificial intelligence provide new opportunities for target-based antibacterial drug discovery. The selection of a 'good' new target for direct-acting antibacterial compounds is the first decision, for which multiple criteria must be explored, integrated and re-evaluated as drug discovery programmes progress. Criteria include essentiality of the target for bacterial survival, its conservation across different strains of the same species, bacterial species and growth conditions (which determines the s

A host-directed macrocyclic peptide therapeutic for MDR gram negative bacterial infections.

The emergence of infections by carbapenem resistant Enterobacteriaceae (CRE) pathogens has created an urgent public health threat, as carbapenems are among the drugs of last resort for infections caused by a growing fraction of multi-drug resistant (MDR) bacteria. There is global consensus that new preventive and therapeutic strategies are urgently needed to combat the growing problem of MDR bacterial infections. Here, we report on the efficacy of a novel macrocyclic peptide, minimized theta-defensin (MTD)-12813 in CRE sepsis. MTD12813 is a theta-defensin inspired cyclic peptide that is highly

Tachypleus lysate test for endotoxin in patients with Gram negative bacterial infections.

Amoebocyte lysate from the horseshoe crabs (Tachypleus gigas) which abounds in the Gulf of Thailand was used to detect endotoxin in patients with Gram-negative bacteremia, in patients with Gram-positive bacterial infections as well as in the control. The Tachypleus lysate test (TLT) was positive in 94.4% of 36 patients with Gram-negative bacteremia before initiation of antibiotic therapy. Only 4% of 50 healthy individuals were positive and all 7 patients with Gram-positive bacterial infections were negative. The threshold sensitivity of TLT was 0.625 micrograms endotoxin per ml of the plasma.

Infection-related hemolysis and susceptibility to Gram-negative bacterial co-infection.

Increased susceptibility to co-infection with enteric Gram-negative bacteria, particularly non-typhoidal Salmonella, is reported in malaria and Oroya fever (Bartonella bacilliformis infection), and can lead to increased mortality. Accumulating epidemiological evidence indicates a causal association with risk of bacterial co-infection, rather than just co-incidence of common risk factors. Both malaria and Oroya fever are characterized by hemolysis, and observations in humans and animal models suggest that hemolysis causes the susceptibility to bacterial co-infection. Evidence from animal models
