Gram-positive anaerobic cocci.

Gram-positive anaerobic cocci (GPAC) are a heterogeneous group of organisms defined by their morphological appearance and their inability to grow in the presence of oxygen; most clinical isolates are identified to species in the genus Peptostreptococcus. GPAC are part of the normal flora of all mucocutaneous surfaces and are often isolated from infections such as deep organ abscesses, obstetric and gynecological sepsis, and intraoral infections. They have been little studied for several reasons, which include an inadequate classification, difficulties with laboratory identification, and the mi

[A consideration on the results of nationwide surveillance of antimicrobial susceptibilities--gram-positive cocci and gram-negative cocci].

The results of the semi-annual nationwide surveillance of antimicrobial susceptibilities, conducted by the Japanese Ministry of Health and Welfare during the period of January 1993 to July 1995, were analyzed for typical Gram-positive cocci and Gram-negative cocci in the purpose of provision of an index for antimicrobial selection. During these 3 years, Streptococcus pyogenes (group A) and Streptococcus agalactiae (group B) showed slightly increasing tendency in susceptibility to ofloxacin (OFLX) and sulfamethoxazole/trimethoprim (ST), while Streptococcus pneumoniae and Moraxella (Branhamella)

Brucella Species Staining as Gram-Positive Rod and Gram-Positive Cocci in Chains.

Two cases of Brucellosis were identified at a hospital in Rhode Island. In both cases, the organisms were isolated from the blood cultures. The bacteria did not appear as the classical textbook description of Brucella spp. as short, Gram-negative rods; instead, Gram-positive rods and Gram-positive cocci in chains were observed. Due to the atypical Gram stain morphology, Brucella spp. were not initially considered as a possible pathogen. Antimicrobial prophylaxes were offered to the technologists who were exposed to the organisms.

Glycopeptide resistance in gram-positive cocci: a review.

Vancomycin-resistant enterococci (VRE) have emerged as important nosocomial pathogens in the past two decades all over the world and have seriously limited the choices available to clinicians for treating infections caused by these agents. Methicillin-resistant Staphylococcus aureus, perhaps the most notorious among the nosocomial pathogens, was till recently susceptible to vancomycin and the other glycopeptides. Emergence of vancomycin nonsusceptible strains of S. aureus has led to a worrisome scenario where the options available for treating serious infections due to these organisms are very

Problems with antimicrobial resistance in gram-positive cocci.

The development of antimicrobial resistance has almost invariably accompanied the therapeutic use of antimicrobial agents. Newer antimicrobials have succeeded partly but not entirely in overcoming the problem of resistance. Antimicrobial resistance in gram-positive cocci has achieved its greatest prominence in the past 15 years. There has been a steady erosion of antimicrobial activity against gram-positive cocci. The presentations in this symposium dealt with some of the important problems of antimicrobial resistance in gram-positive cocci, including methicillin resistance and multidrug resis

Emerging resistance in clinically important gram-positive cocci.

In the first half of the decade of the 1990s, we in the United States have seen the emergence and escalation of substantial antimicrobial resistance in medically important gram-positive cocci. The incidence of methicillin resistance of Staphylococcus aureus continues to increase (now 18%), resulting in many more isolates that are multiply resistant; all S aureus isolates are still susceptible to vancomycin. Enterococci, particularly Enterococcus faecium, have increasingly developed resistance to penicillin, gentamicin, streptomycin, and vancomycin (the last plasmid-mediated). More than a fourt

[Interpretive reading of the antibiogram in gram positive cocci].

Resistance to methicillin in Staphylococcus is related to the expression of the mecA gene, and involves resistance to all beta-lactams, with the exception of the new cephalosporins, ceftobiprole and ceftaroline. Breakpoints for interpretation of this mechanism differ in S. aureus and in coagulase-negative species. For macrolides-lincosamides-streptogramins B, (MLS(B)) the most frequent mechanism among resistant strains is expression of methylases (erm genes). Topoisomerase changes caused by point mutations and expression of the efflux pump NorA determine resistance to quinolones, but there are

Linezolid for the treatment of resistant gram-positive cocci.

OBJECTIVE: To provide a comprehensive review of linezolid, the first of a new class of antibiotics, the oxazolidinones. Therapeutic issues regarding the emergence of multidrug-resistant bacteria and a brief history of the oxazolidinones are also discussed. DATA SOURCES: A MEDLINE search (1966-March 2001) was conducted to identify pertinent literature, including preclinical trials, clinical trials, and reviews. Unpublished clinical data, adverse effects, and dosing information were abstracted from product labeling. STUDY SELECTION: Clinical efficacy data were extracted from clinical trials, c

In vitro activity of teicoplanin against gram-positive cocci.

The glycopeptide susceptibility of 443 clinical isolates of gram-positive cocci collected from nine general hospitals in 1996 was determined according to the recommendations of the CA-SFM (the Antibiogram Committee of the French Society for Microbiology). In total, 234 isolates of Staphylococcus aureus, 84 isolates of coagulase-negative staphylococci (CNS), 98 enterococci and 27 streptococci were collected. The mecA gene confirming resistance to methicillin was found in 42.7% of S. aureus isolates and 51.2% of CNS isolates. No resistance to teicoplanin and vancomycin was found in S. aureus but

Recovery of vancomycin-resistant gram-positive cocci from children.

A cross-sectional survey of vancomycin-resistant gram-positive cocci (VRGPC) in the feces of children was initiated after several bacteremic infections with these organisms occurred at our hospital. A selective medium consisting of colistin-nalidixic acid agar, 5% sheep blood, vancomycin (5 mg/liter), and amphotericin B (8 mg/liter) was developed to isolate VRGPC. A single stool specimen submitted to the clinical microbiology laboratory from each of 48 patients was inoculated onto the medium. Plates were incubated at 35 degrees C with 5% carbon dioxide and examined at 24, 48, and 72 h. Suscept

New receptor for human plasminogen on gram positive cocci.

180 bacterial strains representing 17 different species of gram positive cocci were tested for the ability to interact with human plasminogen. Receptors for plasminogen could be detected on 23/24 strains of S. pyogenes, 15/15 strains of S. equisimilis, 14/16 strains of human group G streptococci and 14/14 strains of S. pneumoniae. Eight of nineteen strains representing five species of alpha-hemolytic streptococci were also positive. S. equisimilis demonstrated the highest uptake with a median value of 58 per cent (20%-67%). On the other hand, all strains of S. agalactiae, the majority of S. fa

Activity of quinolones against gram-positive cocci: clinical features.

The potential role of the commercially available fluoroquinolones in the treatment of Gram-positive infections is discussed on the basis of data obtained from animal experiments and clinical trials. In respiratory tract infections, and particularly in community-acquired pneumonia, it is evident that the presently available quinolones cannot be prescribed empirically as first-line therapy because of their borderline activity against Streptococcus pneumoniae and anaerobes. Reports of pneumococcal seeding in other tissues during quinolone therapy render their administration a debatable issue. Exp

Risk for infective endocarditis in bacteremia with Gram positive cocci.

PURPOSE: Infective endocarditis (IE) is a serious complication of bacteremia and is most often caused by Gram positive cocci. We investigated the prevalence of IE in patients where echocardiography was encouraged when bacteremia with Gram positive cocci was present. METHODS: The study included patients with Gram positive cocci bacteremia hospitalized at two Danish hospitals between March and December 2016. Information concerning echocardiography, demographics and bacterial species was collected from the patients' medical files. Patients without echocardiography were followed for 6 months in o

Anaerobic radiometric detection of facultative Gram-positive cocci in blood.

Radiometric detection of facultative gram-positive cocci, particularly streptococci, has been reported to be suboptimal. A recently marketed anaerobic Bactec bottle (7C) with purportedly improved capability to detect facultative gram-positive cocci in blood was compared with the previously available bottle in a study of simulated bacteremia. The Bactec system detected all beta-hemolytic streptococci and enterococci tested. Detection of alpha-hemolytic streptococci was improved in general. However, detection of Streptococcus mutans, Streptococcus bovis and Streptococcus pneumoniae remained subo

[A case of acute bacterial prostatitis caused by gram positive cocci].

This report describes our experience with sulbactam/cefoperazone in the treatment of a 36-year-old man with acute prostatitis (complicated with bilateral epididymitis) caused by S. salivarius. The patient had no past history suggesting the relationship between this organism and the route of infection. The isolated strain exhibited a high susceptibility to the drug and symptoms subsided after treatment with a daily dose of 4 g sulbactam/cefoperazone for 7 days. Usually, the most causative bacteria of acute prostatitis are either GNR or E. faecalis. However, as in the present case, there may be

Mechanisms of resistance to antimicrobial drugs in pathogenic Gram-positive cocci.

Many species of Gram-positive cocci are pathogenic. The most important are staphylococci, streptococci, and enterococci. Widespread usage of antibiotics was the main cause for the appearance and spread of resistance to almost all antimicrobials. The occurrence, mechanisms, and genetic background of resistance to antimicrobial drugs other than beta-lactams and glycopeptides among pathogenic staphylococci, streptococci, and enterococci are discussed in the text. Well-established agents (such as macrolides, lincosamides, streptogramins, aminoglycosides, quinolones, mupirocin, chloramphenicol) as
