Graves’ ophthalmopathy, also called Graves’ orbitopathy, is a potentially sight-threatening ocular disease that has puzzled physicians and scientists for nearly two centuries.– Generally occurring in patients with hyperthyroidism or a history of hyperthyroidism due to Graves’ disease, Graves’ ophthalmopathy is also known as thyroid-associated ophthalmopathy or thyroid eye disease, because it sometimes occurs in patients with euthyroid or hypothyroid chronic autoimmune thyroiditis. The condition has an annual adjusted incidence rate of 16 women and 3 men per 100,000 population. This review expl

Graves ophthalmopathy (GO) is an autoimmune disease that typically affects the thyroid gland and the orbit. The roles of the thyroid stimulating hormone receptor and cellular and humoral immunity in the pathogenesis of GO are being investigated. Investigators are gaining insights into the epidemiology of GO as it relates to other ocular and autoimmune diseases. Several possible tools for assessing GO disease activity are being studied. Medical and radiation therapy for GO recently have been evaluated prospectively. Orbital surgeons continue to refine orbital decompression techniques to improve

The Graves' ophthalmopathy consists of all the pathological changes in orbital tissues, preceding, accompanying of following hyperthyroidism. Although ophthalmopathy has its clinical expression only in 50% of patients with Graves disease, the ultrasound exploration and the CT scan of the orbital area, as well as the values of intraocular pressure showed that occult changes exist in all patients. The etiopathological autoimmune mechanism responsible of ophthalmopathy doesn't imply the same autoantibodies, though ophthalmopathy is associated with thyrotoxicosis, and the degree of ocular changes

Graves' ophthalmopathy usually occurs in association with hyperthyroidism. Its occasional occurrence in the absence of thyroid disease suggests, however, that it may be a separate autoimmune disorder. While the evidence supporting an autoimmune pathogenesis is considerable for the ophthalmopathy, it is not so impressive as that for Graves' hyperthyroidism: orbital antibodies have not been convincingly demonstrated and autoantigens have not been identified. On the other hand, in patients with Graves' ophthalmopathy the orbital tissues and eye muscle membranes are infiltrated with lymphoid cells

Smoking is the most important risk factor for the occurrence/progression of Graves' ophthalmopathy (GO), as well as for its lower/slower response to immunosuppression. Accordingly, refrain from smoking should be urged, both as primary prevention (removal of risk factors in Graves' patients without GO), secondary prevention (early detection and treatment of asymptomatic/very mild GO) and tertiary prevention (reduction of complications/disability of overt GO). A 6-month course of 200 μg/day sodium selenite can prevent progression of mild GO to more severe GO and is, therefore, a form of secondar

Most patients with Graves' disease have some evidence of ocular involvement, but this is commonly mild, requiring only local measures. A minority of patients (3-5%) have severe Graves' ophthalmopathy, for which the three main treatment procedures are represented by high-dose glucocorticoids, orbital radiotherapy and orbital decompression. Favourable results with medical treatment have been reported in approximately 60% of patients, with particular regard to inflammatory changes, newly developed eye muscle dysfunction and optic neuropathy. Orbital decompression is indicated in severe eye diseas

Fortunately, most patients with thyroid-related eye disease have mild ocular involvement that spontaneously involutes. Less than 5% of patients with hyperthyroidism will develop sufficiently severe ocular abnormalities that will require surgical intervention. Most patients with thyroid eye findings should be symptomatically managed. Some will require use of either topical drops or oral steroids to alleviate their eye problems. In approximately one half of those patients who present acutely with severe thyroid orbital finding, nonsurgical therapies will be sufficient to control their eye diseas

Graves's ophthalmopathy, the commonest extrathyroidal manifestation of Graves' disease is an autoimmune disorder of the orbit. Restoration of thyroid dysfunction, lubricant eye drops and smoking cessation is important. Clinical activity and disease severity determine specific treatment. Referral to specialist centres is urgent in sight-threatning optic neuropathy. Intravenous methylprednisolone is first-line therapy for clinically active disease. Retrobulbar irradiation may help in persistant ocular muscle inflammation. Prompt orbital decompression is warranted for sight-threatning neuropathy

Management of Graves' ophthalmopathy is preferably done in a multidisciplinary setting. Smoking is associated with worse disease outcome. (131)I therapy for hyperthyroidism can also worsen ophthalmopathy, especially if administered during active disease or to patients who smoke or have severe hyperthyroidism, or those with high levels of TSH-receptor-binding inhibitory immunoglobulins. Coadministration of steroids and (131)I therapy is recommended for such high-risk patients. (131)I therapy is safe for patients with inactive Graves' ophthalmopathy. Subtotal thyroidectomy and antithyroid drugs

Ophthalmopathy, the most frequent extrathyroidal manifestation of Graves' disease, results from an increased volume of the orbital tissues (connective and adipose tissue, interstitial enlargement of extraocular muscles) within the enclosed space of the bony orbits. While the primary antigen is still uncertain, the thyrotropin receptor has recently become a prime candidate because it is expressed both by thyroid epithelial cells and by orbital preadipocyte fibroblasts. Tolerance towards the thyroidal and orbital TSH receptor may be disrupted by bacterial or viral antigens through a "molecular m

The pathology of the orbital changes in Graves' ophthalmopathy (GO) has been discussed in detail. The target tissue is the eye muscle and the damage probably results from autoimmune processes. Cell-mediated immune responses have been demonstrated and an antibody to eye muscle is detectable in 70% of patients. There is no direct evidence for an effect of thyroid hormones or TSH on orbital tissues in GO. The relation between GO and autoimmune thyroid disease is discussed. Present evidence suggests that in GO autoimmune responses are directed to orbital tissue antigens and do not cross-react with

PURPOSE: This review aims to discuss the psychological aspects of Graves' ophthalmopathy (GO), estimate the prevalence of depression and anxiety disorders in GO, examine whether these psychiatric disorders are more prevalent in GO than in Graves' disease (GD) without eye disease, and evaluate the main contributors for depression and anxiety in GO.
METHODS: A review of the literature.
RESULTS: Both depression and anxiety are associated with GO. The prevalence of depression and anxiety disorders specifically in GO patients was estimated at 18-33% and 26-41%, respectively. The reported prevalen

The aims of surgical treatment in Graves's orbitopathy (GO) are improvement of function and appearance. Since antiinflammatory treatment of GO rarely results in a complete resolution of symptoms, surgical treatment is very important for patients well being. Rehabilitative surgery includes orbital decompression, squint correction, lid lengthening and blepharoplasty and these procedures have to be performed in centres of expertise. Various techniques have been developed for orbital decompression which allow now a graded approach to proptosis reduction and optic nerve decompression in emergency s

AIMS: To study the ocular manifestations and their severity in children with Graves' disease.
METHODS: All patients with Graves' disease having regular follow up in a paediatric endocrine clinic were recruited for the study. A comprehensive ophthalmic assessment including ocular motility, exophthalmometry, intraocular pressure (IOP), slit lamp, and fundus examinations was performed.
RESULTS: 83 patients (72 female, 11 male) aged 16 years or below were examined. All are Chinese. Ocular symptoms occurred in 12 patients. Ocular signs of ophthalmopathy were documented in 52 patients (62.7%). Mos

Graves' ophthalmopathy is an autoimmune process initiated and maintained by antigen(s) shared by the thyroid and the orbit. A matter of argument concerns the choice of the method of treatment for Graves' hyperthyroidism when clinically evident ophthalmopathy is present. Restoration of euthyroidism appears to be beneficial for ophthalmopathy. On the other hand the continuing disease activity associated with the recurrence of hyperthyroidism appears to adversely affect the course of ophthalmopathy. For these reasons it is our opinion that in patients with Graves' hyperthyroidism and ophthalmopat

In most patients with Graves' hyperthyroidism the eye signs are self-limiting and mostly subclinical. However, about one-third of the patients have clinically relevant ophthalmopathy, which can be disabling and disfiguring. The mechanical causes of the symptoms and signs of the eye disease are largely understood, but the best way to manage the ophthalmopathy is still a matter of much debate. Adequate treatment of hyperthyroidism can aleviate the eye symptoms to some extent, but it is less clear which kind of antithyroid treatment is to be preferred in patients with ophthalmopathy. There is par

The important questions to be answered in the course of decision-making in patients with Graves' ophthalmopathy include the following: 1. Is the eye problem owing to Graves' ophthalmopathy? If not, the cause of the eye problem must be sought. 2. Does the patient have serious medical problems apart from the thyroid and the eyes? Define type and severity, and risk to life and well-being. Do they preclude anesthesia or steroid therapy? 3. Is the patient euthyroid? Define thyroid abnormality and treat. 4. Is the eye problem the highest medical priority for the patient and the physician? If not, tr

BACKGROUND: Graves' disease is an autoimmune disease caused by the production of auto-antibodies against the thyroid-stimulating hormone receptor, which stimulates follicular cell production of thyroid hormone. It is the commonest cause of hyperthyroidism and may cause considerable morbidity with increased risk of cardiovascular and respiratory adverse events. Five per cent of people with Graves' disease develop moderate to severe Graves' ophthalmopathy. Thyroid surgery for Graves' disease commonly falls into one of three categories: 1) total thyroidectomy, which aims to achieve complete macro
