The ultimate goal of periodontal therapy is to restore periodontal tissues lost through disease or trauma. The most predictable way to accomplish this goal is by guided tissue regeneration (GTR). The principle of GTR is to give preference to certain cells to repopulate the wound area to form a new attachment apparatus. Clinically this is accomplished by placing a barrier over the defect thereby excluding gingival tissues from the wound during early healing. The first generation of GTR barriers were non-resorbable which implies that they have to be removed in a second surgical procedure. Resorb

AIM: The aim of the present study was to evaluate, over a period of 1 y, 3 different surgical methods for the treatment of periodontal bone defects.
METHODS: Thirty-six infrabone defects, at least 4 mm in depth, in non-smokers were enrolled in the study. Of these, 12 were treated with guided tissue regeneration (GTR) using a resorbable membrane with collagen, 12 with enamel matrix derivative (EMD) gel and 12 with a modified Widman flap (MWF). The pocket depth, attachment loss and gingival recession both before commencement of therapy and after an interval of 1 y are reported.
RESULTS: The re

Nonabsorbable barriers are considered the material by which all other barriers are judged. They have a well-established record of safety and efficacy. They are not a panacea, however. Practitioners must take special care when selecting both patients and surgical sites for GTR. Extensive experience, superior surgical ability, and meticulous attention to detail are also required if one is to achieve predictably favorable results.

The use of expanded polytetrafluoroethylene (ePTFE) membranes in the treatment of mucogingival problems such as gingival recession, has been recently described. The major issues raised dealt with creating and maintaining space under the membrane and designing a flap that could be coronally positioned and at the same time capable of maintaining a good blood supply. The authors proposed the use of a trapezoidal flap and some technical space-making solutions, such as the use of a suturing technique and different metal structures to support the membranes. BACKGROUND: Guided tissue regeneration (GTR) has the potential to promote periodontal regeneration, which is one of the goals of periodontal surgery. While many successful reports of periodontal regeneration using barrier membranes exist in the literature, considerable heterogeneity of GTR outcomes is more typical of current reports. The reasons for this variability are numerous, but could be attributed to differences in surgical skills and case selection. There is a need for a current analysis of the factors affecting success and the formation of evidence-based treatment guidelines for GTR.

Common cosmetic failures in guided tissue regeneration (GTR) procedures performed in the esthetic zone include an unacceptable postoperative recession of the gingival flap and loss of the papillae. This article describes the dynamics of flap healing that contribute to esthetic failures. Specific technique modifications of GTR procedures that enhance the predictability of successful functional and esthetic outcomes are described.

Chronic periodontitis results in the destruction of the collagen fibre attachment to cementum, apical migration of the epithelial attachment, contamination of the exposed root surface by plaque and loss of alveolar bone. Regeneration can be defined as the restitution of lost tissues to a state which existed before destruction. As such, it is an ultimate and ideal goal and in most circumstances is currently beyond our capabilities. However, research in periodontology over the last 10 years or so has made considerable advances and is perhaps unique in proposing a new and fundamental treatment mo

Periodontitis is a chronic infectious oral disease with a high prevalence rate in the world, and is a major cause of tooth loss. Nowadays, people have realized that the local microenvironment that includes proteins, cytokines, and extracellular matrix has a key influence on the functions of host immune cells and periodontal ligament (PDL) stem cells during a chronic infectious disease such as periodontitis. The above pathological process of periodontitis will lead to a defect of periodontal tissues. Through the application of biomaterials, biological agents, and stem cell therapy, guided tissu

Guided tissue regeneration (GTR) and guided bone regeneration (GBR) are two common dental regenerative procedures used to repair periodontal defects caused by periodontitis. In both procedures, a barrier membrane is placed at the interface between the soft tissue and the periodontal defect, serving to impede the infiltration of soft tissue while creating a secluded space for periodontal regeneration. Recently, barrier membranes based on chitosan (CS) have emerged as a promising avenue for these applications. However, despite numerous studies on the development of CS-based membranes, comprehens

This article reviews the role of barrier membranes in guided tissue regeneration (GTR) and guided bone regeneration (GBR), including the advantages of using absorbable barrier membranes in GTR and GBR and the unique properties of collagen membranes. The indications and contraindications for using collagen membranes for these procedures are examined, and successful cases are presented. Finally, the role of collagen membranes in the future of regenerative therapy is considered.

Biomaterials-driven immunomodulation and epigenetic regulation are increasingly recognized as key determinants of cell behavior and regenerative outcomes, yet they have largely been studied independently. Epigenetic mechanisms, including DNA methylation, histone modifications, chromatin remodeling, and non-coding RNAs, regulate gene expression, cellular plasticity, and lineage commitment in response to biomaterial cues. Meanwhile, immune responses, particularly macrophage polarization and the foreign body response, critically influence the inflammatory and regenerative microenvironment after b

BACKGROUND: Conventional treatment of destructive periodontal (gum) disease arrests the disease but does not regain the bone support or connective tissue lost in the disease process. Guided tissue regeneration (GTR) is a surgical procedure that aims to regenerate the periodontal tissues when the disease is advanced and could overcome some of the limitations of conventional therapy.
OBJECTIVES: To assess the efficacy of GTR in the treatment of periodontal infra-bony defects measured against the current standard of surgical periodontal treatment, open flap debridement.

Case reports of mandibular molars with iatrogenic perforations are presented. Such perforations have been treated with internal amalgam repair or by surgical intervention. However, the potential exists for tissue migration and pocket formation. The use of guided tissue regeneration as a barrier for apical migration is beneficial in surgical treatment of perforations. Follow-up examination of perforated teeth treated with resorbable barriers showed successful repair. Results obtained with this technique offer the possibility of a successful repair for an otherwise hopeless complication of endod

BACKGROUND: Conventional treatment of destructive periodontal (gum) disease arrests the disease but does not usually regain the bone support or connective tissue lost in the disease process. Guided tissue regeneration (GTR) is a surgical procedure that specifically aims to regenerate the periodontal tissues when the disease is advanced and could overcome some of the limitations of conventional therapy.
OBJECTIVES: To assess the efficacy of GTR in the treatment of periodontal infra-bony defects measured against conventional surgery (open flap debridement (OFD)) and factors affecting outcomes.

BACKGROUND: The goal of guided tissue regeneration-based root coverage (GTRC) is to repair gingival recession via new attachment formation. Numerous clinical trials have been conducted utilizing the concept of GTR to promote root coverage. Most GTRC studies have had relatively small sample sizes and have not utilized power calculations to determine appropriate sample size; therefore, it is difficult to draw strong conclusions from them. Hence, the purpose of this study is to combine data from currently available GTRC studies and to use meta-analysis to determine whether GTRC provides significa

This article has discussed and illustrated various applications of GTR associated with dental implants. At present, in the opinion of this author, the material of choice for these procedures is GTAM. Other barriers are currently available while still more are under development. It is not within the scope of this article to include a comprehensive discussion of all available materials but it is noteworthy that resorbable membranes such as GUIDOR (Butler) show promise. At present the predictability of the various GTAM techniques discussed in this article, in the experience of the author are as f

Bioresorbable barrier material, used in guided tissue regeneration, changes the conventional two-stage surgery to a single-step procedure. This article presents a general surgical protocol for application of a bioresorbable matrix barrier (Guidor AB, Huddinge, Sweden). Three case reports are used to illustrate the surgical treatment of a Class II furcation defect, an intrabony defect, and a recession type defect. The learning objective of this article is to enhance the knowledge of surgical procedures and bioresorbable barrier membranes available.

Autotransplantation is an alternative treatment to replace missing or periodontally-involved hopeless teeth. A prerequisite for performing this procedure is having a recipient site with sufficient bone volume to support the transplant. Often, however, crestal bone resorption following tooth extraction or periodontitis results in a reduced alveolar ridge with inadequate dimensions to properly house the transplant. In these cases, the procedure is contraindicated. A case is presented in which the biologic principle of guided tissue regeneration was used to gain periodontal support around an auto

One advanced localized periodontal lesion in each of 10 patients was treated using the guided tissue regeneration procedure. Six weeks after placement of expanded polytetrafluoroethylene (ePTFE) membranes, microbial samples were taken from each treated site and the membranes were removed. Six weeks later the cases were re-evaluated. They had all healed successfully with varying amounts of gain of clinical attachment. Gram-negative, anaerobic rods were found in all samples and made up 31% of all organisms cultivated. In 1 patient, Porphyromonas gingivalis was found in a proportion of 17.5%. Six
