Ophthalmic manifestations of perinatally acquired HIV in a US cohort of long-term survivors.

BACKGROUND/AIMS: To determine the ophthalmic manifestations of HIV in a cohort of long-term survivors of perinatally acquired HIV. METHODS: Twenty-two patients with perinatally acquired HIV who were aged ≥12 years were prospectively studied at a university clinic. They underwent complete ophthalmic examinations and fundus photography. Their medical histories, medications and CD4 counts were abstracted from the medical records. To evaluate for keratoconjunctivitis sicca, both HIV patients and 44 healthy controls (matched by age, gender and contact lens wear) underwent Schirmer testing and ocul

Adolescents with perinatally acquired HIV: emerging behavioral and health needs for long-term survivors.

PURPOSE OF REVIEW: Because of widespread availability of highly active antiretroviral therapy in the developed world, a large proportion of children with perinatally acquired HIV have survived to adolescence and young adulthood. Although their survival is remarkable, many now experience the long-term effects of HIV infection and its treatment. Further, as these youths have entered adolescence, more is known about the impact of normative developmental transitions on health maintenance behaviors. RECENT FINDINGS: Although perinatally infected adolescents are healthier than they were a decade or

The moral-behavioural logic of silence: gender, stigma, and disclosure among long-term HIV survivors.

Silence, the deliberate non-disclosure of HIV status - remains an overlooked yet central aspect of long-term antiretroviral therapy (ART) survivorship. While disclosure is promoted as empowerment, such framings may neglect the gendered, relational, and institutional contexts in which non-disclosure occurs. We conducted in-depth interviews with 21 long-term ART users (10-22 years on treatment) in Tamil Nadu, India, and analysed narratives using stigma, social identity, emotion work, spiritual reframing, and institutional betrayal lenses. Silence emerged not as absence, but as a moral-behavioura

Absence of recoverable infectious virus and unique immune responses in an asymptomatic HIV+ long-term survivor.

We have studied a woman with transfusion-acquired HIV who appears to have contained infectious virus to consistently undetectable levels over a 13-year period without antiviral treatment. She received the infected transfusion for intra- and postpartum blood loss immediately after delivery of her second child in 1981. She had no acute febrile syndrome and has never had HIV-associated clinical signs or symptoms in the 13 years since infection. She was first tested and found positive for HIV antibodies in 1985, and the infected blood donor was diagnosed with AIDS in 1986 and died of AIDS-related

"It's not just HIV anymore": A qualitative exploration of health, research, community and mental health priorities of long-term survivors of HIV in the United States.

ABSTRACTLong-term survivors (LTS) of HIV are people diagnosed before the advent of highly active antiretroviral therapy in 1996 or who have lived with HIV for more than ten years. As they age, many experience comorbidities and effects of lifelong therapy, yet they remain underrepresented in research, which limits person-centered strategies. We examined priorities related to health, aging, safer antiretroviral therapy, community, mental health, and research engagement among LTS in the United States, and explored how social determinants shaped outcomes and access. In 2023 to 2024, we conducted q

Lack of differences in nef alleles among HIV-infected asymptomatic long-term survivors and those who progressed to disease.

Sequences of the human immunodeficiency virus (HIV) nef gene in virus isolates from 12 long-term survivors (LTSs) and 7 progressors were compared to determine if any association existed between the sequences and the corresponding clinical status. The sequences of at least five clones were determined for each subject. Conceptual translations of the open reading frames (ORFs) were examined with respect to a consensus with a prototypic nef sequence (HIV-1SF2) and for conservation of functionally described motifs. Premature stops were observed at equivalent, yet low, frequencies among the differen

Motivations, acceptability and ethical considerations for interventional HIV cure research at the end of life: perspectives from long-term survivors of HIV in the United States.

INTRODUCTION: Despite progress in antiretroviral therapy (ART), an effective cure for HIV remains out of reach. End-of-life (EOL) research studies involving individuals with a prognosis of six months or less offers an opportunity to advance cure science but has so far been limited to observational designs focused on HIV reservoirs. As interventional approaches at the EOL are now being considered, it is essential to assess their acceptability before moving forward. Understanding how long-term survivors (LTS) of HIV perceive these potential interventions, along with their motivations and ethical

Phenotype and envelope gene diversity of nef-deleted HIV-1 isolated from long-term survivors infected from a single source.

BACKGROUND: The Sydney blood bank cohort (SBBC) of long-term survivors consists of multiple individuals infected with attenuated, nef-deleted variants of human immunodeficiency virus type 1 (HIV-1) acquired from a single source. Long-term prospective studies have demonstrated that the SBBC now comprises slow progressors (SP) as well as long-term nonprogressors (LTNP). Convergent evolution of nef sequences in SBBC SP and LTNP indicates the in vivo pathogenicity of HIV-1 in SBBC members is dictated by factors other than nef. To better understand mechanisms underlying the pathogenicity of nef-del

Factors associated with conversion of long-term non-progressors to progressors: a prospective study of HIV perinatally infected paediatric survivors.

BACKGROUND & OBJECTIVES: Survival pattern among children infected with the human immune deficiency virus (HIV) follows a bimodel distribution. Some children survive beyond 9 years age and are known as long term survivors (LTS) while others had a more rapid course to death during the first few years of life. In the LTS group of children, two sub-populations have emerged, the long term non-progressors (LTNP) who have remained asymptomatic over a period of years and those who have survived despite clinical and laboratory evidence of disease progression, the long term progressors (LTP). The aim of

Persistence of attenuated HIV-1 rev alleles in an epidemiologically linked cohort of long-term survivors infected with nef-deleted virus.

BACKGROUND: The Sydney blood bank cohort (SBBC) of long-term survivors consists of multiple individuals infected with nef-deleted, attenuated strains of human immunodeficiency virus type 1 (HIV-1). Although the cohort members have experienced differing clinical courses and now comprise slow progressors (SP) as well as long-term nonprogressors (LTNP), longitudinal analysis of nef/long-terminal repeat (LTR) sequences demonstrated convergent nef/LTR sequence evolution in SBBC SP and LTNP. Thus, the in vivo pathogenicity of attenuated HIV-1 strains harboured by SBBC members is dictated by factors

"A cure might help, but it won't erase it all": a qualitative study of policy challenges and priorities for long-term survivors of HIV in the United States.

INTRODUCTION: Long-term survivors (LTS) of HIV, including individuals diagnosed before the availability of effective antiretroviral therapy (ART), have played a pivotal role in shaping the HIV response. Despite an increase in their number in the United States, their unique medical, social and economic challenges remain underrepresented in HIV policy and research, particularly in the context of HIV cure advancements. While an HIV cure may alleviate ART-related burdens, LTS fear unintended consequences, including the potential loss of critical social benefits, economic support and healthcare acc

"Sometimes They Exclude Us because of Our Age-That's Not Right": Perceptions of HIV Cure Research Among Diverse Long-Term Survivors in the United States.

HIV cure research has advanced, utilizing analytical treatment interruption (ATI) as a research tool alongside therapeutic strategies such as latency-reversing agents, block and lock strategies, immune-based therapies, cell and gene therapies, and combination approaches to overcome viral persistence. While promising, participation in cure trials remains limited, particularly for long-term survivors (LTS) who have lived with HIV for decades. Many LTS are willing to participate but face barriers such as age-based exclusions, comorbidities, and trial design constraints. With over half of the peop

Differences in social and mental well-being of long-term survivors among people who inject drugs and other participants in the Swiss HIV Cohort Study: 1980-2018.

BACKGROUND: People living with HIV who were diagnosed before highly active antiretroviral therapy became available in 1996 and who survived at least 15 years after HIV diagnosis, termed long-term survivors (LTS), form a particularly vulnerable population. We study social, clinical and mental factors of LTS in the Swiss HIV Cohort Study, with a particular focus on people who inject drugs (PWID). METHODS: We quantified differences between PWID LTS, and men who have sex with men (MSM) and heterosexual (HET) LTS. Using phylogenetic methods, we distinguished between heterosexual LTS who most likel

Reevaluation of possible outcomes of infections with human immunodeficiency virus.

Several lines of evidence indicate that HIV infection can result in several possible incomes, including a very small proportion of individuals whose HIV replication is controlled after treatment interruption (known as HIV posttreatment controllers) or spontaneously without any treatment (known as HIV elite controllers). Both types of individuals are HIV RNA negative but HIV DNA positive, with living virus which can be stimulated ex vivo. A review was conducted to assess the literature on yet rarer cases with detectable integrated HIV DNA without HIV infectious virus in HIV-seropositive or -neg

Persistent elite controllers as the key model to identify permanent HIV remission.

PURPOSE OF REVIEW: To summarize the heterogeneity in the elite controllers population with the aim to identify a compatible profile with a persistent HIV remission, making distinction between persistent elite controllers, people with HIV (PWHIV) who permanently maintain virological control in the absence of antiretroviral treatment (ART), and transient elite controllers, PWHIV who eventually lose virological control. For this purpose, it is important to consider the mechanisms and biomarkers that have previously been associated with the maintenance and loss of the natural virological control.

Long term management of people with HIV.

BACKGROUND: Advances in the treatment of human immunodeficiency virus (HIV) have resulted in sustained improvements in the general health and longevity of people living with the virus. Primary care continues to be predominantly delivered by high caseload general practitioners and specialists, but GPs with limited HIV experience are increasingly likely to have contact with HIV positive patients through shared care arrangements. OBJECTIVE: The aim of this article is to review the management of stable patients with HIV and to provide an approach to important elements of their ongoing care. DISC

Aging of adult lifetime survivors with perinatal HIV.

PURPOSE OF REVIEW: We describe the emerging clinical outcomes for adults born with perinatally acquired HIV (PHIV), who have been living with HIV throughout their life. Whilst many comorbidities appear similar to adults with horizontally acquired HIV, they manifest at a younger chronological age. The additional impact of HIV throughout postnatal, childhood and adolescent growth and development requires further consideration. RECENT FINDINGS: There is growing evidence of an increased incidence of metabolic, cardiovascular, respiratory, bone and renal impairment as well as structural brain chan

Learning from nature: HIV elite controllers as blueprints for a functional cure.

Antiretroviral therapy (ART) has transformed HIV infection into a manageable chronic condition but remains non-curative and requires lifelong adherence. Rare cases of long-term remission following stem cell transplantation (SCT) have demonstrated the possibility of viral eradication, yet this approach is not scalable or safe for global implementation. A unique subset of people living with HIV, known as elite controllers (ECs), can naturally suppress replication-competent HIV without ART, often for decades, while maintaining stable CD4 + T cell counts and showing no signs of disease progression
