Complications of Lesser Toe Surgery: How To Avoid Them before Surgery and How To Assess and Treat Them When They Have Occurred.

Complications following lesser toe surgery are challenging to manage. The keys to treatment of any of these conditions are, first, to try to avoid them through identification of patient- and surgeon-related variables that contribute to their development and, second, following the occurance of a complication, to understand what can and cannot be corrected with surgical and nonsurgical management. This review provides a comprehensive assessment of current literature, demonstrates best practices and approaches to lesser toe complications, and provides an illustration of clinical examples.

A systematic review of the claw toe deformity: What is known and what is needed apart from surgical procedures.

The claw toe deformity is a painful condition that mainly affects the adult population. Although there are many different treatments to solve the deformity, there is no optimal procedure to restore the normal foot mechanics. The objective of this review was to identify the technical features of the kinematic evaluation methods used in patients with claw toes. Furthermore, the aim of this review was to clarify what is known and what is needed apart from the surgical procedures to correct the claw toe deformity, with the purpose of reducing risk factors of falling in elderly people. Neuropathic Deformities of the Lesser Toes.

Neuropathic deformities of the lesser toes refer to a variety of neuromuscular disorders in which a stiff lesser toe deformity is encountered. Claw toes, hammer toes, and mallet toes can be found. Each deformity is better described as which joint of a particular toe is in a flexed or extended position. This information guides the surgeon to which joint should be addressed and what additional soft tissue procedure may be needed. Interphalangeal joint fusions are the procedure of choice, together with extensor or flexor tenotomies. Good results can be expected, with low revision rates.

Treatment of Dynamic Claw Toe Deformity Through Lengthening of the Flexor Tendons of the Hallux and Toes at the Midfoot Level.

The claw toe deformity is characterized by the flexion of interphalangeal joints (IPJs) with hyperextension of the metatarsophalangeal (MTP) joint. It can be flexible and reducible or rigid and irreducible, or dynamic. The most common cause of dynamic claw toes is a neurological disorder, like sequelae of an ischemic contracture of the muscle belly after a compartment syndrome. Most of the surgical techniques require multiple procedures and may be associated with complications such as toe stiffness, persisting metatarsalgia, and toe malalignment. The aim of this study is to present an option f

Implants for proximal interphalangeal joint arthrodesis of the lesser toes: Where are we? A systematic review.

INTRODUCTION: Proximal interphalangeal joint (PIPJ) arthrodesis is indicated for the treatment of lesser toe deformities. K-wires have traditionally been the standard of care for PIPJ arthrodesis, however intramedullary implants may be superior. This systematic review evaluates the union rate and outcomes of implants for PIPJ arthrodesis. METHODS: MEDLINE, CENTRAL, EMBASE and Google Scholar databases were reviewed for studies reporting on outcomes of intramedullary implants for PIPJ arthrodesis in hammer or claw toe. The primary outcome measure was union rate. Secondary outcome measures inclu

SmartToe, ToeGrip and buried k-wire versus percutaneous k-wire fixation for 2nd PIPJ arthrodesis: A comprehensive review of outcomes.

BACKGROUND: Proximal interphalangeal joint arthrodesis is a reliable and repeatable method of surgical correction for the semi-rigid and rigid hammer toe deformity. In recent years the authors have noted a significant increase in the use of novel intramedullary devices in place of the traditional percutaneous Kirschner wires (k-wire). This paper set out to critically review three methods of internal fixation; ToeGrip implant, SmartToe implant and buried k-wire technique in comparison to the traditional method of percutaneous k-wire fixation during arthrodesis of the proximal interphalangeal jo

Hammertoes and clawtoes: proximal interphalangeal joint correction.

Hammertoe and clawtoe deformities are common forefoot problems. The deformity exists owing to the underlying pathoanatomy. Hallux valgus, longer metatarsals, and intrinsic imbalance are the most common etiologies. Understanding the cause of the deformity is important to be able to successfully treat the condition, whether nonoperative or with operative intervention. When nonoperative measures fail, PIP correction is best obtained through arthroplasty or arthrodesis. The key to successful PIP correction is obtaining a well-aligned toe and reducing pain as demonstrated by Coughlin and Mann.15 Wh

Fiber-reinforced fixation implant for proximal interphalangeal joint arthrodesis shows advanced implant bio-integration at 2-year follow-up.

BACKGROUND: A bio-integrative fiber-reinforced implant (OSSIOfiber® Hammertoe Fixation Implant, OSSIO Ltd., Caesarea, Israel) for proximal interphalangeal joint (PIPJ) correction-arthrodesis showed partial bio-integration at 1-year follow-up (1FU) in a previous study. The study was prolonged to assess the bio-integration at 2-year-follow-up (2FU). METHODS: Twenty-four patients with proximal interphalangeal joint (PIPJ) correction-arthrodesis using the fiber-reinforced implant and analysed at 1FU, completed 2FU. Follow-up included clinical examination, patient reported outcomes, radiographs, M

Challenges in Offloading Lesser Toe Deformities: A Case Study.

BACKGROUND: Lesser toe deformities such as hammer, mallet, claw, or cross over toe deformities are common in the elderly. They are attributed to mechanical, neuromuscular, congenital, inflammatory arthritis, or other disorders. They become clinically relevant when they produce pressure and friction at the distal ends of the digits leading to preulcerative callus or ulcer formation. These deformities can be difficult to offload with custom orthotics, accommodative footwear, and off-the-shelf devices such as crest pads and toe crutches. CASE: Mr. D was an 82-year-old man with bilateral lesser t

[Reconstructive surgery of sequelae of compartment syndrome of the lower leg and/or foot. Presentation of a new classification].

The sequelae of an undiagnosed insufficiently treated or unpreventable (by crush injury) compartment or postischemic syndrome, most often after lower leg fracture or popliteal artery injury, are caused by necrosis and contracture of the extrinsic foot muscles. Therefore claw toes, pes equinus or other forms, such as a severe pes equino varus related to the compartment involved will decide the kind of foot deformity. In cases of a combined compartment syndrome of the lower leg and foot, not only the extrinsic but also the intrinsic muscles especially the short flexors are involved, leading to e

Women's musculoskeletal foot conditions exacerbated by shoe wear: an imaging perspective.

The predominance of several musculoskeletal foot conditions in women are largely the result of biomechanical alterations caused by ill-fitting shoes. In particular, the altered biomechanics (associated with high-heeled shoes and shoes with a narrowed toe box) has been linked to the genesis of hallux valgus, hammer toe deformity, Haglund syndrome, metatarsal stress fracture, Freiberg infraction, and Morton neuroma. In reviewing the imaging findings of several of these conditions, we emphasize the role of radiographs, special radiographic views, and the utility of more costly studies, such as ma

Intraosseous Technique for Correction of Hammertoe Deformity.

Hammertoe deformities are among the most common conditions treated by foot and ankle surgeons. Many different types of proximal interphalangeal joint arthrodesis fixation techniques have been used. These include implant fixation, absorbable fixation, screw fixation, two-pin fixation, and single-Kirschner wire fixation. Each of these has their own set of associated advantages and disadvantages. One of the most common techniques to address hammertoe deformity is the use of Kirschner wires. Although Kirschner wires have been reliable and produced good outcomes, there are some drawbacks associated

Lesser toe abnormalities.

Lesser toe abnormalities, which can result in significant pain and discomfort, are caused by several intrinsic or extrinsic factors including inflammatory arthritis, trauma, congenital abnormalities, neuromuscular disorders, or poorly fitting shoe wear. Identification of the etiology of the deformity is necessary to determine whether conservative or surgical treatment is warranted and to possibly halt progression of the deformity.
