HINTS battery involving head impulse test (HIT), nystagmus, and test of skew is the critical bedside examination to differentiate acute unilateral peripheral vestibulopathy from posterior circulation stroke (PCS) in acute vestibular syndrome (AVS). The highest sensitivity component of the battery has been reported to be the horizontal HIT, whereas skew deviation is defined as the most specific but non-sensitive sign for PCS. Video-oculography-based HIT (vHIT) may have an additional power in making the differentiation. If vHIT is undertaken, then both gain and gain asymmetry should be taken int

BACKGROUND: Vestibular Migraine (VM) is a frequent cause of recurrent spontaneous vertigo. While some report a normal Video Head Impulse Test (vHIT) in VM, others observed abnormal results on this test. Whether or not methodological discrepancies could be the cause of these differences is not known. There are 2 vHIT methods: subjects fixating an earth-fixed target (HIMP paradigm) or a head-fixed target, the suppression head impulse test (SHIMP paradigm).
OBJECTIVES: The present study aimed to compare VM patients against healthy controls on both HIMP and SHIMP in order to unravel any differenc

The vestibulo-ocular reflex (VOR) is a dynamic system of the human brain that helps to maintain balance and to stabilize vision during head movement. The video head impulse test (vHIT) is a clinical test that uses lightweight, high-speed video goggles to examine the VOR function by calculating the ratio of eye-movement to head-movement velocities. The main problem with a patient's vHIT is that data coming from the goggles may have artifacts and other noise. This paper proposes an impulse classification network (ICN) using a one-dimensional convolutional neural network that can detect noisy dat

Seasickness can have a devastating effect on a crewmember's well-being. In the literature, there is a controversy about whether car sickness could be diagnosed by vestibular evaluation tests, specifically by Video Head impulse test (vHIT) a semi-circular canal (SCC) function test. These consequences might be relevant to seasickness susceptibility. Angular acceleration at the pitch and roll planes lead to the development of seasickness. Normative data from vHIT indicates that the vestibulo-ocular reflex (VOR) gain for the anterior and posterior semicircular canals is lower and more variable com

The functional head impulse test is a new test of vestibular function based on the ability to recognize the orientation of a Landolt C optotype that briefly appears on a computer screen during passive head impulses imposed by the examiner over a range of head accelerations. Here, we compare its results with those of the video head impulse test on a population of vestibular neuritis patients recorded acutely and after 3 months from symptoms onset. The preliminary results presented here show that while both tests are able to identify the affected labyrinth and to show a recovery of vestibular fu

The computerized rotational head impulse test (crHIT) uses a computer-controlled rotational chair to deliver whole-body rotational impulses to assess the semicircular canals. The crHIT has only been described for horizontal head plane rotations. The purpose of this study was to describe the crHIT for vertical head plane rotations. In this preliminary study, we assessed four patients with surgically confirmed unilateral peripheral vestibular abnormalities and two control subjects. Results indicated that the crHIT was well-tolerated for both horizontal head plane and vertical head plane stimuli.

BACKGROUND AND OBJECTIVE: Migraine is an extremely prevalent primary headache disorder that frequently associates parallel symptoms such as dizziness, tinnitus and hearing loss. Our aim is to investigate differences in video head impulse (vHIT) results with patients suffering from vestibular migraine (VM) and healthy people, taking into consideration mean values of vestibule ocular reflex (VOR) gain, occurrence of the compensatory saccades latency and amplitude. According to the results, determine the usefulness of vHIT in vestibular migraine diagnostics.
METHODS: A total number of 120 subjec

The authors highlights the importance of the vestibulo-ocular reflex examination through the head impulse test as a diagnostic method for vestibular dysfunction as well as, and primarily, a bedside semiotic resource capable of differentiating between acute peripheral vestibulopathy and a cerebellar or brainstem infarction in emergency rooms.

The objective of this study was to compare the findings of the bedside head-impulse test (HIT), passive head rotation gain, and caloric irrigation in patients with cerebellar ataxia (CA). In 16 patients with CA and bilaterally pathological bedside HIT, vestibuloocular reflex (VOR) gains were measured during HIT and passive head rotation by scleral search coil technique. Eight of the patients had pathologically reduced caloric responsiveness, while the other eight had normal caloric responses. Those with normal calorics showed a slightly reduced HIT gain (mean ± SD: 0.73 ± 0.15). In those with

BACKGROUND AND OBJECTIVE: In interictal Ménière's disease (MD), a caloric-vHIT dissociation is well established: caloric testing reveals vestibular hypofunction, yet video head impulse test (vHIT) gains in the lateral canal are preserved. Whether this preservation corresponds to preserved functional gaze stabilization remains unknown. This preliminary, exploratory study compared affected and unaffected ear responses of dynamic visual acuity (DVA), functional head impulse test (FHIT), and vHIT in interictal MD, and evaluated the clinical value of each measure.
METHODS: Eleven interictal MD pat

One of the most important questions within the field of vertigo-diagnosis is the proof or the exclusion of a vestibular impairment, the spectrum of appropriate diagnostic tools has been expanded by the head impulse test and the vibratory test in the last years. The head impulse test is a method to examine the functionality of single semicircular canals. As clinical "bedside test" it is an already established part of the diagnostic procedures, as an apparative method with registration and quantitative analysis, however, it is available for general in-office use only recently. The vibratory test

This study investigated the effects of head position on gain values during video head impulse tests (vHITs). Different head positions were used for vHIT of the horizontal semicircular canals of 20 healthy controls and 18 patients with unilateral vestibular loss (UVL), with head velocities ranging from 150°/s to 200°/s. Differences in vestibulo-ocular reflex gain in the control and patient groups according to head position (0° and 30° downward pitch) were analyzed. In the unaffected control group, the 30° pitched-down position resulted in a mean gain increase of up to 1.0 in both ears (right ea

BACKGROUND: The head-impulse test, which is sensitive and specific for detecting severe unilateral peripheral vestibulopathy, is an accepted part of the neurological examination, especially in patients with vertigo and balance disorders.
OBJECTIVE: To discover if the head-impulse test is just as useful diagnostically when patients are asked to rotate their own heads, the active head-impulse test, rather than when the clinician does so as in the standard passive head-impulse test.
METHODS: Clinical observation of compensatory saccades and search coil measurement of compensatory eye rotations,

Head impulse test (HIT) is helpful to understanding high-frequency vestibulo-ocular reflex in patients with dizziness and imbalance. There are some reports on abnormal HITs in cerebellar disorder. To our knowledge, there was no report of transient bilateral positive head impulse related to antiepileptic drugs. A 65-year-old woman developed dizziness and imbalance after treatment with pregabalin for pain control of radiation cystitis. Neurological examination exhibited positive bilateral HIT results, in addition to ataxia and gaze-evoked rebound nystagmus. Pregabalin intoxication can evoke tran
