The glial reaction in closed head injuries.

The development of the glial reaction in human closed head injury has been investigated using morphometry and statistical analysis. The brains of eight individuals that survived less than 48 h following closed head injury were analysed using immunoperoxidase for glial fibrillary acidic protein (GFAP). Controls were eight patients without neurological disease. The density of reactive astrocytes was estimated in 25 fields in each of 10 different areas sampled bilaterally avoiding the subpial and subependymal zones, and the perivascular white matter. There was great variation between the zones wi

Closed head injury.

Closed head injuries vary from the very minor to the catastrophic. It is often difficult to differentiate the severity at initial presentation. Serial assessment is very valuable. Awareness of facial injuries is aided by familiarity with facial bone anatomy and the clinical presentation of orbital, zygomatic, maxillary, and mandibular fracture. Functional injury such as concussion may coexist with other injuries. This article will discuss closed head trauma and outline specific injuries to the face, brain, skull, and its surroundings.

[Injuries of cranial nerves II-VII from mortal closed head injuries].

Brain autopsy was carried out in 12 patients who died within 3 days of closed head injury. A segment of the brain stem with the entire intracranial portions of nerves II-VII was dissected in each case, fixed in formaline and inspected thoroughly under magnification. Injuries to the examined nerves were seen in 9 subjects. The oculomotor nerve was torn out of the midbrain unilaterally in 3 and bilaterally in 2 cases. In one patient only a portion of the superficial fibres on the medial aspect of the nerve was ripped out from the brainstem. In two patients the IV nerve was ruptured. The root of

[Management of closed head injury].

OBJECTIVE: To review current concepts regarding the management of severe closed head injury and determine the possible causes of brain damage in these patients. DEVELOPMENT: Severe closed head injury causes a high rate of mortality and disability in developed countries. The diagnosis and management of these patients has progressed in recent years, from the availability of brain scanning to varied therapeutic methods based on brain metabolism and intracranial dynamics. We have reviewed the different neuropathological processes in relation to closed head injury and emphasized the importance of

Management strategies for severe closed head injuries in children.

Brain injuries represent the most common cause of mortality and long-term morbidity from trauma in children. The management of closed head injuries focuses on prevention of secondary injury by optimizing the delivery of oxygen and nutrients to the injured brain while minimizing neuronal metabolic demand. Despite the known differences between the immature and mature brain, treatments used in head-injured children are mainly extrapolated from those employed in adults due to the paucity of class one and two studies focused on the pediatric age group. Therapies intended to minimize secondary brain

Pediatric closed head injuries treated in an observation unit.

BACKGROUND: Closed head injury (CHI) is common in childhood and frequently results in hospital admission for observation and treatment. Observation units (OUs) have shown significant benefits for patients and physicians. At our institution, a level 1 pediatric trauma center, patients with CHI are often admitted to an OU for up to 24 hours of observation and treatment. STUDY OBJECTIVES: To describe characteristics of patients with a CHI admitted to a pediatric OU and to identify demographic, historical, clinical, and radiographic factors associated with the need for unplanned inpatient admissi

Primary brain stem lesions caused by closed head injuries.

Traumatic lesions of the brain stem are of two types: primary, which are considered to be caused at the moment of impact, and secondary, associated with supratentorial mass lesion. Of the 239 patients with a serious head injury who showed a severe disturbance of consciousness upon admission and who had CT scan carried out immediately, 21 cases were considered to have a primary brain stem lesion with initial CT scan. A primary brain stem lesion was found in 21 of 239 (8.8%) of patients with serious head injury. Their injuries were caused primarily by traffic accidents. Sixteen of the 21 cases s

Traumatic basal ganglia hematoma following closed head injuries in children.

PURPOSE: High-velocity trauma with acceleration/deceleration forces turns into shear stress over lenticulostriate or anterior choroidal arteries that lead to basal ganglia hemorrhage. Traumatic basal ganglia hematoma has rarely been described in pediatric population. The aim of this study was to present our clinical series of pediatric patients with traumatic basal ganglia hematoma and to analyze the prognostic indicators of traumatic basal ganglia hematoma. METHODS: In this retrospective case series, emergency admissions of pediatric patients with traumatic basal ganglia hematoma due to clos

Dabigatran bleed risk with closed head injuries: are we prepared?

OBJECT: The direct thrombin inhibitor dabigatran has recently been approved in the US as an alternative to warfarin. The lack of guidelines, protocols, and an established specific antidote to reverse the anticoagulation effect of dabigatran potentially increases the rates of morbidity and mortality in patients with closed head injury (CHI). Confronted with this new problem, the authors reviewed their initial clinical experience. METHODS: The authors retrospectively reviewed all cases of adult patients (age ≥ 18 years) who sustained CHI secondary to ground-level falls and who presented to the

Closed head injuries in children: is hospital admission always necessary?

The objective of this study was to identify a group of patients with mild closed head injury, lack of other significant trauma, and normal head computerized tomograph (CT), who could be safely observed at home by a reliable caretaker. Data were from a retrospective chart review of pediatric emergency department (PED) and hospital course of an urban university children's hospital. The pediatric trauma registry was used to identify patients one to 17 years old seen in the PED with closed-head injury and normal head CT between June 1991 and August 1992. A total of 746 patients with heads injury w

Visual dysfunction in closed head injury.

Damage to the visual system often occurs after head injury. Clinical manifestations of visual system dysfunction may be monocular or binocular, and may include reduced visual acuity, visual field loss, and in some cases complete blindness. This paper reviews and updates nine cases of visual dysfunction secondary to closed head trauma. Pathophysiology, prognosis and optometric co-management with neurology and neuro-ophthalmology are highlighted. Optometric roles in visual rehabilitation and the prevention of head injury are addressed.

Pediatric minor closed head injury.

Many studies have found conflicting evidence over the use of clinical indicators to predict intracranial injury in pediatric mild head injury. Although altered mental status, loss of consciousness, and abnormal neurologic examination have all been found to be more prevalent among head-injured children, studies have observed inconsistent results over their specificity and predictive value. Children older than 2 years have been evaluated, managed, and studied differently than those less than 2 years old. Evidence strongly supports a lower threshold to perform a CT scan in younger children becaus

Hypopituitarism following closed head injury.

We describe four young patients (age 19-34 years) with hypopituitarism following closed head injury. The diagnosis was made by demonstration of low basal pituitary hormone levels and dynamic tests showing low pituitary reserve. The time interval between the injury and diagnosis of hypopituitarism was between three weeks and two months demonstrating the difficulty and complexity of making this diagnosis. Three of our patients (all patients suffering from anterior pituitary hormone deficiency) had ACTH deficiency, a condition which may be life threatening if left undiagnosed; these patients also

Mutism after closed head injury.

Prospective study of patients admitted to a hospital for closed head injury showed that nine patients (nearly 3%) became mute for varying periods despite recovery of consciousness and communication through a nonspeech channel. Computed tomography (CT) showed subcortical lesions situated primarily in the putamen and internal capsule of four patients, whereas four of the five patients without subcortical lesions had left-hemisphere cortical injury. The patients without subcortical injury visualized by CT exhibited a longer duration of impaired consciousness consistent with severe diffuse brain i

A review of 100 closed head injuries associated with facial fractures.

One hundred closed head injuries associated with facial fractures treated over a 78-month period at a level I trauma center in Northeast Ohio were reviewed. The incidence of closed head injury in patients with facial fractures was 17.5%. Males suffered closed head injuries four times more often than females, and sustained severe intracranial injuries eight times as often. The 16- to 30-year age group predominated (59%). Although motor vehicle accidents were the most frequent cause of injury (61%), motorcycle accidents were associated with the most severe head injury. The mandible/midface fract

Adolescents with closed head injuries. A report of initial cognitive deficits.

Accidents represent the leading cause of morbidity in the adolescent age group. Closed head injuries (CHI) sustained in such accidents are frequently associated with cognitive deficits. The intent of this study was to explore the neuropsychological functioning of adolescents with CHI. Thirty-three teenagers who had sustained CHI as the result of a motor vehicle or motor vehicle/pedestrian accident were compared with orthopedically injured (n = 13) and matched, noninjured (n = 35) control groups. Results indicated that immediately after injury, patients with CHI performed poorer than their coun
