Headache secondary to psychiatric disorders.

Recent literature shows an interest in the relationship between psychiatric disorders and headache. This relationship is complex and multifaceted, with existing studies confirming high rates of comorbidity between psychiatric disorders (especially depression and anxiety) and migraine and tension-type headache, implicating comorbid psychiatric disorders as risk factors for headache progression and chronification, and underscoring the need for assessment and treatment of relevant disorders. A smaller amount of literature has focused on headache that presents exclusively during and secondary to a

Diagnosing Secondary and Primary Headache Disorders.

PURPOSE OF REVIEW: This article provides a systematic diagnostic approach to the patient with headache. RECENT FINDINGS: The vast majority of patients presenting with headache in clinical practice have a primary headache disorder. The most common primary headache disorder in clinical practice is overwhelmingly migraine. Unfortunately, a substantial proportion of patients with migraine do not receive an accurate diagnosis. In addition, the clinical features of migraine overlap with secondary causes of headache, making a careful history and deliberative evaluation for warning symptoms or signs

Headache Disorders: Differentiating Primary and Secondary Etiologies.

In the initial assessment of a headache patient, several dangerous secondary etiologies must be considered. A thorough history and physical examination, along with a comprehensive differential diagnosis may alert a physician to the diagnosis of a secondary headache particularly when it is accompanied by certain clinical features. Evaluation and workup include a complete neurological examination, consideration of neuroimaging, and serum/spinal fluid analysis if indicated. Careful attention to the patients' history and physical examination will guide the diagnostic work-up and management. In thi

Hallmarks of headache disorders: part 5 - secondary headaches.

BACKGROUND AND AIM: Headache is a common symptom that can be frequently caused by multiple secondary causes. Every person with headache fears to have a severe cause, and healthcare providers must be trained to identify which patients may suffer from these. This manuscript continues the series of hallmarks of headache disorders describing the state of the art regarding the pathophysiology, clinical aspects, diagnosis, red flags, biomarkers, acute treatment, and preventive therapies of secondary headache disorders. MAIN RESULTS: Identification of secondary headache disorders is challenging, due

Cognitive Impairment in Primary and Secondary Headache Disorders.

PURPOSE OF REVIEW: To critically evaluate the recent literature on cognitive impairment and headache. RECENT FINDINGS: Neurocognitive symptoms are prevalent, debilitating, and occur often with both primary and secondary headache disorders. This is a "narrative review of the current literature in PubMed on cognitive function and headache." Migraine is associated with cognitive impairment years before a migraine diagnosis. In young and middle-aged adults, migraine is associated with deficits in attention, executive function, processing speed, and memory. It is unlikely that migraine is associat

Developments in distinguishing secondary vascular headache from primary headache disorders in clinical practice.

INTRODUCTION: Vascular headaches are secondary headache disorders with potentially devastating consequences if missed. Clinicians often struggle to distinguish these from primary headache disorders whereby there is no underlying structural pathology. Here, the authors describe the advancement in our understanding of vascular headache disorders, their clinical presentation and the developments in neuroimaging that facilitate diagnosis. AREAS COVERED: Here the authors discuss the definition of primary and secondary headache disorders. They review the literature on the presentation, choice of ne

Headache characteristics in acromegaly: Only a secondary disorder?

OBJECTIVE: To investigate the characteristics and nature of headache in a population of patients with acromegaly. BACKGROUND: Headache is frequently described by patients with pituitary adenomas. Although it is mainly considered a secondary disorder, it can persist despite effective therapy for pituitary disease. A proper description of headache according to the subtype of pituitary adenoma is not available in literature. In this light, we aimed to analyze headache characteristics in a population of patients with acromegaly. METHODS: In this registry-based retrospective cohort study, headach

Secondary Headache Disorders: Approach, Workup, and Special Considerations for Select Populations.

Headache is one of the most common diagnoses in neurology. A thorough understanding of the clinical presentation of secondary headache, which can be life-threatening, is critical. This review provides an overview of the diagnostic approach to a patient with headache, including discussion of "red," "orange," and "green" flags. We emphasize particular scenarios to help tailor the clinical workup to individual circumstances such as in pregnant women, when particular attention must be paid to the effects of blood pressure and hypercoagulability, as well as in older adults, where there is a need fo

A rose by another name? Characteristics that distinguish headache secondary to temporomandibular disorder from headache that is comorbid with temporomandibular disorder.

Co-occurring pain conditions that affect overlapping body regions are complicated by the distinction between primary vs secondary pain conditions. We investigate the occurrence of headache and painful temporomandibular disorder (TMD) in a community-based, cross-sectional study of US adults in the Orofacial Pain: Prospective Evaluation and Risk Assessment (OPPERA-II) study. A specific goal was to determine whether headache attributed to TMD is separable from primary headache. Using DC/TMD and International Classification of Headache Disorders-third edition criteria, 3 groups of individuals were

Migraine-like headache attributed to a causative disorder: primary migraine or secondary headache with a migraine-like phenotype?

BACKGROUND: Migraine-like headaches can accompany or be the initial manifestation of many neurological disorders. When a migraine-like headache develops or worsens in the presence of another neurological disorder, there is often uncertainty about whether the headache is true migraine (i.e., the primary headache) or is a secondary headache with migraine-like symptoms. To answer this question, we analyzed headache characteristics and the effect of treatment in several neurological disorders. MATERIALS AND METHODS: The study was a retrospective analysis of five existing datasets collected by the

Headaches secondary to psychiatric disorders (HSPD): a retrospective study of 87 patients.

OBJECTIVE: We describe a sample of patients receiving a diagnosis of headache attributed to psychiatric disorder (HSPD). BACKGROUND: The international literature to date provides only a few case reports of patients presenting with HSPD. METHOD: A retrospective study of the medical records of all patients having received HSPD when consulting at a headache emergency center during 2009. RESULTS: Out of a total of 8479 patients seen during one year, 25 men and 62 women received an HSPD diagnosis (1.02%), mean age 40.3 ± 14 years. Peripheral nerve blocks for primary and secondary headache disorders: review of current evidence and a practical approach.

Headache is prevalent, disabling, and a frequent neurological referral in the healthcare system. Clinic-based procedures have evolved in recent years to play an important role in headache medicine, with growing evidence on the safety, tolerability and efficacy of peripheral nerve blocks (PNBs). Despite novel headache therapies, PNBs are still widely used in headache services to treat primary and secondary headache disorders, including cluster headache and other trigemino-autonomic cephalalgias, migraine, occipital neuralgia, and other less frequent headache disorders. We aim to provide an upda

Pectoralis Minor Tenotomy with Occasional Secondary Neurolysis Significantly Reduces Self-Reported Pain and Headaches Across Heterogenous Chronic Pain Disorders of the Upper Limb.

Background and Objectives: Many patients suffer from chronic pain of the shoulder, neck, upper back, and/or arm. They may be diagnosed with fibromyalgia, complex regional pain syndrome, myofascial pain, thoracic outlet, subacromial pain, cervical radiculopathy, cervicogenic headaches, post-mastectomy pain, and/or occupational shoulder disorder. The pectoralis minor (PM) is the only muscle of the scapula controlled by the lower trunk of the brachial plexus. In the Human Disharmony Loop (HDL), this neurologic asymmetry produces persistent protraction of the scapula. Protraction deforms the scapu

Refractory primary and secondary headache disorders that dramatically responded to combined treatment of ultrasound-guided percutaneous suprazygomatic pterygopalatine ganglion blocks and non-invasive vagus nerve stimulation: a case series.

In 1981, Devoghel achieved an 85.6% success rate in treating patients with treatment-refractory cluster headaches with alcoholization of the pterygopalatine ganglion (PPG) via the percutaneous suprazygomatic approach. Devoghel's study led to the theory that interrupting the parasympathetic pathway by blocking its transduction at the PPG could prevent or treat symptoms related to primary headache disorders (PHDs). Furthermore, non-invasive vagus nerve stimulation (nVNS) has proven to treat PHDs and has been approved by national regulatory bodies to treat, among others, cluster headaches and mig

Personality in Chronic Headache: A Systematic Review with Meta-Analysis.

BACKGROUND: Chronic headache (CH) is a condition that includes different subtypes of headaches and that can impair different life domains. Personality traits can play a relevant role both in the development and in coping with this medical condition. The first aim of the present study is to realize a systematic review of the personality traits associated with CH compared to healthy controls; the second objective is to carry out a quantitative meta-analysis with the studies using the same instrument to assess personality traits. METHOD: The literature search encompassed articles published from

Medication Overuse Headache.

BACKGROUND: Medication overuse headache (MOH) is one of the highly disabling headache disorder and affects about 1% of the population of the world. It is associated with the development of headache for 15 days or more, with consumption of acute symptomatic medications for 10-15 days (depending on the class of drug, like, simple analgesics, triptans, and opioids) in a month, used for relief of headache for three or more months, in a known patient of primary headache disorder. OBJECTIVE: The aim of this study was to review the topic of MOH and present the details of this disorder with an emphas
