Conceptualization of a Health Care Coalition Framework in Georgia Based on the Existing Regional Coordinating Hospital Infrastructure.

The watershed events of September 11, 2001; the anthrax attacks; Hurricane Katrina; and H1N1 necessitated that the United States define alternative mechanisms for disaster response. Specifically, there was a need to shift from a capacity building approach to a capabilities based approach that would place more emphasis on the health care community rather than just first responders. Georgia responded to this initiative by creating a Regional Coordinating Hospital (RCH) infrastructure that was responsible for coordinating regional responses within their individual geographic footprint. However, i

Bridging Public Health and Health Care for Systems-Level Change: The NYC Coalition to End Racism in Clinical Algorithms, 2021-2023.

The use of race-based clinical algorithms contributes to population-level racial health inequities. To address these inequities, the New York City Department of Health and Mental Hygiene convened 10 health systems in the Coalition to End Racism in Clinical Algorithms (CERCA). Our mixed-methods evaluation showed that health systems implemented algorithm change and provider education and that CERCA members perceived the coalition as a catalyst for systems-level change. CERCA is a replicable model for collaboration between public health and medicine to advance health equity and antiracist policy.

Community coalitions for smoke-free environments in Armenia and Georgia: A mixed methods analysis of coalition formation, implementation and perceived effectiveness.

Effective models for aligning public health and civil society at the local level have the potential to impact various global health issues, including tobacco. Georgia and Armenia Teams for Healthy Environments and Research (GATHER) is a collaboration between Armenia, Georgia and U.S. researchers involving a community randomized trial testing the impact of community coalitions to promote smoke-free policy adoption and compliance in various settings. Community Coalition Action Theory (CCAT) was used to guide and describe coalition formation, implementation and effectiveness. Mixed methods were u

Community Coalitions Implementing and Sustaining Policy, Systems and Environment Strategies for Rural Health Equity.

Rural communities have assets that can be leveraged to address health disparities, particularly through complex policy, systems and environment (PSE) change approaches. Eleven rural community coalitions across Georgia participated in The Two Georgias Initiative, a 5-year health equity project from 2017 to 2022 designed to address rural-urban health disparities. The coalitions addressed community health concerns through implementing county-specific PSE and programmatic strategies tracked through a Community Change Tracking Tool. This mixed-methods analysis assessed the variation in strategies i

Valuing the primary care patient base.

This analysis, albeit limited, can help clinics and integrated health systems with their medical staff planning, and it demonstrates the importance of the primary care patient base as a foundation for health services planning and economic and market strategies. It is important to note, however, that when primary, specialty, subspecialty, surgical, and hospital care are well planned and marketed, it is virtually impossible to identify the relative contribution of each to the overall success of the system. Consequently, primary care physicians should not attribute the value discussed here solely

Healthcare coalitions: the new foundation for national healthcare preparedness and response for catastrophic health emergencies.

After 9/11 and the 2001 anthrax letters, it was evident that our nation's healthcare system was largely underprepared to handle the unique needs and large volumes of people who would seek medical care following catastrophic health events. In response, in 2002 Congress established the Hospital Preparedness Program (HPP) in the U.S. Department of Health and Human Services (HHS) to strengthen the ability of U.S. hospitals to prepare for and respond to bioterrorism and naturally occurring epidemics and disasters. Since 2002, the program has resulted in substantial improvements in individual hospit

Using Collaborative Coalition Processes to Advance Community Health, Well-Being, and Equity: A Multiple-Case Study Analysis From a National Community Transformation Initiative.

Spreading Community Accelerators Through Learning and Evaluation (SCALE) was a Robert Wood Johnson Foundation-funded initiative from 2015 to 2017 to build capability of 24 community coalitions to advance health, well-being, and equity. The SCALE theory of change had three components: develop leadership capability, build relationships within and between communities, and create an intercommunity system to spread promising ideas. The theory was operationalized through training academies, coaching, and peer-to-peer learning that explicitly addressed equity and systems change. In this article, we d

Six components necessary for effective public health program implementation.

Public health programs succeed and survive if organizations and coalitions address 6 key areas. (1) Innovation to develop the evidence base for action; (2) a technical package of a limited number of high-priority, evidence-based interventions that together will have a major impact; (3) effective performance management, especially through rigorous, real-time monitoring, evaluation, and program improvement; (4) partnerships and coalitions with public- and private-sector organizations; (5) communication of accurate and timely information to the health care community, decision makers, and the publ

CDC Grand Rounds: Addressing Preparedness Challenges for Children in Public Health Emergencies.

Recent public health emergencies including Hurricane Katrina (2005), the influenza H1N1 pandemic (2009), and the Ebola virus disease outbreak in West Africa (2014–2015) have demonstrated the importance of multiple-level emergency planning and response. An effective response requires integrating coordinated contributions from community-based health care providers, regional health care coalitions, state and local health departments, and federal agency initiatives. This is especially important when planning for the needs of children, who make up 23% of the U.S. population (1) and have unique need

Building a Rural, Coalition-Based Approach to Youth Substance Use Prevention: The Drug Free Dickson Coalition Model.

Youth substance use remains a critical public health issue in the United States, with rural communities experiencing disproportionate risk because of limited access to prevention resources and persistent barriers related to behavioral and mental health. Rural youth are often exposed to elevated risk factors, including geographic isolation, reduced availability of extracurricular and prosocial activities, transportation limitations, and fewer specialized prevention and treatment services. These challenges underscore the need for comprehensive, community-driven prevention approaches that are res

Financing health care in Ghana.

Attempts to recover some government health care expenses through user charges have produced less revenue than hoped. National health insurance offers an attractive alternative, but needs to include features which check cost escalation. Community involvement and traditional medicine can also help to reduce costs.
