Health Care Rationing in a Just Society: The Clinical Effectiveness Model.

Representing 18% of gross domestic product, and projected to increase to 20% by 2022, health care costs in the United States are an unsustainable expense. The clinical effectiveness model of cost containment is an ethical and self-sustaining paradigm that can assist bending the health care-cost curve. As envisioned by Buyx et al, clinically effective care is aimed at making the practice of medicine more explicitly evidence based with the goals of improving clinical success, efficiency, and value. I provide a vision for applying the clinical effectiveness model to the American health care syste

Raising awareness of disabled lives and health care rationing during the COVID-19 pandemic.

Discriminatory and ableist health care rationing policies have raised serious concern in the disability community during the novel coronavirus pandemic. These concerns reflect the long-standing devaluation of disabled lives and place considerable weight on the disability community during this already stressful time. Nondisabled psychologists should work to advocate for the rights of people with disabilities by amplifying the voices and concerns of disabled activists so that history does not repeat itself. Disability versus futility in rationing health care services: defining medical futility based on permanent unconsciousness--PVS, coma, and anencephaly.

All societies must ration health care services in the face of unlimited demand. The concept of medical futility may appear to be an uncontroversial means by which to ration services. Depending upon how it is applied, however, limiting services based on alleged medical futility may violate prohibitions against disability-based discrimination. In particular, use of medical futility to require removal of life-sustaining interventions has been held to violate the Americans with Disabilities Act. The ADA protects both people with disabilities who are conscious and people in unconscious states, such

[Evaluation of primary health care physicians' knowledge on rational antibiotic use].

Physicians' prescribing practices impact greatly on rational drug use and expenditure. The aim of this cross-sectional study is to determine primary health care physicians' knowledge on rational antibiotic use in Izmir. A total of 475 physicians from primary health care centres (response rate: 100%) participated in this study. Prescription information of physicians was collected with a standardized questionnaire. It was detected that 48.4% of the physicians have prescribed antibiotics in one of each three (30%) prescriptions, while 19.4% in one of each five prescriptions. The most frequent rea

Rational use of medicines in the older adult: the role of trust in the health care system.

BACKGROUND: More than 50% of drugs used by people worldwide are used inappropriately. This rate increases even more in older adults due to reasons such as memory loss, forgetfulness, and polypharmacy. The population of older adults is increasing worldwide, and therefore, rational drug use in older individuals is becoming even more important. OBJECTIVE: This study aims to determine the effects of demographic characteristics and trust in the health care system on the rational use of medicines among older adults. DESIGN: A cross-sectional study design was employed, and data were collected from ol

Rationalizing definitions and procedures for optimizing clinical care and public health in fetal death and stillbirth.

Despite the recent focus on stillbirth, there remains a profound need to address problems associated with the definitions and procedures related to fetal death and stillbirth. The current definition of fetal death, first proposed in 1950, needs to be updated to distinguish between the timing of fetal death (which has etiologic and prognostic significance) and the timing of stillbirth (ie, the delivery of the dead fetus). Stillbirth registration procedures, modeled after live birth registration and not death registration, also need to be modernized because they can be an unnecessary burden on s

Technical Rationality Under Pressure: Nursing Leadership and Clinical Governance in Politically Volatile Health-Care Settings.

Clinical protocols are commonly framed as technical instruments for standardizing care and improving patient safety. Their implementation in practice, however, depends not only on the strength of the evidence that underpins them, but also on the organizational and political conditions that enable their continuity over time. In health-care settings characterized by administrative instability, political interference, patronage-based appointments, and high workforce turnover, adherence to protocols becomes especially fragile. This practice-based analysis examines how such conditions affect clinic

A Comparative Study of the Effects of Different Decellularization Methods and Genipin-Cross-Linking on the Properties of Tracheal Matrices.

BACKGROUND: Universal Health Coverage (UHC) in Indonesia is planned to be fully implemented in 2019 through the National Health Insurance (NHI) launched in January 2014. However, limited financial resources cause health care providers (HCPs) to perform rationing in providing medicine services. The purpose of this study was to analyze rationing strategies performed by HCPs for potentially beneficial essential medicines due to financial constraints and other reasons in the Indonesian NHI Plan and evaluate its fairness. METHODS: A qualitative study was conducted to find out the rationing perform

The cost of living: kidney dialysis, rationing and health economics in Britain, 1965-1996.

How important is research in shaping policy when a new life-saving medical technology becomes available, but happens to be very expensive? Taking the case of kidney dialysis, this paper argues that the emerging discipline of health economics had little influence relative to national differences in health service organization and cultures of expectation of provision. Paradoxically, the most effective covert rationing was achieved under the British NHS which ostensibly provides free care for all, while the uncentralised market system in the US gave way, on this issue, to almost universal state-s

Contested evidence: a Dutch reimbursement decision taken to court.

This paper examines a remarkable lawsuit in health care rationing. The Patients Association for Interstitial Cystitis sued the Dutch National Health Care Institute for alleged misconduct against Interstitial Cystitis patients, as the Institute decided that bladder instillations with chondroitin sulphate or hyaluronic acid are no longer covered by the basic health insurance. The patients' organisation challenged the Institute for basing its standpoint on scientific evidence; overruling clinical expertise and patients' experiences. While scientific advice is often solicited in public health issu

Allocating scarce intensive care resources during the COVID-19 pandemic: practical challenges to theoretical frameworks.

The COVID-19 pandemic strained health-care systems throughout the world. For some, available medical resources could not meet the increased demand and rationing was ultimately required. Hospitals and governments often sought to establish triage committees to assist with allocation decisions. However, for institutions operating under crisis standards of care (during times when standards of care must be substantially lowered in the setting of crisis), relying on these committees for rationing decisions was impractical-circumstances were changing too rapidly, occurring in too many diverse locatio

Simulation of New York City's Ventilator Allocation Guideline During the Spring 2020 COVID-19 Surge.

IMPORTANCE: The spring 2020 surge of COVID-19 unprecedentedly strained ventilator supply in New York City, with many hospitals nearly exhausting available ventilators and subsequently seriously considering enacting crisis standards of care and implementing New York State Ventilator Allocation Guidelines (NYVAG). However, there is little evidence as to how NYVAG would perform if implemented. OBJECTIVES: To evaluate the performance and potential improvement of NYVAG during a surge of patients with respect to the length of rationing, overall mortality, and worsening health disparities. DESIGN,

A survey of Ethiopian physicians' experiences of bedside rationing: extensive resource scarcity, tough decisions and adverse consequences.

BACKGROUND: Resource scarcity in health care is a universal challenge. In high-income settings, bedside rationing is commonly discussed and debated as a means to addressing scarcity. However, little is known about physicians' experiences in resource-limited contexts in low- income countries. Here we describe physicians' experiences regarding scarcity of resources, bedside rationing, use of various strategies to save resources, and perceptions of the consequences of rationing in Ethiopia. METHODS: A national survey was conducted amongst physicians from 49 public hospitals using stratified, mul

"By the Skin of Our Teeth": U.S. Hospital, Regional, and State Experiences of Scarcity During the COVID-19 Pandemic.

BACKGROUND: The COVID-19 pandemic presented unprecedented challenges to hospital system and critical care resources, leading to significant changes to operations and patient care. There are limited national data on these changes and instances of unsanctioned deviations from patient care, yet understanding the COVID response is key to future preparedness efforts. We sought to understand how hospitals and states navigated scarcity during COVID-19, particularly in the absence of a declaration of crisis standards of care. METHODS: Between February 2022 and September 2022 we conducted 34 interview
