Design and Rationale the SCAN-MP (Screening for Cardiac Amyloidosis With Nuclear Imaging in Minority Populations) Study.

Background Transthyretin amyloid cardiomyopathy (ATTR-CM) is an important cause of heart failure in older individuals. Misfolding and deposition of transthyretin or prealbumin protein causes ATTR-CM in the context of a normal (wild-type) or variant TTR sequence. Variant ATTR-CM is most commonly caused by the substitution of valine for isoleucine at position 122 in transthyretin (Val122Ile or pV142I, almost exclusively observed in individuals of West African ancestry), demonstrated in 3.4% of self-identified Black individuals in the United States with an estimated 1.5 million carriers. The Relationship between Health Disparities, Psychosocial Functioning and Health Outcomes in Pediatric Hematology-Oncology and Stem Cell Transplant Populations: Recommendations for Clinical Care.

Not only do racial and ethnic minority children and adolescents with chronic illness experience disparities in health status and health outcomes, they also experience significant healthcare disparities, including differences in healthcare coverage, access to care, and quality of care. It is well known that the interaction between psychosocial functioning, health behaviors and ethnic and racial disparities, ultimately leads to worse health and psychosocial outcomes in pediatric and AYA chronic illness patient populations, including increased rates of morbidity and mortality. Investigating the i

Stressors and Impact of the COVID-19 Pandemic on Vulnerable Hispanic Caregivers and Children.

Psychological sequelae are important elements of the burden of disease among caregivers. Recognition of the impact of adversity and stress biomarkers is important to prevent mental health problems that affect rearing practices and child well-being. This cross-sectional study explored social determinants of health (SDoH)-mediated stressors during COVID-19 and risks for mental health problems among caregivers of children with prenatal Zika virus exposure. Twenty-five Hispanic caregivers completed surveys assessing SDoH vulnerabilities, COVID-exposures and impact, post-traumatic stress disorder (

Clinical Penetrance of the Transthyretin V122I Variant in Older Black Patients With Heart Failure: The SCAN-MP (Screening for Cardiac Amyloidosis With Nuclear Imaging in Minority Populations) Study.

Background Transthyretin amyloid cardiomyopathy (ATTR-CM) is an underdiagnosed cause of heart failure (HF) among patients ≥60 years of age. Although the V122I (valine to isoleucine substitution at position 122 of the transthyretin protein) variant associated with hereditary ATTR-CM is present in 3.4% of self-identified Black individuals in the United States (or 1.5 million people), the phenotypic penetrance is not known. Methods and Results The SCAN-MP (Screening for Cardiac Amyloidosis With Nuclear Imaging in Minority Populations) study is a currently accruing prospective multisite study desi

High prevalence of MUTYH associated polyposis among minority populations in Israel, due to rare founder pathogenic variants.

BACKGROUND: Autosomal recessive conditions are common in consanguineous populations. Since consanguinity is common in the Israeli Arab population, we evaluated the rate of MUTYH polyposis (MAP) among polyposis patients in this population and studied Pathogenic Variants (PVs) spectrum. METHODS: We reviewed health records of all Arab and Druze polyposis patients referred for counseling during 2013-2020 who fulfilled the Israeli Genetic Society criteria for MUTYH/APC testing, in a tertiary center in Northern Israel and four additional gastro-genetic clinics in Israel. RESULTS: The Northern coho

Improving access to care and delivery to marginalized and vulnerable populations: a committee opinion.

It has been estimated that only a quarter of persons with infertility in the United States can sufficiently access infertility care. Against this backdrop of disparity, specific populations, including persons of color, sexual and gender minorities, immigrants, and lower-income persons, face barriers that further constrain access to care. This document outlines these communities' barriers and reviews best practice recommendations to extend inclusive access to care for marginalized populations. This reference is intended to support health professionals with knowledge of barriers that limit acces

Impact of Limited Access to Surgical Care in Medically Underserved Communities.

Summary/BackgroundMedically underserved communities and ethnic minorities constitute a significant portion of the vulnerable population within the United States. Recent changes in the health care structure, rising inflation with a decline in median household income, and the SARS-CoV-2 pandemic have disproportionately impacted communities of low socioeconomic status. Healthcare providers and federal organizations must be aware of how these factors influence access to surgical care to tailor treatment, interventions, and policies better to meet the needs of these populations.MethodWe systematica

Ensemble machine learning of factors influencing COVID-19 across US counties.

Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) the causal agent for COVID-19, is a communicable disease spread through close contact. It is known to disproportionately impact certain communities due to both biological susceptibility and inequitable exposure. In this study, we investigate the most important health, social, and environmental factors impacting the early phases (before July, 2020) of per capita COVID-19 transmission and per capita all-cause mortality in US counties. We aggregate county-level physical and mental health, environmental pollution, access to health care,

Remote Patient Monitoring for Hypertension: Feasibility and Outcomes of a Clinic-Based Pilot in a Minority Population.

BACKGROUND: In the US 48% of adults have hypertension, with direct costs in excess of $130 billion per year. Remote patient monitoring (RPM) has been discussed as a useful tool in the treatment of hypertension, but few studies evaluate its cost effectiveness or efficacy in minority, lower socio-economic (SES) populations. Our study aims to evaluate the clinical and financial outcomes of RPM in hypertension management in a primarily minority, low-SES population. METHODS: In this prospective cohort pilot study, patients with uncontrolled primary hypertension (defined via Joint National Committe

The Impact of Coronavirus Disease 2019 (COVID-19) on Vulnerable Communities: What the Clinician Should Know.

The COVID-19 pandemic unmasked striking health disparities within the US healthcare system and significantly impacted vulnerable populations. While much attention on vulnerability has been directed toward older individuals with underlying medical conditions, key populations-including the uninsured, racial and ethnic minorities, and pregnant individuals-were considerably affected and require intervention. This review examines the barriers these populations encountered and outlines how clinicians can engage at the provider, practice, community, and national levels to advance health equity and im

Psychosocial risks of prescription drug misuse among U.S. racial/ethnic minorities: A systematic review.

Prescription drug misuse (PDM) is the leading cause of accidental death in the U.S. One in five Americans report at least one lifetime incident of PDM. PDM has been studied extensively, yet there is limited inclusion of racial/ethnic minorities due to purportedly lower rates of PDM. However, health disparate groups often face more detrimental consequences of substance abuse including behavioral, social, and medical/mental health (e.g., injury, HIV/AIDS, incarceration, educational attainment, and comorbidity). Failing to characterize risk factors for and consequences of PDM in racial/ethnic min

A Systematic Review and Meta-Analysis on Oral Health Disparities Among the Indigenous Paediatric Population.

There is a knowledge gap in the literature regarding oral health disparities (OHD) in minority and indigenous (IG) paediatric cohorts that needs to be addressed. Disparities in oral health among children are a pressing concern, highlighting inequities in access to dental care and meeting needs. The current systematic review aims to provide a comprehensive synthesis of the prevailing understanding of OHD in the minority and IG strata. A meticulous search strategy was formulated by a team of reviewers to identify pertinent studies from databases of PubMed, MEDLINE, Scopus, Google Scholar and EMB

Underrepresentation of blind and deaf participants in the All of Us Research Program.

Blind and deaf individuals comprise large populations that often experience health disparities, with those from marginalized gender, racial, ethnic and low-socioeconomic communities commonly experiencing compounded health inequities. Including these populations in precision medicine research is critical for scientific benefits to accrue to them. We assessed representation of blind and deaf people in the All of Us Research Program (AoURP) 2018-2023 cohort of participants who provided electronic health records and compared it with the Centers for Disease Control and Prevention 2018 national esti

Health access, health disparities, and Medicare's transforming episode accountability model (TEAM) in the field of spine surgery.

In the past, many health reform efforts have been touted as a means to tangentially reduce healthcare disparities. Few have shown any demonstrable efficacy in this arena. There is reasonable concern that the machinations of Medicare's Transforming Episode Accountability Model (TEAM) may also exert unintended effects on health access and delivery, potentially worsening existent disparities for racial and ethnic minorities as well as other vulnerable populations. As TEAM has yet to be implemented, this review intends to prognosticate potential pitfalls and behaviors that may be motivated by this

Area-Level Disparities in Pneumococcal Vaccination Among Publicly Insured US Adults: 2016-2019.

OBJECTIVES: Understanding pneumococcal vaccine uptake across county-level social vulnerability is important for identifying geographic disparities and informing strategies to increase vaccine uptake and reduce the incidence of pneumococcal disease (PD). Using a retrospective cohort design, we characterized differences in vaccine uptake among publicly insured US adults, comparing vaccination rates among beneficiaries in counties with high versus low social vulnerability. METHODS: We assessed uptake and disease risk using claims for Medicare (aged ≥65 y; 2016-2019) and Medicaid (low income or d

Nutrition and Health in the Lesbian, Gay, Bisexual, Transgender, Queer/Questioning Community: A Narrative Review.

Sexual and gender minorities have a higher risk for health and nutrition-related disparities across the life course compared to the heterosexual or cisgender population. Experiences of stigmatization and discrimination are associated with diminished mental health quality and psychological distress, which are risk factors for developing various eating disorders. Other nutrition disparities include increased risk for food insecurity, body dissatisfaction, and weight complications, such as those experienced by the transgender population in association with gender-affirming hormone therapies. End-of-life dementia care: a qualitative study of the experiences and perceptions of minority ethnic and economically disadvantaged groups.

BACKGROUND: Dementia is a leading cause of morbidity and mortality amongst ageing populations. However, palliative care is often poor or non-existent. People from minority ethnic and economically disadvantaged communities are especially likely to miss out. Research exploring how inclusive end-of-life (EOL) care should be provided for people living with dementia (PLWD) is limited and commonly fails to include sufficient representation of people from minority ethnic groups and those living in poverty. AIMS: Our aim was to understand the experiences and perceptions of EOL dementia care amongst u
