INTRODUCTION: Emergency department (ED) closures have become increasingly common across health systems worldwide, reflecting mounting pressures from staffing shortages, resource constraints, and rising patient demand. Closures, whether temporary or permanent, pose potential risks to timely access to emergency care and may significantly impact patient outcomes and healthcare system performance. Despite growing attention from policymakers and the public, academic evidence on real-world impacts of ED closures remains fragmented. In this scoping review we compiled and summarized the current litera

This study estimates the effect of nursing home closure on occupancy, net profit margin, and operating margin of nearby nursing homes. We use national nursing home data from 2009 to 2019 from Medicare cost reports, Medicare Provider of Services (POS), and LTCfocus.org data. Using the Callaway and Sant'Anna difference-in-differences model, we compare the changes in occupancy, net profit margin, and operating margin between incumbent nursing homes in markets with any closure and nursing homes in markets without a closure, overall, and across rurality. Our findings suggest that nursing home closu

In the 1990s, many hospitals will continue to be confronted with financial, regulatory, and medical staff issues that threaten their survival. Inadequate reimbursement, HCFA certification problems, and aging medical staffs are just a few examples of the many difficult issues health care institutions face today and that have contributed to the phenomenal number of failing hospitals. Failing hospitals must consider all their options, such as turnaround process, modification of service mix, change to a specialty hospital, transfer to a new owner, or closure. Selection of the most appropriate opti

INTRODUCTION: As a result of the COVID-19 pandemic, outpatient diagnostic imaging (DI) facilities experienced decreased operations and even unprecedented closures. The purpose of this study was to examine the impact of COVID-19 on the practices of DI clinics, and investigate the reasons for the change in their operations during the initial period of the pandemic starting in mid-March 2020.
MATERIALS AND METHODS: A questionnaire was created and distributed to the managers of eighteen outpatient DI clinics in London, Hamilton, and Halton, Ontario, Canada. The managers indicated whether their cl

Between 1990 and 2020, 334 rural hospitals closed in the United States, and since 2011 hospital closures have outnumbered new hospital openings. This scoping review evaluates peer-reviewed studies published since 1990 with a focus on rural hospital closures, synthesizing studies across six themes: 1) health care policy environment, 2) precursors to rural hospital closures, 3) economic impacts, 4) effects of rural hospital closures on access to care, 5) health and community impacts, and 6) definitions of rural hospitals and communities. In the 1990s, rural hospitals that closed were smaller, wh

Objectives. To characterize youth detention and incarceration facility closure attempts in the United States from January 2010 to February 2023 and inform ongoing attempts for facility closure. Methods. We conducted a landscape analysis of youth facilities with stated intent to close during our study period. For each facility, we coded for jurisdiction (state vs local), year of attempt, stated reasons for closure, outcome of the facility (closed, remained open, or other), facility size, and what happened to youths after the facility closed. Results. We identified a total of 118 facilities in 3

The U.S. healthcare safety net, a critical lifeline for millions of vulnerable individuals, is facing mounting financial and systemic pressures. Safety-net hospitals (SNHs) have supported the nation's safety net. In this perspective piece birthed from a Society of General Internal Medicine workshop, we examine SNH closures, why this matters for patients, trainees, and clinicians, and offer actionable steps forward. From 2013 to 2023, over 60% of SNH closures occurred in Medicaid non-expansion states and disproportionately impacted rural areas. The closures impact access and care quality in alr

BACKGROUND: Rural hospitals are increasingly at risk of closure. Closure reduces the availability of hospital care in rural areas, resulting in a disparity in health between rural and urban citizens, and it has broader economic impacts on rural communities as rural hospitals are often large employers and are vital to recruiting new businesses to a community. To combat the risk of closure, rural hospitals have sought partnerships to bolster financial performance, which often results in a closure of services valuable to the community, such as obstetrics and certain diagnostic services, which are

The purpose of this paper is to review past and present roles and to speculate on the future of the Ontario provincial psychiatric hospitals (PPHs). Currently, and for the very immediate future only, there are 10 PPHs that are owned and operated by the Ministry of Health of Ontario, each serving a specified population ranging from 250,000 to over 3,000,000. In addition to clinical expertise, provincial psychiatric hospitals contribute greatly to teaching and research. Ontario's mental health reform movement has called for a shift of resources to the community and a downsizing of PPHs by 2003.

BACKGROUND: Hospital closures have become commonplace in the United States but remain controversial. Memorial Hospital of Rhode Island was a 294-bed hospital in a disadvantaged community that closed in 2018 amid falling patient volume and rising costs.
METHODS: Immersion/crystallization method of qualitative analysis was employed in reviewing semi-structured interviews, public testimony, and public documents. Themes that emerged were organized into discrete narrative typographies, represented by illustrative quotations.
RESULTS: Three main narratives of the hospital's closure arose: 1.) fina

This paper traces the implementation of Michigan's program for hospital bed reduction through four phases in the critical first 30 months following enactment: standard-setting, plan development, plan approval, and legislative oversight. Procedural complexity and goal conflict complicated implementation from the start: what began as a simple proposal to close unneeded beds soon became enmeshed in efforts to address long-standing issues of equity in access to care. A combination of administrative, political, and economic factors peculiar to Michigan, as well as the more generic problems incurred

From the legal perspective, closing a hospital can be as difficult as opening one. Whether a hospital is closing or downsizing, it should become familiar with the relevant legal issues before the process begins and develop a strategy for addressing them. Hospitals planning to discontinue services often must give advance notice to federal, state, and local governmental agencies, as well as to nongovernmental organizations such as the Joint Commission on Accreditation of Healthcare Organizations and to contractors. Failure to notify the applicable state licensing agency may violate the hospital

IMPORTANCE: US nursing home closures, whether planned or unexpected, can disrupt continuity of care, place additional burdens on remaining facilities, and disproportionately affect vulnerable populations. It is important to understand the trends and factors associated with these closures, as such knowledge can inform policy decisions, guide resource allocation, and help develop strategies to mitigate negative effects on both residents and the broader health care system.
OBJECTIVE: To examine nursing home operational dynamics in the US and to identify how facilities transition through openings

In April 1995, a public sexually transmitted disease (STD) clinic in the northwest (NW) ward of the District of Columbia closed, leaving one public STD clinic in the southeast (SE) ward to provide public STD services for the entire city. This report summarizes an investigation by CDC following a request from the District of Columbia Department of Health's STD program to evaluate the impact of the NW STD clinic closure on STD case reports. The findings of this investigation indicate that the clinic closure resulted in a marked decrease in reported syphilis in the NW ward, and suggest that NW wa

INTRODUCTION: The time between seeking emergency care and receiving the first healthcare service is referred to as delays in reaching health facilities. Delay in reaching health facilities had three main sub-themes in different studies. These include (i). Availability of transportation and infrastructure, (ii). Distance from health facilities and (iii). Lack of finance for transportation. Although there are plenty of findings about maternal delays in reaching health facilities, studies do not address the delay situation of women in conflict zones all over the world. In Ethiopia, specifically,

IMPORTANCE: Temporary disruptions in health care access are common, but their associations with chronic disease control remain unknown.
OBJECTIVE: To evaluate whether long-term changes in chronic disease control were associated with a temporary 6-month decrease in access to health care services.
DESIGN, SETTING, AND PARTICIPANTS: This cohort study examined the long-term changes in chronic disease control associated with the 6-month closure of the Manhattan facility of the Veterans Affairs (VA) New York Harbor Healthcare System after superstorm Sandy, which caused a significant disruption in
