Harnessing the Power of Health Information Exchange.

In the complex landscape of health care, the journey of a patient with end stage kidney disease (ESKD) is representative of the challenges common to those with chronic health conditions. This article delves into the fragmented nature of health care data and the pivotal role of health information exchange (HIE) in enhancing care coordination. While HIE has the potential to improve patient outcomes by facilitating the seamless transfer of information among providers, its success depends on having a strong operational framework in place. This article provides a foundational review of HIE, tracing

Identifying homelessness using health information exchange data.

BACKGROUND: Homeless patients experience poor health outcomes and consume a disproportionate amount of health care resources compared with domiciled patients. There is increasing interest in the federal government in providing care coordination for homeless patients, which will require a systematic way of identifying these individuals. OBJECTIVE: We analyzed address data from Healthix, a New York City-based health information exchange, to identify patterns that could indicate homelessness. METHODS: Patients were categorized as likely to be homeless if they registered with the address of a ho

Establishing a health information exchange research network (HIERN) to support population health studies.

We propose the Health Information Exchange Research Network (HIERN) to advance population health research by leveraging state and regional HIEs and their representative real-world data. HIERN builds on existing governance and infrastructure to create centralized or distributed data repositories for longitudinal, multisource data integration. It helps study important clinical and epidemiological topics, such as Alzheimer's Disease and Related Dementias, one of the fastest-growing chronic conditions in the United States.

Physician attitudes toward health information exchange: results of a statewide survey.

OBJECTIVE: To assess physicians' attitudes toward health information exchange (HIE) and physicians' willingness to pay to participate in HIE. DESIGN: We conducted a cross-sectional mail survey of 1296 licensed physicians (77% response rate) in Massachusetts in 2007. MEASUREMENTS: Perceptions of the potential effects of HIE on healthcare costs, quality of care, clinicians' time, patients' privacy concerns, and willingness to pay for HIE. RESULTS: After excluding 253 physicians who did not see any outpatients, we analyzed 1043 responses. Overall, 70% indicated that HIE would reduce costs, whi

Building Bridges in Palliative Care: Perspectives on a Community-Health Information Exchange.

INTRODUCTION: Individuals experiencing homelessness or unstable housing conditions face many challenges in accessing palliative and end-of-life care. Although supports may exist in hospitals and community settings, service providers face communication barriers with different agencies and may struggle to connect clients with appropriate community and hospital resources. Community-health information exchanges (CHIEs) are one possible vehicle to enhance communication and understanding between service providers and recipients. They hold potential to support more personalized palliative and end-of-

Development and Assessment of a Public Health Alert Delivered through a Community Health Information Exchange.

Timely communication of information to health care providers during a public health event can improve overall response to such events. However, current methods for sending information to providers are inefficient and costly. Local health departments have traditionally used labor-intensive, mail-based processes to send public health alerts to the provider community. This article describes a novel approach for delivering public health alerts to providers by leveraging an electronic clinical messaging system within the context of a health information exchange. Alerts included notifications relate

Qualitative study of patient consent for health information exchange in an HIV clinic.

INTRODUCTION/BACKGROUND: Health information exchange (HIE) is the secure, electronic transfer and/or accessibility of clinical data among healthcare providers. In the United States (US), the consent process for participation varies state to state. New York State (NYS) mandates written patient consent. The purpose of this study was to examine workflow and perceptions related to obtaining HIE consent in an HIV clinic. METHODS: We used contextual inquiry to observe the HIE consent-related workflow of four registration clerks for a total of 4 hours on two weekdays and subsequently created a flow

Leveraging public health's participation in a Health Information Exchange to improve communicable disease reporting.

BACKGROUND: Infectious diseases can appear and spread rapidly. Timely information about disease patterns and trends allows public health agencies to quickly investigate and efficiently contain those diseases. But disease case reporting to public health has traditionally been paper-based, resulting in somewhat slow, burdensome processes. Fortunately, the expanding use of electronic health records and health information exchanges has created opportunities for more rapid, complete, and easily managed case reporting and investigation. To assess how this new service might impact the efficiency and

Implementation of Health Information Exchange at the Pima County Adult Detention Complex: Lessons Learned.

Successful transition of patients between health care entities is difficult. Historically, the process of transitioning individuals through the corrections system with regard to health care was made even more challenging by the limitation of paper records. Recently, the advent of electronic medical records has improved health care nationwide. The use of health information exchange (HIE) systems in areas such as emergency medicine has also impacted patient outcomes. To date, this technology has not been used in the corrections system. Semi-structured interviews were conducted with 12 correction

Generalization of Machine Learning Approaches to Identify Notifiable Conditions from a Statewide Health Information Exchange.

Healthcare analytics is impeded by a lack of machine learning (ML) model generalizability, the ability of a model to predict accurately on varied data sources not included in the model's training dataset. We leveraged free-text laboratory data from a Health Information Exchange network to evaluate ML generalization using Notifiable Condition Detection (NCD) for public health surveillance as a use case. We 1) built ML models for detecting syphilis, salmonella, and histoplasmosis; 2) evaluated generalizability of these models across data from holdout lab systems, and; 3) explored factors that in

Evaluation of a health information exchange system for microcephaly case-finding - New York City, 2013-2015.

BACKGROUND: Birth defects surveillance in the United States is conducted principally by review of routine but lagged reporting to statewide congenital malformations registries of diagnoses by hospitals or other health care providers, a process that is not designed to rapidly detect changes in prevalence. Health information exchange (HIE) systems are well suited for rapid surveillance, but information is limited about their effectiveness at detecting birth defects. We evaluated HIE data to detect microcephaly diagnosed at birth during January 1, 2013-December 31, 2015 before known introduction

Information exchange between patients with Lynch syndrome and their genetic and non-genetic health professionals: whose responsibility?

Individuals at high risk for Lynch syndrome (LS) should be offered genetic counselling, since preventive options are available. However, uptake of genetic services and follow-up care are currently suboptimal, possibly caused by inadequate exchange of information. Therefore, this qualitative study aims to gain insight in the process of information exchange between patients diagnosed with LS and their non-genetic (i.e., general practitioner, gastroenterologist, gynaecologist) and genetic (i.e., clinical geneticist or genetic counsellor) health professionals concerning referral for genetic counse
