Participant-based studies face fragmented workflows. We describe implementation of the Aims Innovations Research Management System, a cloud-based unified platform that supports the operations pipeline and includes participant-facing AI agents for outreach, screening, consent, and follow-up. Controls include private AI endpoints with no-training assurances, study isolation, human handoff thresholds, HIPAA (Health Insurance Portability and Accountability Act) safeguards, and tamper-evident audit logging. As of June 2026, 10 academic institutions completed governance reviews and initiated studies

PURPOSE: To assess the impact of the Health Insurance Portability and Accountability Act (HIPAA) on research in ophthalmology.
DESIGN: A personal perspective with a review of relevant publications.
METHODS: Review of experience at a single institution as it transitioned to enforcing HIPAA guidelines.
RESULTS: HIPAA has been costly to institutions and will continue to be so. At Johns Hopkins alone, nearly 26,000 employees have had to take HIPAA compliance training and pass examinations with an overall estimated cost of nearly $2 million in the first year. At the same time, complying with HIP

The Health Insurance Portability and Accountability Act (HIPAA) Safe Harbor method requires removal of 18 types of protected health information (PHI) from clinical documents to be considered "de-identified" prior to use for research purposes. Human review of PHI elements from a large corpus of clinical documents can be tedious and error-prone. Indeed, multiple annotators may be required to consistently redact information that represents each PHI class. Automated de-identification has the potential to improve annotation quality and reduce annotation time. For instance, using machine-assisted an

BACKGROUND: The last few years have witnessed an increasing number of clinical research networks (CRNs) focused on building large collections of data from electronic health records (EHRs), claims, and patient-reported outcomes (PROs). Many of these CRNs provide a service for the discovery of research cohorts with various health conditions, which is especially useful for rare diseases. Supporting patient privacy can enhance the scalability and efficiency of such processes; however, current practice mainly relies on policy, such as guidelines defined in the Health Insurance Portability and Accou

PURPOSE: The aim of this study was to assess the effects of team training on operational efficiency during outpatient MRI.
METHODS: In this institutional review board-approved, HIPAA-compliant study, six MRI outpatient sites of a midwestern hospital system were randomized to serve as controls or have their teams trained in advanced communication skills. The fourth quarter of fiscal year 2015 was the trial baseline. The trial ended in the third quarter (Q3) of fiscal year 2016 (FY16). Equipment utilization (completed scans/available slots), hourly scan rates (total orders completed per machine

OBJECTIVE: In response to COVID-19, the informatics community united to aggregate as much clinical data as possible to characterize this new disease and reduce its impact through collaborative analytics. The National COVID Cohort Collaborative (N3C) is now the largest publicly available HIPAA limited dataset in US history with over 6.4 million patients and is a testament to a partnership of over 100 organizations.
MATERIALS AND METHODS: We developed a pipeline for ingesting, harmonizing, and centralizing data from 56 contributing data partners using 4 federated Common Data Models. N3C data qu

BACKGROUND: Health care organizations increasingly rely on business associates (BAs) to provide clinical, administrative, and technology services that require access to protected health information. While the Health Information Technology for Economic and Clinical Health (HITECH) Act and the Health Insurance Portability and Accountability Act (HIPAA) Omnibus Rule extended legal liability to BAs, the frequency and characteristics of data breaches involving BAs have not been systematically tracked across the entire post-HITECH reporting era. Understanding these trends is critical for health info

Inmates in cramped living quarters, a situation common to correctional facilities, are especially vulnerable to disease. Cramped living conditions, coupled with above-average rates of HIV, tuberculosis, and other communicable diseases, increase inmates' risk of problematic health outcomes. Thus, high-quality health care and sustained efforts to prevent disease are especially important to improve inmate health within correctional facilities. Compliance with federal privacy restrictions pursuant to the Health Insurance Portability and Accountability Act (HIPAA) Privacy Rule and state disease rep

Nonsyndromic cleft lip with or without cleft palate (CL/P) is a prevalent congenital anomaly worldwide that poses significant health challenges. Currently, the United States lacks a unified approach to CL/P coverage. This article explores the U.S. medical-legal framework surrounding CL/P, identified via the PubMed, Google Scholar, Lexis, and Westlaw databases. It examines the Affordable Care Act, the Health Insurance Portability and Accountability Act, and state-level variations in CL/P coverage. The findings suggest a need for legal reform to ensure equitable access for patients with CL/P acr

Generative AI, particularly large language models (LLMs), is reshaping clinical workflows in dermatology. However, cloud-based commercial models pose persistent challenges to Health Insurance Portability and Accountability Act (HIPAA) compliance, especially in dermatology, where protected health information (PHI) extends beyond text to clinical photographs, dermoscopic images, and total-body photography that may capture identifiable anatomical features and document conditions carrying social stigma. Locally hosted, open-weight LLMs that are run within the institution's own infrastructure offer

PURPOSE OF REVIEW: Telemedicine uses technology to connect patients with providers at a distance. While the clinical aspects of this have been reviewed extensively, the technical aspects are not as well understood by allergists when they consider adding telemedicine to their practice. This review focuses on the technical aspects of setting up a telemedicine program.
RECENT FINDINGS: With the advent of new technologies including high-speed Internet, video conferencing solutions, and digital examination equipment, facilitated virtual visits as replacements for in-person visits are now possible.

Patients increasingly expect timely, secure access to health information, a right reinforced by the Health Insurance Portability and Accountability Act and expanded under the 21st Century Cures Act. Yet millions of individuals receiving care in jails and prisons remain excluded from these advances. Incarcerated populations bear a disproportionate burden of chronic disease, psychiatric illness, and substance use disorders. For many, incarceration presents consistent contact with healthcare, though access to personal medical records during and after detention remains limited. Although the Cures

The year opened on a somber note, with the nation still struggling to cope with the aftermath of the Sept. 11, 2001, attacks and subsequent anthrax scare. The healthcare industry was scrambling to be ready for any future attacks and pondering the best preparedness measures. Meanwhile, compliance and quality of care simmered on the back burner, along with soaring malpractice insurance costs and growing budget deficits in the states. And debate over provider reimbursement rates, staffing shortages and more double-digit increases in the cost of healthcare coverage also made headlines.

BACKGROUND: The Health Insurance Portability and Accountability Act (HIPAA) aims to safeguard patient information; however, complex legal language may lead to confusion and mistrust, and hinder enrollment in clinical trials.
OBJECTIVE: To evaluate the effect of a standard HIPAA authorization included in mailed survey packets on study enrollment for a multi-site pragmatic trial.
DESIGN: This study is nested within an advance care planning pragmatic trial at 50 primary care clinics across three University of California (UC) Health Systems.

The continued presence and growing rates of individuals infected with human immunodeficiency virus (HIV) and acquired immunodeficiency syndrome (AIDS) in the United States has come to reflect an epidemic of significant proportion. Unfortunately, federal legislation has been eerily silent regarding the establishment of protections against health status-based discrimination for asymptomatic HIV and AIDS sufferers. Congress has done little to change this reality, despite the institution of major healthcare system and insurance reform by the Obama Administration in 2010. This Note argues that "Oba

A 19-year-old woman on immunomodulatory therapy presents to her provider for a routine physical. She is diagnosed with chlamydia trachomatis during screening for sexually transmitted infections (STIs) and returns to the clinic for directly observed treatment with azithromycin. She later calls the practice to express dismay that her parents questioned her about her sexual activity after they received an explanation of benefits for services rendered.

Nontraumatic subperiosteal orbital hemorrhages (NTSOHs) are rare events, but pose a grave risk of vision loss and must be evaluated carefully for optic nerve compromise. To the author's knowledge, only 2 cases of NTSOH after cardiovascular procedures have been reported. The authors report a unique case of bilateral NTSOH with optic neuropathy that developed after cardiothoracic surgery and reaccumulated after orbitotomy. The case presentation conforms to the tenets of the Declaration of Helsinki and is Health Insurance Portability and Accountability Act compliant. The etiology and management o
