A framework for supporting health capability-based planning: Identifying and structuring health capabilities.

The COVID-19 pandemic has highlighted that health security systems must be redesigned, in a way that they are better prepared and ready to cope with multiple and diverse health threats, from predictable and well-known epidemics to unexpected and challenging pandemics. A powerful way of accomplishing this goal is to focus the planning on health capabilities. This focus may enhance the ability to respond to and recover from health threats and emergencies, while helping to identify the level of resources required to maintain and build up those capabilities that are critical in ensuring the prepar

Maternal health planning and prioritization in Chad: developing a supportive tool.

The Republic of Chad has one of the highest rates of maternal mortality in the world. With scarce resources to respond to competing demands, pragmatic evidence-based planning tools are needed to aid planning and support priority setting. This action research aimed to develop a tool to support maternal health (MH) planning and prioritization decisions and identify priority regions/provinces for intervention in Chad based on aggregate MH coverage gap scores (Target-Coverage = Coverage Gap). A rapid review was conducted to identify key indicators and relevant national targets. The 2019 Multiple I

Communicating stroke survivors' health and further needs for support in care-planning meetings.

AIMS AND OBJECTIVES: This study will illustrate how stroke survivors, their relatives and different professionals communicated in care-planning meetings when planning care for patients after their discharge from hospital. We wanted to know what topics participants were talking about, to what extent they were involved in the discussion and how the communication was organized. BACKGROUND: Communication in health care is sometimes problematic because of the participants' asymmetrical positions when negotiating how to understand the patients' future care. METHODS: A qualitative and a quantitativ

Supporting breast and cervical cancer screening follow-up: qualitative health plan interviews.

BACKGROUND: Breast and cervical cancer represent a significant disease burden. Health care quality measures captured through electronic clinical data systems could be used to assess follow-up of abnormal breast and cervical cancer screening results. The National Committee for Quality Assurance (NCQA) wanted to explore the feasibility of using health plan electronic clinical data systems to develop quality measures for follow-up of abnormal breast and cervical cancer screening test results. METHODS: NCQA interviewed quality improvement leaders from ten health plans, recruited through a purposi

Assessing Health and Human Services Needs to Support an Integrated Health in All Policies Plan for Prince George's County, Maryland.

With evolving demographics and a changing health system landscape, the Prince George's County Council, acting as the County Board of Health, is considering its future policy approaches and resource allocations related to health and well-being. To inform this path forward, the authors of this study used primary and secondary data to describe both the health needs of county residents and drivers of health within the county, inclusive of the social, economic, built, natural, and health service environments. This study integrates these findings, an analysis of budget documents, and a review of pro

Risk-need-responsivity and its application in behavioral health settings: A feasibility study of a treatment planning support tool.

The risk-need-responsivity (RNR) model suggests several key practices for justice-involved populations under correctional supervision. Behavioral health treatment planning aligned with RNR principles for offender populations with co-occurring mental health and substance use disorders (CODs) could be one method for integrating RNR into clinical care. To explore a unique approach to working with behavioral health and RNR principles, the authors implemented a mixed-methods feasibility study of the acceptability, usability, and utility of a newly developed RNR treatment planning support tool (RNR

Implementation and 12-Month Health Service Utilization and Cost Outcomes from a Managed Care Health Plan's Permanent Supportive Housing Program.

Homelessness, which refers to the lack of a fixed, regular, and adequate nighttime residence, is a pervasive public health issue. This article presents results from an implementation and outcome study of an ongoing permanent supportive housing (PSH) program-including service utilization and associated costs review-operated by a large not-for-profit Medicaid and Medicare managed care plan serving more than 1 million members in the Inland Empire area of Southern California. This PSH program combines a long-term housing subsidy with intensive case management services for adult plan members experi

A Complex Digital Health Intervention to Support People With HIV: Organizational Readiness Survey Study and Preimplementation Planning for a Hybrid Effectiveness-Implementation Study.

BACKGROUND: Evaluating implementation of digital health interventions (DHIs) in practice settings is complex, involving diverse users and multistep processes. Proactive planning can ensure implementation determinants and outcomes are captured for hybrid studies, but operational guidance for designing or planning hybrid DHI studies is limited. OBJECTIVE: This study aimed to proactively define, prioritize, and operationalize measurement of implementation outcomes and determinants for a DHI hybrid effectiveness-implementation trial. We describe unique advantages and limitations of planning the t

Addition of mHealth (mobile health) for family planning support in Kenya: disparities in access to mobile phones and associations with contraceptive knowledge and use.

BACKGROUND: Recently mobile health (mHealth) has been implemented in Kenya to support family planning. Our objectives were to investigate disparities in mobile phone ownership and to examine the associations between exposure to family planning messages through mHealth (stand-alone or combined with other channels such as public forums, informational materials, health workers, social media and political/religious/community leaders' advocacy) and contraceptive knowledge and use. METHODS: Logistic and Poisson regression models were used to analyze the 2014 Kenya Demographic and Health Survey. RE

HealthMap: a cluster randomised trial of interactive health plans and self-management support to prevent coronary heart disease in people with HIV.

BACKGROUND: The leading causes of morbidity and mortality for people in high-income countries living with HIV are now non-AIDS malignancies, cardiovascular disease and other non-communicable diseases associated with ageing. This protocol describes the trial of HealthMap, a model of care for people with HIV (PWHIV) that includes use of an interactive shared health record and self-management support. The aims of the HealthMap trial are to evaluate engagement of PWHIV and healthcare providers with the model, and its effectiveness for reducing coronary heart disease risk, enhancing self-management

Usability testing of Avoiding Diabetes Thru Action Plan Targeting (ADAPT) decision support for integrating care-based counseling of pre-diabetes in an electronic health record.

PURPOSE: Usability testing can be used to evaluate human-computer interaction (HCI) and communication in shared decision making (SDM) for patient-provider behavioral change and behavioral contracting. Traditional evaluations of usability using scripted or mock patient scenarios with think-aloud protocol analysis provide a way to identify HCI issues. In this paper we describe the application of these methods in the evaluation of the Avoiding Diabetes Thru Action Plan Targeting (ADAPT) tool, and test the usability of the tool to support the ADAPT framework for integrated care counseling of pre-d

The feasibility and acceptability of implementing simplified cognitive behavioral therapy approaches to support postpartum mental health and address associated social and behavioral barriers to postpartum family planning in Amhara, Ethiopia: a qualitative study.

BACKGROUND: Poor mental health can negatively impact health outcomes across diverse health areas, including in the first year postpartum. Yet, the intersection of postpartum mental health and postpartum family planning (FP) is understudied. Cognitive Behavioral Therapy (CBT) is an evidence-based practice that has proven helpful for improving mental health and supporting positive behavior change across health areas, including in low-resource settings. Drawing on existing CBT tools, we created and piloted an intervention called Mothers Time, designed to be delivered in three sessions by a commun

The Likely Impact of Congenital Zika Syndrome on Families: Considerations for Family Supports and Services.

Congenital Zika virus infection has obvious implications for infants, and considerable research has addressed the nature and consequences of congenital Zika syndrome (CZS). Children with classic CZS meet the criteria for "children with medical complexity," and ongoing research is required to understand the range of needs and optimal treatment options. Far less attention has been given to the consequences of CZS for families, which are both immediate and lifelong. Although families of children with CZS have much in common with families of other children with disabilities, at least 4 features of

Streamlining Hospital Planning: A User-Friendly Decision Support Tool for Enhanced Healthcare Delivery.

Healthcare providers worldwide face challenges in maintaining a delicate balance between rising demand and limited resources. Decision-makers often struggle with planning and implementing new services, leading to costly consequences and hindering efficiency, patient outcomes, and staff well-being. Current systems lack a comprehensive modelling approach that encompasses all aspects of the hospital and fails to generate crucial outputs. Moreover, these tools often require expert modelers, limiting their usability for end users such as service managers, clinicians, and management. To address this

Management of neuronopathic Gaucher disease: revised recommendations.

The original guidelines drawn up for the management of the neuronopathic forms of Gaucher disease were felt to be in need of revision; in particular, the role of high-dose enzyme replacement therapy (120 IU/kg of body weight every 2 weeks) in stabilizing neurological disease. The existing published evidence was analysed; it was concluded that it did not support the role of high-dose ERT, although this might be required to treat severe visceral disease.
