The healthy worker effect in US chemical industry workers.

BACKGROUND: Occupational studies typically observe a 20% deficit in overall mortality, broadly characterized as the healthy worker effect (HWE). Components of the HWE may be addressed by various analytical approaches. AIMS: To explore the HWE in a modern industrial cohort. METHODS: Standardized mortality ratios (SMRs) were calculated for 114,683 US chemical industry employees, who worked at least 3 days between 1960 and 2005. RESULTS: SMRs were 79 (95% confidence interval 78-80) for all causes, 81 (95% confidence interval 79-82) for heart disease, 70 (95% confidence interval 67-73) for non-

Anatomy of the healthy worker effect: a critical review.

The healthy worker effect is the composite result from factors such as (1) selection of the work force, (2) changes in lifestyle accompanying employment, and (3) methodological characteristics of the standardized mortality ratio (SMR). While the choice of the general population as the comparison population is the underlying reason for this effect, no more feasible, widely accepted or "better" alternative is yet available. Much of the current understanding of the healthy worker effect has been limited to an examination of selection of the work force. The purpose of this report is to illustrate,

Healthy worker effect in a cross-sectional study of lead workers.

To study the role lead may play in the development of renal disease, we performed a cross-sectional study of workers at a lead smelting plant. Renal function was defined based on calculated creatinine clearance using the prevalence of values under the 3rd percentile to compare groups. The prevalence of calculated creatinine clearance values under the 3rd percentile in these workers (n = 1782) as a whole was 2.81%, a result comparable to that which has to be expected for the general population. Closer analysis, however, showed significantly lower prevalence of calculated creatinine clearance un

Healthy worker survival effect at a high-altitude mine: prospective cohort observation.

Very little is known about the factors of healthy worker survival effect at high-altitude mines. We conducted this cohort observation of the new hires for a high-altitude gold mine in Kyrgyzstan with the aim to ascertain predictors of survival at work. All new hires in 2009 through 2012 for a high-altitude gold mine (3600-4500 m above sea level) were followed up to January 2022. We tested the association of demographic, physiological predictors and diagnoses at the pre-employment screening with non-survival at work in Cox proportional hazards yielding hazard ratios (HR) with their 95% confiden

Standardized mortality ratios and the "healthy worker effect": Scratching beneath the surface.

The age-standardized mortality ratio (SMR) is a relative index of mortality, expressing the mortality experience of the study population relative to that of a comparison ("standard") population. With the general population as the "standard", the SMR for an occupational population will underestimate the mortality experience of that latter population (since it comprises individuals necessarily healthy enough to be employable --and whose mortality risk is therefore initially lower than the general population average). However, this "healthy worker effect" does not equally to all groups within the

Migrants, healthy worker effect, and mortality trends in the Gulf Cooperation Council countries.

The Gulf Cooperation Council (GCC) countries namely, Bahrain, Kuwait, Oman, Qatar, United Arab Emirates (UAE), and Saudi Arabia, have experienced unique demographic changes. The major population growth contributor in these countries is young migrants, which has led to a shift in the population age pyramid. Migrants constitute the vast proportion of GCC countries' population reaching >80% in Qatar and UAE. Using Global Burden of Disease Study 2015 (GBD 2015) and United Nations data, for the GCC countries, we assessed the association between age-standardized mortality and population size trends

Healthy worker effect and pregnancy: role of adverse obstetric history and social characteristics.

Data from a survey conducted in 1981 on a national sample of 5508 births in France were used to analyse the role of a history of previous adverse pregnancy outcome (spontaneous abortion, perinatal death or adverse fetal condition) in the selection mechanisms of women with regard to occupational activity, and the impact on the relation between work and preterm delivery. The study, carried out separately in each parity group, showed that occupational activity was associated with a more favourable outcome for women of parity one: the preterm delivery rate was significantly higher among women who

Cardiovascular composite events and the healthy worker effect among noise-exposed workers: a retrospective cohort study.

BACKGROUND: Occupational noise exposure is prevalent, but its cardiovascular effects are under recognized. Most studies are cross-sectional, focus on single endpoints, and rarely account for the healthy worker effect (HWE). This study aimed to identify risk factors for cardiovascular composite events in noise-exposed workers and further assess this effect. METHODS: This retrospective cohort study enrolled 1,381 noise-exposed workers with normal blood pressure and normal electrocardiogram at baseline in 2020. All participants were followed annually through 2023. The primary endpoint was a comp

Structure and Control of Healthy Worker Effects in Studies of Pregnancy Outcomes.

Much of the literature on the healthy worker effect focuses on studies of chronic disease and mortality; however, when studying pregnancy outcomes, these effects might differ because of the short, defined risk periods of most pregnancy outcomes. Three pregnancy-specific healthy worker effects have also been described, but the structure of these effects has not yet been investigated when occupational exposure, and not employment status, is the exposure of interest. We used directed acyclic graphs to examine healthy worker effects in studies of occupational exposures and pregnancy outcomes: the

A Mailed-questionnaire Survey of Ex-workers: A Tool to Evaluate the Healthy-worker Effect in Cross-sectional Studies.

In order to evaluate a possible healthy-worker effect in a cross-sectional study in a viscose rayon plant, a survey of ex-workers was conducted. From the personnel records of this factory, 475 male, Caucasian, Dutch-speaking, carbon disulfide (CS&inf2;)-exposed workers who had been employed ten years prior to the cross-sectional study were selected for study. Of these, 142 were still employed in the same plant, and 64 were deceased. Retrieval of the causes of death proved impossible. A questionnaire was mailed to the remaining 269 men, of whom 149 completed and returned it. Eighty-nine subject

Mortality by cause of death and risk behaviors in farmers versus non-farmers: the importance of avoiding the healthy worker effect.

OBJECTIVES: To compare mortality by cancer sites and by other specific causes of death, and the prevalence of risk behaviors in farmers and non-farmers in Spain. METHODS: Mortality by cause of death was calculated based on a longitudinal study with 10-years follow-up of 9.5 million men and 6 million women aged 20-64 years who were employed in 2001. The prevalence of risk behaviors was calculated from the 2001 National Health Survey in the 6464 employed men and 5573 employed women aged 20-64. The study subjects were grouped as farmers and non-farmers. For each cause of death, we estimated the

Effect of age and gender on high - Sensitivity C - Reactive protein levels serum on health worker with latent tuberculosis and healthy control.

BACKGROUND: Latent tuberculosis is defined as a state of persistent immune response stimulated by Mycobacterium tuberculosis antigens with no evidence and signs of active TB . Health workers have a high risk of developing latent TB disease due to occupational exposure from patients. High sensitivity CRP (hs-CRP) assays have been developed for special values that may indicate low-grade inflammatory lesions as is true in measurement of latent tuberculosis infection. Factors that affect CRP levels are gender and age. Our study is conducted to asses effect of age and gender on Hs- CReactive protei

Trends in silicosis prevalence and the healthy worker effect among gold miners in South Africa: a prevalence study with follow up of employment status.

BACKGROUND: Given the intimate association between silicosis and tuberculosis, understanding the epidemiology of the South African gold mining industry silicosis epidemic is essential to current initiatives to control both silicosis and tuberculosis in this population, one of the most heavily affected globally. The study's objectives were to compare the prevalence of silicosis among working black gold miners in South Africa during 2004-2009 to that of previous studies, including autopsy series, and to analyse the influence of silicosis and/or tuberculosis on exiting employment. METHODS: Routi

[Effect of the serum of workers directly engaged in an automobile factory in Tychy on the transformation of lymphocytes in healthy persons].

The effects of serum collected from 20 workers involved directly in manufacturing processes upon lymphocytes blastic transformation in healthy subjects have been tested. The controls included: spontaneous blastic transformation of lymphocytes found in healthy subjects (15) and lymphocytes blastic transformation in healthy subjects in the presence of serum from other healthy subjects (13). The tests have demonstrated that the serum of the Motor Car Factory workers statistically significantly stimulated lymphocytes blastic transformation, unlike the serum of healthy subjects.

Evaluating the effect of electronic monitoring and feedback on hand cream use in healthcare workers: Healthy Hands Project.

BACKGROUND: Healthcare workers (HCWs) are at high risk of developing hand dermatitis (HD). Current guidelines on HD prevention recommend the use of emollients; however, in practice, adherence is poor. OBJECTIVE: To assess whether the provision of creams, electronic monitoring and feedback on cream consumption can improve skin care in HCWs. METHODS: A cluster randomized controlled trial was conducted on 19 academic hospital wards, including 501 HCWs, for 12 months. The intervention wards (n = 9; 285 HCWs) were provided with hand cream dispensers equipped with an electronic system to monitor u

The Effect of a Healthy Lifestyle Intervention on Shift Workers With Acid Peptic Disease: A Quasi-experimental Study in Karaikal, South India.

Acid peptic disease (APD) is the most common health issue among rotational shift workers, including night shifts. Alterations in diet patterns from the routines of life play a key role in the development of APD in this population. This study aims to determine food habits and evaluate the effect of a healthy lifestyle intervention on reducing the frequency of the occurrence of APD among rotational shift workers. A quasi-experimental study was carried out for six months in Karaikal, South India. About 37 employees involved in rotational shift work were recruited consecutively. A healthy lifestyl

A comparison of standard methods with g-estimation of accelerated failure-time models to address the healthy-worker survivor effect: application in a cohort of autoworkers exposed to metalworking fluids.

BACKGROUND: Studies of autoworkers exposed to straight metalworking fluids report excess risks of several cancers. These studies, however, have not addressed the healthy-worker survivor effect. Most methods proposed to address this bias do not consider that it may be caused by time-varying confounders affected by prior exposure. G-estimation of accelerated failure-time models was developed to handle this issue but has never been applied to account for the healthy-worker survivor effect. METHODS: We compare results from Cox models and g-estimation in 38,747 autoworkers exposed to straight meta
