The first heart transplantation (SHT) was performed by Professor Ch. Barnard in 1967 but it was not until 1980s that this method became an established approach to treatment of patients with end-stage heart failure. Considering the limited number of donor organs and the number of potential post-transplantation complications, the decision to perform heart transplantation at the right time in an indicated patient is difficult and complex. Subsequent pharmacological management with immunosuppressive agents and other medication becomes everyday life reality. Knowledge of drug interactions and colla

Heart transplantation remains the best treatment for terminal heat failure. Ischemic and idiopathic cardiomyopathies are the main indications. Contraindications are due to immunosuppressive treatment and fixed pulmonary hypertension. After clinical evaluation, the patient is put on the waiting list controlled by the French Biomedicine Agency. Acute rejection had decrease due to the improvement of immunosuppressive treatments. The incidence of severe acute rejection is 5%. Chronic rejection is the main cause of morbidity and mortality. The rate at 5 years is about 40-50%. Survival at one year i

Although the number of available donor hearts severely limits the epidemiologic impact of heart transplantation on patients with heart failure, patients with end-stage heart failure unresponsive to medical management currently have no other viable alternatives. Destination therapy with a ventricular assist device is the closest toward approaching clinical reality but has been plagued with problems of infection and stroke. The purpose of this review is to summarize recent developments in the field that may broaden the clinical impact of heart transplantation. For example, novel methods of cardi

The first heart transplantation in the Nordic countries was performed at Rikshospitalet in Oslo, Norway in 1983 and the method is now accepted as a good treatment for selected patients. In this article we present a review of heart transplantation as it is practiced in our hospital. Because of donor shortage, the selection criteria are strict. It is most important that the patients are motivated and able to cooperate. They should be in heart failure class IV (III-IV) on the best medical treatment with no other treatment alternatives. Estimated survival without transplantation should be less tha

In carefully selected patients with end-stage heart failure heart transplantation has developed from an experimental procedure to standard therapy during the last 30 years. It is currently accepted as a procedure for prolonging life and also for improving quality of life. According to the Registry of the International Society for Heart and Lung Transplantation the overall one-year actuarial survival is 79% and 10-year survival barely 50%. Nine years after the start of the Heart Transplant Program at Rigshospitalet the overall actuarial survival of 157 consecutive patients is 66%. Due to the li

Heart transplantation is presently the only treatment capable of substantially changing the natural history of endstage heart failure, achieving a 5-year survival rate of 60%. The relative scarcity of available donors is the main limitation to this procedure. The standard surgical technique is that referred to as "orthotopic", described by Shumway and Lower. Research efforts now focus on the design of more efficient methods of preservation that will permit longer storage periods once the grafts have been harvested. Rejection and infection are the most common causes of post-transplantation mort

Cardiac transplantation is now accepted as the therapy of choice for irreversible, advanced heart failure. Improving results have been observed since 1980. This is the consequence of better defined criteria for selection of recipients, refined use of antilymphocyte serum, improved myocardial preservation methods, and the introduction of cyclosporine. Cyclosporine, a metabolite of a soil fungus, is one of the most potent and specific immunosuppressants yet discovered. Its main drawbacks are nephrotoxicity and hepatotoxicity. The immunosuppressive protocol usually includes cyclosporine and low d

Heart transplantation (HTx) remains the last therapeutic resort for patients with advanced heart failure. The present work is a clinically focused review discussing current issues in heart transplantation. Several factors have been associated with the outcome of HTx, such as ABO and HLA compatibility, graft size, ischemic time, age, infections, and the cause of death, as well as imaging and laboratory tests. In 2018, UNOS changed the organ allocation policy for HTx. The aim of this change was to prioritize patients with a more severe clinical condition resulting in a reduction in mortality of

Over the past 40 years, heart transplantation has become a routine treatment for end-stage heart failure. Over 3000 heart transplants are performed annually. Despite improvements in short-term outcomes, long-term survival remains limited. Current efforts remain directed at alleviating the shortage of donor organs, developing effective treatments for chronic rejection, and improving mechanical circulatory support for heart failure.

Since February 1999, organ transplantation from brain-dead donors has started in Japan. During the more than 3 years since, 77 patients have received organ transplantations from 19 donors (heart in 13, lung in 10, liver in 17, pancreas-kidney in 6, pancreas alone in 1, small intestine in 1, and kidney in 29). As of April 2002, 116 patients were registered with the Japan Organ Transplant Network as heart transplant (HTx) candidates, and 13 and 8 underwent HTx in Japan and abroad, respectively, while 32 died while on the waiting list. Ten of 13 HTx recipients were implanted with a left ventricul

From September 1988 until March 1990, 22 orthotopic heart transplantations (HTX) were performed in 20 patients (18 male and 2 female). Median age was 56.5 years (23-66). The indication for HTX was an end-stage ischemic disease in 7 pts. a dilated cardiomyopathy in 13 pts, and a retransplantation in 2 pts. The mean waiting time was 58 days. Immunosuppressive therapy included OKT3, prednisone and azathioprine. Cyclosporine was introduced at day 10. Donor hearts were obtained from our institution in 5 cases, from other hospitals in Belgium in 9 cases, and from other European countries in 8 cases.

Modern concepts of cardiac transplantation are reviewed. Selection of potential recipients and donors is discussed with particular attention to those aspects of procurement that involve coordination with other transplant teams. Technical aspects of the operation, postoperative management, immunologic regimens, and late results, including physiologic testing and complications, are also reviewed.

Despite efforts to improve donor heart allocation policies, geographic disparities, logistical challenges and patient criteria limit organ availability. However, recent surgical innovations offer potential solutions. These include partial heart transplantation and xenotransplantation. Partial heart transplantation is a new procedure with the potential to address several clinical challenges in treating congenital heart defects. Cardiac xenotransplantation involves the transplantation of genetically modified porcine hearts into humans, offering a potential solution to the shortage of donor organ

The advances in the treatment of many different heart diseases have on the one side led to a significant prolongation of life expectancy but have also contributed to an increase of patients with heart failure. This tendency is supported even more so by the demographic development of our population. The replacement of insufficient organs has always been in the focus of medical research. In the 1960's Shumway and Lower developed the technique of cardiac transplantation and also worked intensively on the treatment and diagnosis of rejection. However, it was Barnard who, in 1967 performed the firs

BACKGROUND: Currently, a high percentage of patients with congenital heart disease (CHD) reach adulthood. The consequence is that more and more patients will require a heart transplant (HTx) or heart-lung transplant (HLTx). The objective of the study was to analyze the evolution and temporary trend of the number of HTxs and HLTxs in patients with and without CHD.
METHODS: We performed a retrospective analysis of all HTxs and HTLxs from a Spanish transplant hospital. Retransplant and other combined transplants were excluded. HTx and HLTx were divided into 2 groups (CHD or non-CHD). The number

The article reviews history, indication and follow-up after heart transplantation, including the mechanical assist devices. Various complications of posttransplant follow-up are mentioned, e.g. rejection, infection, vasculopathy, meta-bolic disorders, hypertension or malignities. Pharmacotherapy used for immunosuppression is discussed. Heart transplantation improves the prognosis of patients with previous heart failure and also their quality of life. Key words: heart transplantation - immunosuppression - mechanical assist devices - rejection - terminal heart failure.

Heart transplantation is now a treatment option with good outcome for infants and children with end-stage heart failure or complex, inoperable congenital cardiac defects. One-year and 5-year actuarial survival rates are high, approximately 75% and 65%, respectively, with overall patient survival half-life greater than 10 years. To date, survival has been improving as a result of reducing early mortality. Further reductions in late mortality, in part because of graft coronary artery disease and rejection, will allow achievement of the goal of decades-long survival. Quality of life in surviving

Neonatal heart transplantation was developed and established in the 1980's as a durable modality of therapy for complex-uncorrectable heart disease. Patients transplanted in the neonatal period have experienced unparalleled long-term survival, better than for any other form of solid-organ transplantation. However, the limited availability of neonatal and young infant donors has restricted the indications and applicability of heart transplantation among newborns in the current era. Indications for heart transplantation include congenital heart disease not amenable to other forms of surgical pal

Heart transplantation is currently the definitive gold standard surgical approach in the treatment of refractory heart failure. However, the shortage of donors limits the achievement of a greater number of heart transplants, in which the use of mechanical circulatory support devices is increasing. With well-established indications and contraindications, as well as diagnosis and treatment of rejection through defined protocols of immunosuppression, the outcomes of heart transplantation are very favorable. Among early complications that can impact survival are primary graft failure, right ventri
