Right Ventricular Assist Devices After Heart Transplantation.

BACKGROUND: Severe right ventricular failure (RVF) has a significant incidence among cardiac transplant patients. It is a serious complication and an independent risk factor for postoperative mortality. In this setting, ventricular assist devices (VADs) must be considered if conservative medical management fails. This study sought to examine our series of patients with early RVF after heart transplantation requiring VAD support. METHOD: We analyzed consecutive, adult heart transplant recipients at a third level intensive care unit who underwent transplantation from January 2011 to March 2019

Cardiac assist devices for end-stage heart failure.

Cardiac assist devices have become an important therapy for patients with end-stage cardiac failure. These devices continue to evolve. Most current devices provide temporary, left-sided support for patients with cardiogenic shock from postcardiotomy syndrome or for patients awaiting cardiac transplantation. The two most frequently used left ventricular assist devices as a bridge to transplant are the Thermocardiosystems (HeartMate I) and the Novacor devices. The selection criteria for the use of these devices and treatment of these patients will be reviewed. Additionally, other currently avail

Biventricular assist devices and total artificial heart: Strategies and outcomes.

In contrast to the advanced development of the left ventricular assist device (LVAD) therapy for advanced heart failure, the mechanical circulatory support (MCS) with biventricular assist device (BVAD) and total artificial heart (TAH) options remain challenging. The treatment strategy of BVAD and TAH therapy largely depends on the support duration. For example, an extracorporeal centrifugal pump, typically referred to as a temporary surgical extracorporeal right ventricular assist device, is implanted for the short term with acute right ventricular failure following LVAD implantation. Overview of Ventricular Assist Devices and the Total Artificial Heart.

BACKGROUND: Patients with isolated left ventricular failure may have positive outcomes after being implanted with a left ventricular assist device. Unfortunately, almost half of patients with heart failure and reduced ejection fraction also have evidence of right ventricular dysfunction. For a subset of this population with severe biventricular failure, or those who develop right ventricular dysfunction after left ventricular assist device implantation, patients may necessitate biventricular assist devices or the total artificial heart. OBJECTIVES: This overview of mechanical circulatory supp

Left ventricular assist devices: evolving devices and indications for use in ischemic heart disease.

PURPOSE OF REVIEW: The mortality with end-stage heart failure is extremely high, especially when patients become refractory to conventional medical therapy and require frequent hospitalization. Ischemic heart disease remains the primary cause of advanced heart failure. Mechanical pumps or devices have been developed called ventricular assist devices and are being used to support an increasing number of patients with refractory heart failure. RECENT FINDINGS: The use of ventricular assist devices has evolved from initially only support of patients unable to be weaned from a heart-lung machine

Heart transplantation after longest-term support with ventricular assist devices.

The use of mechanical circulatory support devices to keep patients alive until transplantation has become essential in the face of an increasing organ shortage. We report successful heart transplantations after 841 days of left ventricular assist device (LVAD) support in a child with hypoplastic left heart syndrome, and after 547 days of biventricular assist device (BVAD) support in another child with cardiomyopathy. To our knowledge, this report is the first on the longest-term (841 days) LVAD and the longest-term (547 days) BVAD support in children who were mobile and awake during the suppor

Diaphragmatic hernias associated with ventricular assist devices and heart transplantation.

BACKGROUND: Diaphragmatic hernias in heart transplant recipients with prior left ventricular assist device (LVAD) placement are a potentially life-threatening complication. This review examined the incidence, diagnostic strategies, surgical management, and prevention of diaphragmatic hernias in these patients. METHODS: A retrospective review of patients receiving a Thoratec HeartMate (Thoratec Laboratories Corp, Pleasanton, CA) LVAD who developed a diaphragmatic hernia after VAD explantation and orthotopic heart transplantation (OHT). Two groups were identified and their results compared. In

Left ventricular heart failure model for testing cardiac assist devices.

During the development of a new circulatory support system, we have used different animal models with and without heart failure for hemodynamic testing. As the normally functioning heart does not always allow for proper evaluation of a circulatory support system, a good and adjustable animal heart failure model is needed. The aim of the study was to develop a left ventricular failure model in calves for acute hemodynamic studies. The principle is based on the negative inotropic effect of drugs with bypass of the right ventricle. Seven calves were used in the study. Metoprolol and verapamil wer

Implantation of biventricular assist devices for chronic heart transplant rejection.

Mechanical circulatory support has been used to treat graft failure caused by rejection after heart transplantation, but the prognosis remains bleak. Organ failure induced by the addition of or an increase in immunosuppressive medication is known to be a cause of the poor outcomes. Thus we tried a new therapeutic technique that allows complete withdrawal of immunosuppressive medication, as the donor heart is removed and circulatory support is maintained with a biventricular assist device.

Polymeric heart valves for surgical implantation, catheter-based technologies and heart assist devices.

Efficient function and long-term durability without the need for anticoagulation, coupled with the ability to be accommodated in many different types of patient, are the principal requirements of replacement heart valves. Although the clinical use of valves appeared to have remained steady for several decades, the evolving demands for the elderly and frail patients typically encountered in the developed world, and the needs of much younger and poorer rheumatic heart disease patients in the developing world have now necessitated new paradigms for heart valve technologies and associated material

"The heart supporters": systematic review for ventricle assist devices in congenital heart surgery.

Ventricular assist device (VAD) implantation is a widely used procedure in children with cardiac failure refractory to medical therapy as a long-term bridge to recovery or transplant. This strategy has proved to be of an enormous advantage in the cure of these children. The aim of this review is to evaluate the current strategies used for clinical monitoring of paediatric patients with a VAD, focusing on the management of several aspects such as anticoagulant and antiplatelet therapy, haemorrhagic and thrombotic complications, as well as the effects that VADs have on the exposure, effectivenes

In Vivo Biocompatibility of an Innovative Elastomer for Heart Assist Devices.

Cardiac surgical approaches require the development of new materials regardless of the polyurethanes used for pulsatile blood pumps; therefore, an innovative biomaterial, a copolymer of poly(ethylene terephthalate) and dimer fatty acid (dilinoleic acid) modified with D-glucitol, hereafter referred to as PET/DLA, has been developed, showing non-hemolytic and atrombogenic properties and resistance to biodegradation. The aim of this work was to evaluate in vivo inflammatory responses to intramuscular implantation of PET/DLA biomaterials of different compositions (hard to soft segments). Two copol

[Treatment of end-stage heart failure by implantable ventricular assist devices].

The strategy for severe heart failure treatment has changed dramatically since 2011 by the initiation of clinical use of implantable continuous flow ventricular assist devices( LVADs) in Japan. The continuous flow LVADs include DeBakey ventricular assist device (VAD), HeartMate II, HeartWare, Dura-Heart, and EVAHEART, although EVAHEART alone is clinically available in Japan currently. Hear-Mate II is the most popular LVAD with more than 10,000 worldwide implants, while its clinical result is superior to implantable pulsatile LVADs. Crucial complications with implantable LVADs include infection

Supportive psychotherapy for patients with heart transplantation or ventricular assist devices.

OBJECTIVE: Both heart transplant (HTX) candidates and patients on ventricular assist device (VAD) support suffer from severe heart failure and have to fear life-threatening complications. For both patient groups, the importance of screening for and treatment of psychosocial problems and mental disorders to optimize compliance and survival has been acknowledged. We compared the use of psychotherapeutic support by patients with primary HTX, on VAD support, either on destination therapy or to be transplanted, and successfully bridged to transplant. In addition, we evaluated the use for supportive

Pathological findings in explanted prosthetic heart valves from ventricular assist devices.

AIMS: Ventricular assist devices (VADs) are now a mainstay in the management of patients with end-stage heart failure. An important consideration in the long-term durability of these devices is the structural integrity of the prosthetic valves. Herein, we report the morphological findings in inflow and outflow explanted bioprostheses from seven such devices. METHODS: The porcine bioprostheses (n = 7; HeartMate, Novacor) were examined from inflow and outflow valve conduits. Cusp tears were assessed on gross examination. Tissues were then processed for histology and graded for pannus, thrombus,

Selection of long-term ventricular assist devices in heart failure patients.

Selection of long-term ventricular assist devices in heart failure patients depends on indication, intention of mechanical support, and availability of devices. Even if many more issues of mechanical support like infection, thromboembolism, and device failure are still unresolved, acceptable results can already be achieved with the current systems.

Valvular heart disease in patients supported with left ventricular assist devices.

Patients with advanced HF and coexisting VHD who undergo LVAD implantation constitute a growing population and pose unique challenges in clinical and surgical management. Contemporary continuous-flow LVADs alter blood flow dynamics and cardiovascular physiology and can induce progression of preexisting VHD or lead to de novo VHD. Although we have defined our recommendations for treatment options (Table 2), continued changes to device technology, patient selection, and surgical techniques will undoubtedly lead to further changes in practice. Finally, the rapid growth in the field of mechanical

Psychiatric problems of heart transplant candidates with left ventricular assist devices.

Most heart transplant candidates are equipped with left ventricular assist devices (LVADs). LVAD therapy is associated with characteristic psychiatric and psychosocial problems. To investigate the mental states of heart transplant candidates, psychiatric diagnosis, psychological or behavioral problems, and the need for treatment were evaluated around the time of registration to the waiting list and during follow-up. Saitama Medical University Hospital has been designated a hospital for heart transplantation since October 2002. The subjects were 14 heart transplant candidates (9 male candidates

Influence of different assist devices on survival after orthotopic heart transplantation.

BACKGROUND: In 1995, a risk factor of 1.88 was indicated for one-year mortality in connection with bridging to heart transplantation. Both one-year and three-year survival rates in patients bridged to transplantation were less than 80%. METHODS: From 3/89 to 12/98, 903 orthotopic heart transplantations were performed at our center in 888 recipients. Bridging was necessary in 142 patients. RESULTS: The one-year survival rate was 76% in pts without VAD, 86% in pts bridged with VAD and 66% in pts with VAD due to postcardiotomy syndrome. The three-year survival rates were 73%, 80% and 55% respec

Outcomes after heart transplantation in sensitized patients bridged with ventricular assist devices.

OBJECTIVE: Sensitization has been associated with worse outcomes following heart transplantation (HTx). The use of ventricular assist devices (VAD) is a risk factor for the development of sensitization. We investigated the impact of left ventricular assist devices (LVAD) and sensitization in HTx recipients. METHODS: We queried the UNOS database for all heart transplants performed from January 2000 through December 2016. Patients were considered highly sensitized and included if panel-reactive antibody (PRA) activity was 25% or higher. Patients were separated by pretransplant LVAD utilization
