Continuous arteriovenous hemofiltration.

CAVH is a bedside form of dialysis that is used in the treatment of fluid and electrolyte disorders seen in critically ill patients. The major advantages of the procedure include (1) gradual, continuous therapy, which is ideal in hemodynamically unstable patients; (2) control of fluid balance; and (3) ease of administration in the ICU. The major disadvantages of CAVH include (1) a requirement for arterial access, (2) the need for anticoagulation, (3) the risks of infection from long-term indwelling vascular lines, and (4) the potential for significant volume depletion. The effectiveness of CAV

Chronic hemofiltration treatment.

Fourteen patients (six males and eight females) have been treated with chronic hemofiltration three times weekly for three to 27 months with the post-dilution technique. All patients had previously been on regular dialysis treatment. Patients were selected for hemofiltration because of dialysis-resistent hypertension (eight), symptoms of dialysis discomfort (five), hypertriglyceridemia (five) and polyneuropathy (seven). Hypertension improved in six of eight patients, symptoms of dialysis discomfort markedly diminished in all five patients, hypertriglyceridemia did not change consistently, poly

Hemofiltration treatment of psoriasis.

Eleven patients with severe psoriasis were treated with hemofiltration once weekly for 4 consecutive weeks. Hemofiltration is a method of extracorporeal dialysis that uses a pressure gradient, rather than passive diffusion, to remove small and medium molecular weight substances from the blood. Six of the patients had very substantial clearing of their psoriatic lesions. Five patients had little or no apparent benefit during the course of hemofiltration treatments. They subsequently had dramatic clearing, however, when previously ineffective topical therapy was resumed. No patient had worsening

Hemofiltration: an alternative to dialysis.

Continuous arteriovenous hemofiltration (CAVH) is a relatively new procedure designed to relieve fluid overload. It has proved useful in many patients whose ability to maintain fluid homeostasis is compromised. Burn victims, patients with acute renal failure, patients with pulmonary edema, and trauma patients have all received benefit from CAVH. CAVH is an extracorporeal circuit in which blood is propelled by the force of the patient's blood pressure through a filter, ultrafiltered, and returned to the patient. CAVH removes fluid and solutes more slowly than hemodialysis, eliminating the dange

[Extrarenal hemofiltration in pediatric resuscitation].

Peritoneal dialysis, continuous hemofiltration (HF) and continuous hemodiafiltration (HDF) are the most convenient methods for blood purification in pediatric intensive care units. HF is based on the ultrafiltration process and removes not only water but also Na, K, bicarbonate, urea and creatinine with sieving coefficient close to 1. HDF is based on both ultrafiltration and diffusion processes and increases the removal of low molecular weight solutes (urea). These procedures are indicated in: 1) neonates presenting with oligo-anuric acute renal failure (ARF) or fluid overload withstanding diu

Continuous arteriovenous hemofiltration in children.

Continuous arteriovenous hemofiltration (CAVH) is an extracorporeal technique for the treatment of hypervolemia and electrolyte disturbances in the critically ill patient with oligoanuria. The patient's cardiac output provides the blood flow through the circuit; no pumps are necessary. A range of hemofilters is now available extending the applicability of CAVH to the pediatric population, including premature newborns. In this report the treatment of 15 neonates and 8 older children is described. Fluid overload was reduced in all cases. Reflecting the very grave clinical conditions of these pat

Clearance of metformin by hemofiltration in overdose.

BACKGROUND: Metformin is prescribed with an increasing frequency for patients with Type II diabetes mellitus; the increasing availability increases the risk of intentional overdoses. Metformin may cause severe lactic acidosis in overdose, especially when accompanied by co-ingestants or other medical conditions that alter lactate handling or metformin elimination. Though the clearance of therapeutic metformin by hemodialysis is known, the clearance in the setting of a large overdose has not been reported. CASE REPORT: A 58-year-old man with a history of Type II diabetes, hypertension, bipolar

Continuous hemofiltration for the failing heart.

The effect of hemofiltration (HF) was studied in three different groups of patients with severe heart failure refractory to inotropic support. Group I consisted of 72 patients who were treated preoperatively with HF. In 1,350 patients (group II) undergoing several kinds of open heart surgery, HF was performed during cardiopulmonary bypass. In a third group (520 patients), HF was used postcardiotomy cardiogenic shock. Hemodynamic, metabolic, and PO2 measurements were obtained before, during, and after continuous HF. During 2 to 24 hrs of continuous HF, an increase in mean arterial pressure left

Urea removal by hemofiltration and hemodiafiltration.

Continuous hemofiltration is usually regarded as a safe method for controlling fluid overload in neonates presenting acute renal failure. We considered that continuous hemodiafiltration with a hemofilter type especially designed for use in the neonatal period could improve urea removal as compared with hemofiltration. Continuous arteriovenous hemofiltration (CAVH) and hemodiafiltration (CAVHD) were consecutively performed on 7 anesthetized adult rabbits which were given an urea infusion. The hemofilter was a 800-cm2 polysulfone model (Amicon Minifilter Plus). Mean values for physiological and

[Ultrafiltration and hemofiltration in the cardiology unit].

The utilization of renal replacement therapies in cardiac patients has received increasing attention in recent years. In fact, isolated ultrafiltration has been proposed in patients with heart failure as a means for rapidly relieving fluid overload while preserving renal function; moreover, periprocedural hemofiltration (HF) has been suggested for radiocontrast-induced nephropathy (RCIN) prophylaxis. As a matter of fact fluid overload, with the ensuing systemic and pulmonary congestion, remains a major problem in patients with heart failure, and diuretic resistance is not an uncommon feature i

Treatment of hydrops fetalis with hemofiltration.

The treatment of hydrops fetalis, a critical state of extravascular fluid overload in the newborn, poses a great medical challenge. The aim of this study was to investigate the use of continuous arteriovenous hemofiltration (CAVH) in the treatment of five critically ill newborns with hydrops fetalis of different etiology. All patients had anasarca, oliguria not responsive to diuretics, were on mechanical ventilation, and were treated with inotropic drugs and sedation. The duration of treatment ranged from 42 to 114 h and all patients, except one who died from congenital heart disease, achieved

[Hemofiltration and dialysis in a case of rhabdomyolysis].

The effect of CAVDH was evaluated in a patient with rhabdomyolysis due to extreme low temperature and also to carbon monoxide poisoning. Since renal failure im myoglobinuria has a relatively good prognosis, we recommend in this case the use of a soft technique such as continuous arteriovenous hemofiltration with dialysis.

Clinical and technical aspects of hemofiltration.

The removal of uremic substances in hemofiltration, in contrast to hemodialysis, is achieved by means of a convective transport across membranes of high porosity. Since 1974, more than 30 patients with chronic renal insufficiency have been treated with regular hemofiltration three times weekly for four to five hours each. After completing a pilot study, a controlled study to compare hemodialysis and hemofiltration was initiated during January, 1978. A normalization of blood pressure in patients with severe hypertension, and remarkable stability of the circulatory system, even after dehydration

Continuous arteriovenous hemofiltration in premature infants.

Five critically ill premature infants with acute renal failure (ARF) and hypervolemia were treated by continuous arteriovenous hemofiltration (CAVH). Prostacyclin was used to prevent hemofilter clotting. Mean treatment duration was 53.6 +/- 14 h. Mean blood flow rates of 1.6 +/- 0.22 ml/min and filtration fractions of 17.2 +/- 3.7% produced mean ultrafiltration rates of 8.3 +/- 3.1 ml/kg.h. Fluid overload was easily corrected by means of CAVH. The mean prehemofiltration serum creatinine and urea levels were 2.3 +/- 0.4 and 77 +/- 29.7 mg/dl; the mean posthemofiltration levels were 2.38 +/- 0.4

Citrate anticoagulation for continuous venovenous hemofiltration.

OBJECTIVE: Continuous venovenous hemofiltration (CVVH) is applied in critically ill patients with acute renal failure for renal replacement. Heparins used to prevent circuit clotting may cause bleeding. Regional anticoagulation with citrate reduces bleeding, but has metabolic risks. The aim was to compare the safety and efficacy of the two. DESIGN: Randomized, nonblinded, controlled single-center trial. SETTING: General intensive care unit of a teaching hospital. PATIENTS: Adult critically ill patients needing CVVH for acute renal failure and without an increased bleeding risk. INTERVENTIO

[Hemofiltration in acute neonatal metabolic crisis].

Continuous arteriovenous hemofiltration (CAVH) and venovenous hemofiltration (CVVH) were successfully used in the treatment of two neonates, one with maple syrup urine disease and the other with ornithine transcarbamylase deficiency. The plasma concentrations of branched-chain amino acids as well as of ammonium decreased to non-toxic values within a few hours and the clinical improvement in the encephalopathy was striking. Our results and a review of the literature show that hemofiltration is more effective than peritoneal dialysis in removing toxic metabolites. The best clearance rates are ac

Hemofiltration in myoglobinuric acute renal failure.

The frequency of myoglobinuric renal failure is estimated between 8 and 20%. Despite early onset of therapy often the use of renal substitution by hemodialysis or hemofiltration is required. This study of the clinical course of nine patients with myoglobinuric acute renal failure reveals continuous arterio-venous hemofiltration (CAVH) to have an effective clearance for myoglobin. Thus, the time until recovery of renal function as well as the frequency of secondary complications in rhabdomyolysis induced acute renal failure can be distinctly reduced.
