Historical and statistical data on traumatic diaphragmatic hernias are presented. It was shown that frequency of occurrence of the pathology in question was dependent on principal causes of its development, anatomic features of diaphragmatic hernias and mechanisms of damage of the diaphragm. In the article the author notes that the worsening of criminogenic situation, increased number of road and traffic accidents were principal causes of the development of diaphragmatic hernias. Special place in the review is given to strangulated diaphragmatic hernias. The basic diagnostic methods for this p

BACKGROUND: Traumatic diaphragmatic hernias commonly occur after blunt and penetrating trauma. Due to coexisting injuries and the silent nature of the diaphragmatic injuries, the diagnosis is easily missed. We undertook this study to describe the clinical characteristics of consecutive patients with traumatic diaphragmatic hernias treated surgically at our institution.
METHODS: We conducted a prospective cohort of patients treated during a 6-year period. We assessed variables such as age, gender, type of mechanism of trauma, type of hernia, methods of diagnosis, herniated organs and associate

Traumatic intrapericardinal diaphragmatic hernia is a rare occurrence following blunt thoracoabdominal injuries. We have reviewed 59 cases reported in the literature and added three. Emphasis is placed on the diagnostic difficulties encountered, with some discussion regarding the management and surgical approach.

Because of its rarity in children the diagnosis of diaphragmatic hernia due to trauma is often delayed, sometimes resulting in gastrointestinal or respiratory complaints or obstruction of the small intestine. Eleven children with rupture of the diaphragm were seen between 1950 and 1977. All injuries were due to blunt trauma sustained in automobile-related accidents; all involved the left hemidiaphragm. Nine children had associated injuries, most commonly fractures, and injury to the head and spleen. In six children the diaphragmatic rupture was recognized and treated at the time of injury. The

Traumatic rupture of the diaphragm is an uncommon condition. The prevalence of diaphragmatic rupture among blunt trauma victim ranges from 0.8 to 8%. The etiologic factors are blunt trauma (for example, in motor vehicle accidents) and penetrating trauma. The diagnosis is often missed because of non-specific clinical signs, and the absence of additional intra-abdominal and thoracic injuries. We present a case which was misdiagnosed as a case of left sided hemopneumothorax and treated with tube thoracotomy in other center.

Traumatic diaphragmatic hernia is rare, but is of utmost importance due to its high morbidity and mortality. It is markedly important in patients with blunt abdominal trauma, and diagnosis is difficult because of the numerous associated injuries. A patient with few symptoms of chronic traumatic diaphragmatic hernia is described, who underwent surgery due to a gastric volvulus. Laparoscopic surgery permits repair of these injuries through an abdominal approach, avoiding a thoracic incision or selective intubation.

Experience with 42 cases of traumatic diaphragmatic hernia is reviewed. The correct diagnosis was most readily made when: (1) the injury was recent, (2) the tear was left sided and large with readily identifiable structures herniated, (3) appropriate diagnostic procedures were carried out (chest film, upper gastrointestinal examination, barium enema study, nuclear liver scan, computed tomography), and (4) a high index of suspicion was maintained. The diagnosis was likely to be missed when: (1) the history of trauma, usually remote, was not obtained or was disregarded, (2) the hernia was right

BACKGROUND: Traumatic diaphragmatic hernias are serious complications of blunt abdominal or thoracic trauma. In the early posttraumatic period, they are often missed, and they may be followed by a variety of subacute or chronic symptoms due to pulmonary or intestinal obstruction.
METHODS: We present three cases of traumatic diaphragmatic hernias. Two of them were successfully treated by laparoscopy and direct suturing during the early posttraumatic period; the other was treated 10 years after the trauma.
RESULTS: We found that laparoscopy is a safe, successful, and gentle procedure not only

AIMS: This study classifies cases of traumatic diaphragmatic hernias (TDH) in patients admitted to the Intensive Care Unit (ICU) of the Coimbra University Hospitals (HUC) from 1990 to 2004.
METHODS: Retrospective analysis of 34 cases of TDH, studying anatomical location, place and time of diagnosis, complementary tests aiding diagnosis, herniated organs, associated traumatism, morbidity and mortality.
RESULTS: Twenty-eight male and six female patients with an average age of 40.5 years +/- 20.5, average SAPS score 38.8. Average length of stay was 19.1 +/- 13.6 days, all suffered from closed t

A 65-year-old female was sent to our hospital by ambulance car after being struck by a car. Chest X-ray showed a elevation of the right diaphragm. Computed tomography and liver scintigram revealed herniation of the liver into the right thorax. As the blood gas analysis was in normal range and cerebral contusion coexisted, she was observed without operation. During 5 year after the injury, the hernia was gradually progressive and dyspnea appeared finally. She was readmitted to the hospital and operated upon through thoraco-abdominal approach. There are no report of spontaneous cure of the herni

Diaphragmatic rupture following blunt or penetrating thoraco-abdominal trauma is frequently missed. It presents years later with herniation of abdominal viscera. Surgical treatment should be sought for when diagnosed. A 56 year old female presented to emergency with traumatic right diaphragmatic hernia, road traffic accident 8 years ago when she sustained multiple rib fractures on the right side. Upon diagnosis, successful primary hernia repair was performed. Non-specific clinical and radiological features of diaphragmatic hernia (diminished breath sounds, respiratory distress, orthopnoea, dys

A 3-yr-old boy presented with respiratory distress of 2 days duration. There was a history of blunt trauma to the lower chest having occurred 5 days earlier. Although missed initially, serial chest X-rays and a computed tomographic (CT) scan revealed an isolated traumatic right-sided diaphragmatic hernia without any injury to the viscera or the ribcage. Laparotomy with reduction of the herniated right lobe of the liver and the transverse colon was performed. Recovery was uneventful. The presentation, diagnosis and management of this relatively uncommon injury is discussed. The need for a high

Diaphragm rupture is associated with approximately 5% of blunt abdominal trauma. However, rupture of the central tendon of the diaphragm leading to an intrapericardial diaphragmatic hernia is very rare, with less than 100 cases reported in the world literature. All previously reported cases have been repaired via laparotomy or thoracotomy. In this paper, we present the first laparoscopic repair of a traumatic intrapericardial diaphragmatic hernia.

Intercostal hernias are rarely reported in the literature. We report a case of intercostal hernia secondary to a ruptured right hemidiaphragm and fractured costal margin caused by blunt trauma. The patient was ventilated at initial hospitalization because of rib fractures and advanced age, and the intercostal hernia was not evident. After physical rehabilitation treatment elsewhere, a painful chest wall bulge developed. A chest film and computed tomographic scan revealed the hernia. Surgery with a thoraco-abdominal incision in the line of the hernia allowed reduction of the hernial contents.

INTRODUCTION: Intrapericardial diaphragmatic hernia is a rare injury. We present a case of an intrapericardial diaphragmatic hernia from blunt trauma. In this report we will review the current literature and also describe the first report of a primary laparoscopic repair of the defect.
CASE DESCRIPTION: A 38-year-old unrestrained male passenger had blunt chest and abdominal trauma from a motor vehicle collision. Two months later, on a computed tomography scan, he was found to have an intrapericardial diaphragmatic hernia. The defect was repaired primarily through a laparoscopic approach.
