[Research progress of Hoffa fracture].

Hoffa fracture is a rare type of fracture confined to the coronal plane of the femoral condyle. High-energy is a common reason of Hoffa fracture, and low-energy trauma and iatrogenic injury can also cause Hoffa fracture in some cases. The commonly used classifications include Letenneur classification, CT classification, AO classification and modified AO classification. X-ray is the first choice to diagnose Hoffa fractures, fracture lines can be found. If the X-ray is negative, CT scan and MRI should be performed. Nondisplaced fractures can be managed conservatively with cast immobilization, ho

The Hoffa fracture--a fracture not to miss.

The Hoffa fracture is a rare unicondylar fracture of the lower femur that occurs in the coronal plane. The nature of the fracture pattern makes it both radiologically and clinically difficult to diagnose. The mechanism of injury is usually high energy, often associated with multiple injuries and so can easily be overlooked. A high degree of suspicion is warranted to avoid missing these fractures which have a propensity to displace if not fixed. This report highlights the clinical presentation and management of a Hoffa fracture in a patient presenting with polytrauma and reviews the current lit

Treatment of an osteoporotic Hoffa fracture.

Two patients with Hoffa fracture of the knee, both suffering from poliomyelitis are reported. Both had unicondylar coronal plane fracture of the medial femoral condyle. The patients were treated with open reduction and screw fixation. Due to poor screw purchase, reoperation was necessary in one patient. The results were satisfactory in both patients.

[Medial Hoffa fracture: a case report].

INTRODUCTION: knee femoral condyle coronal fractures or Hoffa fractures are among the rarest fractures of the pelvic limb, being even more rare those of the medial condyle. The success in the management of these patients lies in the timely diagnosis and the consequent anatomical reduction of the femoral articular surface. CLINICAL CASE: the objective of this article is to present a patient with right medial Hoffa fracture, registered as a single case in the archives of our hospital. The patient was referred to us on November 2011, with a 10-day course after suffering a fall from horse ridding

The Hoffa fracture: Three case reports.

Unicondylar fractures of the lower end of the femur are uncommon injuries that usually occur in the saggital plane 2. Coronal (tangential) plane fractures, first described by Hoffa in 1904, are unusual.(1,2, 3,4) We report our experience with three cases and the mechanism of injury is discussed. Open reduction and internal fixation is mandatory for good long-term results.

Hoffa fracture in a 14-year-old.

Coronal shear fractures of the femoral condyle (ie, Hoffa fracture) are an uncommon clinical entity typically seen in adults after higher-energy trauma. Historically poor outcomes have been reported in the literature with nonoperative treatment of these fractures. Conversely, open reduction and internal fixation of these fractures has been shown to produce good long-term clinical results in adults. These fractures appear to be even more uncommon in skeletally immature patients, with only 3 case reports documented in the literature to date. Two of the 3 cases presented as a symptomatic nonunion

Hoffa-mimicking fracture of the tibial plateau.

We report the case of a teenage male who had a slip and fall from a motorcycle and sustained injury to his left knee. A clinicoradiological evaluation revealed a closed displaced osteochondral fragment of the posterolateral tibial plateau. The patient underwent open reduction and internal fixation with two cannulated headless screws. The patient recovered uneventfully on follow-up. The Hoffa-like displaced posterolateral tibial plateau fracture with no associated ligament damage is a rare injury. Further, there are very few reported cases on the surgical approach and fixation techniques in thi

Nonunion of a Hoffa fracture in an adult.

The coronal fractures of femoral condyles, known as Hoffa fractures, are a rarity. Conservative management often leads to unsatisfactory results and nonunion. In this study, we present a case of a 35-year-old patient with an established nonunion of medial Hoffa fracture of right knee treated with open reduction and internal fixation.

[Difficulties involved in the Hoffa fractures].

To study the outcome in Hoffa fractures regarding the type of fracture, concomitant injuries, and different treatments. From January 1996 until December 2002, 7 Hoffa fractures (6 patients) from a total of 79 distal femoral fractures were treated in the Department of Trauma Surgery, University Hospital of Giessen. The results were evaluated by Neer's score. For the evaluation of postoperative vascularization of the distal femur, clinical and radiological findings were compared with anatomical studies of the bone arteries by corrosion preparations. Fracture consolidation was seen in all cases.

Nonunion of a Hoffa fracture in a child.

The authors report a case of a Hoffa fracture of the lateral femoral condyle that subsequently went on to nonunion in an eight-year-old child. The child presented with symptoms of knee pain and snapping five years after a motor vehicle accident. The nonunion fragment involved most of the lateral femoral condylar articular surface but spared the physis. After treatment by open reduction and limited internal fixation, the nonunion has healed, and the child has virtually full range of motion of the knee and no evidence of growth disturbance.

[Discussion on internal fixation of Hoffa fractures].

Hoffa fracture is an unstable intra-articular fracture with significant redisplacement tendency. It is easy to be missed diagnosis when accompanied by distal intercondylar or supracondylar fracture of femur. CT scan is the gold standard for the diagnosis of Hoffa fracture. The treatment principles are anatomic reduction of the articular surface, reliable internal fixation, and early functional activity. At present, the main treatment is arthroscopic screw fixation. During screw fixation, the tail cap of screw should be buried, resulting in non-healing iatrogenic injury of articular cartilage.

Case Report: Bicondylar conjoined Hoffa fracture with incarcerated patella.

CASE: Hoffa fractures, uncommon injuries of the femoral condyle, sometimes involve both condyles, forming bicondylar fractures, typically from high-velocity trauma. We describe a 17-year-old male with an open conjoined bicondylar Hoffa fracture and a patellar fracture with incarceration following a road traffic accident. Emergency treatments included debridement, irrigation, and cannulated screw fixation. Postoperative care involved controlled range-of-motion exercises and specialized rehabilitation. CONCLUSION: Despite its rare and severe nature, this conjoined bicondylar Hoffa fracture with

Novel approach of plate assisted buttressing in Hoffa fracture.

PURPOSE: Hoffa fracture is a femoral condyle fracture in the coronal plane. The lateral condyle is more commonly involved. The diagnosis is often difficult to detect with routine radiographs. Conservative management in this type of fracture resulted in nonunion, malunion, and other complications, such as stiff knee. Therefore, surgical management is mandatory in displaced fractures. Previous studies suggest only application of cancellous screw fixation, but these are not enough to counter vertical shear stress. Therefore, this study will evaluate the clinical outcomes of open reduction and int

Non -Union of Lateral Hoffa Fracture - A Case Report.

INTRODUCTION: External fixation, various subcutaneous screw fixations, and plate fixation can be considered as fixation methods for unstable pelvic ring fractures. We describe a first case of treated unstable pelvic ring injury using a dual internal anterior subcutaneous fixator we called "dual INFIX," comprising four screws, two subcutaneous rods, and two cross-link connectors, without posterior fixation. CASE REPORT: An 81-year-old man sustained an unstable pelvic injury (AO type B2) with fracture of the left ilium and pubis. Dual INFIX was used to stabilize the pelvic ring injury. Polyaxia

Operative management of Hoffa fracture of the femoral condyle.

Hoffa fracture is a rare injury consisting of unicondylar tangential posterior fracture of the distal femur and only very few cases have been reported in the literature. These fractures are due to high energy trauma and conservative treatment generally yields poor results, but rigid internal fixation allows early functional rehabilitation and decreases the incidence of complications. The purpose of the study was to prospectively analyse the clinico-radiological and functional outcome following open surgical treatment. From July 2005 to July 2010, 22 patients (14 males and 8 females) were recru

Open bicondylar Hoffa fracture associated with extensor mechanism injury.

Two cases of open bicondylar Hoffa fracture of the knee associated with extensor mechanism injury are described in two active young patients with multiple fractures. The level of the fracture was determined by the proximal insertion of the posterior cruciate ligament and anterior cruciate ligament in the medial and lateral condyle. The level of the extensor mechanism injury was determined by the degree of flexion of the knee at the moment of impact. No ligament or meniscal tears were found. Open reduction and internal fixation with four lag screws and bone-to-tendon repair of the patellar and

Repair of Displaced Partial Articular Fracture of the Distal Femur: The Hoffa Fracture.

PURPOSE: The Hoffa fracture is a rare fracture pattern consisting of a unicondylar posterior fracture of the distal femur. This video demonstrates a case of Hoffa fracture repair using headless screw compression. METHODS: Hoffa fractures are intra-articular in nature. Given that isolated Hoffa fractures are rare, there is little information available as to the best management of this injury. However, several small studies have demonstrated good-to-excellent functional results after anatomical reduction and rigid fixation of the Hoffa fracture, followed by early mobilization. RESULTS: This vi
