Detangling experiences of agency in welfare technology use by home care recipients and their staff.

Welfare technology (WT), defined as digital technology enabling individuals to live securely, actively and independently at home, is often viewed as a solution to care resource shortages. This solution discourse has increasingly been problematised, and ethical issues specifically connected to WT have emerged, e.g., regarding quality of care. The existing array of WT is developed with limited user involvement; primary user perspectives are thus inadequately voiced in research. This study aimed to disentangle how older adults and their homecare staff experience and influence WT use in homecare s

Resistant, adaptive, and supported agency - Examining narratives of Finnish older home care clients.

The purpose of this article is to increase our understanding of agency in older home care clients living in Finland. Twenty older home care clients (12 women, eight men) aged 73 and over were interviewed using a thematic interview. The stories narrated in the interview data were analyzed using thematic analysis, resulting in three forms of agency: resistant, adaptive, and supported. The findings show that older adults see themselves - despite their need for help and support - as capable individuals who challenge perceptions of being just objects of care. The stories of the older home care clie

Characteristics of care management agencies affect expenditure on home help and day care services: A population-based cross-sectional study in Japan.

AIM: The financial interests of care management agencies can affect how care managers assist clients' use of long-term care insurance services. The present study examined the relationship between clients' service expenditures, and whether the home help and day care service agencies belonged to the same organization as the care management agency. METHODS: Population-based data were obtained from a suburban municipality in Japan. We investigated 4331 persons with care needs certificates (levels 1-5), including those using home help (n = 1780) or day care (n = 2141) services. Data on the service

Do Higher-Cost Settings Deliver Better Outcomes? Post-Acute Care Efficiency in Medicare Skilled Nursing Facilities and Home Health Agencies.

Determining the value of different post-acute care settings for clinically similar patients is important for providers and payers. This study examines the causal effect of post-acute care (PAC) setting on Medicare spending and short-term patient outcomes among beneficiaries discharged from hospitals following treatment for stroke or congestive heart failure (CHF). Using 2013-2017 Medicare claims data, we implement an instrumental variable strategy that exploits daily variation in skilled nursing facility (SNF) bed occupancy at the hospital service area level to estimate the local average treat

Hepatitis B outbreak associated with a home health care agency serving multiple assisted living facilities in Texas, 2008-2010.

We investigated a multifacility outbreak of acute hepatitis B virus infection involving 21 residents across 10 assisted living facilities in Texas during the period January 2008 through July 2010. Epidemiologic and laboratory data suggested that these infections belonged to a single outbreak. The only common exposure was receipt of assisted monitoring of blood glucose from the same home health care agency. Improved infection control oversight and training of assisted living facility and home health care agency personnel providing assisted monitoring of blood glucose is needed.

Depression Agency-Based Collaborative: Effect of Problem-Solving Therapy on Risk of Common Mental Disorders in Older Adults With Home Care Needs.

BACKGROUND: Interventions to prevent depression in older adults have mainly focused on young-old ambulatory adults, not on the old-old with disabilities who receive supportive services in their homes. OBJECTIVE: The Depression Agency-Based Collaborative (Dep-ABC) is a single-blind pilot randomized controlled trial assessing the effect of an intervention-development strategy using problem-solving therapy (PST) on the risk of common mental health disorders in this vulnerable population. METHODS: The intervention involved six to eight sessions of PST over 12 weeks. Participants were followed up

Breaking the cycle: Effect of a multi-agency maternity service redesign on reducing the over-representation of Aboriginal and Torres Strait Islander newborns in out-of-home care: A prospective, non-randomised, intervention study in urban Australia.

BACKGROUND: Strategies to reduce over-representation of Indigenous children in out-of-home care must start in pregnancy given Indigenous babies are 6 % of infants (<1 year), yet 43 % of infants in out-of-home care. OBJECTIVE: To determine if an Indigenous-led, multi-agency, partnership redesign of maternity services decreases the likelihood of babies being removed at birth. PARTICIPANTS AND SETTING: Women carrying an Indigenous baby/babies who gave birth at the Mater Mothers' Public Hospital, Brisbane (2013-2019). METHODS: A prospective, non-randomised, intervention trial evaluated a multi-

Home care in New York City: providers, payers, and clients.

Estimates based on 1980 data indicate that almost two-thirds of the disabled elderly in New York City receive care only from informal sources. Another one-fifth use a combination of informal and formal sources of care. Thus, only about 15 percent are exclusively dependent on various agencies and programs that constitute the formal home care system. Over 200 organizations in 1986 offered some form of home care service in New York City. Among these were at least 100 proprietary businesses, 61 nonprofit personal care agencies, and 36 nonprofit certified home health agencies. In 1984, 166,739 diff

The state of infection prevention and control at home health agencies in the United States prior to COVID-19: A cross-sectional study.

BACKGROUND: Home health care is a rapidly growing healthcare sector worldwide. Home health professionals face unique challenges related to preventing and controlling infections, which are likely to amplify during an infectious disease outbreak (e.g. SARS-CoV-2). Little is known about the current state of infection prevention and control-related policies and outbreak preparedness at U.S. home health agencies. OBJECTIVES: In this study, we conducted a national survey to assess infection prevention and control-related policies, infrastructure, and procedures prior to the SARS-CoV-2 pandemic. DE

Quality of Medicare Skilled Home Health for People Living With Dementia in the US: National Patterns and Implications.

BACKGROUND: People living with dementia frequently use Medicare skilled home health care and have unique usage patterns as compared to people without dementia, but little is known about variation in measured quality of home health care received by this population. METHODS: Using 2021 Medicare Fee-for-Service Claims data, we examined receipt of high-quality home health (i.e., care from an agency with a star rating > 3.5) as determined by two publicly available measures: the Quality of Patient Care Star Rating (based on standardized clinical status measures) and the Patient Survey Star Rating (
