Home-based infusion therapy for patients with Fabry disease.

Fabry disease is an inherited, progressive, life-threatening disease; therefore, lifelong therapy is needed. By replacing the deficient enzyme, disease progression may be delayed or halted, thereby avoiding serious complications. Hospital-based agalsidase therapy is generally perceived as inconvenient and home-based infusion therapy is greatly appreciated by patients, their families and healthcare professionals. Patients can get familiar with infusion therapy in a hospital setting and, if specific requirements are fulfilled, routine nurse-assisted infusion, or self-care, at the patient's home

Hyperemesis gravidarum: implications for home care and infusion therapies.

Nausea and vomiting afflicts up to 80% of pregnancies, with the more severe form, hyperemesis, complicating about 1%. Hyperemesis in pregnancy causes fluid volume deficit, starvational ketoacidosis, and at times, metabolic alkalosis with hypokalemia. Significant weight loss may occur and reflect fluid as well as lean tissue loss. The cause is unknown, and although prognosis is generally good with aggressive fluid replacement and nutritional support, there is potential for decreased birth weight. Management strategies for hyperemesis gravidarum, including hydration, enteral and parenteral nutri

[Home infusion therapy for Pompe disease: Recommendations for German-speaking countries].

BACKGROUND: Pompe disease is a lysosomal multisystem disorder with predominant proximal myopathy. Treatment with enzyme replacement therapy (ERT) is available requiring life-long biweekly infusions of recombinant α-glucosidase. To minimize the burden of ERT patients ask for home infusion therapy. AIMS AND METHODS: Pompe disease experts from Germany, Austria, and Switzerland discussed in two consensus meetings in 2019 and 2020 requirements for home infusion therapy, adequate execution of treatment, and the legal situation for delegating physicians. RESULTS AND DISCUSSION: Home infusion therap

Home therapy with subcutaneous immunoglobulin infusions in children with congenital immunodeficiencies.

OBJECTIVES: Patients with congenital, humoral immunodeficiencies are usually treated with intravenous immunoglobulin infusions. Subcutaneous infusions have emerged as an alternative treatment modality also in children. Our institution has run a subcutaneous infusion home therapy program for 6 years, and the purpose of this report is to describe our experience with this regimen. METHODS: The subcutaneous therapy of eight patients with immunodeficiency (three with agammaglobulinemia, one with common variable immunodeficiency, one with severe combined immunodeficiency and bone marrow transplanta

Home infusion therapy is safe and enhances compliance in patients with mucopolysaccharidoses.

The transition to home infusion therapy is described in 11 patients with mucopolysaccharide (MPS) disorders initially treated with enzyme replacement therapy (ERT) in an out-patient infusion center. Home therapy was well tolerated with no infusion-related reactions (IRR) occurring in 671 patient-weeks of treatment. In addition, compliance with the weekly treatment regimen was improved in patients on home therapy.

Rapid home therapy infusion of velaglucerase alfa in naïve patients with Gaucher disease.

BACKGROUND: Enzyme replacement therapy (ERT) has revolutionised the management of patients with Gaucher disease (GD). In 2018, we published the safety and efficacy of rapid 10-min infusion of velaglucerase alfa in previously treated patients, mostly on low-dose therapy. AIM: To improve quality of life (QoL) for patients needing lifelong bi-weekly infusions by introducing a 10-min infusion instead of 1 h per label in patients naive to ERT and on high-dose therapy. METHODS: Fifteen naive patients were enrolled; all received bi-weekly infusions of 60 units/kgBW velaglucerase alfa; the infusion

Home Dobutamine Infusion Combined with Opioid Therapy in Patients with Advanced Heart Failure.

Home-based palliative care for patients with advanced heart failure remains challenging because refractory dyspnea and hemodynamic instability often require continuous medical support. Continuous dobutamine infusion is occasionally used to facilitate discharge to home in patients with end-stage heart failure; however, symptom relief with inotropic therapy alone may become insufficient during the terminal stage of illness. Reports describing the combined use of home dobutamine infusion and opioid therapy remain limited, particularly in Japan. We report two elderly patients with advanced heart f

Immunomodulatory therapy to achieve maximum efficacy: doses, monitoring, compliance, and self-infusion at home.

INTRODUCTION: The Oxford Programme for Immunomodulatory Immunoglobulin Therapy has been operating since 1992 at Oxford Radcliffe Hospitals in the UK. Initially, this program was set up for patients with multifocal motor neuropathy or chronic inflammatory demyelinating poly-neuropathy to receive reduced doses of intravenous immunoglobulin (IVIG) in clinic on a regular basis (usually every 3 weeks). The program then rapidly expanded to include self-infusion at home, which monitoring showed to be safe and effective. It has been since extended to the treatment of other autoimmune diseases in which

Home-based factor infusion therapy and hospitalization for bleeding complications among males with haemophilia.

Information from the medical records of 2650 US males with haemophilia living in six states was used to examine the influence of infusing factor concentrate at home (home therapy) and other variables on rates of hospitalization for a haemorrhagic bleeding complication (HBC) over a 4-year period. Bleeding complications included actual and suspected haemorrhagic events but excluded elective admissions for procedures necessitated by haemorrhage (e.g. joint synovectomy). Other risk determinants considered in the analyses included age, race, employment status, health insurance type, care received i

Stability of therapeutic monoclonal antibodies during infusion with elastomeric pumps intended for home-based therapy.

The majority of monoclonal antibodies (mAbs) are administered through intravenous infusion, typically in hospitals. A shift towards home-based therapy, initially driven by the COVID-19 pandemic, has been underway in recent years. Elastomeric pumps are devices that can be used for home-based therapy. They have been used for analgesics and antibiotics but not for delicate APIs such as proteins. While there is interest in using elastomeric pumps for administration of mAbs, and a few reports indicate that some hospitals have already started using them, valid stability data are lacking. [Home-based infusion therapy by Broviac catheter after total parenteral nutrition therapy in a patient with a severe skin condition].

In collaboration between the in-hospital nutrition support team and infection control team, we attempted to standardize the management of infusion therapy. We report on a simple and effective at-home infusion therapy, after total parenteral nutrition(TPN)therapy, by using a Broviac catheter in a discharged patient with a severe skin condition. The patient was a man in his 50s who had amyloidosis. Because of dysphagia and complications of the digestive organs, TPN was chosen as the method for nutritional management. At the beginning the patient repeated infection and a trouble of the skin, but

Association of overweight and obesity with the use of self and home-based infusion therapy among haemophilic men.

An elevated body mass index (BMI) may make venipuncture more difficult, potentially impacting the use of home infusion (HI) and self-infusion (SI). We sought to determine whether above-normal BMI is associated with decreased use of HI treatment and SI of clotting factor concentrate among haemophilic persons. We analysed data from 10,814 male patients with haemophilia A and B (45% with severe disease) aged 6-79 years enrolled in the Centers for Disease Control and Prevention Universal Data Collection surveillance project between 1998 and 2008. Associations between the use of HI and SI and BMI w
