Over a 10-year period, 47 infants with congenital hypertrophic stenosis underwent Ramstedt's pyloromyotomy in a district general hospital. The operations were carried out by 10 surgeons-6 consultants and 4 registrars. There were 3 major wound infections and 3 incisional hernias. One infant was referred to a specialist centre for reoperation.

Among 10 hospitals of the North East Thames Region, 84 patients were operated on for inflammatory bowel disease in the years 1977-81. There was one death in the 51 patients treated electively. The postoperative mortality was 24% in 33 patients coming to urgent surgery. The factors leading to this continuing high mortality are analysed as a contribution to an improvement in management.

A survey of 31 district general hospitals found a large variation in hours of medical and nursing time available per inpatient episode in general medicine and its associated specialties. These differences could not be attributed to case-mix variation or severity. Thirteen general ENT clinics are held for a population of 600,000 people. The present study comprises an individual experience in managing patients with dizziness seen in three general ENT clinics. Eighty-six patients with a complaint of dizziness were seen over a period of 10 years from 1974 to 1984. Postural vertigo in elderly patients was the commonest cause of vertigo (50 per cent) in the present study. Most of these patients suffered from momentary dizzy spells with excessive neck movements. Cholelithiasis is a rare finding in children, even though recent series show increased detection of this disease. A retrospective study was performed in children with a diagnosis of cholelithiasis between 1993 and 2005 in the Reina Sofia Hospital in Tudela (Spain). A retrospective analysis of all deliveries that occurred at Jimma hospital, south western Ethiopia from September 1990 to May 1999 was conducted to determine the incidence, maternal and perinatal outcome, sociodemographic and clinical characteristics of mothers with obstructed labor. A referral service for echocardiography was established in a district general hospital. One hundred and three patients were studied and the benefits to defined groups of referrals were evaluated. It is concluded that the introduction of echocardiography into this setting is of advantage to many patients with cardiac disease and should therefore be encouraged.

All cases of nasal polyposis seen in a district hospital in Nigeria over a 5 year period were analysed in a prospective study to determine the aetiological pattern, the prevalence, occurrence and pathological types as well as predisposing factors. Thirty-three cases of infective endocarditis presenting during a 6.5 year period to a district general hospital were analysed retrospectively. A retrospective study of 32 patients with colovesical fistula was carried out. The most frequent presenting symptoms were urinary; an abdominal mass was found in 40% of patients. Urine culture grew bowel organisms in 93% of patients and the diagnosis was most accurately confirmed by barium enema which showed evidence of a fistula in 55% of patients examined. Surgical treatment varied from staged resection to primary resection and anastomosis, disconnection of the fistulous tract or colostomy alone. The carotid surgical experience of Cheltenham General Hospital over a 13 year period (1968-81) is presented. This includes 42 operations for stenosis, and 12 further operations for carotid body tumour, carotid aneurysm, subclavian steal syndrome and trauma to the internal carotid artery. The operative techniques and complications are briefly discussed and reasons advanced for a more agressive approach to the problems of extra-cerebral carotid disease in this country.
