Do Pediatric Hospitals Improve Operative Efficiency?

BACKGROUND: In recent years there has been a push towards developing free standing pediatric facilities to provide care specifically towards pediatric patients. The purpose of this study was to determine if moving pediatric cases from a general hospital to a dedicated pediatric facility improved the quality and efficiency of surgical procedures. METHODS: A retrospective review of pediatric patients undergoing posterior spinal fusion (PSF) was performed. All procedures were performed by one orthopaedic surgeon (SLW) from 2015 to 2019. The procedures were performed at a general hospital (GH) th

Pediatric hospital condition consolidation in US hospitals.

BACKGROUND AND OBJECTIVES: While children's hospitals (CH) tend to be the locus of specialized hospital care, they also care for common conditions. There is no system to understand the distribution of hospital days within CHs and non-CHs (NCH) based on how ubiquitous conditions are across hospitals. We develop a method to classify conditions based on their commonality and consolidation within hospitals. METHODS: We performed a retrospective study of the 2022 Healthcare Cost and Utilization Project (HCUP) Kids' Inpatient Database, excluding normal newborns. For the 441 conditions, the volume o

Utilization of pediatric hospitals in New York State.

BACKGROUND: Little published information is available regarding the proportion of hospitals that offer pediatric services at specified levels, or the actual utilization of such services. OBJECTIVE: To determine if an existing national accreditation process created for educational purposes identifies hospitals with a distinct utilization pattern, in a sample of pediatric inpatient activity from one state. METHODS: Hospitals in New York State were classified as "pediatric" hospitals according to their sponsorship or major participation with an accredited pediatric residence program. Institutio

Pediatric outpatient parenteral antibiotic therapy: an important option for pediatric hospitals in Germany.

Outpatient parenteral antibiotic therapy (OPAT) has been established for decades in children and adolescents with cystic fibrosis. Today, prolonged intravenous antibiotic treatment is less frequently required due to the increased use of oral sequential therapy with easily absorbable antibiotics. However, when it is needed in special situations, pediatric OPAT (pOPAT) can be a beneficial and safe alternative for various reasons (especially quality of life, limited inpatient resources, treatment costs). This article takes a critical look at the current status of OPAT in children and adolescents

Navigating Limited Resources-Outpatient Pediatric Cholecystectomies at Rural Hospitals.

BACKGROUND: Rural facilities that provide pediatric surgical services are a critical resource to local communities. Our aim was to characterize differences in outpatient pediatric cholecystectomy outcomes performed at rural and urban hospitals with the hypothesis that rural hospitals would have similar outcomes. METHODS: The Nationwide Ambulatory Surgery Sample (NASS), which contains ambulatory surgery encounters at hospital-owned facilities, was used to perform a retrospective cohort analysis of pediatric patients age 18-years and younger who had a cholecystectomy (n = 15,449) between 2016 a

Assessing the Respect of Children's Rights in Pediatric Hospitals.

Background and Objectives: In 1989, the United Nations (UN) General Assembly adopted the United Nations Convention on the Rights of the Child (UNCRC), with a considerable number of the Articles of the Convention being related to the health status of children. Therefore, adhering to and assessing the implementation of the rights of children during hospitalization is a very important step towards child protection. Herein, we attempt to highlight the depth of knowledge of employees working in children's hospitals with regard to children's rights as well as the degree of adherence to the UNCRC wit

Pediatric liver transplantation at the University of Chicago Hospitals.

The concept of graft-reduction surgery in liver transplantation has revolutionized pediatric transplantation and soon may carry over into adult transplantation. Reduced-size liver transplants have virtually eliminated patient deaths while waiting for donor organs to become available by increasing the potential donor pool. This has shifted the distribution of pediatric liver transplant recipients to a younger age which more closely matches the needs of pediatric patients with end-stage liver disease. Split-liver transplants can create new grafts without increasing the donor supply. Living-donor

Variation in In-Hospital Emergency Transfer Rates Across Pediatric Hospitals.

BACKGROUND: Clinical deterioration with late escalation, termed emergency transfer (ET), is a key safety metric. However, national benchmarking is limited by variable definitions. We sought to estimate pooled ET rates and assess variation across pediatric hospitals. METHODS: We performed a cross-sectional analysis of ET events (2020-2022) at 7 pediatric tertiary care hospitals. ET was defined as unplanned transfer to the pediatric intensive care unit with intubation, vasopressors, and/or at least 60 mL/kg fluid resuscitation within 1 hour before or after transfer. We calculated ET rates per 1

Use of antimicrobials in pediatric wards of five Brazilian hospitals.

The use of antimicrobials (AMs) in pediatric infections is common practice and use may be inappropriate leading to antimicrobial resistance. Off-label AM use is also common in this group and can result in drug-related problems. There is lack of DUR data in Brazil and in Latin America, specially for AM pediatric use. The aim of this study was to describe the utilization of AMs in hospitalized children in five hospitals in Brazil. We conducted an observational study of the utilization of AMs in pediatric wards in hospitals in the states of Ceará (CE), Sergipe (SE), Rio de Janeiro (RJ), Rio Grand

Variation in post-adenotonsillectomy admission practices in 24 pediatric hospitals.

OBJECTIVES/HYPOTHESIS: There is controversy about which children should be admitted after adenotonsillectomy (T&A) and limited clinical evidence to help with this decision. Current practice has evolved based on empirical or anecdotal evidence. We sought to identify practice variations in postoperative admission after T&A in tertiary care pediatric hospitals. STUDY DESIGN: Retrospective database study using administrative information stored in the Pediatric Health Information System (PHIS) database. METHODS: There were 29,920 T&As performed in 24 pediatric hospitals included in the PHIS datab

Amiodarone monitoring practices in pediatric hospitals in the United States.

Laboratory monitoring of amiodarone therapy is recommended due to the high incidence of adverse events associated with the drug. The use of appropriate monitoring is unknown at pediatric hospitals. The Pediatric Health Information System database was queried during a 5-year period for all patients who received amiodarone while hospitalized. Use of thyroid function testing, hepatic function testing, electrocardiogram, pulmonary function testing, and chest X-ray for patients was identified. Incidence of individual monitoring tests and complete monitoring profile was identified and compared acros

Pediatric Hospitalization for Varicella in an Italian Pediatric Hospital: How Much Does It Cost?

BACKGROUND: Varicella is a common pediatric infection. Even if it generally has a benign course, it may complicate and require hospitalization. The aim of our study was to estimate the acute hospitalization cost (AHC) for varicella in the acute phase in a pediatric population. METHODS: We calculated the AHC of pediatric patients admitted for varicella at Bambino Gesù Children Hospital, Rome, Italy, from 1 November 2005 to 1 November 2020. RESULTS: In the study period, 825 pediatric patients affected by varicella were hospitalized. The mean hospitalization cost was EUR 4015.35 (range from EUR

Pediatric Hyperacute Arterial Ischemic Stroke Pathways at Canadian Tertiary Care Hospitals.

BACKGROUND: Childhood acute arterial ischemic stroke (AIS) is diagnosed at a median of 23 hours post-symptom onset, delaying treatment. Pediatric stroke pathways can expedite diagnosis. Our goal was to understand the similarities and differences between Canadian pediatric stroke protocols with the aim of optimizing AIS management. METHODS: We contacted neurologists at all 16 Canadian pediatric hospitals regarding AIS management. Established protocols were analyzed for similarities and differences in eight domains. RESULTS: Response rate was 100%. Seven (44%) centers have an established AIS p

Increasing Ketamine Use for Refractory Status Epilepticus in US Pediatric Hospitals.

Ketamine is an emerging therapy for pediatric refractory status epilepticus. The circumstances of its use, however, are understudied. The authors described pediatric refractory status epilepticus treated with ketamine from 2010 to 2014 at 45 centers using the Pediatric Hospital Inpatient System database. For comparison, they described children treated with pentobarbital. The authors estimated that 48 children received ketamine and pentobarbital for refractory status epilepticus, and 630 pentobarbital without ketamine. Those receiving only pentobarbital were median age 3 [interquartile range 0-
