Outcomes of Veterans Treated in Veterans Affairs Hospitals vs Non-Veterans Affairs Hospitals.

IMPORTANCE: Many veterans enrolled in the Veterans Affairs (VA) health care system have access to non-VA care through insurance and VA-purchased community care. Prior comparisons of VA and non-VA hospital outcomes have been limited to subpopulations. OBJECTIVE: To compare outcomes for 6 acute conditions in VA and non-VA hospitals for younger and older veterans using VA and all-payer discharge data. DESIGN, SETTING, AND PARTICIPANTS: This cohort study used a repeated cross-sectional analysis of hospitalization records for acute myocardial infarction (AMI), coronary artery bypass graft (CABG),

Management of Bacteriuria in Veterans Affairs Hospitals.

BACKGROUND: Bacteriuria contributes to antibiotic overuse through treatment of asymptomatic bacteriuria (ASB) and long durations of therapy for symptomatic urinary tract infections (UTIs), yet large-scale evaluations of bacteriuria management among inpatients are lacking. METHODS: Inpatients with bacteriuria were classified as asymptomatic or symptomatic based on established criteria applied to data collected by manual chart review. We examined frequency of treatment of ASB, factors associated with treatment of ASB, durations of therapy, and frequency of complications including Clostridium di

Veterans Hospitals vs Non-Veterans Centers in the Prevention of Limb Loss.

IMPORTANCE: US veterans remain an at-risk population for limb loss, given the prevalent rates of coexisting diabetes and peripheral arterial disease (PAD). Moreover, despite the perception that a healed minor amputation might obviate the need for a future major amputation, previous work documented increased longitudinal rates of limb loss among non-veteran Medicare beneficiaries. OBJECTIVE: To document the corresponding incidence of major amputation among veterans after a prior minor amputation and accordingly benchmark Veterans Affairs (VA) health care delivery compared to non-VA civilian ho

Histoplasmosis in Veterans Administration hospitals in middle America.

A series of clinical studies of histoplasmosis based on demonstrations of etiology led to descriptions of chronic pulmonary histoplasmosis, the principal type in adults, and amphotericin B as an effective therapy. A participant recreates the atmosphere of discovery during the contributions of a team comprising a mycologist, a surgeon and two physicians using cultures of excised lung and sputum to clearly identify cases of chronic pulmonary histoplasmosis. Subsequently cooperating physicians in several V.A. Hospitals carried out randomized studies of treatment, dosage, and side effects that est

Inpatient suicide and suicide attempts in Veterans Affairs hospitals.

BACKGROUND: Suicide is the eleventh leading cause of death in the United States. Approximately 1,500 suicides occur in inpatient hospital units in the United States each year. In an attempt to determine the methods and environmental factors involved in inpatient suicide and suicide attempts in Department of Veterans Affairs (VA) hospitals, all root cause analysis (RCA) reports of inpatient suicides and suicide attempts submitted to the VA National Center for Patient Safety (NCPS) before June 2006 were reviewed. METHODS: VA medical centers are required to conduct RCAs on all inpatient suicides

Overutilization of acute-care beds in Veterans Affairs hospitals.

The authors tested the hypothesis that the Department of Veterans Affairs (VA) hospitals would have substantial overutilization of acute care beds and services because of policies that emphasize inpatient care over ambulatory care. Reviewers from 24 randomly selected VA hospitals applied the InterQual ISD* (Intensity, Severity, Discharge) criteria for appropriateness concurrently to a random sample of 2,432 admissions to acute medical, surgical, and psychiatry services. Reliability of hospital reviewers in applying the ISD* criteria was tested by comparing their reviews with those of a small g

Outpatient Parenteral Antimicrobial Therapy Practice Variability Across Veterans Affairs Hospitals.

IMPORTANCE: Challenges providing outpatient parenteral antimicrobial therapy (OPAT) can compromise patient safety and care quality, yet little is known about how OPAT is delivered by health care systems across the US. OBJECTIVE: To understand and compare OPAT delivery at US Department of Veterans Affairs Medical Centers (VAMCs). DESIGN, SETTING, AND PARTICIPANTS: This qualitative study using a mixed-methods approach was performed from February 6, 2024, through April 15, 2025. Six midwestern VAMCs were purposively selected, identifying physician and pharmacist leaders to participate in qualit

Use of and Outcomes in Veterans Affairs and Other Hospitals Among Veterans Experiencing Homelessness After Expanded Access.

OBJECTIVE: Veterans in the U.S. Department of Veterans Affairs (VA) health care system experiencing homelessness have high rates of hospitalization. The 2014 Veterans Choice Act and Medicaid expansion programs under the Affordable Care Act expanded access to VA-purchased care and Medicaid for veterans. The authors aimed to examine-in VA and non-VA hospitals-changes in medical-surgical and psychiatric hospitalizations and hospital outcomes after these expansions for homeless veterans versus other veterans. METHODS: A longitudinal study (2012-2017) of veterans in 11 states was conducted. Non-VA

BRUCELLOSIS IN A VETERAN'S HOSPITAL, 1963.

In Canada, about 100 sporadically occurring cases of brucellosis are reported yearly. Three patients were admitted to one Montreal hospital in the first seven months of 1963; all were employed in or around a packing plant. One had pain and electrocardiographic changes suggestive of Brucella myocarditis; he recovered promptly. Symptoms of neurasthenia and anxiety are common and were observed in two of these three cases. Two had positive blood cultures; the third showed conclusive agglutination to Br. abortus (1:10,240). In some patients with frequent previous exposures to infection the agglutin

Do discharge delays explain longer stays at veterans health administration hospitals?

BACKGROUND: Discharge delays are difficult to quantify without standardized indicators for when patients are medically ready for discharge. We aimed to estimate the proportion of increased hospital length of stay attributable to discharge delays, as proxied by increases in 'avoidable' days. METHODS: We conducted a retrospective cohort study of Veterans Health Administration hospitals in the continental United States with emergency departments between 1 March 2019 and 28 February 2023. We included Veterans who were discharged from an acute medicine service without a COVID-19 diagnosis. We used

Reexamining Differences Between Black and White Veterans in Hospital Mortality and Other Outcomes in Veterans Affairs and Other Hospitals.

OBJECTIVES: To examine Black-White patient differences in mortality and other hospital outcomes among Veterans treated in Veterans Affairs (VA) and non-VA hospitals. BACKGROUND: Lower hospital mortality has been documented in older Black patients relative to White patients, yet the mechanisms have not been determined. Comparing other hospital outcomes and multiple hospital systems may help inform the reasons for these differences. METHODS: Repeated cross-sectional analysis of hospitalization records was conducted for Veterans discharged in VA and non-VA hospitals from January 1, 2013 to Dece

The Whipple resection for cancer in U.S. Department of Veterans Affairs Hospitals.

OBJECTIVE: The authors compiled the results after Whipple resection for cancer from a large U.S. national hospital system. METHODS: Computerized hospital and death benefits records for patients treated with Whipple resection for cancer from 1987 to 1991 in U.S. Department of Veterans Affairs hospitals were analyzed, excluding lymphomas and neuroendocrine tumors. Institutional tumor registrar reports allowed TNM staging in 45% of these cancers. RESULTS: Whipple resections were performed in 252 patients with pancreatic cancer and 117 with other periampullary cancers. Complications occurred in

Appropriateness of short-stay admissions for procedures in six Veterans Affairs hospitals.

Admissions records were reviewed for six acute care Veterans Affairs Medical Centers (VAMC's) in New England to determine appropriateness of short-stay admissions (two days or less) in fiscal years 1986 and 1987 for certain medical and surgical procedures. Results indicated that such admissions accounted for 18,588 (22%) of a total of 84,266 admissions for the six hospitals; of these admissions, 4,362 were for procedures commonly performed on an outpatient basis. Criteria developed by a peer review board of physicians was applied to a sample of 728 (16.7%) of the 4,362 short-stay admissions fo

Periampullary cancer treatment in U.S. Department of Veterans Affairs hospitals: 1987-1991.

BACKGROUND: Reported treatment outcomes for painless jaundice caused by ampullary, distal biliary, or duodenal (ABD) cancer are better than those caused by pancreatic cancer. METHODS: Outcomes after resection, bypass, or biliary intubation were compared in 432 patients with ABD cancer and 1753 with periampullary pancreatic cancer from U.S. Veterans Affairs hospitals. Computer and tumor registrar records from 1987 to 1991 were compiled; follow-up to death was complete in more than 93%. RESULTS: Operative mortality and complication rates were similar for each diagnosis. Adenocarcinomas staged

Housing outcomes for hospitalized homeless veterans.

This study examines housing status at the time of hospital discharge in a national sample of 3,502 veterans who were homeless at admission to a Veterans Health Administration (VHA) medical center. A supplemental survey on homelessness was added to the annual VHA inpatient census over a 4-year period (1995 through 1998). Data from this survey were used to identify homeless veterans hospitalized on the final day of the fiscal year and to document their housing status at discharge. A signal detection technique, Receiver Operating Characteristic analysis, was used to identify predictors of housing
