Controlled human infection models are powerful tools for evaluating pathogenesis, immune responses, vaccines, and therapeutics, including parasitic helminths. Helminths have also been tested as inflammatory disease therapeutics. This review focuses on immune responses during controlled infections with Trichuris species, Necator americanus, and Schistosoma mansoni. Over 700 participants have been experimentally infected across 33 studies to date. These models reveal significant heterogeneity in type 2-associated responses among individuals for all infections. While eosinophilia is most frequent

Controlled human infection models (CHIMs) can accelerate vaccine development for infectious diseases. Mycobacterium tuberculosis is a human-adapted pathogen that is the leading infectious cause of death worldwide. M tuberculosis infection results in a spectrum of clinical outcomes that are incompletely modelled in animals. To date, the risks of infection, prolonged treatment, and sequelae related to CHIMs with M tuberculosis have been considered ethically unacceptable. However, recent advances in bacterial engineering have resulted in safe strains that could permit M tuberculosis CHIM studies

Antibody-mediated immunity directed against Plasmodium falciparum erythrocyte membrane protein 1 (PfEMP1) is an important immune mechanism for protection against clinical malaria and control of parasitemia. Current studies on PfEMP1-based immunity are constrained by the protein's high molecular weight, complex multidomain architecture, and extensive polymorphism. Controlled human malaria infection (CHMI) studies uniquely enable the assessment of strain-specific PfEMP1 immunity prior to exposure and its correlation with infection outcomes using the same parasite strain. In this study, different

Chlamydia trachomatis remains the most common sexually transmitted bacterial infection worldwide. Recent phase I trials of the C. trachomatis vaccine candidate CTH522 in women and men have shown that the adjuvanted and non-adjuvanted formulations are safe and elicit systemic (immunoglobulin G) and mucosal (immunoglobulin A) antibodies, as well as vaccine-specific cell-mediated immunity. In advance of a standard phase II randomized controlled trial to determine the vaccine's efficacy, this perspective advocates for a carefully and ethically designed human challenge model as an innovative approa

INTRODUCTION: Human challenge studies (HCS) have been proposed to accelerate the development of health interventions and strengthen research capacity in low- and middle-income countries. However, implementation in settings such as Vietnam, where clinical care facilities and research capacity are limited and no specific regulatory framework for HCS exists, raises special ethical, social, and practical concerns. This study examined healthcare professional students' perceptions of and attitudes toward HCS, focusing on ethical considerations and factors associated with willingness to participate.

Controlled human infection studies play a critical role in advancing our knowledge of infectious diseases and developing therapeutics. However, these studies involve knowingly exposing research participants to pathogens, raising significant ethical, social, environmental and logistical concerns. Controlled human infection studies are being conducted in settings where individuals are economically disadvantaged and may have limited access to healthcare, education, and basic social amenities, making community engagement a central element in ensuring the ethical conduct of controlled human infecti

Controlled human infection models can play an important role in vaccine development, particularly for neglected tropical diseases such as helminth infections. Currently, controlled infection models have been established for schistosomiasis and hookworm. This review highlights the developments in the controlled human schistosomiasis infection model (CHI-S) and the controlled human hookworm infection model (CHHI) and their contributions to vaccine development. In general, both models are considered safe and well-tolerated. Measures to decrease risk of potential adverse events were taken when dev

Dengue viruses are mosquito-borne human pathogens whose global incidence in human infections is rising. Surveillance is hampered by the requirement for blood-based diagnostics. Interestingly, in several prior research studies, dengue virus nucleic acids and proteins could be detected in saliva, making saliva-based diagnostics a plausible alternative. However, the temporal relationships between exposure, viremia, salivary accumulation, and symptom onset remain poorly defined. To address this knowledge gap, nine participants were enrolled in clinical trial NCT04298138 to receive an inoculation w

INTRODUCTION: Rates of sexually transmitted infections are on the rise globally, including those caused by Neisseria gonorrhoeae. Retrospective analyses across clinical settings around the world have indicated that rates of gonorrhoea declined following mass campaigns with vaccines made with outer membrane vesicles (OMV) from N. meningitidis serogroup B, a pathogen closely related to N. gonorrhoeae. The US Food and Drug Administration (FDA)-approved N. meningitidis serogroup B vaccine (4CMenB) contains N. meningitidis OMV and five recombinant protein components: factor H-binding protein (fHbp)

Malaria remains a significant global health burden, particularly among pediatric populations in endemic regions. Recent advances in preventive interventions highlight the complementary roles of vaccines and long-acting monoclonal antibodies (mAbs) in reducing infection risk. Vaccines such as RTS,S/AS01 and R21/Matrix-M induce active immunity through polyclonal antibody and CD4+ T-cell responses against the circumsporozoite protein (CSP), providing durable, population-level protection. In contrast, mAbs including CIS43LS and L9LS confer immediate, passive immunity, with protection directly link

BACKGROUND: Controlled human infection (CHI) trials deliberately expose healthy participants to a pathogen. Although they share design features with phase I clinical trials, CHI trials may differ in participant burden and administrative burden. Literature on CHI participant motivations and trial experience compared to phase I participants is limited.
METHODS: We conducted a survey comparing motivation, decision-making, and ethical views among CHI and phase I participants, using a previously validated questionnaire. Descriptive and comparative analyses assessed motivational drivers and ethical

BACKGROUND: The zoonotic parasite Plasmodium knowlesi is an increasing cause of human malaria in Southeast Asia. We aimed to develop a P. knowlesi induced blood-stage malaria (IBSM) model to facilitate study of parasite biology, host responses to infection, and activity of antimalarial treatments.
METHODS: We manufactured and characterized a P. knowlesi parasite bank using the P. knowlesi YH1 strain adapted to grow in human serum, and evaluated this bank in an IBSM study involving four adults intravenously inoculated with P. knowlesi-infected erythrocytes using a dose escalation strategy (10-

BACKGROUND: The immunopathogenesis of dengue infection and immune correlates of protection are uncertain, no therapeutic anti-viral is available, and the long-term risks of severe breakthrough infection after vaccination remain of concern. Dengue controlled human infection models (DCHIM) have found increasing utility and have the potential to address these unmet needs. We reviewed the clinical, biochemical and immunologic features of modern day DCHIMs.
METHODS: A systematic review protocol was developed and registered with PROSPERO [CRD42024558534]. We searched MEDLINE, Cochrane and Embase fo

A previous controlled human influenza transmission trial produced minimal transmission using nasal inoculation of an egg adapted virus. Therefore, we implemented a new trial with naturally infected Donors. We recruited healthy Recipients for four, two-week hotel quarantine cohorts and naturally infected, qRT-PCR confirmed Donors for two cohorts. Five Donors (mean age: 21; 80% female; two H1N1, three H3N2, one for cohort 24b and 4 for 24c, Jan-Feb 2024) exposed Recipients (mean age: 36; 54% female, eight in cohort 24b and 3 in 24c) in a hotel room with limited ventilation but a high air recircu

Mycobacterium tuberculosis infection is a dynamic continuum. Clinical outcomes reflect complex host-pathogen interactions. Epidemiological and animal studies have suggested influenza coinfection as a risk factor for progression from contained infection to active disease, but human studies have been lacking. Using a whole blood luminescent mycobacterial growth inhibition assay within a human influenza challenge study, we show that influenza infection reduces immunological control of mycobacterial growth. Transcriptome-wide RNA sequencing, cytokine and cellular analyses of subjects' blood before

BACKGROUND: Cyclical epidemics of pertussis, also known as whooping cough, continue to occur despite vaccination, because current vaccines fail to suppress colonisation by, and transmission of, the causative agent Bordetella pertussis. We aimed to show the safety of B pertussis controlled human infection delivered in an outpatient setting, as well as identify immunological correlates of protection against B pertussis colonisation, to guide the future development of pertussis vaccines.
METHODS: This open-label, phase 1, controlled human infection study was conducted at University Hospital Sout

Identifying conserved, immunogenic proteins that confer protection against Streptococcus pneumoniae (pneumococcus) colonization could enable development of serotype-independent vaccines. In our controlled human infection model, no individual IgG or cytokine/chemokine response correlated significantly with protection against colonization with pneumococcus, suggesting that effective immunity reflects a coordinated, multi-antigen response. To capture these complex patterns, we trained independent Random Forest models on humoral and cellular datasets. The humoral model identified IgG responses to

Health research plays a crucial role in advancing medicine and public health, but ethical constraints shape its conduct. This study presents findings from the Views in Bioethics Survey (VIBeS), the first large-scale systematic survey of bioethicists in the United States, which aimed to assess perspectives on key research ethics issues. We surveyed 824 bioethicists on topics including human challenge trials, research without explicit consent, financial incentives for participation, local standards of care, and the publication of high-risk research. Our findings reveal areas of consensus-such as

BACKGROUND: Experimental infection with Plasmodium falciparum in malaria-naive European adults induces CD25+FoxP3+ regulatory CD4+ T cell (Treg) expansion. Increased Treg activity is associated with reduced inflammatory cytokine responses and increased parasite growth. Whether similar associations occur in semi-immune adults from malaria-endemic regions remains unclear. In this study, we aimed to examine how inflammatory cytokines, regulatory cytokines, and Tregs influence parasitic and clinical outcomes during controlled human malaria infection in semi-immune adults from three regions in Keny

BACKGROUND: Bacterial mucosal immunotherapy has shown protection of children and adults from both viral and bacterial respiratory infections, offering the potential to reduce antimicrobial use, and hence also control antimicrobial resistance (AMR). Pneumococcal carriage of vaccine type Streptococcus pneumoniae remains high in Malawi despite infant conjugate vaccination and AMR is increasing. We compared nasal inflammation following sublingual bacterial immunotherapy including S. pneumoniae (MV130, Inmunotek, Spain) or placebo and determined the effect in an experimental human pneumococcal carr
