BACKGROUND: Distal humerus fractures constitute 2% of all fractures in the adult population. Although historically, these injuries have been treated non-operatively, advances in implant design and surgical technique have led to improved outcomes following operative fixation.
METHODS: A literature search was performed and the authors' personal experiences are reported.
RESULTS: This review has discussed the anatomy, classifications, treatment options and surgical techniques in relation to the management of distal humeral fractures. In addition, we have discussed controversial areas including

For acute distal humeral fractures not amenable to open reduction and internal fixation, total elbow arthroplasty has become an established alternative. However, lifelong activity restrictions designed to prevent early mechanical failure make this a poor option for some patients. This has led to a renewed interest in distal humeral hemiarthroplasty. Using modern implants and techniques, distal humeral hemiarthroplasty has shown outcomes comparable to those of total elbow arthroplasty at short- to mid-term follow-up, with an overall higher but different complication rate. Long-term data are nee

The treatment of comminuted, distal humeral fractures in elderly patients with osteoporotic bone is challenging. Total elbow arthroplasty or hemiarthroplasty are reliable treatment options with favorable outcomes for fractures that are not amenable to open reduction and internal fixation. Total elbow arthroplasty is a reliable option for a comminuted distal humeral fracture in an elderly patient with osteoporosis and low functional demands. Longer-term studies have shown good to excellent results and a low risk of complications. Specific indications for hemiarthroplasty are evolving, but inclu

Distal humeral fractures in the adult often are complicated by pseudarthrosis, pain, and limitation of motion. Of 29 adult patients with distal humeral fractures treated in teaching hospitals, 22 obtained acceptable results. Twenty-two percent of those treated by open methods and 36% of those treated by closed methods had unacceptable results. Ninety percent of the patients starting physiotherapy within six weeks of injury recovered well, while over half of those starting physiotherapy after six weeks had unsatisfactory results. Even comminuted fractures healed when treated by internal fixatio

Approaching the elbow joint posteriorly is the most used method to visualize closed fractures of the distal humerus in adults. Treatment can span from open reduction and internal fixation to elbow hemiarthroplasty and total elbow arthroplasty. Different approaches were developed in the necessity of balancing an adequate visualization with minimizing complications and these surgical exposures can be divided into four main categories: paratricipital, triceps reflecting, triceps splitting and olecranon osteotomy. In this article are described the positive and negative features of the principal po

The incidence of complex articular fractures of the distal humeral in adults has increased and will be growing in the future due to the greater incidence of high-energy trauma and to the higher percentage of the elderly population. Successful treatment is challenging for the needed balance between the stability of often comminuted fractures and early motion. Malunion is a common complication after distal humerus fractures that is influenced by a variety of factors, such as biology, particularly the blood supply of the metaphysis, the nonanatomical reduction of the fracture, the methods of fixa

Coronal shear fractures of the distal humerus are rare, complex fractures that can be technically challenging to manage. They usually result from a low-energy fall and direct compression of the distal humerus by the radial head in a hyper-extended or semi-flexed elbow or from spontaneous reduction of a posterolateral subluxation or dislocation. Due to the small number of soft tissue attachments at this site, almost all of these fractures are displaced. The incidence of distal humeral coronal shear fractures is higher among women because of the higher rate of osteoporosis in women and the diffe

This article describes a technique of open reduction and internal fixation of intra-articular fractures of the distal humerus in adults. The historical treatment of these fractures is surveyed, including current classification systems. Anatomy is reviewed, specifically as it pertains to surgical intervention. The results in 23 consecutive patients stabilized through an intra-articular olecranon osteotomy exposure are reported.

We have reviewed 12 cases of fracture-separation of the distal humeral epiphysis, three of which were initially misdiagnosed as fractures of the lateral condyle and one as an elbow dislocation. Cubitus varus deformity is as common after this fracture-separation as it is following supracondylar fracture, and is most common in children under two years of age. Closed reduction and simple immobilisation is adequate for the older child, but we recommend for those under two years of age that closed reduction should be followed by percutaneous pinning, so that the carrying angle can be assessed immed

BACKGROUND: Posterior shear fractures of the distal humerus are rare and have a complex, controversial injury mechanism. This study aims to investigate the morphological characteristics, injury mechanisms, and treatment outcomes of these fractures.
METHODS: From March 2021 to October 2024, 13 patients with posterior shear fractures of the distal humerus were treated with open reduction and internal fixation. Among them, 7 were male and 6 were female, with a mean age of 40 years (range: 25-66 years). Five cases involved the left side, and 8 involved the right side. All fractures were closed. M

The aim of this study was to assess outcomes following open reduction and internal fixation in complex fractures of the distal humerus. Between 2000 and 2006, 34 patients were operated for complex fractures of the distal humerus. Bone fixation was obtained with a reverse Y-shaped reconstruction plate in 13 cases and with double plating in 21 cases. At final follow-up, all the patients were assessed with the Mayo Elbow Performance Score. Satisfactory results were observed in 71% of the cases despite a high rate of complications. Age over 65 years is correlated with increased risk for an inferio

PURPOSE: Distal metaphyseo-diaphyseal humeral fractures are challenging particularly if open, comminuted, or associated with nerve injury. In cases of open distal complex metaphyseo-diaphyseal humeral fractures inamenable for traditional methods of fixation, retrograde intramedullary locked humeral nail with a new lateral condylar point of entry has been proposed.
METHODS: Two phases of study were conducted; phase I comprised computerized tomography evaluation of right humeri of 120 adult subjects, while phase II entailed prospective analysis of 18 patients who sustained firearm injuries resu

The number of fractures of the distal humerus in older adults is rising due to an ageing population and an increasing incidence of fragility fractures. The management is based on the patient's preference, the pattern of the fracture, the quality of the bone and soft-tissue, the patient's comorbidities and potential for rehabilitation, as well as balancing the risk of complications such as infection, nonunion, instability, or loosening. Nonoperative treatment remains appropriate for a few low-demand or medically unfit patients. Open reduction and internal fixation (ORIF) or arthroplasty, either

PURPOSE: To review the functional outcome of AO type C distal humeral fractures (bicondylar fractures) managed with dual orthogonal plate fixation.
METHOD: Twenty-three patients were reviewed retrospectively at a mean of 45.1 months. Patient-rated outcomes (Disabilities of the Arm Shoulder and Hand [DASH], Patient Rated Ulnar Nerve Evaluation [PRUNE], American Shoulder and Elbow Surgeons Elbow form [ASES-e], and Short Form-36 [SF-36]), clinical, radiographic, and objective evaluations were used to assess outcomes.
RESULTS: Almost 40% of patients required a third plate for adequate fixation.

We reviewed 17 patients with distal intraarticular humeral type C fractures, treated by open reduction and internal fixation, followed by an early rehabilitation programme. The results, as judged by the criteria of Jupiter, were excellent in ten, good in five and fair in two. Non-union of the olecranon osteotomy occurred in one patient.

BACKGROUND: Distal humeral fractures in adults are relatively uncommon injuries that require surgical intervention in most cases. There is a lack of consensus regarding the best management of distal humeral fractures in adults, including the role of conservative treatment, appropriate surgical approach, fixation strategies, the role of total elbow arthroplasty and handling of nerves such as the ulnar nerve.
OBJECTIVES: To assess the effects (benefits and harms) of surgical interventions for distal humeral fractures in adults.
SEARCH METHODS: We searched the Cochrane Bone, Joint and Muscle Tr
