Hydromorphone.

Hydromorphone is a semi-synthetic opioid that has been used widely for acute pain, chronic cancer pain and to a lesser extent, in chronic nonmalignant pain. Its pharmacokinetics and pharmacodynamics have been well studied, including immediate release oral preparations, a variety of slow release oral preparations, as well as administration through intravenous, subcutaneous, epidural, intrathecal and other routes. It is known to be metabolized to analgesically inactive metabolites that have been associated with neuroexcitatory states and other toxicity. There is no evidence that hydromorphone ha

Hydromorphone neuroexcitation.

Continuous parenteral hydromorphone is used to treat pain in palliative care. Case reports have suggested that neuroexcitatory symptoms, such as agitation, myoclonic activity, and even seizures may occur during administration. However, little information exists on the incidence of these side effects or their relationship to the dose or duration of parenteral hydromorphone. A retrospective chart review was performed on 48 terminally ill hospice patients who received continuous parenteral hydromorphone for pain control. Chart reviews were conducted searching for three neuroexcitatory symptoms: a

Hydromorphone for cancer pain.

BACKGROUND: This is an update of the original Cochrane Review first published in Issue 10, 2016. For people with advanced cancer, the prevalence of pain can be as high as 90%. Cancer pain is a distressing symptom that tends to worsen as the disease progresses. Evidence suggests that opioid pharmacotherapy is the most effective of these therapies. Hydromorphone appears to be an alternative opioid analgesic which may help relieve these symptoms. OBJECTIVES: To determine the analgesic efficacy of hydromorphone in relieving cancer pain, as well as the incidence and severity of any adverse events.

Hydromorphone for cancer pain.

BACKGROUND: Cancer pain is an important and distressing symptom that tends to increase in frequency and intensity as the cancer advances. For people with advanced cancer, the prevalence of pain can be as high as 90%. It has been estimated that 30% to 50% of people with cancer categorise their pain as moderate to severe, with between 75% and 90% of people with cancer experiencing pain that they describe as having a major impact on their daily life. Epidemiological studies suggest that approximately 15% of people with cancer pain fail to experience acceptable pain relief with conventional manage

[Excessive sweating related to hydromorphone].

Diffuse and abundant sweating in a middle age patient evolving for several weeks should raise suspicion of malignant lymphoma and infectious or neuroendocrine disorders before considering a drug origin. We report a patient who presented with severe and invalidating excessive sweating related to hydromorphone therapy for vertebral pain. Amongst their many reported side-effects, excessive sweating disappearing with discontinuation of the drug have been reported with some opiates.

Hydromorphone for neuropathic pain in adults.

BACKGROUND: Opioid drugs, including hydromorphone, are commonly used to treat neuropathic pain, and are considered effective by some professionals. Most reviews have examined all opioids together. This review sought evidence specifically for hydromorphone, at any dose, and by any route of administration. Other opioids are considered in separate reviews. This review is part of an update of a previous review, Hydromorphone for acute and chronic pain that was withdrawn in 2013 because it needed updating and splitting to be more specific for different pain conditions. This review focuses only on ne

Hydromorphone for acute and chronic pain.

BACKGROUND: While morphine is the gold standard for the management of severe cancer pain, some patients either do not achieve adequate analgesia, or suffer intolerable morphine-related toxicity. For these patients alternatives such as hydromorphone are recommended. However, there appear to be gaps in our understanding of the efficacy and potency of hydromorphone. OBJECTIVES: This review explores and assesses the evidence for the efficacy of hydromorphone in the management of pain. SEARCH STRATEGY: Randomised trials which included hydromorphone were sought using electronic databases and by ha

Unintentional administration of intra-articular hydromorphone.

Opioid overdose is a very common cause of presentation to the emergency department (ED) in the United States every year, with an average of over 200 ED visits per 100,000 people in 2018. This case demonstrates a scenario with which few emergency physicians are familiar: an inadvertent postoperative administration of intra-articular hydromorphone leading to delayed presentation for altered mental status. In this case a patient erroneously received 10 mg intra-articular hydromorphone immediately after right knee hemiarthroplasty. She was difficult to rouse for 6 hours postoperatively and require

[Hydromorphone in Cancer Patients-A Retrospective Study].

We retrospectively investigated the use of oral hydromorphone for cancer pain. Nineteen patients treated for cancer pain with oral hydromorphone were reviewed in this study. Cancers had occurred in the gastrointestinal (n=4), lung(n=3), breast(n=2), bone and soft tissue(n=2), hematological(n=2), and others(n=6). The administered opioids before switching to hydromorphone were morphine, oxycodone, and tapentadol. The mean oral morphine equivalent daily dose (OMEDD)was 89.3 mg. The average dose of hydromorphone administered was 16.4 mg/day, and average NRS 10(numerical rating scale: 0-10)scores o

A case report of sweating caused by hydromorphone.

Thermoregulatory control of shivering and sweating is a complex process that can be affected by centrally acting medications. Opioids, in particular fentanyl and methadone, have been associated with sweating, but it remains a relatively rare occurrence in clinical practice. Under-recognition of this medication side-effect may lead to patient discomfort as well as a potentially unnecessary work-up to determine the etiology of the sweating. Here, we discuss severe sweating caused by hydromorphone that resolved upon the medication's discontinuation.

A myoclonic reaction with low-dose hydromorphone.

OBJECTIVE: To report a case of myoclonus associated with the use of a low dose of the opioid analgesic hydromorphone. CASE SUMMARY: A 55-year-old white man with a history of nonobstructive hypertrophic cardiomyopathy was hospitalized for treatment of severe chest pain. On hospital day 1, intravenous hydromorphone (6 doses, total dose of 4 mg) was administered. The pain continued and, on day 2, the dose was increased, with a total of 6 mg administered on day 2. The patient developed uncontrollable jerking movements of the head, neck, arms, and legs after he received the hydromorphone. The myoc

Radioimmunoassay of hydromorphone and hydrocodone in human plasma.

A radioimmunoassay for hydromorphone in human plasma was developed using a commercially available morphine-6-antiserum and tritiated dihydromorphine. In the assay, free and bound drug are separated using dextran-coated charcoal. The method quantitates hydromorphone in the 2.5-20-ng/ml range with minimum sensitivity at 1.0 ng/ml. Within-run precision for hydromorphone as shown by the standard error of the estimate of the linear regression equation is +/- 1.01 ng/ml. Between-run precision data for hydromorphone at the 2,5-, 10-, and 20-ng/ml levels gave percent relative standard deviations of 22

Analgosedation: The Use of Fentanyl Compared to Hydromorphone.

BACKGROUND: The 2018 Society of Critical Care Medicine guidelines on the "Prevention and Management of Pain, Agitation/Sedation, Delirium, Immobility, and Sleep Disruption in Adult Patients in the ICU" advocate for protocol-based analgosedation practices. There are limited data available to guide which analgesic to use. This study compares outcomes in patients who received continuous infusions of fentanyl or hydromorphone as sedative agents in the intensive care setting. METHODS: This retrospective cohort study evaluated patients admitted into the medical intensive care unit, the surgical int

Intradialytic clearance of opioids: methadone versus hydromorphone.

Opioids are commonly prescribed to patients with chronic pain associated with end-stage renal disease requiring hemodialysis. The stability of opioid analgesia during dialysis may vary among different opioids. No studies to date have corroborated this clinical observation by directly comparing plasma concentrations of different opioids during dialysis. We compared changes in peridialysis plasma concentrations of 2 pharmacokinetically distinct opioids, methadone and hydromorphone (HM). Fourteen dialysis patients with chronic pain received either methadone or HM for at least 2 weeks before begin
