Hypodermoclysis versus intravenous hydration in palliative medicine: clinical effectiveness.

OBJECTIVES: Palliative care patients struggle to cope with many symptoms. Malnutrition and dehydration may develop either as a consequence of the disease itself or as a result of its treatment. Parenteral methods, such as the intravenous method, are advanced treatment options, but sometimes patients experience problems related to venous depletion. Although hypodermoclysis (HDC) has been known for a long time, it is often overlooked in clinical practice. The aim of this study was to analyse the clinical differences between the HDC method and the intravenous hydration method. METHODS: This pros

A Prospective Study of Hypodermoclysis Performed by Caregivers in the Home Setting.

CONTEXT: Decreased oral intake is very common at the end of life. Dehydration can aggravate symptoms, such as fatigue, myoclonus, and confusion. Intravenous hydration at home can be logistically difficult and expensive. Hypodermoclysis is easy to provide and inexpensive; however, it is rarely used to provide hydration at the end of life in the home setting. OBJECTIVES: The purpose of this study was to determine if caregivers were capable of administering hypodermoclysis in the home hospice setting. METHODS: All caregivers underwent a 45-minute training session on hypodermoclysis administrati

Development and Validation of an Educational Tool on Hypodermoclysis for Palliative Care Professionals.

Background/Objectives: Hypodermoclysis has gained increasing recognition as a safe, effective, and minimally invasive method for administering medication and fluids in palliative care. Despite its advantages, its adoption remains limited, primarily due to a lack of structured training resources for healthcare professionals. This study aimed to develop and validate an educational tool for training clinical nurses in hypodermoclysis administration in palliative care. Methods: This is a methodological study with a multi-methods approach. Study development involved a needs assessment with 48 profe

Hypodermoclysis (subcutaneous infusion) effective mode of treatment of dehydration in long-term care patients.

Dehydration is associated with morbidity and mortality in frail elderly patients. Intravenous fluid administration in these patients is sometimes hard, especially in agitated patients. The purpose of this study was to review of 57 long-termed care patients received hypodermoclysis infusion on 118 different occasions during the 9 months. Data collection included: demographic data, functional and mental status, indication hypodermoclysis, duration of the infusion, type of solution, laboratory data, adverse effects, and outcome. The main reasons for initiation of the hypodermoclysis were either d

Development of an evidence based pediatric subcutaneous rehydration "hypodermoclysis" protocol for mildly dehydrated children with a history of difficult venous access.

BACKGROUND: Establishing peripheral intravenous access in children who present to a hospital setting with mild to moderate dehydration is often challenging. These children may not require hospitalization especially when unable to tolerate rehydration orally. An alternate access option for rehydration therapy is hypodermoclysis, commonly referred to as subcutaneous rehydration (SCR), where fluids can be infused and absorbed through the subcutaneous tissue. Unlike achieving peripheral vascular access, subcutaneous needle insertion is not complex and results in minimal venous attempts and discomf

Approach to subcutaneous infusions in primary care: Indications, practical considerations, acceptability, and tolerability.

OBJECTIVE: To provide updated and practical guidelines for the use of subcutaneous (SC) infusions, or hypodermoclysis, in primary care. SOURCES OF INFORMATION: A thorough literature review of the MEDLINE database was conducted for articles published from 1945 to 2025. Most retrieved articles provided level II or III evidence; well-designed randomized controlled trials were scarce. MAIN MESSAGE: Compared with the intravenous (IV) route, hypodermoclysis is less invasive, better tolerated, easier to implement in outpatient settings, and associated with a lower risk of serious complications. [Subcutaneous infusion: Indications, practical considerations, and tolerability].

Subcutaneous (SC) infusion, or hypodermoclysis, is a route of administration involving the injection of fluids or medications into the hypodermis. Historically used and subsequently abandoned due to complications arising from poor technique, it has regained interest since the 1990s, particularly in geriatrics and palliative care. Compared with the intravenous (IV) route, it is less invasive, better tolerated, easier to implement in outpatient settings, and carries a lower risk of serious complications. The pharmacokinetics of the SC route show slightly slower absorption than IV administration,

[Subcutaneous rehydration for efficient treatment of elderly people during heat-waves].

Geriatric patients were more often treated in hospitals for Dehydration in the last years resembling a rising burden for Health systems. Elderly people are more vulnerable for Dehydration during Heat Waves especially when there are complicating factors like febrile episodes or multiple medications. In private care situations as in homes for elderly Strategies have to be implemented in Germany to prevent hospital admissions in the case of transient Dehydration. By this a rise of costs and a waste of resources should be diminished. Prehospital structures have to be implemented to maintain oral R

Subcutaneous Infusion of Fluids for Hydration or Nutrition: A Review.

Subcutaneous infusion, or hypodermoclysis, is a technique whereby fluids are infused into the subcutaneous space via small-gauge needles that are typically inserted into the thighs, abdomen, back, or arms. In this review, we provide an overview of the technique, summarize findings from studies that have examined the use of subcutaneous infusion of fluids for hydration or nutrition, and describe the indications, advantages, and disadvantages of subcutaneous infusion. Taken together, the available evidence suggests that, when indicated, subcutaneous infusion can be effective for administering fl

Use of subcutaneous fluids in palliative care with children: a case study.

Quality of life is a major consideration in children's palliative care, particularly at the end of life. Optimal symptom management is crucial in maintaining quality of life, with the aim being to ensure the child is as comfortable as possible. Ensuring adequate hydration will often be part of symptom management but may be associated with several practical and ethical challenges. Subcutaneous fluid administration in children's palliative care is relatively uncommon, so there is a lack of evidence on the topic. This article demonstrates that it is feasible to use subcutaneous fluid therapy in t

Thalassemia Major and Priapism: A Case Report of an Adolescent.

Priapism is defined as a prolonged pathologic penile erection without sexual stimulation. In children, priapism secondary to sickle cell disease or hematological malignancy is a frequent condition. Appropriate treatment of priapism varies; the treatment is primarily etiological, conservative management. In the present report, we aimed to present a case of asplenic thalassemia major who developed priapism, improved with hydration and ibuprofen treatment. Clinicians should take into account that priapism can be encountered in patients with thalassemia major. To our knowledge this is the second p

Absorption rate of subcutaneously infused fluid in ill multimorbid older patients.

BACKGROUND: Subcutaneous (SC) hydration is a valuable method for treating dehydration in the very old patients. Data are absent on the absorption rate, and the availability of SC infused fluid in the circulation in this group of patients where SC hydration is particularly relevant. METHODS: We performed an explorative study on ill very old (range 78-84 years old) geriatric patients with comorbidities who received an SC infusion of 235 ml isotonic saline containing a technetium-99m pertechnetate tracer. The activity over the infusion site was measured using a gamma detector to assess the absor

Initial experiences with subcutaneous recombinant human hyaluronidase.

We report here our retrospective observations on the use of recombinant human hyaluronidase (rHuPH20) for the facilitation of subcutaneous hydration and drug infusion. Thirty-two patients were treated with rHuPH20 in a hospice setting over a 6-month period. Of these, 26 received this agent to enhance hypodermoclysis with standard hydration fluids for symptom control of delirium, myoclonus and mild to moderate dehydration. Flow rates up to 500 mL/hr were attained without difficulty. Electrolyte replacement in hydration fluid was achieved without incident in 5 patients receiving potassium and in
