Hypothermia.

Significant hypothermia is an increasing clinical problem that requires a rapid response with properly trained personnel and techniques. Although the clinical presentation may be such that the victim appears dead, aggressive management may allow successful resuscitation in many instances. Initial management should include CPR if the victim is not breathing or is pulseless. Further core heat loss should be prevented by removing wet garments, insulating the victim, and ventilating with warm humidified air/oxygen to help stabilize core temperature. Core temperature and cardiac rhythm should be mo

Hypothermia.

Experimental studies show that, following hypoxic ischaemic injury, mild induced hypothermia-a reduction of body temperature by about 3 degrees C -- preserves cerebral energy metabolism, reduces cerebral tissue injury and improves neurological function. Randomized trials in full-term and near-full-term newborns suggest that treatment with mild hypothermia is safe and improves survival without disabilities up to 18 months of age. Although the optimal time of initiation, the depth and duration, and the method of cooling are uncertain, in the absence of specific treatments many clinicians will wi

Hypothermia.

Accidental hypothermia is defined as an unintentional decline in the core temperature below 35 degrees C. The population of patients at risk is very heterogeneous. Common thermal stressors include both primary exposures and secondary contributory diseases or injuries. As the core temperature progressively declines, the compensatory metabolic, adrenergic, and cardiovascular responses that attempt to maintain thermal homeostasis fail. At this juncture, therapeutic intervention must occur. An understanding of the pathophysiological variables impacting rewarming is critical. For example, the effec

[Severe hypothermia].

Severe hypothermia is a rare phenomenon, the incidence of which is increasing due in part to an ageing population at risk of falls. Its management is complex, given the hemodynamic impairment and consciousness disorders that are often concomitantly present. A systematic approach is key to improving outcome. Management focuses on rewarming, which can be associated with adverse effects depending on the patient'sunderlying condition and the degree and duration of hypothermia. This article reviews the management of severe hypothermia.

Environmental Hypothermia.

Although a rare diagnosis in the Emergency Department, hypothermia affects patients in all environments, from urban to mountainous settings. Classic signs of death cannot be interpreted in the hypothermic patient, thus resulting in the mantra, "No one is dead until they're warm and dead." This comprehensive review of environmental hypothermia covers the clinical significance and pathophysiology of hypothermia, pearls and pitfalls in the prehospital management of hypothermia (including temperature measurement techniques and advanced cardiac life support deviations), necessary Emergency Departme

[Therapeutic hypothermia].

The International Liaison Committee on Resuscitation (ILCOR) recommended therapeutic hypothermia treatment as follows. Comatose adult patients with spontaneous circulation after out-of-hospital VF cardiac arrest should be cooled to 32 to 34 degrees C for 12 to 24 hours. Induced hypothermia might also benefit comatose adult patients with spontaneous circulation after out-of-hospital cardiac arrest from a nonshockable rhythm, or cardiac arrest in hospital. Therapeutic hypothermia may be beneficial for adolescents who remain comatose following resuscitation from sudden witnessed out-of-hospital V

[Therapeutic hypothermia].

The use of therapeutic hypothermia has been shown to improve survival and neurological outcome following cardiac arrest. Patients with traumatic brain injury or ischemic stroke also responded positively to therapeutic hypothermia, which may be induced by various procedures including surface cooling, endovascular cooling catheter and cold infusion. Possible side effects include infection and hemorrhage, as well as changes in water and electrolyte levels. It is the aim of this article to provide an overview of studies to date, as well as practical guidance for the application of therapeutic hypo

[Accidental hypothermia].

Accidental hypothermia in the elderly is a rather unknown clinical syndrome in Germany. Manifestations are those of a generalized disease without striking symptoms or signs. To assure the clinical diagnosis, a low graded thermometer is necessary. Treatment consists of slow and passive rewarming and of correction of the accompanying disturbances. As important as correct diagnosis and treatment are preventive efforts against exogenous and endogenous risk factors.

Accidental hypothermia.

Accidental hypothermia was studied in 11 patients, five of whom died. This condition occurs spontaneously, usually in a cold environment, and should be suspected in the comatose hypotensive patient. It is easily missed because regular clinical thermometers do not record extremely low body temperatures. It is distinguished from other forms of hypothermia by the very low body temperature and by the absence of recognized causes of hypothermia. Ten of the 11 patients were either alcoholic or diabetic. Intravascular thrombosis was the most common complication leading to death.

Accidental hypothermia.

Accidental hypothermia is an unintentional drop in core body temperature below 35 °C. It can occur at any time of year and in any climate, and can affect all age groups. The epidemiology of accidental hypothermia reflects the interaction between biological susceptibility, social conditions and exposure to environmental factors. As core temperature falls and thermoregulation mechanisms become insufficient, the metabolism slows, consciousness deteriorates and the hypothermic myocardium becomes increasingly prone to arrhythmias and cardiac arrest. The prognosis is variable, and treatment outcomes

Therapeutic hypothermia.

Pioneer works on therapeutic hypothermia (TH) half a century ago already showed promising results but clinical application was limited by a lack of understanding of the underlying pathophysiology, lack of reliable method for temperature control and lack of intensive care facilities to deal with possible complications. More recently, 2 studies in 2002 supported the application of moderate TH (32.0-34.0℃) in post-cardiac arrest patients. Although the studies included only patients suffering from out-of-hospital VF, many ICUs world-wide are applying the therapy to all post-cardiac arrest patients

Therapeutic hypothermia.

Therapeutic hypothermia has been used for millennia, but in recent years was not in much clinical use due to an apparent high risk of complications. More recently, the benefits of induced therapeutic hypothermia have been rediscovered, mainly with the improvement in neurological outcome in out-of-hospital cardiac arrest victims. In addition, therapeutic hypothermia has been suggested to improve outcome in other neurological conditions such as traumatic brain injury, neonatal asphyxia, cerebrovascular accidents and intracranial hypertension. This article reviews the history of the discovery of

Accidental hypothermia.

Knowledge of the effects of hypothermia has increased greatly over the past 25 yr. Thousands of patients have been cooled intentionally in the operating room, and hundreds of thousands of living hearts have been temporarily stopped by cold cardioplegia and restarted without difficulty or apparent ill-effect. Yet in spite of the acquisition of this vast body of clinical experience an aura of mystery stills surrounds the patient who becomes hypothermic accidentally. The best treatment in any particular case is not always clear, and published accounts do not always give the impression that the hy

[Management of hypothermia -- Severe Accidental Hypothermia Centre in Krakow].

Severe accidental hypothermia is a condition associated with significant morbidity and mortality. In the years 2009–2012 the Polish National Statistics Department reported 1836 deaths due to exposure to excessive natural cold. The Severe Accidental Hypothermia Centre (CLHG, Centrum Leczenia Hipotermii Glebokiej) was set up in Krakow in 2013. It is a unit functioning within the structure of the Cardiac Surgery Clinic, established in order to improve the effectiveness of the treatment of patients in the advanced stages of severe hypothermia. Early identification of hypothermia, binding algorithm

[Pathophysiology in accidental hypothermia. Hypothermia is not only a cold body].

Knowledge of the pathophysiology in accidental hypothermia is essential for clinical decision-making. The prognosis should be favourable provided the condition is recognized and treated accordingly. Progressive organ dysfunction is associated with a declining core temperature which is reversible on rewarming. Other reactions occur during rewarming of which rewarming collapse has received much attention. More detailed knowledge of rewarming collapse which has come out is examined in this paper.

Hypothermia in Trauma.

Hypothermia in trauma patients is a common condition. It is aggravated by traumatic hemorrhage, which leads to hypovolemic shock. This hypovolemic shock results in a lethal triad of hypothermia, coagulopathy, and acidosis, leading to ongoing bleeding. Additionally, hypothermia in trauma patients can deepen through environmental exposure on the scene or during transport and medical procedures such as infusions and airway management. This vicious circle has a detrimental effect on the outcome of major trauma patients. This narrative review describes the main factors to consider in the co-existin
