Image-guided breast biopsy has become an attractive alternative to wire-localized excisional biopsy to evaluate women with nonpalpable abnormalities detected by breast imaging. We have organized a database from our institution that includes over 3,500 procedures. We have reviewed our institutional results and the literature pertaining to image-guided breast biopsy. Discussed in this review are the indications and contraindications for image-guided biopsy, common techniques employed, accuracy based on pathology, reimbursement issues, and the multidisciplinary approach used at our institution. PURPOSE OF REVIEW: The most efficient and cost-effective approach to pleural exudates not diagnosed by means of thoracocentesis remains uncertain. Both closed pleural biopsy and thoracoscopy may be utilized for the acquisition of pleural tissue. This review will focus on the developments in image guidance of closed pleural biopsy.
RECENT FINDINGS: Recent studies suggest that computed tomography and ultrasound guidance improve the yield and safety of closed pleural biopsy. Imaging is best suited to reduce the rate of false-negative biopsy in malignant pleural disease by enhanced targeting of l

Widespread application of mammography and breast ultrasound has led to improved recognition and delineation of breast lesions. Localization of these lesions with specialized, computerized stereotactic radiographic equipment or ultrasound imaging with subsequent use of this image to guide percutaneous core sampling of the abnormal area is less invasive, less painful, highly accurate, and less expensive than incisional breast biopsy with preoperative needle localization. These image-guided techniques have progressively matured in recent years, are used regularly by more and more surgeons, and ar

Transthoracic image guided biopsy is now firmly established in the armamentarium of diagnostic tests to establish the nature of a mass in the lung, pleura, mediastinum or chest wall. It may be performed under computed tomographic, ultrasonic or fluoroscopic control, and provided a fine needle technique is employed, morbidity is low. Worldwide reports show a 90% true positive yield and a false positive rate of less than 1%.

OBJECTIVE: A variety of benign and neoplastic lesions can affect the synovium, including pigmented villonodular synovitis (PVNS) and synovial chondromatosis. Prior to surgical resection, accurate characterization of synovial lesions is necessary for appropriate treatment planning. Additionally, recent advances in potential medical therapies for PVNS could decrease or eliminate the need for surgery in some cases. Such treatment options demand accurate characterization of synovial lesions prior to treatment.
METHODS AND MATERIALS: Institutional IRB approval was obtained. We identified 54 synovi

Image-guided biopsy of the lung or of the mediastinum is a minimally invasive method for acquiring material for microbiologic or histologic examinations. Given careful selection and preparation of the patient, image-guided biopsy is a safe procedure with a relatively low risk of complications. The aim of this tutorial is to summarize the special aspects concerning the intervention, including evaluation, the technical needs, different kinds of access to the target, and risk and complication management.

BACKGROUND AND OBJECTIVES: Image-guided navigation is an effective intra-operative technology in select surgical sub-specialties. Laparoscopic and open lymph node biopsy are frequently undertaken to obtain adequate tissue of difficult lesions. Image-guided navigation may positively augment the precision and success of surgical lymph node biopsies.
METHODS: In this prospective pilot study, pre-operative imaging was uploaded into the navigation platform software, which superimposed the imaging and the subject's real-time anatomy. This required anatomical landmarks on the subject's body to be sp

We report a case of littoral cell angioma (LCA) diagnosed by percutaneous core needle biopsy during the workup of a patient with multiple splenic lesions. Splenectomy was not performed. The patient has remained asymptomatic during 4 years of follow-up. Our findings raise interesting questions about the feasibility of core needle biopsy for the diagnosis of LCA. Clinicians should be aware of LCA as an unusual benign entity in the differential diagnosis of multiple splenic lesions.

During the last 30 years, the techniques of percutaneous biopsy have evolved dramatically and its indications have expanded accordingly. Additionally, recent treatments in oncology have made it necessary to use larger cutting needles in biopsies for tissue analysis. These developments force radiologists to perform biopsies in areas they are not very familiar with. However, lesions in such organs and regions are traditionally considered to be dangerous because of the risk of bleeding and trauma to the critical structures along the needle tract. As a result, radiologists are generally reluctant

Image-guided biopsy of the synovium is a relatively uncommon but safe procedure with a high-diagnostic yield in the correct clinical scenario. Whilst surgical and arthroscopic techniques are still commonly performed and remain the gold standard, they are more invasive, expensive and not widely available. Ultrasound and X-ray-guided synovial biopsy are being increasingly performed by radiologists to diagnose both native and periprosthetic joint infection (PJI) to guide surgical and microbiological management. The purpose of this review article is to present the historical background to synovial

Image-guided core-needle breast biopsy (IGCNBB) is widely used to evaluate patients with abnormal mammograms; however, information is limited regarding the reliability of a benign diagnosis. The goal of this study was to demonstrate that a benign diagnosis obtained by IGCNBB is accurate and amenable to mammographic surveillance. Records of all patients evaluated by IGCNBB from July 1993 through July 1996 were reviewed. Biopsies were classified as malignant, atypical, or benign. All benign cases were followed by surveillance mammography beginning 6 months after IGCNBB. Of the 1110 patients eval

In a series of 44 image guided stereotactic biopsy from August 1995 until March 1997, findings were as follows (frequency order). Tumors, glioblastoma was the most frequent. Primary lymphoma and other conditions associated to AIDS. Metastasis, three cases, Vasculites, two cases, Arachnoid cyst, Creutzfeldt-Jakob, cortical degeneration, inespecific calcification (one case each). The age varied from 1 to 83 years. Forty one lesions were supratentorial, two infratentorial, and one was outside the brain (dura and skull) and we used stereotaxy to localize it. There was no mortality and morbidity wa

Imaging guided percutaneous core needle biopsy of the breast is an accurate and cost-effective method for the diagnosis of indeterminate breast lesions. It is also useful for the preoperative confirmation of suspected carcinoma. Depending on the location of the lesion in the breast, its imaging characteristics, and the configuration of the breast, biopsy using sonographic or stereotactic guidance may be preferable. However, for many lesions either technique can be used. A variety of biopsy probes are available. For many lesions the ability to make a diagnosis does not appear to depend on the p

Image-guided percutaneous needle biopsy has become the preferred diagnostic modality for bone and soft tissue tumors. However, to the authors' knowledge, the levels of patient anxiety, pain, and satisfaction before and after the procedure have not been studied. Sixty-five patients undergoing image-guided needle biopsy of a possible bone or soft tissue tumor were prospectively surveyed to quantify preprocedure and postprocedure levels of anxiety and pain and to determine demographic and clinical correlates of anxiety, pain, and satisfaction. Anxiety was measured with the Spielberger State-Trait

Image-guided lung needle biopsy allows for minimally invasive diagnosis of lung pathology. In the setting of suspected malignancy, the biopsy not only confirms the diagnosis but also allows for molecular profiling, a requisite for tailored systemic therapy. Needle biopsy can also characterize non-neoplastic entities such as infections not responding to treatment and other inflammatory processes. A successful and safe lung needle biopsy starts with lesion and patient selection and careful pre-procedural evaluation. Here we review the indications and contraindications, diagnostic alternatives, a

OBJECTIVE: To examine the histologic correlation between image-guided large-core needle biopsy (LCNB) and excisional biopsy of nonpalpable breast lesions.
DESIGN: Histologic findings of LCNB and excision specimens were reviewed for patients who underwent image-guided LCNB of nonpalpable breast lesions from April 1, 1993, to March 31, 1996. Histologic diagnosis of the excision specimen was used as a criterion standard. Follow-up ranged from 1 to 4 years.
SETTINGS: A dedicated breast imaging clinic on the campus of an urban community hospital.
PATIENTS: Five hundred fifty-two patients with no

INTRODUCTION: Studies on the use of image-guided percutaneous biopsy for hepatoblastoma (HB), and recommendations put forth by the pediatric hepatic international tumor trial (PHITT), are limited. It is unknown if sufficient tissue can be obtained for trial enrollment as well as molecular profiling, which will likely play a key role in informing future treatment strategies.
METHODS AND MATERIALS: Patients with HB who underwent percutaneous biopsy at initial diagnosis in interventional radiology (IR) over a 12-year period at a single center were included. Patient demographics, pretreatment ext
