The role of immune system exhaustion on cancer cell escape and anti-tumor immune induction after irradiation.

Immune surveillance seems to represent an effective tumor suppressor mechanism. However, some cancer cells survive and become variants, being poorly immunogenic and able to enter a steady-state phase. These cells become functionally dormant or remain hidden clinically throughout. Neoplastic cells seem to be able to instruct immune cells to undergo changes promoting malignancy. Radiotherapy may act as a trigger of the immune response. After radiotherapy a sequence of reactions occurs, starting in the damage of oncogenic cells by multiple mechanisms, leading to the immune system positive feedbac

Influence of Hesperidin on Systemic Immunity of Rats Following an Intensive Training and Exhausting Exercise.

Intensive training and exhausting exercise can disrupt innate and acquired immunity. The flavanone hesperidin has shown immunomodulatory properties in physiological and some pathological conditions, and positive effects on exercise-induced oxidative stress. Nevertheless, it remains uncertain whether it also prevents exhausting exercise-induced immune alterations. The aim of this study was to establish the effect of oral hesperidin supplementation on the systemic immune system in rats following an intensive training and exhausting exercise. For this purpose, female Wistar rats were randomized i

Serum sPD-1 and sTIM-3 as T-cell exhaustion biomarkers in systemic lupus erythematosus: The IL-6-associated exhaustion axis and the SLE Immune Exhaustion Index.

BACKGROUND: T-cell exhaustion marked by soluble PD-1 (sPD-1) and soluble TIM-3 (sTIM-3) has been implicated in systemic lupus erythematosus (SLE), yet the cytokine mechanisms driving checkpoint-receptor upregulation, the relationship with complement consumption, and the clinical utility of these markers remain incompletely characterised. METHODS: In a case-control with cross-sectional study of 120 participants (40 active SLE, 40 quiescent SLE, 40 healthy controls), serum sPD-1, sTIM-3, IL-6, ferritin, complement proteins (C3, C4, C1q), anti-dsDNA, CRP, ESR, and urinary protein-to-creatinine r

Viral load-driven systemic immune exhaustion is an enabler of antibody breadth in HIV infection.

A broadly neutralizing response to a human immunodeficiency virus (HIV) vaccine is a major goal of the field, yet the determinants of antibody breadth remain poorly defined. Antibody responses to HIV evolve over years of infection, developing unusual features such as high mutation rates in the subset of individuals that acquire breadth. Using single-cell RNA sequencing, single-cell proteomics, and plasma neutralization assays in a longitudinal Kenyan cohort of treatment-naive women living with HIV, we identify systems-level immune correlates of antibody breadth. Broad and narrow neutralizers d

Systemic immune classification related to immune exhaustion evaluates the clinical response of patients with HBV-HCC after transarterial chemoembolization.

Transarterial chemoembolization (TACE) has the potential to activate the immune system and regulate the tumor microenvironment. This study assesses the clinical response of patients with HBV-related hepatocellular carcinoma (HBV-HCC) after TACE treatment based on systemic immune classification (SIC). A total of 80 patients with HBV-HCC were assessed for the peripheral blood immune exhaustion phenotype and immune proteins through a combination of "Olink High Sensitivity Plasma Proteomics" and "Multicolor Flow Cytometry." An unsupervised clustering algorithm was employed to classify various immu

Impact of endotoxin exposure after exhausting exercise on the immune system in solid organ transplant recipients.

Subsequent to prolonged exhausting exercise a transient immunosuppression is often observed in athletes. This so-called "open window" results in a reduced resistance of the athletes to viral and bacterial infections after an exhaustive exercise bout. Concerning the effect of bacterial endotoxin contact after exhausting exercise in transplant recipients, who are innately immunosuppressed by their medication, no data exists at present. After performing 81 km cycling, including ascending more than 1800 m in altitude, peripheral blood from 10 male kidney transplant recipients and from 10 healthy c

Torque teno viruses exhaust and imprint the human immune system via the HLA-E/NKG2A axis.

The ubiquitous Torque teno virus (TTV) establishes a chronically persistent infection in the human host. TTV has not been associated with any apparent disease, but, as part of the human virome, it may confer a regulatory imprint on the human immune system with as yet unclear consequences. However, so far, only few studies have characterized the TTV-specific immune responses or the overall immunological imprints by TTV. Here, we reveal that TTV infection leads to a highly exhausted TTV-specific CD8+ T-cell response, hallmarked by decreased IFN-γ production and the expression of the inhibitory N

Exhaustion of the immune system by Group A Streptococcus necrotizing fasciitis: the occurrence of late secondary infections in a retrospective study.

BACKGROUND: Necrotizing fasciitis is a potentially lethal condition for which early and adequate treatment with surgical debridement and broad-spectrum intravenous antibiotics are essential for survival. It is hypothesized that Group A Streptococcus (GAS) necrotizing fasciitis causes exhaustion of the immune system, making these patients more susceptible for late secondary infections. METHODS: A retrospective study was conducted of all patients with necrotizing fasciitis between 2002 and 2016. Patients with necrotizing fasciitis based on macroscopic findings, positive Gram staining, culture o

Macroscopic fractal dynamics characterize the "physical-metabolic" dual barriers and systemic immune exhaustion associated with primary resistance to immunotherapy in liver metastases.

BACKGROUND: Liver metastases are associated with systemic immune tolerance and primary resistance to immune checkpoint inhibitors (ICIs) by establishing complex physical and metabolic barriers within the tumor immune microenvironment (TIME). We developed a non-invasive macroscopic fractal dynamics framework to map these microenvironmental barriers across scales, aiming to predict ICI efficacy in colorectal cancer liver metastases (CRLM) and lung squamous cell carcinoma (SCC). METHODS: This single-center, retrospective, proof-of-concept cohort study consecutively enrolled 472 patients with CRL

Mitigating non-genetic resistance to checkpoint inhibition based on multiple states of immune exhaustion.

Despite the revolutionary impact of immune checkpoint inhibition on cancer therapy, the lack of response in a subset of patients, as well as the emergence of resistance, remain significant challenges. Here we explore the theoretical consequences of the existence of multiple states of immune cell exhaustion on response to checkpoint inhibition therapy. In particular, we consider the emerging understanding that T cells can exist in various states: fully functioning cytotoxic cells, reversibly exhausted cells with minimal cytotoxicity, and terminally exhausted cells. We hypothesize that inflammat

Effects of immune exhaustion and senescence of innate immunity in autoimmune disorders.

Innate immune system activation is crucial in the inflammatory response, but uncontrolled activation can lead to autoimmune diseases. Cellular exhaustion and senescence are two processes that contribute to innate immune tolerance breakdown. Exhausted immune cells are unable to respond adequately to specific antigens or stimuli, while senescent cells have impaired DNA replication and metabolic changes. These processes can impair immune system function and disrupt homeostasis, leading to the emergence of autoimmunity. However, the influence of innate immune exhaustion and senescence on autoimmun

BACKGROUND: The mechanisms underlying refractory Mycoplasma pneumoniae (M. pneumoniae) pneumonia (RMPP) and its association with adaptive immune dysfunction remain incompletely defined. This study investigated clinical features and peripheral lymphocyte profiles in children with RMPP versus those with common M. pneumoniae pneumonia (CMPP) to identify immune disturbances that may serve as early predictive indicators. METHODS: Of 622 children diagnosed with M. pneumoniae pneumonia between January 2019 and January 2024, 139 with single infection were enrolled: 72 RMPP and 67 CMPP. Clinical data

Cancer and HIV-1 Infection: Patterns of Chronic Antigen Exposure.

The main role of the human immune system is to eliminate cells presenting foreign antigens and abnormal patterns, while maintaining self-tolerance. However, when facing highly variable pathogens or antigens very similar to self-antigens, this system can fail in completely eliminating the anomalies, leading to the establishment of chronic pathologies. Prototypical examples of immune system defeat are cancer and Human Immunodeficiency Virus-1 (HIV-1) infection. In both conditions, the immune system is persistently exposed to antigens leading to systemic inflammation, lack of generation of long-t

The multifaceted landscape of T cell exhaustion in organ transplantation: From molecular mechanisms (epigenetics, transcription, metabolism) to induction strategies.

T cell exhaustion is a state of T cell dysfunction resulting from persistent antigenic stimulation, characterized primarily by the high expression of inhibitory receptors, metabolic reprogramming, and epigenetic remodeling. T cell exhaustion is closely associated with immune responses in chronic infections, tumor escape, and organ transplantation. In transplantation immunology, T cell exhaustion plays a dual role: moderate exhaustion can promote immune tolerance and reduce graft rejection, while excessive exhaustion may weaken the defensive capabilities of the immune system, increasing the ris

Multiscale modeling of T cell exhaustion: A mathematical framework integrating continuous dynamics with spatial heterogeneity.

Continuous antigen exposure drives T cells into a progressive state of dysfunction known as exhaustion, enabling tumors to evade immune surveillance and promoting disease progression. Despite its importance, predictive modeling of T cell exhaustion remains a major challenge due to the complexity of its regulatory dynamics. To address this challenge, we developed a mathematical framework that characterizes the dynamic regulation of T cell exhaustion and its impact on tumor-immune interactions. Here, we integrate multi-source data, population dynamics modeling, and agent-based modeling to track

B-cell exhaustion in HIV infection: the role of immune activation.

PURPOSE OF REVIEW: To discuss a component of the pathogenic mechanisms of HIV infection in the context of phenotypic and functional alterations in B cells that are due to persistent viral replication leading to aberrant immune activation and cellular exhaustion. We explore how B-cell exhaustion arises during persistent viremia and how it compares with T-cell exhaustion and similar B-cell alterations in other diseases. RECENT FINDINGS: HIV-associated B-cell exhaustion was first described in 2008, soon after the demonstration of persistent virus-induced T-cell exhaustion, as well as the identif

The cost of persistent alarm: temporal transcriptional reprogramming of macrophages from acute activation to chronic immune suppression by day 11.

INTRODUCTION: Chronic inflammation has long been associated with cancer initiation, yet the mechanisms linking sustained immune activation to an immune-permissive tumor microenvironment remain incompletely defined. Prevailing explanations such as immune exhaustion (IEX) or free radical mediated tissue damage, fail to account for the active state of immune tolerance, a process driven by potent negative feedback loops that systematically suppress host effector responses. METHODS: To address this gap, we developed an in vitro model of macrophage tolerance driven by sustained Toll-like receptor 4

Insights into Immune Exhaustion in Chronic Hepatitis B: A Review of Checkpoint Receptor Expression.

Hepatitis B, caused by the hepatitis B virus (HBV), often progresses to chronic infection, leading to severe complications, such as cirrhosis, liver failure, and hepatocellular carcinoma. Chronic HBV infection is characterized by a complex interplay between the virus and the host immune system, resulting in immune cell exhaustion, a phenomenon commonly observed in chronic viral infections and cancer. This state of exhaustion involves elevated levels of inhibitory molecules, cells, and cell surface receptors, as opposed to stimulatory counterparts. This review aims to elucidate the expression p

Mechanisms and strategies for reversing NK cell exhaustion in tumor immunotherapy.

Natural Killer (NK) cells play a crucial role in the body's innate immune system, distinguished by their ability to target a wide array of tumors through diverse killing strategies. Following the intense focus on T cells, NK cells have emerged as a key player in immunotherapy. Recent advancements in NK cell-based cancer treatments, including adoptive cell therapy, immune checkpoint inhibition, recombinant cytokine therapies, and nanomedicines, have demonstrated promising outcomes both in vitro and in vivo. As research into NK cell-based therapies intensifies, evidence is mounting that NK cells

Monocyte exhaustion associated with systemic lupus erythematosus.

Monocytes are major contributors to systemic lupus erythematosus (SLE) pathogenesis, modulating B- and T-cell autoreactivity through inflammatory cytokine secretion and disrupted immune clearance. Under prolonged inflammatory pressure, monocytes develop a pathogenic "exhausted" immune memory state defined by paradoxical proinflammatory and immunosuppressive gene expression and diminished immune effector functions. To determine whether chronic inflammation in SLE elicits a similar monocyte exhaustion phenotype, we analyzed bone marrow and splenic reservoir monocytes from lupus-prone MRL and MRL
