The immunocompromised patient, with or without superimposed granulocytopenia, provides a wide range of life-threatening challenges for the primary medical and nursing care management group. In the context of haematological malignancy, particularly leukaemia and bone marrow transplantation, special expertise needs to be developed which includes competence in many aspects of general medical, nursing, and intensive care techniques. Clearly, the range of essential knowledge extends to other disciplines including respiratory, cardiac, and renal medicine, with important contributions from microbiolo

Sporotrichosis is a global implantation or subcutaneous mycosis caused by several members of the genus Sporothrix, a thermo-dimorphic fungus. This disease may also depict an endemic profile, especially in tropical to subtropical zones around the world. Interestingly, sporotrichosis is an anthropozoonotic disease that may be transmitted to humans by plants or by animals, especially cats. It may be associated with rather isolated or clustered cases but also with outbreaks in different periods and geographic regions. Usually, sporotrichosis affects immunocompetent hosts, presenting a chronic to s

This article deals with the management of the immunocompromised host. Mechanisms of immunocompromise include alterations in skin and mucosal barriers, normal oral and intestinal flora, splenic function, and number or function of T cells, B cells, granulocytes, and monocytes. Discussed in this article are ways for maintaining those defenses not altered by the primary disease, minimizing the environmental risks to the patient, anticipating potential infections in order to institute appropriate prophylactic measures, and diagnosing and aggressively treating infections as they occur.

The immunocompromised host represents a growing and heterogeneous population within the intensive care unit. Advances in oncologic therapies, organ transplantation, and immunomodulatory treatments for autoimmune diseases have expanded survival but altered host immune architecture. Sepsis in this population is not merely sepsis with an added risk factor; rather, immune changes may fundamentally modify host-pathogen interactions, inflammatory phenotypes, diagnostic accuracy, and therapeutic response. This article focuses on sepsis in patients with solid and hematologic malignancies, organ transp

PURPOSE OF REVIEW: Novel treatment modalities for previously fatal diseases, including newer chemotherapeutic and biologic agents and the expansion of the indications for solid organ and stem cell transplantation, have resulted in prolonged patient survival and a significant increase in the population of immunocompromised hosts (ICHs).
RECENT FINDINGS: This review discusses the increasing spectrum of opportunistic infections in the ICH, the general approach for early diagnosis and treatment of pulmonary infections in this population, and the current and novel diagnostic modalities available t

Vaccinations are safe and effective in immunocompromised patients. Apparently most vaccines in this patient population are underutilized. General vaccination recommendations are expressed for influenza, diphtheria and tetanus. Pneumococcal, meningococcal und Haemophilus influenzae B immunizations are specially indicated for patients with or developing B-cell-deficiency. Live attenuated vaccines are usually contraindicated. The efficacy of the immunization and its indication can be additionally measured by antibody response, which is usually decreased compared to healthy subjects. Immunocomprom

Congenital and acquired states of immunodeficiency are associated with an increased incidence of ill-defined lymphoproliferations. These are mainly B-cell, often extranodal, lymphoproliferations commonly associated with EBV. Their incidence is increasing with the rapid development of organ transplantation and spreading of the HIV infection. The association with EBV partly explains the pathogeny of these affections. This ubiquitous virus immortalizes B lymphocytes in vitro and is tumorigenic for new world primates. An EBV-specific cytotoxic T-cell memory prevents uncontrolled proliferation of i

Adenoviruses are among the many pathogens and opportunistic agents that cause serious infection in the congenitally immunocompromised, in patients undergoing immunosuppressive treatment for organ and tissue transplants and for cancers, and in human immunodeficiency virus-infected patients. Adenovirus infections in these patients tend to become disseminated and severe, and the serotypes involved are clustered according to the age of the patient and the nature of the immunosuppression. Over 300 adenovirus infections in immunocompromised patients, with an overall case fatality rate of 48%, are re

We are presenting two typical case histories of mucormycosis in patients with acute leukaemia which illustrate some of the important features of this infection in compromised hosts. In such patients, mucormycosis is mostly pulmonary or widely disseminated and often has a rapidly fatal course. A microbiological diagnosis from blood or sputum is difficult to establish. Mucormycosis or other fungal infections should be suspected in compromised patients with infections which do not respond to adequate antibiotic therapy.

Immunocompromised individuals are at increased risk for opportunistic infections including pneumonia. Pneumonia has long been known to be a leading cause of mortality during induction chemotherapy for acute leukemia and was the first recognized presentation of human immunodeficiency virus (HIV). Even with adequate treatment, there is a wide breadth of postpneumonia sequelae, which is of particular interest in immunocompromised hosts given their increased risk for pneumonia. In this review, we describe the varying complications, presentations, and systems involved in the sequelae of immunocompr

PURPOSE OF REVIEW: To highlight recent advances in our understanding of Trypanosoma cruzi infection in immunocompromised individuals, a condition that is increasingly recognized as populations shift and use of immunosuppressive medications becomes more commonplace.
RECENT FINDINGS: Chagas disease screening programs should include people at risk for both Chagas disease and immunocompromise, e.g. people who have resided for ≥6 months in endemic Latin America who have an immunocompromising condition such as HIV or who are planned to start an immunosuppressive medication regimen. The goal of iden

A 40-year-old woman with a history of poorly controlled HIV presented to a district referral hospital in rural Botswana for a generalized skin rash of several months duration. The highly pruritic rash predominantly involved her hands and feet and was associated with bullae that were present for days at a time before rupturing without drainage or discharge. The patient endorsed night sweats, periodic fevers, occasional cough productive of blood-tinged sputum, fatigue, and weight loss. On admission, CD4 count was 46 cells/mm3 and viral load was >750000 copies/mL. Pulmonary tuberculosis testing v

Immunocompromised hosts are at high risk for bloodstream infections (BSIs); the outcome is unpredictable (even with adequate therapy), frequently severe (mortality 27%). A 26 months survey in a haematology/oncology hospital was set-up; it comprised 158 patients and detected 171 positive blood cultures. The origin of the infections was primitive in 27.22% and secondary in 72.78% of the cases; the most frequent cause was the presence of a vascular catheter (23.41%). The second most important cause were the respiratory infections (19.62%), followed by the urinary tract infections (UTIs) (11.39%).

PURPOSE OF REVIEW: Expanding access to antiretroviral treatment has dramatically improved the long-term prognosis of patients with HIV-associated cryptococcal disease who survive the acute infection. However, the incidence and acute mortality of HIV-associated cryptococcal meningitis remain high. In this context, this review summarizes urgently needed recent work aimed at improving the acute management of cryptococcal infection in immunocompromised hosts.
RECENT FINDINGS: Studies have started to optimize antifungal regimens and address the complications of raised cerebrospinal fluid pressure

Candida infections in the immunocompromised patients represent a major therapeutic challenge. In immunocompromised host candidiasis can range from asymptomatic oral thrush to life-threatening candidemia. Invasive Candida infections parallel increase in the immunocompromised patient population and are characterized by high mortality. Diagnosis of Candida infections is difficult and is based on clinical picture, microbiological and histological examinations. Mainstay of treatment are systemic antimycotic agents (fluconazole, amphotericin B, itraconazole, voriconazole and caspofungin). Empirical

The most important species of Cryptosporidium in humans, C. parvum and C. hominis, cause severe and chronic life-threatening gastroenteritis in immunocompromised patients. Despite a certain efficacy shown by passive immunotherapy or by some chemotherapeutic agents (e.g. paromomycin and nitazoxanide), no significant benefit has been demonstrated. The use of highly active antiretroviral therapy (HAART) in persons with the acquired immunodeficiency syndrome drastically reduces the prevalence of Cryptosporidium infection. This result seems to be due to aspartyl protease inhibitors of the human imm

The data from the most recent studies concerning infection with human herpes virus 8 strongly support a pathogenic role for this virus in the tumoral hyperplasia which is a feature of Kaposi's sarcoma. The results from the first studies of the sensitivity of human herpes virus 8 to antiviral drugs were also published last year. This review also concerns bacterial and mycobacterial infections showing unusual presentation, important trials in the prevention of infection with varicella zoster virus by using vaccination in children with leukemia, and a controlled therapeutic study in HIV-infected
