Nasal intermittent positive pressure ventilation in preterm infants: Equipment, evidence, and synchronization.

The use of nasal intermittent positive pressure ventilation (NIPPV) as respiratory support for preterm infants is well established. Evidence from randomized trials indicates that NIPPV is advantageous over continuous positive airway pressure (CPAP) as post-extubation support, albeit with varied outcomes between NIPPV techniques. Randomized data comparing NIPPV with CPAP as primary support, and for the treatment of apnea, are conflicting. Intrepretation of outcomes is limited by the multiple techniques and devices used to generate and deliver NIPPV. This review discusses the potential mechanism

Equipment and modifications that enabled infant child-care by a mother with C8 tetraplegia: a case report.

PURPOSE: In the past, rehabilitative effort after spinal cord injury has focused on maximizing physical functioning related to activities of daily living and self-care, with less emphasis on issues such as care-giving. The number of women becoming pregnant and rearing children after spinal cord injury has increased in recent years, but descriptions of equipment and modifications used in child-care are scarce. Equipment and techniques that enable physical independence in providing infant-care by persons with low-level tetraplegia has not previously been described. METHODS: This case report des

Screening for Cronobacter species in powdered and reconstituted infant formulas and from equipment used in formula preparation in maternity hospitals.

BACKGROUND/AIMS: Cronobacter spp. have been identified as being of considerable risk to neonates. The occurrence of organisms in infant formulas is therefore of considerable interest. METHODS: The occurrence of Cronobacter spp. in infant feeds (formulas and fortified cow's milk) was determined using most probable number (MPN) analysis, and from formula preparation utensils. Ninety-nine samples were analyzed, of which 42 were unopened cans of powdered infant formula (PIF), 25 reconstituted infant formulas in feeding bottles, 27 utensils used in the preparation of infant formula and 5 samples o

Comparison of Miller and Airtraq laryngoscopes for orotracheal intubation by physicians wearing CBRN protective equipment during infant resuscitation: a randomized crossover simulation study.

BACKGROUND: The purpose of this study was to evaluate the performance of orotracheal intubation with the Miller laryngoscope compared with the Airtraq laryngoscope by emergency and pediatric physicians wearing CBRN-PPE type III on infant manikins with conventional airway. We hypothesized that in this situation, the orotracheal intubation with the Airtraq laryngoscope would be faster and more effective than with the Miller laryngoscope. METHODS: This was a prospective, randomized, crossover, single-center study who recruited emergency department physicians on a voluntary basis. Each physician

Feasibility and early outcomes of intact cord resuscitation without special equipment in extremely preterm infants born at 23-25 weeks of gestation: A case series.

UNLABELLED: Deferred cord clamping (DCC) enhances placental transfusion, yet its feasibility in extremely preterm infants requiring resuscitation remains uncertain. Intact cord resuscitation (ICR) may allow simultaneous placental transfusion and stabilization. This study evaluated the feasibility and short-term outcomes of ICR in infants born at 230/7 to 256/7 weeks of gestation. This single-center case-series study compared two consecutive two-year periods before and after the introduction of ICR (September 2023) in extremely preterm infants requiring delivery room resuscitation. Infants born

Study protocol of "A preterm infant life-saving package including a simple and affordable incubator and a portable flow generator mask-CPAP in resource-limited settings" SAVE preterm trial, saving preterm infants by adopting vital equipment.

INTRODUCTION: Despite a significant global decline in the mortality rate for children under the age of 5, the neonatal mortality rate (NMR) has not significantly improved over the last thirty years. Globally, 250 million newborn babies die each year, and 98% of the neonatal deaths occur in middle- and low-income countries. The leading cause of death is preterm birth; and complications due to the prematurity of the infant include respiratory failure, hypothermia, infections, and dehydration. Treatment for these complications may require respiratory support devices, temperature management using

The development of a computer controlled system to simulate in rats, the rapid, frequent changes in oxygen experienced by preterm infants developing retinopathy of prematurity.

Preterm infants that develop severe ROP have significantly more fluctuations in their transcutaneous oxygen compared to mild or no ROP, despite the fact that all these infants are kept within clinically 'safe' limits. Current animal models do not accurately reflect this oxygen environment. Our aim was to custom build equipment capable of reproducing the transcutaneous oxygen (TcPO2) levels recorded by infant cotside monitoring equipment in a rat model and assess the equipment's precision. Using previously published data for the rat that translates TcPO2 into the equivalent inspired FiO2, a pro

Transmission of neonatal listeriosis in a delivery room.

Two cases of neonatal listeriosis occurred in a hospital within a two-week period. Both infants were infected with the same organism, Listeria monocytogenes, type 1a, bacteriophage type 6 (lysotype 1652). Both infants were born in the same delivery room, 17 hours apart. The index patient became septic shortly after birth and died after 48 hours despite antibiotic therapy. The mother of the index patient was febrile and had chorioamnionitis. The second infant became ill with meningitis at 13 days of age. Neither infants nor mothers were attended by the same medical or nursing staff nor were the

Time-Driven, Activity-Based Costing of Presurgical Infant Orthopedics: A Critical Component of Establishing Value of Latham Appliance and Nasoalveolar Molding.

BACKGROUND: Value-based health-care reform requires assessment of outcomes and costs of medical interventions. In cleft care, presurgical infant orthopedics is still being evaluated for clinical benefits and risks; however, the cost of these procedures has been largely ignored. This study uses robust accounting methods to quantify the cost of providing two types of presurgical infant orthopedics: Latham appliance treatment and nasoalveolar molding. METHODS: This is a prospective study of patients with nonsyndromic cleft lip and/or palate who underwent treatment with presurgical infant orthope

Airway emergency management in a pediatric hospital before and during the COVID-19 pandemic.

OBJECTIVE: Children's hospitals frequently care for infants with various life-threatening airway anomalies. Management of these infants can be challenging given unique airway anatomy and potential malformations. Airway emergency management must be immediate and precise, often demanding specialized equipment and/or expertise. We developed a Neonatal-Infant Emergency Airway Program to improve medical responses, communication, equipment usage and outcomes for all infants requiring emergent airway interventions in our neonatal and infant intensive care unit (NICU). PATIENTS AND METHODS: All patie

Nosocomial transmission of congenital tuberculosis in a neonatal intensive care unit.

Congenital tuberculosis is uncommon, and nosocomial transmission from a congenitally infected infant to another infant has not been reported in the English literature. We report an investigation of 2 infants with tuberculosis who were cared for in the same neonatal intensive care unit. Isolates from both infants were genetically indistinguishable. Transmission between the 2 infants was likely due to contaminated respiratory equipment.

Serratia marcescens and its role in Pink breast milk and upper GI bleeding in an infant: a Tanzanian case report.

BACKGROUND: Serratia marcescens, a gram-negative bacterium once considered non-pathogenic, is now recognized as a cause of hospital-acquired infections, particularly in immunocompromised patients with outbreaks reported in neonatal intensive care units. In newborns, it can result in severe illness, including sepsis and meningitis. It is well recognized by the production of pink pigment found on drains and other equipment, including breast pumps. CASE REPORT: We report a case of a two-month-old previously healthy infant with upper gastrointestinal bleeding, whose mother had pink-stained expres

Brainstem Auditory Evoked Potentials in infants aged 1 to 24 months during a hearing health care service.

OBJECTIVES: Assessing infants' hearing is of utmost importance, as hearing at this phase is required for the development of oral language. Through hearing, human beings are capable of developing communication. The Brainstem Auditory Evoked Potentials are an indispensable test to diagnose deafness in infants. This study aimed to analyze the results of the Brainstem Auditory Evoked Potentials in children with risk factors for hearing loss. METHODS: This cross-sectional study analyzed the Brainstem Auditory Evoked Potentials in 123 infants aged 1 to 24 months at a hearing health care service. Th

Nosocomial septicemia and meningitis in neonates.

The past decade brought with it both highly sophisticated neonatal intensive care with improved perinatal mortality rates, and increased risk for nosocomial septicemia and meningitis among survivors. Although most of these infections were caused by multiple antibiotic-resistant gram negative enteric bacteria, Staphylococcus aureus "outbreaks" appeared sporadically. Risk for nosocomial infection was related primarily to factors which enhance infant contact with these bacteria (crowding and high nurse to infant ratios, poor handwashing practices, contaminated life support equipment, antibiotic e

Gray scale ultrasonography in the evaluation of hydrocephalus and associated abnormalities in infants.

Improved gray scale ultrasonic equipment has made two-dimensional echoencephalography a useful diagnostic tool in the examination of the infant brain for fluid-containing structures such as dilated ventricles and cysts. Eighteen infants have been examined to exclude hydrocephalus and other abnormalities. Ten had significantly dilated lateral ventricles and three of these were shown to have associated intracerebral abnormalities. Another infant was demonstrated to have an arachnoid cyst or subdural hygroma with normal size ventricles. Ten of the 11 abnormalities were confirmed by some other neu

Double surface phototherapy on a fluid bed.

Jaundice is a common neonatal problem with a higher incidence in premature infants. Phototherapy is an established treatment modality, effective and safer than exchange transfusion. Double surface phototherapy is more effective. The present methods have drawbacks, viz limited availability, high cost of equipment and difficulties in nursing infants during phototherapy. A modification to the technique which overcomes these difficulties made with equipment available in most neonatal units is described with three case reports of hyperbilirubinaemia successfully treated using this method.

Resuscitation of the newborn.

Resuscitation of the newborn is not different from resuscitation of any other patient, but in newborns, resuscitation requires special skills and considerations because of the size of the patient. If properly done, it can save many lives. If done incorrectly, however, it can save many lives. If done incorrectly, however, it can result in death or permanent neurologic damage to the infant. Because it is difficult to predict the status of the infant before birth, any delivery room must have experienced personnel and adequate equipment necessary for the resuscitation of the newborn.

A rare case of necrotizing fasciitis after early infant male circumcision.

Even though circumcision is low-risk, complications may occur, necessitating careful management. Here, we describe a 5-month-and-28-day-old infant who developed necrotizing fasciitis following Plastibell circumcision. This report emphasizes that clinicians should use standard equipment and appropriate-sized rings to prevent these unusual complications following circumcision.
