The intersection of infant substance exposure, foster care placement & child welfare policy.

OBJECT: The author studied the specific issue of ventral tethering in cases of type II split cord malformation (SCM). He also discussed the issue of ventral tethering in patients with type I SCM in whom a bony septum and dural sleeve have been resected. METHODS: Eleven (21%) of 52 patients with type II SCM were found to have a ventral fibrous septum tethering the hemicords to the ventral dura. Four categories of ventral septa were noted: 1) pure ventral intradural septa (three patients); 2) complete dorsoventral intradural septa (three patients); 3) dorsoventral septa continuous with a dermal

Routine screening for sickle cell haemoglobinopathy by electrophoresis in an infant welfare clinic.

The experience of a Nigerian infant welfare clinic in offering sickle cell haemoglo-binopathy (SCH) screening by electrophoresis to its clinic population over a 12-year period is reported. Overall 10,115 children were screened. Homozygous haemoglobin S was detected in 3.1% and haemoglobin S + C in 1.1%. Carriers of the sickle cell gene (haemoglobin A + S) comprised 23.7% of the children screened. Gene frequencies for the A, S and C genes were 0.814, 0.155 and 0.031 respectively. The gene frequencies of the S and C genes were higher than those reported in infants and under-five children from ot

Child Welfare Involvement and Health Outcomes in Infants With Prenatal Substance Exposure.

IMPORTANCE: The child welfare system (CWS) is often contacted when a child is born with prenatal substance exposure, but there is limited evidence on the beneficial and iatrogenic effects of CWS involvement. OBJECTIVE: To examine whether involvement with the CWS at birth is associated with injuries and moderate- to high-complexity emergency department (ED) use in the first year of life among infants identified with prenatal substance exposure. DESIGN, SETTING, AND PARTICIPANTS: Retrospective cohort study using probabilistically linked administrative data of Pennsylvania statewide child welfa

Improving outcomes for babies and toddlers in child welfare: A model for infant mental health intervention and collaboration.

Children under three comprise a sizable and growing proportion of foster care placements. Very young children who enter the child welfare system experience disruptions of critical attachments that are essential to this formative stage of brain development, as well as other traumatic events, leaving them at great risk for lifelong impairments. To reverse these concerning outcomes, babies who have been removed from their homes require intensive, relationship-based interventions that promote secure attachment to a primary caregiver and holistic attention the child's developmental needs. Infants Investigated by the Child Welfare System: Exploring a Distinct Profile of Risks, Service Needs, and Referrals for Support in Ontario.

The science of early childhood development underscores that maltreatment and other adversities experienced during infancy heightens the risk for poor developmental and socio-emotional outcomes. Referrals to supportive services by the child welfare system are particularly critical during infancy given the rapidity of brain development and infants' sensitivity to their environment. The main objectives of the current study are to: (1) examine age-specific differences in clinical and case characteristics; (2) determine the factors associated with the service referral decision involving infants; an

Impacts of natural history and exhibit factors on carnivore welfare.

To improve the welfare of nonhuman animals under professional care, zoological institutions are continuously utilizing new methods to identify factors that lead to optimal welfare. Comparative methods have historically been used in the field of evolutionary biology but are increasingly being applied in the field of animal welfare. In the current study, data were obtained from direct behavioral observation and institutional records representing 80 individual animals from 34 different species of the order Carnivora. Data were examined to determine if a variety of natural history and animal manag

Whose Rights Count? Negotiating Practice, Policy, and Legal Dilemmas Regarding Infant-Parent Contact When Infants are in Out-of-Home Care.

This article takes a human rights perspective with a view to articulating the infant's perspective when the infant has been subjected to abuse, neglect, or both and is reliant on the state to ensure his or her health and well-being. When a young child is removed from parental care, important and often difficult decisions have to be made about subsequent contact between child and parent. We consider a number of dilemmas which may arise for practitioners when they are assisting child welfare decision makers in relation to contact, and acknowledge the limited empirical follow-up studies of the im

[An epidemiologic study on an outbreak of measles in a particular group of infants].

An outbreak of measles broke out and prevailed in the Beijing Children Welfare House from March 23rd to April 27th, 1992. The 18 victims accounted for 4.8% of the whole infant population in the Welfare House, 11 of them being babies of less than 8 months. One third of the attacked babies were in bad condition and were hospitalized in the observation room. The clinical manifestations indicated that severe symptoms, more complications but favourable prognosis were characteristic of the infant patients. Based on the epidemiologic study and serological test, it was found that the outbreak and spre

The Postpartum Specific Anxiety Scale: Confirmatory factor analyses and relationships with birth experience.

The Postpartum Specific Anxiety Scale [PSAS] was developed and validated as a research tool with a four-factor structure; with predictive validity corroborated in studies examining infant-feeding and maternal bonding outcomes. The PSAS has not been examined in relation to birth experiences. We aimed to confirm the PSAS four-factor structure and examine these domains of anxiety in relation to subjective and objective birth experiences. Postpartum mothers (≤ 12-months; N = 500) completed the PSAS alongside measures of subjective birth satisfaction and objective obstetric interventions/complicati

A decolonial perspective on Indigenous infant and early childhood mental health: Reclaiming Indigenous ways for the next seven generations.

Indigenous lifeways, perspectives, and ways of knowing in the field of infant and early childhood mental health are underrepresented, especially given the inequitable and unjust prevalence of removal and separation of American Indian and Alaska Native (AI/AN) children from their families and communities by the child welfare system in the United States. Strengthening the infant and early childhood mental health field requires uncovering and addressing the ways in which colonization has intentionally attempted to disrupt and destroy Indigenous family relationships, especially bonds with young ch

Interventions and programs that support the health and development of infants with prenatal substance exposure in foster care: a scoping review.

OBJECTIVE: The objective of this scoping review was to describe the characteristics of interventions and programs that support the health and development of infants in foster care who have prenatal substance exposure, their foster care providers, and birth families. INTRODUCTION: Infants in foster care may have experienced prenatal substance exposure, neglect, and maltreatment, as well as disruptions in their relationships with primary caregivers. Despite multiple vulnerabilities, they also have great capacity for overcoming early adversities. Enhanced foster care has been identified as a key

Economic inequality, working-class power, social capital, and cause-specific mortality in wealthy countries.

This study tests two propositions from Navarro's critique of the social capital literature: that social capital's importance has been exaggerated and that class-related political factors, absent from social epidemiology and public health, might be key determinants of population health. The authors estimate cross-sectional associations between economic inequality, working-class power, and social capital and life expectancy, self-rated health, low birth weight, and age- and cause-specific mortality in 16 wealthy countries. Of all the health outcomes, the five variables related to birth and infan
