Managing occupational risks for hepatitis C transmission in the health care setting.

Hepatitis C virus (HCV) infection is a significant contemporary health problem in the United States and elsewhere. Because it is primarily transmitted via blood, hepatitis C infection presents risks for both nosocomial transmission to patients and occupational spread to health care workers. Recent insights into the pathogenesis, immunopathogenesis, natural history, and treatment of infection caused by this unique flavivirus provide a rationale for the use of new strategies for managing occupational hepatitis C infections when they occur. This article reviews this developing information. Recent

Current issues and update on human immunodeficiency virus infection in the orthopaedic setting.

The risk of HIV transmission in the health care setting is a composite of risks, with exposure to blood being the most important source of infection and percutaneous injuries posing the greatest risk of transmission. The frequency of blood contact varies by occupation, procedure performed, and precautions used. Data suggest that PEP may be effective in reducing the risk of occupational HIV transmission. The risk for transmission of HIV from HCW to patients is very small.

Immunization status of residents in pediatrics at the Federal University of São Paulo, Brazil.

Vaccination of health care workers is an efficient way to reduce the risk of occupational infection and to prevent nosocomial transmission to vulnerable patients. Despite this, achieving high immunization rates among those professionals is a challenge. We assessed the immunization status of Residents in Pediatrics at the Federal University of São Paulo from June to December 2008. Their immunization records were checked and evaluated according to the Brazilian Immunization Schedule for health care workers. Considering all required vaccines, only 3.1% of the 64 Residents were up-to-date with the

[The safety of patients and staff in the operating room].

The risk of postoperative infection is determined by a number of factors including the underlying health of the patient, the type of surgery and the environment in which the procedure take place. The predominant infective organisms are Staphylococci. Coagulase-negative staphylococci are regarded as emerging pathogens, especially in clean surgery. The operating team is the major source of the contaminated bacteria. Bacteria dispersed from the skin is of greater importance than bacteria from the respiratory tract, so masks are not always necessary in the theatre during an operation except by tho

Modeling control strategies of respiratory pathogens.

Effectively controlling infectious diseases requires quantitative comparisons of quarantine, infection control precautions, case identification and isolation, and immunization interventions. We used contact network epidemiology to predict the effect of various control policies for a mildly contagious disease, such as severe acute respiratory syndrome, and a moderately contagious disease, such as smallpox. The success of an intervention depends on the transmissibility of the disease and the contact pattern between persons within a community. The model predicts that use of face masks and general

Risk of methicillin-resistant Staphylococcus aureus transmission in the dental healthcare setting: a narrative review.

OBJECTIVE: Information on the risk of methicillin-resistant Staphylococcus aureus (MRSA) infection transmission in dental healthcare settings was incomplete only few years ago; therefore, MRSA infection control guidelines were necessarily based on data extrapolated from other fields. Recently, publication of specific studies have made it possible to review such risk. METHODS: Studies of MRSA infection in dentistry were searched for using EMBASE, MEDLINE, and Google and were allocated into the following sections: (1) direct evidence: documented cases of MRSA transmission in dentistry; (2) indi

Transfer of pathogens to and from patients, healthcare providers, and medical devices during care activity-a systematic review and meta-analysis.

OBJECTIVE: The transfer of pathogens may spread antimicrobial resistance and lead to healthcare-acquired infections. We performed a systematic literature review to generate estimates of pathogen transfer in relation to healthcare provider (HCP) activities. METHODS: For this systematic review and meta-analysis, Medline/Ovid, EMBASE, and the Cochrane Library were searched for studies published before July 7, 2017. We reviewed the literature, examining transfer of pathogens associated with HCP activities. We included studies that (1) quantified transfer of pathogens from a defined origin to a de

[The problem of cross infection and risk of infection for the practitioner].

Epidemiologic data show that in Belgium, the number of HIV-infected persons increases with an average of 3 a day. This increase will pose a real problem to all health workers including the dentists. The chance that the general dental practitioner comes in contact with HIV-infected patients will increase in the years to come. Therefore, each dental practitioner should have a scheme in his office to prevent cross infection. The transmission of HIV is analogue to HBV: blood, blood products, injury, wound, use of non-sterile instruments and needles... Transmission of HIV by saliva has not been pro

[HIV infection in the operating room conditions].

Several cases of occupational HIV transmission during surgical procedures have been reported all over the world in the last few years. Orthopaedic surgeons are at particularly high risk for blood contamination and skin injuries. This article addresses the risk of HIV transmission in the surgical setting with a focus on specific risk factors: the prevalence of the infection in general population, the stage of the disease, virulence, inoculum size, the nature and frequency of blood contacts. It also shows how the last two factors depend on surgical specialty, duration and kind of the procedure,

Double gloving to reduce surgical cross-infection.

BACKGROUND: The invasive nature of surgery, with its increased exposure to blood, means that during surgery there is a high risk of transfer of pathogens. Pathogens can be transferred through contact between surgical patients and the surgical team, resulting in post-operative or blood borne infections in patients or blood borne infections in the surgical team. Both patients and the surgical team need to be protected from this risk. This risk can be reduced by implementing protective barriers such as wearing surgical gloves. Wearing two pairs of surgical gloves, triple gloves, glove liners or c

Guidelines for preventing the transmission of Mycobacterium tuberculosis in health-care settings, 2005.

In 1994, CDC published the Guidelines for Preventing the Transmission of Mycobacterium tuberculosis in HealthCare Facilities, 1994. The guidelines were issued in response to 1) a resurgence of tuberculosis (TB) disease that occurred in the United States in the mid-1980s and early 1990s, 2) the documentation of several high-profile health-care--associated (previously termed "nosocomial") outbreaks related to an increase in the prevalence of TB disease and human immunodeficiency virus (HIV) coinfection, 3) lapses in infection control practices, 4) delays in the diagnosis and treatment of persons

[Considerations on vaccinations for the dental staff].

In this article the available vaccinations in the Netherlands that might be of value to dentistry are discussed. This in view of protection of both the patient and the medical worker. Furthermore vaccines that are necessary but do not exist or have not been developed yet, are mentioned. Of the available vaccinations, the one against hepatitis B is of the utmost importance. This has been stressed in the Netherlands from all sides for a long time already; nonetheless not all dental workers have been vaccinated up until now. The vaccines against human immunodeficiency virus (HIV) and viral hepati

[The spread of a 'Dutch' methicillin-resistant Staphylococcus aureus strain].

Following isolation of a methicillin-resistant Staphylococcus aureus (MRSA) in a patient subjected to amputation below the knee because of a vasculopathy, further investigations were carried out. Nose, throat and wound cultures were taken from staff and patients who had had contact with the index patient. After taking inventory cultures and cleansing of the two wards involved, these wards were quarantined. The bacterial strain was characterized using microbiological standard methods. The MRSA was encountered in a total of nine patients and two nurses. An infection due to this MRSA was found in

[Nosocomial scabies outbreak].

A scabies outbreak is reported which appeared in the Inpatient Pneumology Department in a 500-bed hospital. The outbreak began after the admission of a 90-year old woman referred from a nursing home. A total of 27 secondary cases occurred among health care personnel (14) and patients (13); other 7 cases of likely scabies occurred among 198 contacts. Patients were treated with 5% permetrine cream, in three applications one week apart from each other. It was also applied as prophylaxis in non involved patients in a single application. A discussion follows on the medical and preventive recommenda

AIDS, emergency operations, and infection control.

Acquired immunodeficiency syndrome (AIDS) caused by the human immunodeficiency virus (HIV) may turn out to be the largest lethal epidemic of infection ever. The estimated global number of HIV-infected adults in 1993 was 13 million, with projections of up to 40 million by the year 2000. Human immunodeficiency virus infections and AIDS are relevant to surgeons with respect to the surgical management of AIDS patients in general, the treatment of the increasingly long list of surgical complications specific to AIDS patients in particular, and the risks of patient-to-surgeon and surgeon-to-patient
