Solitary fibrous tumour extending both supratentorially and infratentorially.

Solitary fibrous tumours (SFT) of the central nervous system are rare. Such lesions are mesenchymal neoplasms that resemble meningioma. To date, 73 cases of SFT have been reported in the literature, but there is no reported case of an SFT that extends into both the supratentorial and infratentorial spaces. A 76-year-old man presented with headache, dysarthria and ataxia of 2 months' duration. MRI revealed a right cerebellar tumour, extending superiorly to the occipital lobe. During surgery, a huge, solid and well-capsulated tumour was observed. The tentorium cerebelli was also damaged by the t

Glioneuronal Tumor With Features of Ganglioglioma and Neurocytoma Arising in the Fourth Ventricle: A Report of 2 Unusual Cases and a Review of Infratentorial Gangliogliomas.

Infratentorial glioneuronal neoplasms are overall quite rare and are more commonly low-grade with surgical excision usually being curative. Multiple distinct histologic entities have been described including rosette-forming glioneuronal tumor, papillary glioneuronal tumor, neurocytoma, dysplastic gangliocytoma of the cerebellum (Lhermitte-Duclos disease), cerebellar liponeurocytoma, and ganglioglioma. While each of these entities has distinct findings, in some instances a tumor may demonstrate overlapping histologic features with mixed components. Herein, we report 2 unusual adult cases of a f

Supratentorial and Infratentorial Approaches to Pineal Surgery: A Database Analysis.

Objectives  Neoplasms involving the pineal gland are rare. When they do occur, tumor resection is anatomically challenging and is traditionally addressed by either a supratentorial or an infratentorial approach. To date, no large, multicenter studies have been performed that systematically analyze outcomes comparing these two approaches. This study aimed to evaluate outcomes for patients undergoing pineal neoplasm resection, comparing supratentorial and infratentorial approaches. Design  Retrospective database review. Setting  Multi-institutional database. Participants  From 2005 to 2016, 60 p

Congenital supratentorial and infratentorial peripheral neurogenic tumor: a clinical, ultrastructural, and immunohistochemical study.

Congenital neoplasms of the central nervous system are extremely rare, although they have been well documented since the earliest reports from the mid-19th century. Medulloblastoma, other primitive neuroectodermal tumors, and various types of gliomas have comprised the majority of cases. This report describes a highly unusual infratentorial and supratentorial tumor presenting as a scalp mass in a neonate who experienced in utero distress. The supratentorial mass extended through a defect in the skull to the parietooccipital lobe, and the infratentorial aspect involved the 9th and 10th cranial

The imaging findings of infratentorial primitive neuroectodermal tumour: A case report.

Central primitive neuroectodermal tumour (cPNET), a rare malignant neoplasm of embryonal origin, often occurs in children younger than 15 years. This is the first case report of the imaging findings of an infratentorial cPNET to be reported in a patient. Here, is reported the case of a 6-year-old boy presenting with symptoms of diplopia for 14 days. Magnetic resonance imaging revealed a solid mass in the fourth ventricle. The postoperative pathological diagnosis was cPNET. To conclude, whenever a child is diagnosed to have an infratentorial solid tumour in the fourth ventricle, cPNET should al

[Primary intracranial tumors: analysis of a series of consecutive autopsies and biopsies].

A retrospective study of primary intracranial tumors found in 19.760 consecutive autopsies and 72.972 consecutive biopsies during a period of over 41 years was carried out. Two hundred and five neoplasms comprising 69,7% of the primary intracranial tumors were located in the supratentorial compartment whereas 84 tumors (28,6%) were infratentorial in site. Gliomas constituted the largest group of primary tumors comprising 60,9% of the intracranial primary neoplasms. Most of gliomas were found in the cerebral hemispheres, particularly in the frontal lobes. Of the total number of gliomas 40,2% we

Volumetric stereotactic surgical resection of intra-axial brain mass lesions.

From August 1984 to August 1987 at the Mayo Clinic, 226 computer-assisted stereotactic resections based on computed tomography or magnetic resonance imaging (or both) were performed on 203 supratentorial and 23 infratentorial lesions in various deep-seated or essential brain locations. Histologic examination revealed 112 glial neoplasms, 70 nonglial tumors, and 44 nonneoplastic lesions. The overall morbidity was 9.3% (21 of 226 patients were worse after the procedure), and the 30-day operative mortality was 1% (2 patients). The procedure provides maximal cytoreduction in high-grade glial neopl

RETROSPECTIVE COMPARISON OF THREE-DIMENSIONAL CONFORMAL RADIATION THERAPY VS. PREDNISOLONE ALONE IN 30 CASES OF CANINE INFRATENTORIAL BRAIN TUMORS.

Infratentorial tumors are relatively infrequent in dogs and a lack of data makes it difficult to offer prognostic information. Untreated, dogs with these neoplasms have shorter survival times than those with supratentorial tumors. The role of radiation therapy (RT) in the management of infratentorial tumors is poorly defined and tumoral/peritumoral swelling in this site is a potential cause of serious acute side effects. The aim of this retrospective, cohort study was to describe cases of infratentorial tumors treated with fractionated three-dimensional conformal RT (3D CRT) and glucocorticoid

Mature posterior fossa teratoma mimicking infratentorial meningioma: a case report.

Intracranial teratomas are congenital neoplasms mostly diagnosed in the pediatric hood and usually involve supratentorial midline structures. These teratomas, especially those involving the posterior fossa are an uncommon and representing less than 0.5% of all intracranial tumors. We report a case of mature posterior fossa teratoma in an adult patient diagnosed in the 4th decade of life. This lesion was taken for a huge infratentorial meningioma.

Surgical Approaches to Pineal Region Tumors.

Pineal region tumors represent a diverse group of neoplasms arising from the pineal gland and its surrounding structures, posing unique diagnostic and surgical challenges due to their deep midline location and proximity to critical venous and neural anatomy. Advances in neuroimaging, microsurgical technique, and endoscopic technology have transformed their management, substantially improving outcomes compared to historical experiences of high morbidity and mortality. This chapter reviews the anatomical, diagnostic, and surgical principles essential to the contemporary management of pineal regi

Infratentorial choroid plexus tumors in children.

OBJECTIVE: Choroid plexus tumors (CPTs) are rare pediatric intracranial neoplasms, and mostly occur in the lateral ventricle. CPTs located in the infratentorial location are considered to be rare in the pediatric population. We present a series of eight patients treated in the last decade at our institution focusing on clinical presentations and their outcome after excision. METHODS: We performed an institutional retrospective review of patients who underwent surgical resection of infratentorial CPTs during the period from 2008 to 2017. Patients' charts were reviewed for demographic data, cli

Unusual co-occurrence of hypertrophic inferior olivary degeneration with infratentorial cavernomatosis and orbital cavernous hemangioma.

Hypertrophic olivary degeneration (HOD) is a rare condition resulting from a lesion in the Guillain-Mollaret triangle (GMT), causing transsynaptic degeneration and hypertrophy of the inferior olivary nucleus (ION). The GMT is composed of the dentate nucleus, red nucleus, and ION, and is commonly affected by ischemic and hemorrhagic strokes, vascular malformations, neoplasms, or surgical trauma. Cavernomas are a frequent type of cerebral vascular malformation associated with HOD, while orbital cavernous hemangiomas are another rare vascular malformation. The association of these two malformatio

Tumor-related hydrocephalus in infants: a narrative review.

Primary brain tumors diagnosed during infancy present several challenges for the pediatric neurosurgeon including the management of tumor-related hydrocephalus. Infant tumor-related hydrocephalus (iTRH) is present in over 70% of new diagnoses overall and in over 90% of infants with infratentorial neoplasms. iTRH may be related to direct obstruction of cerebrospinal fluid (CSF) pathways, shedding of protein and/or cells leading to dysfunctional subarachnoid, tumor-related CSF production, or, most frequently, a combination of these mechanisms. Treatment plans must consider the acuity of presenta

[Masquerade syndrome in a patient with a primary central nervous syndrome lymphoma].

We report the case of a 37-year-old, white male with a primary central nervous system lymphoma with multiple supra and infratentorial locations. The patient developed manifestations of intraocular inflammation secondary to the intracranial neoplasm (masquerade syndrome) and lymphocytopenia--with a low CD4 cell count--representing an immunodeficiency state which etiology was undiagnosed. The diagnosis of lymphoma was established by vitreous cytology. The patient died 10 months after the beginning of the symptoms.

Combined supra/infratentorial approach to tentorial meningiomas.

In spite of significant advancements in imaging technology, monitoring, and microsurgical techniques, complete and safe removal of tentorial meningiomas remains a challenge for most neurosurgeons. Classifications of tentorial meningiomas are revised. The combined supra/infratentorial approach to resect tentorial meningioma is discussed. This approach provides a wider exposure of the supra/infratentorial region with less brain retraction. With this approach the occipital lobe and the cerebellum are exposed along the tentorium. Two illustrative cases are presented. The patients were studied with

Intra-fourth-ventricular choroid plexus papilloma miming ependymoma.

Choroid plexus papilloma (CPP), a low incidence central nervous system (CNS) tumor, typically develops as an intraventricular neoplasm arising from the epithelium of the choroid plexus. Infratentorial CPP is predominantly clustered in adults with roughly 70% in the fourth ventricle, while supratentorial CPP commonly found in the lateral ventricles, is the most frequent location in children. The clinical and imaging features of CPP are not typical and may induce the misdiagnosis as other types of primary brain tumors. In this paper, we described a fourth-ventricular CPP that was misdiagnosed as

Chromosome arm 17p deletion analysis reveals molecular genetic heterogeneity in supratentorial and infratentorial primitive neuroectodermal tumors of the central nervous system.

The current World Health Organization (WHO) classification groups together both infratentorial neoplasms (medulloblastomas) and their supratentorial counterparts as primitive neuroectodermal tumors (PNETs), implying a common origin. Previous analyses of medulloblastoma have shown loss of chromosome arm 17p as the most frequent genetic abnormality: the molecular genetic constitution of supratentorial PNETS has not been systematically studied. We therefore examined 8 hemispheric PNETs and 35 medulloblastomas with 17p restriction fragment length polymorphism (RFLP) and microsatellite markers. We

Incidentally Discovered Cerebellar Ganglioglioma in an Adult Following a Motor Vehicle Accident: A Case Report and Literature Review.

Gangliogliomas (GGs) are rare central nervous system neoplasms composed of neuronal and glial cells. This report details a rare case of a 32-year-old male patient with an incidentally discovered cerebellar GG following a motor vehicle accident. The literature review highlights the variability in symptoms, age, and imaging findings, emphasizing the importance of considering GG in differential diagnoses for infratentorial tumors. The patient's treatment involved subtotal resection due to the risk of significant neurological deficits, with ongoing monitoring for recurrence. This case demonstrates
