Pros, cons, and costs of INFUSE in spinal surgery.

BACKGROUND: INFUSE (recombinant human bone morphogenetic protein-2 [rh-BMP-2]; Medtronic, Memphis, TN, USA) is approved by the Federal Drug Administration (FDA) only for use with the lumbar tapered fusion device (LT Cage; Medtronic) to perform single-level anterior lumbar interbody fusions (ALIF: L2-S1 levels). INFUSE, however, is widely utilized in an "off-label" capacity for anterior and/or posterior cervical, thoracic, and lumbar surgery. Nevertheless, Medicare and other insurance companies, are now increasingly denying reimbursement (average cost of a "large" INFUSE to the hospital without

Propofol infusion for sedation during spinal anesthesia.

PURPOSE: The dose and time course of propofol infusion required to induce rapid sedation without oversedation during spinal anesthesia were investigated. METHODS: Forty patients scheduled for spinal and epidural anesthesia were studied. After premedication with intramuscular midazolam 0.04 mg.kg(-1), an epidural catheter was inserted, followed by spinal anersthesia at L4-L5 with 0.5% hyperbaric tetracaine with epinephrine. The infusion of propofol was started with 10 mg.kg(-1).h(-1) and was decreased to 5 mg.kg(-1).h(-1) at spontaneous eye closure. According to the increase or decrease of the

Spinal cord stimulation versus spinal infusion for low back and leg pain.

The relative roles of spinal cord stimulation and the spinal infusion of opioids in the treatment of chronic, non-cancer lower body pain remains unclear. This report contains a retrospective analysis of patients with chronic lower body, neuropathic pain and treated over a 5 year period. Unilateral leg and/or buttock pain was treated initially with spinal stimulation and bilateral leg or mainly low back pain was treated initially with spinal infusions. 26 patients received spinal stimulation. Pain relief was > or = 50% in 16 (62%) with increased activity levels. Stimulator coverage was most dif

Potency of infused spinal antinociceptive agents is inversely related to magnitude of tolerance after continuous infusion.

By employing a constant-rate (1 microliter/hr) constant-dose intrathecal infusion pump in rats, groups received a 7-day infusion of one of three concentrations (in nmol/hr) of each of the following receptor preferring ligands: morphine (mu: 2, 6, 20), sufentanil (mu: 0.06, 0.2, 0.6), DAMGO (mu: 0.1, 0.3, 1.0), DADLE (delta: 2, 6, 20) or ST-91 (alpha-2: 3, 10, 30). On day 7 the magnitude of tolerance was assessed by establishing i.t. dose-response curves for the effect of the chronic drug given as a bolus. Each infused animal was used for one bolus injection. It was observed that for all drugs,

Morphine distribution in the spinal cord after chronic infusion in pigs.

BACKGROUND: Continuous intrathecal drug delivery provides new options for chronic delivery of drugs that target the spinal cord, but therapeutic efficacy is highly variable. Using an acute porcine model, we have previously demonstrated that continuous intrathecal drug delivery efficacy may be highly variable because of severely limited drug distribution in the cerebrospinal fluid and spinal cord. We designed this study to determine whether the limited drug distribution observed in our acute studies occurs with chronic administration as well. METHODS: Four farm-bred pigs were implanted with in

Concentrations of propofol in cerebral spinal fluid: target-controlled infusion.

BACKGROUND: Although the performance of target-controlled infusion (TCI) have been studied extensively, the accuracy and safety of a TCI system that targets the effect site remains to be demonstrated. This study was to investigate the relations of TCI of propofol to its concentrations in cerebral spinal fluid (CSF), the effect-site concentrations and bispectral index (BIS). METHODS: Twelve mongrel dogs were used for investigations. The target effect-site concentration was set at 3 microg/ml and the infusion was lasted for 15 minutes. CSF and blood samples were then collected and propofol conc

[A case of spinal infarction related to hepatic arterial infusion chemotherapy].

A 65-year-old male underwent iliocecal excision and hepatic posterior segmentectomy for cecum cancer and synchronous liver hepatic metastasis in September and October 2001, respectively. A reservoir was implanted by the GDA-coil method from the right femoral artery in November, and WHF (5-FU 1,000 mg/m2) was administered 8 times. Because of the remnant liver recurrence, WHF was restarted in April 2002. Left leg paralysis appeared suddenly after the 3rd administration. Heparin and urokinase were administrated continuously after hospitalization. Also, liver function tests showed a worsening cond

Toxicology of baclofen continuously infused into the spinal intrathecal space of the dog.

The continuous spinal administration of baclofen has been shown to have therapeutic benefit in the management of spasticity in humans with neuraxial injuries. The present study systematically investigated the potential spinal neurotoxicity of continuous intrathecally-infused baclofen in dogs. Male beagle dogs were prepared with chronic lumbar intrathecal catheters connected to subcutaneously implanted infusion pumps. Three groups of dogs received 28 days of infusion of saline (vehicle: 1 ml/24 hrs; N = 10), 200 micrograms/ml/24 hrs baclofen (N = 10) or 2000 micrograms/ml/24 hrs baclofen (N = 1

Effect of continuous spinal remifentanil infusion on behaviour and spinal glutamate release evoked by subcutaneous formalin in the rat.

Injection of formalin into the hind paw of the rat evokes a biphasic nociceptive behavioural response, which is considered to be an animal model of postoperative pain in humans. The initial response (phase 1) is caused by activation of peripheral nociceptors and is followed by a second phase attributed to ongoing activity in primary afferents and increased sensitivity of dorsal horn neurones. The latter effect is thought to result from glutamate-mediated N-methyl-D-aspartate receptor activation. In studies to date it has been difficult to discriminate mechanisms underlying phase 1 and phase 2

Concurrent spinal infusion of MK801 blocks spinal tolerance and dependence induced by chronic intrathecal morphine in the rat.

BACKGROUND: MK801, an N-methyl-D-aspartate receptor antagonist, has recently been reported to attenuate tolerance to, and withdrawal from morphine. This study analyzes tolerance and withdrawal in a chronic intrathecal coinfusion model of morphine and MK801. METHODS: Intrathecal catheters, attached to 7-day miniosmotic infusion pumps, were implanted in rats and infused with saline, 20 nM/h morphine, MK801 (10 and 3 nM/h) + morphine; and 10 nM/h MK801. Analgesia was measured on the hot plate daily. On the day 7, groups received 3 mg/kg intraperitoneal naloxone and six signs of withdrawal were a

Infusion of neostigmine-glycopyrrolate for bowel evacuation in persons with spinal cord injury.

Defecatory complications are common after spinal cord injury (SCI) and have been attributed, in part, to an imbalance of the autonomic nervous system between parasympathetic and sympathetic effects on the colon. Because parasympathetic (i.e., cholinergic) input to the bowel may be downregulated after SCI, it was hypothesized that neostigmine, a medication that increases cholinergic tone by blocking the metabolism of acetylcholine, might promote bowel evacuation in these persons. Since neostigmine is known to cause bradycardia and bronchoconstriction, we also assessed whether these side-effects

Spinal Contusion Injury-Induced Ejaculatory Dysfunction Is Reduced by Intrathecal Infusions of Gastrin Releasing Peptide in Male Rats.

PURPOSE: Spinal cord injury (SCI) has devastating effects on urogenital functions, including severe deficits in ejaculation. SCI treatment development is hindered by limited understanding of effects of SCI on the spinal ejaculation generator (SEG). The SEG consists of a population of lumbar spinothalamic (LSt) cells that control ejaculation. LSt cells control ejaculation via release of neuropeptides, notably gastrin releasing peptide (GRP). In control male rats, intrathecal infusions of GRP strongly facilitate ejaculatory reflexes, while GRP antagonists prevent ejaculation triggered by stimula

Low-Dose Ketamine Infusions for Highly Opioid-Tolerant Adults Following Spinal Surgery: A Retrospective Before-and-after Study.

Managing acute-on-chronic pain in opioid-tolerant individuals is complex and challenging; exploring new analgesia regimens for this population is essential. Ketamine is an N-methyl D-aspartate antagonist that blocks transmission of painful stimuli and could be a useful medication for this patient population. A new low-dose ketamine protocol as an adjunct to conventional pain therapy was implemented in a major urban Level 1 trauma center in Canada. A retrospective before-and-after chart review was conducted to explore the research question, "What is the effect of low-dose ketamine continuous in
