Cutaneous and subcutaneous complications of calcium infusions.

Five infants with hypocalcemia experienced complications after treatment with calcium gluconate intravenously. Inadvertent soft tissue extravasation resulted in erythema, subcutaneous calcification, tissue necrosis, skin slough, and transient radial nerve damage with wrist drop, the latter previously unreported. The soft tissue lesions may be mistaken for cellulitis, abscess, calcified hematoma, or osteomyelitis, resulting in unnecessary antibiotic therapy or surgical intervention. Initially, no clinical abnormality may be apparent. The lesions appear from days to weeks following extravasation

Subcutaneous ketamine analgesia: postoperative analgesia using subcutaneous infusions of ketamine and morphine.

A series consisting of 32 women undergoing total abdominal hysterectomy received a standard narcotic-free anaesthetic. For the first 24 h postoperatively, eight were given the standard regimen of intramuscular morphine sulphate whilst the other three groups received continuous subcutaneous infusions of either morphine sulphate, ketamine hydrochloride or the two drugs combined. The amount of time they were pain free, the incidence of sleep and nausea, together with cardiovascular and respiratory changes were recorded. All three subcutaneous regimens produced significantly more pain-free reading

Hyperkalaemia, ketoacidosis and subcutaneous or intravenous insulin infusions.

We report a case of a previously stable Type 1 insulin-dependent diabetic who developed diabetic ketoacidosis with marked hyperkalaemia after starting continuous subcutaneous insulin infusion for neuropathy. On two occasions during his hospital admissions he had a rapid onset of hyperkalaemia after short interruptions to this intravenous insulin infusion. Hyperkalaemia can occur rapidly in patients who are treated with short acting insulin only.

Furosemide in end-stage heart failure: community subcutaneous infusions.

BACKGROUND: In chronic heart failure many patients have recurrent hospital admissions and it is the leading cause of admission in people aged over 65 years. In those with end-stage heart failure, there is limited evidence that furosemide can be given subcutaneously to relieve symptoms and avoid hospital admission. METHOD: We initiated a community-based continuous subcutaneous infusion (CSCI) furosemide service for the treatment of advanced heart failure. We aimed to increase patient choice, offer an alternative to hospital admission and, in patients at the end of their life, allow them to die

Comparing subcutaneous fluid infusion with intravenous fluid infusion in children.

INTRODUCTION: This study examined subcutaneous (SC) infusion as a parenteral alternative for children with mild to moderate illness. The purpose was to compare the difference in infusion start time of parenteral fluid between an initial SC order and an initial intravenous (IV) order. In addition, the number of needlesticks a child received for each method was evaluated. This study also sought to address the following question: If SC fluids were given and an IV attempt was made later, did the administration of SC fluids enhance the success of venous cannulation? METHODS: A retrospective descri

Home therapy with subcutaneous immunoglobulin infusions in children with congenital immunodeficiencies.

OBJECTIVES: Patients with congenital, humoral immunodeficiencies are usually treated with intravenous immunoglobulin infusions. Subcutaneous infusions have emerged as an alternative treatment modality also in children. Our institution has run a subcutaneous infusion home therapy program for 6 years, and the purpose of this report is to describe our experience with this regimen. METHODS: The subcutaneous therapy of eight patients with immunodeficiency (three with agammaglobulinemia, one with common variable immunodeficiency, one with severe combined immunodeficiency and bone marrow transplanta

[Continuous subcutaneous deferoxamine infusions in thalassemia major. Improvement in glucose tolerance].

Exocrine and endocrine pancreatic functions were studied in 30 patients with homozygotic beta-thalassaemia. All were treated with continuous subcutaneous deferoxamine infusions for a mean period of 30 months. Three patients (aged 18-22 years) had insulin-dependent diabetes, two before and one shortly after the onset of deferoxamine administration. There was no improvement during the treatment. An abnormal glucose tolerance test was demonstrated in 14 patients (47%) before and in seven (23%) during deferoxamine infusion. Enzyme activity of alpha-amylase and lipase as an expression of exocrine p

Subcutaneous midazolam infusion in palliative care.

This article describes the use of a subcutaneous infusion of midazolam to control restlessness and agitation in 23 patients during the final stages of advanced cancer. Midazolam effectively controlled symptoms in 22 of the patients at an initial dose of 0.4-0.8 mg/hr rising to a mean maximum dose of 2.9 mg/hr. Midazolam mixed in the same syringe as diamorphine was well tolerated at injection sites. The wide dose range emphasizes the need for careful titration of dose for individual patients. At high doses, the volume of injection required presented some practical difficulties, but otherwise th

Hyaluronidase for localised infusion site reaction during continuous subcutaneous infusion.

Local skin site reactions may occur at continuous subcutaneous infusion (CSCI) sites. Factors that contribute these reactions include the concentration of medications, the pH of the solution, type and duration of cannula insertion and site selection. The case of a patient with advanced metastatic cholangiocarcinoma is described. Medications were delivered subcutaneously via a syringe driver and multiple infusion site reactions occurred. Hyaluronidase temporarily depolymerises hyaluronic acid in connective tissue and been has been shown to increase the dispersion and absorption of coadministere

Approach to subcutaneous infusions in primary care: Indications, practical considerations, acceptability, and tolerability.

OBJECTIVE: To provide updated and practical guidelines for the use of subcutaneous (SC) infusions, or hypodermoclysis, in primary care. SOURCES OF INFORMATION: A thorough literature review of the MEDLINE database was conducted for articles published from 1945 to 2025. Most retrieved articles provided level II or III evidence; well-designed randomized controlled trials were scarce. MAIN MESSAGE: Compared with the intravenous (IV) route, hypodermoclysis is less invasive, better tolerated, easier to implement in outpatient settings, and associated with a lower risk of serious complications. Bioavailability of gamma-globulin after subcutaneous infusions in patients with common variable immunodeficiency.

Replacement therapy, using subcutaneous infusions of gamma-globulin, is being applied increasingly for antibody-deficient patients, as this form of treatment has been found to be related to a very low frequency of adverse systemic reactions. However, the uptake of IgG from subcutaneous tissue may be low, owing to degradation locally, especially for the IgG3 molecule. Therefore, the kinetics of IgG and IgG-subclass concentrations in the sera of 23 patients with common variable immunodeficiency was investigated during 18 months of subcutaneous infusions of gamma-globulin (100 mg/kg/week). Comparison of continuous subcutaneous and intravenous hydromorphone infusions for management of cancer pain.

To compare the safety and efficacy of subcutaneous and intravenous infusion of opioid analgesics, a randomised, double-blind, crossover trial was carried out in inpatients. 15 patients with severe cancer pain received two 48 h infusions of hydromorphone--one subcutaneously and one intravenously in randomly allocated order. The study was made double-blind by the use of two infusion pumps throughout; during the active subcutaneous infusion the intravenous pump delivered saline and vice versa. Serial measurements of pain intensity, pain relief, mood, and sedation by means of visual analogue scale

[Subcutaneous infusion: Indications, practical considerations, and tolerability].

Subcutaneous (SC) infusion, or hypodermoclysis, is a route of administration involving the injection of fluids or medications into the hypodermis. Historically used and subsequently abandoned due to complications arising from poor technique, it has regained interest since the 1990s, particularly in geriatrics and palliative care. Compared with the intravenous (IV) route, it is less invasive, better tolerated, easier to implement in outpatient settings, and carries a lower risk of serious complications. The pharmacokinetics of the SC route show slightly slower absorption than IV administration,

Administration of heparin by subcutaneous infusion with a programmable pump.

Heparin has a short half-life (8 to 12 hours) and therefore must be administered by continuous infusion or by intermittent subcutaneous injection. Intermittent subcutaneous injection may lead to fluctuation in the levels of anticoagulation attained. In correcting this deficiency, the programmable automated subcutaneous infusion pump in conjunction with weekly home nursing visits has been used. Eight pregnant women with documented deep venous thrombosis or embolic events before pregnancy who received such therapy were studied. Eight similar subjects who received intermittent subcutaneous inject

Subcutaneous Infusion of Fluids for Hydration or Nutrition: A Review.

Subcutaneous infusion, or hypodermoclysis, is a technique whereby fluids are infused into the subcutaneous space via small-gauge needles that are typically inserted into the thighs, abdomen, back, or arms. In this review, we provide an overview of the technique, summarize findings from studies that have examined the use of subcutaneous infusion of fluids for hydration or nutrition, and describe the indications, advantages, and disadvantages of subcutaneous infusion. Taken together, the available evidence suggests that, when indicated, subcutaneous infusion can be effective for administering fl

Subcutaneous infusion ports in the pediatric patient with hemophilia.

Central venous access devices have become important tools in the management of pediatric patients with malnutrition, malignancy, and infections requiring long-term antibiotic treatment. Hemophilia presents a lifetime challenge for venous access and at times can be an urgent or life-threatening situation. Since 1986, the authors have implanted 11 subcutaneous infusion ports in nine patients with hemophilia. The systems have remained in place for up to 7 years, without major complications or problems. Two catheters were replaced, after 4 and 6 years, because of skin erosion and blockage, respect

Treatment of camptocormia with continuous subcutaneous infusions of apomorphine: 1-year prospective pilot study.

Camptocormia is defined as a marked dystonic flexion of the trunk in the sagittal plane. Camptocormia responds poorly to botulinum toxin injections, manipulation with dopaminergic treatment, and deep brain stimulation. We designed a prospective pilot study to assess the effect of apomorphine infusions on camptocormia. Five patients were enrolled. All five patients responded well to this treatment. The fact that camptocormia responds so well to apomorphine may be explained by the sustained stimulation of the ventrolateral striatal D1 receptors, alleviating this type of dystonia.
