[Breathlessness of the elderly and its managements].

It is certainly difficult to understand the clinical significance of dyspnea or breathlessness among the elderly, though this symptom is quite routine. Setting a conjugate of the dyspnea index as an indicator of ventilatory reserve ratio, its distribution was tested along an age abscissa upon the twenty senile dyspnea and demonstrated almost all of them below the 30% level. Their arterial blood oxygen tensions were grouped onto the severity rank of dyspnea, clarifying that some senile lived of no complaint by the blood oxygen tensions within the range of pulmonary insufficiency, despite there

Performance during constant workrate cycling exercise in women with COPD and hyperinflation.

We aimed to characterize gender differences in exercise endurance, operating lung volumes and symptoms limitation during exercise in patients with COPD. Ninety-three women and 93 men with COPD matched for age and disease severity were evaluated during symptom-limited constant-work rate cycle exercise at 75% of peak capacity. Breathing pattern, inspiratory capacity, dyspnoea and leg discomfort Borg scores were recorded during exercise. Endurance time was shorter in women compared to men. Inspiratory capacity decreased at a similar rate during exercise in women and men (0.71 vs. 0.81 ml x s(- 1)

From respiratory limitation to dynamic depletion of mechanical-ventilatory reserves: a paradigm shift to probe exertional dyspnea with clinical exercise testing.

Cardiopulmonary exercise testing (CPET) is frequently requested in the hope that detecting the maximal limits of cardiovascular or respiratory function will provide clinically relevant information on the genesis of exertional dyspnea. We provide a concise review of emerging evidence that analyzing whole test data, accounting for the dynamic (mis)match between requirements and capabilities (i.e., progressive reserve depletion), is more accurate and valuable for clinical decision-making than the traditional respiratory limitation paradigm. In this context, a pattern of excessive breathing emerge

Sex- and age-adjusted reference values for dynamic inspiratory constraints during incremental cycle ergometry.

Activity-related dyspnea in chronic lung disease is centrally related to dynamic (dyn) inspiratory constraints to tidal volume expansion. Lack of reference values for exertional inspiratory reserve (IR) has limited the yield of cardiopulmonary exercise testing in exposing the underpinnings of this disabling symptom. One hundred fifty apparently healthy subjects (82 males) aged 40-85 underwent incremental cycle ergometry. Based on exercise inspiratory capacity (ICdyn), we generated centile-based reference values for the following metrics of IR as a function of absolute ventilation: IRdyn1 ([1-(

Thermal effects on ventilation in cats: participation of carotid body chemoreceptors.

In pentobarbitone anesthetized cats, raising body temperature from 37 to 40 degrees C by external heat increased respiratory frequency, tidal volume, frequency of spontaneous gasps and mean inspiratory flow. It reduced end-tidal CO2 pressure, together with inspiratory and expiratory durations. After bilateral section of the carotid nerves, raising body temperature still induced hyperventilation, but the increase in gasp frequency was less pronounced and no significant change in tidal volume was observed. In comparison to steady ventilatory values in the intact condition, significant reductions

Effect of sacrum-backrest distance on respiratory function during wheelchair sitting: implications for seating assessment.

PURPOSE: Wheelchair seating configuration may influence respiratory mechanics by altering pelvic alignment, trunk posture, and diaphragmatic excursion. Deep sitting is generally recommended to maintain pelvic neutrality; however, the quantitative impact of sacrum-backrest distance on ventilatory performance remains unclear. This study aimed to investigate the effect of sacrum-backrest distance on respiratory function and thoracic expansion during wheelchair sitting. MATERIALS AND METHODS: Nineteen healthy young females were assessed under three wheelchair sitting conditions defined by sacrum-

Laminoplasty improves respiratory function in elderly patients with cervical spondylotic myelopathy.

Respiratory insufficiency after acute cervical trauma is well documented, but the relationship between respiratory function and chronic lesions, such as cervical spondylosis, has received scant attention. This clinical study investigated the effect of cervical spondylosis on respiratory function in 12 patients over 65 years of age who underwent expansive laminoplasty. Functional and neurological status were assessed using the Japanese Orthopaedic Association (JOA) scale and Neurosurgical Cervical Spine Scale (NCSS). To assess the effect of laminoplasty on respiratory function in patients with

Depression of respiratory function by pneumatic antishock trousers in traumatic quadriplegia.

A case of mild hypoventilation associated with an incompletely inflated pneumatic antishock suit in traumatic quadriplegia is presented. Pulmonary function studies were performed on the same patient 23 days later. Base line studies were performed and then repeated with the antishock suit inflated to 40 and then 100 mm Hg. Measurements were made of tidal volume (Vt), inspiratory reserve volume (IRV), expiratory reserve volume (ERV), forced vital capacity (FVC), forced vital capacity in one second (FEV1), and maximum voluntary ventilation (MVV). With increasing suit pressures, all parameters dec

Intratracheal pulmonary ventilation and congenital diaphragmatic hernia: a report of two cases.

Previous studies from our institution have shown that neonates with congenital diaphragmatic hernia (CDH), whose best postductal PaO2 (BPDPO2) was less than 100 mm Hg while on maximal conventional mechanical ventilation (CMV), had a mortality exceeding 90%. When combined with extracorporeal membrane oxygenation (ECMO), the mortality rose to 100% in those infants who developed hypercarbia following decannulation. Historically, those patients have required increasing ventilator support, leading to iatrogenic lung damage, and eventual death. Intratracheal pulmonary ventilation (ITPV) using the re

The effects of aminophylline on sleep and sleep-disordered breathing in patients with obstructive sleep apnea syndrome.

The methylxanthine derivatives are known to have respiratory stimulant properties. To determine whether these drugs would improve obstructive sleep apnea, 10 male patients with obstructive sleep apnea (OSA) (Apnea Index greater than 15/h) were given infusions of aminophylline and a saline placebo on 2 separate nights a week apart, using a randomized crossover design. There was a significant decrease during aminophylline infusion in the frequency of those apneas, which contained periods of complete respiratory inactivity (central and mixed apneas; placebo, 4.3 +/- 1.8/h; aminophylline, 0.7 +/-

A preliminary study on the feasibility of community game-based respiratory muscle training for individuals with high cervical spinal cord injury levels: a novel approach.

BACKGROUND: Respiratory disorders result in rehospitalization and premature death of patients with cervical spinal cord injuries (CSCI). Community game-based respiratory muscle training (RMT) programs could reduce secondary complications. METHODS: We examined the feasibility and preliminary efficacy of RMT as a community-based exercise program. Among the 10 included participants (eight male and two female), four, one, one, and four reported C3, C4, C5, and C6 complete injuries, respectively (eight graded by American Spinal Injury Association impairment scale [ASIA] A and two by ASIA B). Imposed expiratory resistance, dynamic hyperinflation and locomotor power and fatigue.

Expiratory flow limitation results in dynamic hyperinflation, dyspnoea and premature exercise intolerance. We aimed to measure whether expiratory resistance reduces locomotor power via limiting maximal voluntary motor activity, exacerbating muscle fatigue, or both. Healthy volunteers (n = 14; 23 (3) years) performed a series of very heavy-domain constant power cycling exercise tests with and without an imposed expiratory flow resistance (7 cmH2O/L/s). The decline in maximal evocable isokinetic power at intolerance during each experimental condition was apportioned to: (1) the power equivalent

Mechanisms to dyspnoea and dynamic hyperinflation related exercise intolerance in COPD.

Expiratory flow limitation can develop in parallel with the progression of COPD, and as a consequence, dynamic hyperinflation and lung mechanical abnormalities can develop. Dynamic hyperinflation can cause increased breathlessness and reduction in exercise tolerance. Achievement of critical inspiratory reserve volume is one of the main factors in exercise intolerance. Obesity has specific lung mechanical effects. There is also a difference concerning gender and dyspnoea. Increased nerve activity is characteristic in hyperinflation. Effects of partial anti-G suit inflation on thoracic volume and breathing pattern.

The purpose of this study was to determine the changes in thoracic volume and pattern of breathing during partial anti-G suit (PAGS) inflation by respiratory inductive plethysmography (RIP). Nine normal subjects donned the PAGS, with bladders over legs and thighs, and rested for about 10 min in 60 degrees head-up tilt position. The subjects breathed with closed glottis at functional residual capacity while PAGS was suddenly inflated to 140 mmHg using a calves to thighs sequence. The increase in thoracic volume, as measured from deflection of RIP baseline was 252 ml (S.D. 43 ml), which reflecte

Physiological underpinnings of exertional dyspnoea in mild fibrosing interstitial lung disease.

The functional disturbances driving "out-of-proportion" dyspnoea in patients with fibrosing interstitial lung disease (f-ILD) showing only mild restrictive abnormalities remain poorly understood. Eighteen patients (10 with idiopathic pulmonary fibrosis) showing preserved spirometry and mildly reduced total lung capacity (≥70% predicted) and 18 controls underwent an incremental cardiopulmonary exercise test with measurements of operating lung volumes and Borg dyspnoea scores. Patients' lower exercise tolerance was associated with higher ventilation (V̇E)/carbon dioxide (V̇CO2) compared with con

Hyperventilation with He-O2 breathing is not decreased by superimposed external resistance.

The purpose of this study was to determine the effect of imposed external resistance on the ventilatory response to He-O(2) breathing during peak exercise. To accomplish this purpose, separate inspiratory and expiratory external resistances were applied to offset for the decrease in intrapulmonary airway resistance with He-O(2) breathing. Seven men and three women (69+/-3 years, mean+/-S.D.) with normal pulmonary function performed graded cycle ergometry to exhaustion breathing room air, He-O(2) (79% He, 21% O(2)), He-O(2) with imposed expiratory resistance, and He-O(2) with imposed inspirator

Overweight Is an Independent Risk Factor for Reduced Lung Volumes in Myotonic Dystrophy Type 1.

BACKGROUND: In this large observational study population of 105 myotonic dystrophy type 1 (DM1) patients, we investigate whether bodyweight is a contributor of total lung capacity (TLC) independent of the impaired inspiratory muscle strength. METHODS: Body composition was assessed using the combination of body mass index (BMI) and fat-free mass index. Pulmonary function tests and respiratory muscle strength measurements were performed on the same day. Patients were stratified into normal (BMI < 25 kg/m(2)) and overweight (BMI ≥ 25 kg/m(2)) groups. Exertional inspiratory capacity and operating lung volumes in health: a population-based study.

RATIONALE: Inspiratory capacity (IC) should increase during exercise to allow unrestrained tidal volume expansion, mitigating uncomfortable respiratory sensations. However, preliminary evidence suggests that this is not universally seen in healthy individuals. Whether this negatively impacts on inspiratory reserves and dyspnoea remains unclear. To gain novel insights on the regulation of operating lung volumes in community-dwelling individuals, we determined the association of a lack of increase in IC during incremental cardiopulmonary exercise testing (dynamic inspiratory capacity (ICdyn)) wi
