Impact of elevated pooling levels in basic medical insurance on insured individuals' inpatient service utilization-An equity analysis.

BACKGROUND: China has continuously promoted the transition of basic medical insurance from municipal-level pooling to provincial-level pooling to strengthen fund risk resilience, mutual-aid capacity, and benefit equalization. Although existing studies suggest that higher pooling levels may influence healthcare utilization, limited micro-level evidence exists on how provincial-level pooling of the Basic Medical Insurance Scheme for Urban and Rural Residents (BMISURR) affects inpatient service utilization, hospitalization costs, and income-related equity. This study examines the impact of provin

Does Provincial Pooling of the Resident Medical Insurance Fund Improve the Efficiency of Medical Resource Allocation of China?

Background: Provincial pooling of the Urban-Rural Resident Basic Medical Insurance (URRBMI) fund has been promoted in China to reduce fragmentation and improve system performance, yet its impact on the efficiency of medical resource allocation remains unclear. Objective: To evaluate the impact of provincial pooling on medical resource allocation efficiency and to examine its heterogeneity across regions and institutional arrangements. Methods: Using provincial panel data from 2010 to 2022, this study measures efficiency with a super-efficiency slack-based measure (SBM) model and applies a stag

The Digital Amplifier in Medical Insurance: How Chinese Provincial Pooling Policy Optimizes Chronic Disease Management.

Background: Chronic diseases have proliferated worldwide and become one of the foremost public health challenges. The provincial pooling policy of Chinese Basic Medical Insurance Program (BMIP) (hereinafter the Policy) is conducive to coordinating healthcare resources more broadly and containing medical costs more effectively, which creates opportunities to improve chronic disease patients' health outcomes. Against this backdrop, this study aims to identify how the Policy affects chronic disease patients' health outcomes. Methodology: Utilizing data from the China Family Panel Studies (CFPS) a

FEMA phase-out? Catastrophic extremes challenge decentralization of US flood insurance.

The U.S. National Flood Insurance Program (NFIP) faces growing solvency and affordability challenges amid proposals to decentralize the Federal Emergency Management Agency (FEMA) and shift disaster management to states. Catastrophic floods often span state boundaries, exposing multiple decentralized insurance pools simultaneously. Using a path-independent simulation framework that integrates risk-based premiums, hydrometeorologically clustered flood losses, and 2025 reinsurance contracts, we evaluate the stability of national and state-level pooling using historical data. National pooling mark

Risk equalisation and voluntary health insurance markets: The case of Australia.

In April 2007, Australia introduced a risk equalisation (RE) scheme (de facto a claims equalisation scheme), which replaced an extant reinsurance scheme that had operated since 1976. This scheme is one of a number of policy measures that the Australian Government has instituted to support the voluntary private health insurance (PHI) market which is subject to mandatory community rating and the attendant problem of selection. The latter has been a persistent concern in the Australian PHI market since the introduction of Australia's universal, compulsory national health insurance scheme Medicare

Health Insurance Trends and Access to Behavioral Healthcare Among Justice-Involved Individuals-United States, 2008-2014.

BACKGROUND: A large proportion of justice-involved individuals have mental health issues and substance use disorders (SUD) that are often untreated due to high rates of uninsurance. However, roughly half of justice-involved individuals were estimated to be newly eligible for health insurance through the Affordable Care Act (ACA). OBJECTIVE: We aimed to assess health insurance trends among justice-involved individuals before and after implementation of the ACA's key provisions, the dependent coverage mandate and Medicaid expansion, and to examine the relationship between health insurance and t

Biologic Disease-Modifying Antirheumatic Drugs and Risk of High-Grade Cervical Dysplasia and Cervical Cancer in Rheumatoid Arthritis: A Cohort Study.

OBJECTIVE: Recent research showed an increased risk of high-grade cervical dysplasia and cervical cancer associated with rheumatoid arthritis (RA). The purpose of this study was to examine whether this risk was associated with the use of biologic versus nonbiologic disease-modifying antirheumatic drugs (DMARDs). METHODS: We identified RA patients in the US Medicaid and commercial insurance databases (for the years 2000-2012) who were starting treatment with either a biologic or a nonbiologic DMARD. High-grade cervical dysplasia or cervical cancer was identified with a validated claims-based a

Thin-catheter Surfactant Application for Respiratory Distress Syndrome in Spontaneously Breathing Preterm Infants: A Meta-analysis of Randomized Clinical Trials.

BACKGROUND: Surfactant application by a thin catheter represented by the term less invasive surfactant administration (LISA) for respiratory distress syndrome in spontaneously breathing preterm infants was developed as an alternative to endotracheal intubation. METHODS: We conducted a meta-analysis to assess the effects of LISA when compared to the socalled intubation-surfactant-extubation (INSURE) and the standard endotracheal intubation and mechanical ventilation (MV). The primary outcome was the composite incidence of death or bronchopulmonary dysplasia at a postmenstrual age of 36 weeks.

Insurance status and health-related quality-of-life disparities after trauma: results from a nationally representative survey in the US.

PURPOSE: To determine whether insurance status modifies the association between injuries and health-related quality of life (HRQOL) in a nationally representative sample of US adults. METHODS: This is a longitudinal, observational study using the pooled 2000-2006 Medical Expenditure Panel Survey (MEPS). A total of 50,225 adults (age ≥ 18) with or without injuries were included in the study. HRQOL was evaluated using the EuroQoL Health Index (EQ-5D), visual analog scale (VAS), and the SF-12 physical component score (PCS) and mental component score (MCS). A categorical variable of injury-insura

Medical screening and monitoring as noted by the insurance industry.

Medical screening and monitoring have a long and varied history as part of the insurance industry's safety and health relations with its policyholders. Many workers' compensation insurance carriers have assisted policyholder management in understanding the requirements of cost-effective health programs, in planning and undertaking steps necessary to comply with state-of-the-art medical practices, and in locating knowledgeable physicians to supervise employee health systems. Managing employees' health and associated records has become, for many employers, a complex operational and regulatory pr

Pursuing Affordability, Fairness, and Efficiency in the U.S. Commercial Health Insurance Market.

The high cost of health insurance and health care services in the US is affecting not only the uninsured and lower income households, but is beginning to reach into the middle class. Current health insurance benefit designs often fail to address two forms of market failure: poor consumer information and distorted prices. These market failures distort the market for health care services, and contribute to unaffordable health insurance. When combined with the pooling function of health insurance, those sources of market failure also result in income redistributions that arguably are unfair. Usin

Hospitalizations for near drowning in California: incidence and costs.

California hospital discharge data for 1991 were examined to describe persons hospitalized for near drownings (i.e., a submersion incident for which the victim was admitted to a hospital). Among residents with near-drowning injuries, there were 865 discharges, regardless of outcome (rate = 2.8/100,000); 785 persons survived the hospitalization, and 80 (9%) did not. Swimming pools were the most common submersion site (62%). Highest rates per 100,000 were found among males (3.6), Blacks (3.6), and children 1 through 5 years old (18.4). Charges for the initial hospitalization (excluding physician

Effects of health insurance on out-of-pocket payments: a coarsened exact matching analysis using Kenya's 2022 demographic and health survey.

OBJECTIVES: Out-of-pocket payments remain a major source of healthcare financing in Kenya, yet it is unclear whether health insurance coverage is associated with lower out-of-pocket spending due to non-random enrolment. This study uses coarsened exact matching (CEM) to compare out-of-pocket costs (OOPCs) between insured and uninsured individuals. DESIGN, SETTING AND PARTICIPANTS: This study used cross-sectional data from the 2022 Kenya Demographic and Health Survey which covered 80 780 household members. Participants with missing or incomplete information on health insurance status and OOPC w
