Interferon-gamma-release assays: Better than tuberculin skin testing?

Although the tuberculin skin test has long been the standard for detecting latent tuberculosis infection, it has many limitations. Interferon-gamma-release assays are gaining acceptance as an alternative. In this paper we present cases to illustrate how these new tests can be used and how to interpret the results.

In vitro interferon-gamma release test in patients with drug-induced pemphigus.

BACKGROUND: Drug-specific CD8+ TH1 lymphocytes have been found in the peripheral blood and involved skin of patients with drug-induced bullous exanthems. OBJECTIVES: To determine whether the interferon-gamma release test can identify culprit drugs in pemphigus patients. METHODS: Clinical and laboratory workup for pemphigus was performed in 14 pemphigus vulgaris patients who had been exposed to drugs, and the IFN-gamma release test was conducted on their lymphocytes from heparinized venous blood cultured with medium, phytohemagglutinin and one of 32 drugs, or medium and phytohemagglutinin alo

Interferon-gamma release assay as a diagnostic test for tuberculosis-associated uveitis.

BACKGROUND: To study the use of interferon-gamma release assay (IFN-γ) (IGRAs) as a diagnostic test for tuberculosis (TB)-associated uveitis (TAU). DESIGN: Prospective cohort study. PARTICIPANTS: Consecutive new patients (n=162) with clinical ocular signs suggestive of TAU, seen >1 year period at a single tertiary center. METHODS: All subjects underwent investigations to rule out underlying disease, including T-SPOT.TB and tuberculin skin test (TST). Twenty-one subjects with underlying disease and three with interdeterminate T-SPOT.TB results were excluded. Those with T-SPOT.TB- or TST-posi

Cost-effectiveness of interferon gamma release assays vs tuberculin skin tests in health care workers.

BACKGROUND: Interferon gamma release assays (IGRAs) offer alternatives to tuberculin skin tests (TSTs) for diagnosing latent tuberculosis infection (LTBI). Unlike TSTs, IGRAs require only a single patient visit and are not affected by prior BCG vaccination, providing greater specificity. Of 2 Food and Drug Administration-approved IGRAs, the newer QuantiFERON-TB Gold in Tube test (QFT-GIT) requires less manual processing time than the QuantiFERON-TB Gold test (QFT-G). We compared the cost-effectiveness of the QFT-G, QFT-GIT, and TST for detecting LTBI in new health care workers (HCWs). METHODS

Patch testing versus interferon-gamma release assay in evaluation of drug eruptions.

Delayed-onset T-cell-mediated cutaneous adverse drug reactions are an uncommon but potentially serious result of medication exposures. Identification of culprit medications is crucial, but clinical diagnosis is often difficult. Patch tests and interferon-gamma release assays (IFNγ-RA) were previously reported as potentially useful ancillary tests, while rechallenges remain the reference standard test. We compared the number of positive test results with patch testing and IFNγ-RA for drugs implicated as possible causes of cutaneous reactions. Fifty-one patients with a suspected cutaneous drug e

Cost-effectiveness of interferon-gamma release assay testing for the treatment of latent tuberculosis.

The aim of the present study was to assess the cost-effectiveness of the new T-SPOT.TB assay versus the tuberculin skin test (TST) for screening contacts for latent tuberculosis (TB) infection in Switzerland. Health and economic outcomes of isoniazid treatment of 20- and 40-yr-old close contacts were compared in a Markov model over a 20-yr period following screening with TST only (at three cut-off values) and T-SPOT.TB alone or in combination with the TST. T-SPOT.TB-based treatment was cost-effective at (Euro)11,621 and (Euro)23,692 per life-year-gained (LYG) in the younger and older age group

High reproducibility of the interferon-gamma release assay T-SPOT.TB in serial testing.

Longitudinal studies regarding the reproducibility of Interferon-gamma release assay (IGRA) T-SPOT.TB for the diagnosis of Mycobacterium tuberculosis (M. tb) infection in serial testing are limited. We retrospectively analysed results of serially tested subjects in a medical laboratory in Germany over a time period of 14 years. From October 2004 to December 2018, a total of 5440 subjects were identified with a second T-SPOT.TB test after a median time interval of 258 days (interquartile range [IQR] 62-665). Consistently negative (n = 4520) or positive results (n = 682) were observed in 5202 (9

An interferon-gamma release assay test performs well in routine screening for tuberculosis.

INTRODUCTION: A positive interferon-gamma release assay (IGRA) is regarded as proof of latent Mycobacterium tuberculosis infection. We conducted an evaluation of the IGRA test "T-SPOT.TB" to test its performance during clinical routine use by analysing the positivity rate and odds, effect of season and sensitivity. MATERIAL AND METHODS: Data from T-SPOT.TB testing together with age and test indications (anti-tumour necrosis factor alpha (TNFα) candidate, contact investigation or suspicion of tuberculosis (TB)) were combined with mycobacteria culture results. RESULTS: A total of 1,809 patient

Comparison of interferon-gamma release assay and tuberculin test for screening in healthcare workers.

Healthcare workers (HCWs) have an increased risk of tuberculosis (TB). Screening for latent tuberculosis infection and active TB is therefore essential in infection control programs. Tuberculin skin test (TST) and Interferon -gamma Release Assay (IGRA) were used simultaneously in 1686 HCWs between May 2007 and April 2009. A chest X -ray was performed in order to exclude active TB when TST was >or=10mm or IGRA was positive and in HCWs with TB contact or symptoms. IGRA was positive in 33.1% and TST was >10mm in 78.3% of the HCWs. The proportion of positive IGRA results increased with the TST dia

Interferon-Gamma Release Assay Testing for Latent Tuberculosis Infection: A Health Technology Assessment.

BACKGROUND: Many people infected with the Mycobacterium tuberculosis complex (the bacteria that cause tuberculosis [TB]) have an inactive stage of infection known as latent tuberculosis infection (LTBI). A person with LTBI is at risk of developing active TB. Screening for, and treating people with, LTBI is an important part of preventing adverse health outcomes, reducing the risk of reactivation and the further spread of tuberculosis in a community. We conducted a health technology assessment of interferon-gamma release assay (IGRA) for the detection of LTBI, compared to the standard tuberculi

Tuberculin skin test versus interferon-gamma release assays for the diagnosis of tuberculosis infection.

OBJECTIVE: Accurate detection of latent tuberculosis infection (LTBI) is becoming increasingly important due to the increasing use of immunosuppressive medications and the human immunodeficiency epidemic, which have increased the risk for reactivation to active tuberculosis (TB) infection. LTBI is detected by tuberculin skin test (TST) and interferon-gamma release assays (IGRAs). The latter include T-SPOT(®).TB (Oxford Immunotec) and QuantiFERON(®)-TB Gold In-Tube (QFT-GIT; Cellestis). We examined the value of TST versus IGRAs in the diagnosis of TB infection by meta-analysis based on data der

Interferon-gamma release assay test characteristics depend upon the prevalence of active tuberculosis.

BACKGROUND: Tuberculin skin tests (TSTs) may be negative in the absence of latent tuberculosis infection (LTBI) or in the presence of active tuberculosis (TB). The same phenomenon likely holds for interferon-gamma release assays (IGRAs). METHODS: A mathematical model of LTBI test specificity was developed using elementary probability theory. Implications for IGRA study design, and data collection and analysis are presented. RESULTS: The model demonstrates that the specificity of any test for LTBI is theoretically affected by its sensitivity to active TB, and by the distribution of TB, LTBI a

Optimizing interpretation of the tuberculin test using an interferon-gamma release assay as a reference standard.

The interferon-gamma release assays have greater specificity than the tuberculin skin test (TST), and at least equal sensitivity. We analyzed the sensitivity and specificity of the TST in immunocompetent children considering QuantiFERON as the referent standard. A TST cut-off point of ≥ 5 mm indicates excellent sensitivity (100%) and specificity (93%) in children without Bacillus Calmette-Guérin. In Bacillus Calmette-Guérin-vaccinated children, the TST cut-off point of ≥ 10 mm had poorer specificity (86%), and a cut-off point of ≥ 15 mm resulted in reduced sensitivity (60%).

Making sense of agreement among interferon-gamma release assays and tuberculosis skin testing.

BACKGROUND: Numerous studies of interferon-gamma release assays (IGRAs) and tuberculin skin testing (TST) to assess latent tuberculosis infection have been published without a framework to understand the extent to which these two tests should agree. Analyzing the causes of variability in agreement levels is crucial. METHODS: A mathematical model of agreement between dichotomous tests was used to understand variations in the level of agreement between IGRA and TST results. The effect of cut-off point selection on agreement was also explored using the model. Model-based predictions are illustra
