Intermittent urethral catheterization in the elderly.

Thirty-five elderly patients receiving intermittent urethral catheterization in a Veterans Administration Hospital and attached nursing home care unit were prospectively studied for development of bacteriuria and/or urinary tract infection. Thirty-one of the 35 patients (88.6%) developed urinary tract colonization. The mean time from initiation of catheterization to development of colonization was 5.7 +/- 1.3 days. Persistent bacteriuria with one or several different microorganisms developed in 17 patients. The most common colonizing organisms were coagulase negative staphylococci, Klebsiella

Urethral false passage as a complication of intermittent catheterization.

Intermittent catheterization is used commonly to treat bladder dysfunction. We treated 10 patients who were experiencing difficulty with intermittent catheterization, 9 of whom had a false urethral passage. Of these patients 6 had previously undergone a bladder neck or urethral operation. Endoscopy was helpful to diagnose the condition. Treatment consisted of stenting in 3 patients, transurethral incision and stenting in 3, and fulguration and stenting in 4. An indwelling catheter was left in place for 2 to 3 weeks, after which intermittent catheterization was resumed with a softer catheter. T

[Urethral indwelling catheter, intermittent self-catheterization and urinary infection].

Once a catheter has been passed into the bladder without contamination, there are several possible routes of subsequent infection during drainage period, such as: 1. Entry of bacteria alongside the catheter in the urethra. 2. Introduced bacteria adhered easily to the indwelling catheter and drainage system and colonized. 3. The catheter tip is covered rapidly by various nutrient materials which becomes a good culture medium of stuck bacteria, which is a supply source of bacteria into the bladder urine. 4. Although the motile bacilli ascend very slowly through the stagnant tube and no bacteria

Prevention of urethral stricture recurrence using clean intermittent self-catheterization.

OBJECTIVE: To investigate the effect of clean intermittent catheterization (CIC) on prevention of urethral stricture recurrence after internal urethrotomy. PATIENTS AND METHODS: Of 55 men who were randomly selected, 43 completed the investigation. Of these, 21 patients performed CIC weekly for 1 year following Sachse's operation for urethral stricture and 22 patients formed the control group after the same operation. All had an objective examination for urethral stricture every 2 months after surgery. RESULTS: Significantly fewer (P < 0.01) patients developed recurrence of urethral stricture

Management of urethral stricture in patients practising clean intermittent catheterization.

Clean intermittent catheterization (CIC) is used commonly to treat patients with impaired bladder emptying. We treated 6 male patients who were experiencing difficulty with intermittent catheterization. Retrograde urethrography demonstrated stricture of the urethra in all of them and a false passage as well in one patient. Treatment consisted of urethral dilatation using filiform and followers in 4 patients and optical internal urethrotomy in the remaining 2 patients followed by urethral stenting using a 16 Fr. Teflon Foley catheter in all. The stenting Foley catheter was removed after 2 weeks

Self-catheterization after urethrotomy. Prevention of urethral stricture recurrence using clean intermittent self-catheterization.

Following urethrotomy approximately 50% develop recurrence within 6 months. Twenty-nine consecutive patients were offered clean intermittent self-catheterization (CISC) as a preventive measure against urethral stricture recurrence following internal urethrotomy. Ten patients dropped out during the scheduled 6-month regimen. Of the 19 patients left, a significantly improved flow was maintained over the period of observation. No reoperations were required. Prior to the trial, 14 of the 19 patients had a history of recurrent urethral stricture; 92% of these recurrences were diagnosed within 6 mon

Clean intermittent self-catheterization after urethrotomy for recurrent urethral strictures.

Although endoscopic optical urethrotomy is the primary treatment for urethral stricture, it is associated with a high recurrence rate, and the essential problem is how to stop the scar from shrinking after cutting. In a controlled study the effect of treatment of recurrent urethral stricture by internal urethrotomy followed by clean intermittent self-catheterization (CIC) for 6 or 12 months was compared in 25 and 24 patients, respectively. Patients learnt easily how to perform CIC: only one patient was not able to do it at home. All patients were evaluated by uroflowmetry before and immediatel

Recurrent urethral stricture disease managed by clean intermittent self-catheterization.

We studied 41 patients with urethral stricture who had failed at least 1 operation, and were being maintained with filiform and follower dilation every 6 to 12 weeks. The patients were instructed in clean intermittent self-catheterization with a 16F red rubber catheter, which was performed every 1 to 30 days. Followup from 9 to 36 months revealed excellent compliance and average peak uroflow rates increased from 5.5 cc per second before dilation to 17.1 cc per second at the last followup visit. Clean intermittent self-catheterization is a simple method to maintain a patent urethra and obviates

Female urethral strictures: successful management with long-term clean intermittent catheterization after urethral dilatation.

OBJECTIVE: To report our experience in the diagnosis and treatment of urethral stricture in women. PATIENTS AND METHODS: A retrospective review of records and video-urodynamics identified women treated for urethral stricture between 1999 and 2004 at one institution by one surgeon. Urethral stricture was defined as a fixed anatomical narrowing between the bladder neck and distal urethra of <14 F preventing catheterization, and the diagnosis was confirmed by cysto-urethroscopy, and/or video-urodynamics. Women with a history of external beam radiotherapy to the pelvis, or of gynaecological, uret

[Benefit of clean intermittent self-catheterization in the management of urethral strictures].

OBJECTIVES: Urethral stenosis has a recurrent character. The urethroplasty is often proposed for repeat stenosis. This study seeks to explore the interest of clean intermittent self-catheterization in the evolution of urethral stenosis after urethrotomy. METHODS: Single-center retrospective study from 2008 to 2013, concerning patients who received urethrotomy to treat urethral stenosis which was confirmed by endoscopy and a flow chart. Some accomplished self-catheterization in addition to urethrotomy to prevent recurrence. The monitoring was provided during consultation by a subjective assess

Intermittent catheterization in the treatment of acute purulent urethritis: case report.

Acute purulent urethritis is a complication associated with an indwelling. Foley catheter in male patients. A refractory purulent urethral discharge, fever and the sequelae of urethral stricture, periurethral abscess and epididymitis are familiar problems to the urologist. The usual therapy is removal of the catheter, antibiosis and provision for another type of vesical drainage. Our recent successful experience using clean intermittent catheterization to treat an acute urethritis is presented.

Renal Replacement Therapy and Intermittent Catheterization Risk in Posterior Urethral Valves.

OBJECTIVES: Posterior urethral valves predispose children to renal replacement therapy (RRT) and bladder dysfunction. Researchers of single-institutional series were unable to refine risk stratification because of rarity of the disease. We aimed to identify clinical variables associated with the risk of RRT and clean intermittent catheterization (CIC) in a large multicenter cohort study. METHODS: Children with posterior urethral valves born between 1995 and 2005 who were treated before 90 days of life at 5 children's hospitals were retrospectively reviewed. Outcomes included RRT and recommend

[Urethral pain during clean intermittent self catheterization: Retrospective analysis of 77 patients].

OBJECTIVE: Clean self-intermittent catheterization (CIC) is the gold standard of the therapeutic approach of chronic urinary retention. Usually, CIC are safe, effective but in some cases catheterization can determine urethral pain during catheter insertion or withdrawal leading to poor adherence and compliance. To determine prevalence of pain during CIC and verify its impact on adherence to treatment. METHOD: Retrospective study with evaluation of pain during CIC one month following CIC teaching session by means specific and validated questionnaires: ICDQ (Intermittent Catheterization Difficu

Intermittent self catheterization versus regular outpatient dilatation in urethral stricture: a comparison.

BACKGROUND: The results of regular intermittent self catheterization were compared with regular outpatient dilatation after endoscopic internal urethrotomy for urethral strictures. METHODS: The records of patients who were treated for urethral stricture disease over a 4-year period from 1991 to 1994 were reviewed. They were either on regular urethral dilatation or were advised to carry out self calibration. A follow-up questionnaire was sent to them and they were asked to attend a review. Student's t-test and Proportion Test were used to find out if there was any significant difference betwee

Intermittent catheterization in neurologic patients: Update on genitourinary tract infection and urethral trauma.

Intermittent catheterization is considered the standard of care in most neurologic patients with lower urinary tract disorders. However, in this context, genitourinary tract infection and urethral trauma represent specific challenges. Such conditions have been found to significantly deteriorate quality of life and complicate subsequent treatments. Only optimal prevention associated with appropriate treatment allows for the long-term continuation of such bladder management. Here, we discuss the diagnosis and therapeutic and preventive approaches associated with genitourinary tract infection and
