Internal fixators: a safe option for managing distal femur fractures?

OBJECTIVE: Evaluate safety and reliability of internal fixator for the treatment of intra-articular and periarticular distal femur fractures. METHODS: Retrospective data evaluation of 28 patients with 29 fractures fixed with internal fixator was performed. There was a predominance of male patients (53.5%), with 52% of open wound fractures, 76% of AO33C type fractures, and a mean follow up of 21.3 months. Time of fracture healing, mechanical axis deviation, rate of infection and postoperative complications were registered. RESULTS: Healing rate was 93% in this sample, with an average time of

[Dynamic forces of Mitkovic self-dinamysible trochanteric Internal fixators (SIF)].

Dynamic trochanteric fractures implants allow fracture fragments to be compressed. Dynamisation can be realized if the axial pin force overcome friction force between pin and body of the implant. Examination of sliding iniciation forces in Mitkovic Selfdinamysible Trochanteric Internal Fixator (SIF). SIF was attached for angle block in the position with vertical orientation of pins. The transversal load of 5 kg was connected to pins by a rope. A dynamometer was used to measure force during the movement of angle block in up direction. Regression coefficients were a1 = 4,052 i b1 = 0,623 for SIF

Biomechanical evaluation of a healed acetabulum with internal fixators: finite element analysis.

BACKGROUND: Treatment of complicated acetabular fracture with internal fixation usually has high risk of failure because of unbefitting fixation. However, evaluation of the biomechanical effect of internal fixation under physiological loading for fracture healing is still generally rarely performed. The purpose of this study is to analyze the biomechanical characteristics of a healed acetabulum with designed internal fixators under gait and to explore the biomechanical relationship between the healed bone and the internal fixator. METHODS: A patient-specific finite element model of whole pelv

Biomechanical testing of the LCP--how can stability in locked internal fixators be controlled?

New plating techniques, such as non-contact plates, have been introduced in acknowledgment of the importance of biological factors in internal fixation. Knowledge of the fixation stability provided by these new plates is very limited and clarification is still necessary to determine how the mechanical stability, e.g. fracture motion, and the risk of implant failure can best be controlled. The results of a study based on in vitro experiments with composite bone cylinders and finite element analysis using the Locking Compression Plate (LCP) for diaphyseal fractures are presented and recommendati

Internal fixation of nonunions.

We review the two major types of internal fixation of nonunions (plating and intramedullary nailing), and analyze nonunions in different locations (upper and lower extremities). Depending on the type and the location of the nonunion, plating or intramedullary nailing may be selected. Both have advantages and disadvantages: plating requires opening the nonunion site, which entails some damage to the soft tissues, and carries with it a risk of secondary infection. With plating, it often is impossible to do a real decortication because the periosteum may be thin and poorly adherent to bone, and t

Comparison of the stability of various internal fixators used in the treatment of osteochondritis dissecans--a mechanical model.

The purpose of this study was to determine what characteristics of fixation devices used in the treatment of osteochondritis dissecans (OCD) contribute to improved stability to resist shear loading. An OCD model was designed using rigid polyurethane foam. Each specimen consisted of two components, an osteochondral fragment and a corresponding defect. A total of 40 specimens were prepared and assigned to one of four groups: control (no extrinsic stabilizer); two 2-mm-diameter Kirschner wires (K-wires), 40 mm in length; one threaded washer and a 28-mm screw; and one threaded washer and a 38 mm s

Internal anterior fixators for pelvic ring injuries: Do monaxial pedicle screws provide more stiffness than polyaxial pedicle screws?

OBJECTIVES: Little is known about the mechanical properties of internal anterior fixators (known as INFIX), which have been proposed as subcutaneous alternatives to traditional anterior external fixators for pelvic ring disruptions. We hypothesised that INFIX has superior biomechanical performance compared with traditional external fixators because the distance from the bar to the bone is reduced. METHODS: Using a commercially available synthetic bone model, 15 unstable pelvic ring injuries were simulated by excising the pubic bone through the bilateral superior and inferior rami anteriorly a

[The internal fixation of fractures in children].

The internal fixation of fractures of shaft bones was spread out by a group of surgeons working together with Prevot. The principle of this method is a three point load carrying paragraph sign(entry point, height of fracture and cancellous bone of the metaphysis of the opposite fracture part). From 1996 to 1999 86 children with fractures of the humerus, the fore-arm and the femur were operated in the technique of intramedullary nailing. The fractures affected the humerus to 3.5 %, the fore-arm to 65.1 % and the femoral shaft to 31.4 %. The postoperative duration of stay in hospital after inter

Internal fixation of scaphoid fractures.

Scaphoid fracture fixation is indicated in certain acute situations and for scaphoid nonunion. Internal fixation requires an understanding of the distinctive scaphoid anatomy with relation to its shape, blood supply, fracture healing, and radiographic evaluation. The choice of surgical approach varies with the fracture configuration and procedure planned. A variety of innovative implants are available to accomplish internal fixation of the scaphoid. The ultimate goal is to achieve union and restore stability to the carpus.

Internal fixation in the osteoporotic spine.

The presence of osteoporosis in patients who are candidates for spinal surgery can affect preoperative planning. These patients may not need instrumentation for a successful surgical result, but if use of instrumentation is indicated because of instability or deformity, certain principles should be observed. These include using multiple sites of fixation, accepting lesser degrees of deformity correction, and avoiding ending the instrumentation within kyphotic segments. Advances in perioperative medical management, as well as improved instrumentation systems, may also contribute to improving pa

The AO spinal internal fixator.

The AO spinal internal fixation system uses 5.0 mm diameter transpedicular Schanz screws with 7.0 mm diameter fully threaded stainless steel rods. Originally developed for the treatment of thoracolumbar and lumbar spine fractures, it can be used for a variety of spinal disorders. A prospective study has been carried out, and the first 48 patients have been reviewed. The results for those patients with fractures are excellent. In burst fractures, it has been possible to decompress the canal and restore lordosis. In only half of the cases of spondylolisthesis has it been possible to improve the

Charcot Reconstruction: Internal Fixation.

Charcot neuroarthropathy is a condition characterized by progressive joint dislocation and bone fragmentation, which if not managed early with off-loading, often results in the destruction of the normal foot architecture, an unstable foot deformity and development of neuropathic ulceration. It is most frequently seen in patients with diabetes mellitus; however the clinician should also consider this diagnosis in patients who develop peripheral neuropathy due to chemotherapy, alcoholism, and infectious diseases such as syphilis, leprosy, and human immunodeficiency virus. This article aims to de

Internal fixation of intracapsular fractures.

The main indications for internal fixation of intracapsular fractures are undisplaced and minimally displaced fractures and displaced intracapsular fractures in those aged less than about 70 years. For displaced fractures, closed reduction is to be preferred to open reduction. Numerous different implants may have to be used; with current practice favouring two or three parallel cannulated cancellous screws. These may be inserted either percutaneously or with minimal surgical exposure. Attention to surgical details of fracture reduction and implant positioning will minimise the risk of fracture

Osteonecrosis secondary to internal fixation.

Allergies to metals as a cause of failure of joint prostheses has historically been a controversial issue. Loosening of a prosthesis may be due to trauma, infection of the surrounding area, or a faulty implantation of the device. However, cases of loosening of the device do occur without any history of the above criteria. This leads one to consider other explanations for this phenomenon, such as sensitivity of the tissues to one of the metals in the implant. The following presentation demonstrates that metal allergies are a surgical concern when considering joint prostheses, or internal fixati

[The reversed locked internal plate fixator as an alternative internal fixation of problematic proximal femur fractures].

AIM: The aim of the study was to evaluate the application of a locked internal fixator in complex fractures of the proximal femur, in which the internal fixation with standard implants was not possible due to poor quality of bone or already failed internal fixation in the past. METHOD: Ten patients suffering from a pertrochanteric (n = 5), periprosthetic (n = 1) or subtrochanteric (n = 4) femural fracture between 2003 and 2008 were prospectively registered, underwent open reduction and internal fixation with an "upside-down" femur LISS (less invasive stabilisation system) and were followed up

A Case of Atlanto-Axial Rotatory Fixation Requiring Internal Fixation.

INTRODUCTION: Most cases of atlanto-axial rotatory fixation (AARF) respond well to conservative treatment. Few reports have described initial management strategies. Almost no cases of AARF with recurrence requiring internal fixation have been reported. Here, we describe a case of recurrent AARF requiring internal fixation. CASE REPORT: A 5-year-old boy presented with a history of 22q.11.2 deletion syndrome, DiGeorge syndrome, congenital left-sided clubfoot, and periodic vomiting. He was referred to a local pediatrician with neck pain and torticollis and was prescribed analgesics. After 5 days
