Intraabdominal infections represent a diagnostic and therapeutic challenge in the elderly population. Atypical presentations, diagnostic delays, additional comorbidities, and decreased physiologic reserve contribute to high morbidity and mortality, particularly among frail patients undergoing emergency abdominal surgery. While many infections are the result of age-related inflammatory, mechanical, or obstructive processes, infectious complications of feeding tubes are also common. The pillars of treatment are source control of the infection and judicious use of antibiotics. A patient-centered

Although peritonitis has been recognized as a common and complex disease entity since ancient times, the true understanding and pathophysiology, as well as treatment of peritonitis, continue to plague surgeons and physicians. The clinical course and outcome of peritonitis is dependent upon the struggle between the quantity and virulence of the pathogen and host's physiologic reserve, including the ensuing inflammatory response. The current multimodality treatment of intraabdominal infections is based upon the fundamental principles established by Polk in 1979: surgical source control, fluid re

There are many causes of peritonitis. Primary peritonitis is rare, but may complicate cirrhosis of the liver or may, in developing countries, be caused by Mycobacterium tuberculosis. It is occasionally iatrogenic, particularly as a complication of continuous ambulatory peritoneal dialysis. It is, however, most frequently the result of perforation of a hollow viscus, caused by either trauma or disease. Its most lethal form follows breakdown of an intestinal anastomosis. Primary and iatrogenic peritonitis should be treated with antibiotics, and localized abscesses can sometimes be cured by percu

Intraabdominal sepsis most frequently follows penetrating or blunt abdominal trauma or perforated appendicitis or diverticulitis. The initial escape of the endogenous gastrointestinal microflora into the peritoneal cavity results in peritonitis and secondary septicemia, which is frequently followed by localized intraabdominal abscesses. These infections are most frequently polymicrobial and relate directly to the unique endogenous microflora at the various levels of the gastrointestinal tract. The treatment of intraabdominal sepsis is primarily centered around prompt, appropriate surgical inte

Intraabdominal infections are derived from the flora of the gastrointestinal tract. The upper intestine, from the stomach to the mid-ileum, harbors a sparse microflora. The lower tract harbors a luxuriant flora comprised predominantly of anaerobic bacteria. Bacteriodes fragilis is the leading pathogen in anaerobic infections. In a recent nationwide survey of the susceptibility patterns of B. fragilis the most active beta-lactam antibiotics were cefoxitin, piperacillin and moxalactam. The other active drugs included metronidazole, clindamycin, and chloramphenicol. Many other penicillins and cep

Studies on intraabdominal infections have been difficult to compare in the past due to a missing system of classification for peritonitis. According to a recently developed classification system, secondary peritonitis, including spontaneous acute peritonitis, postoperative peritonitis and posttraumatic peritonitis, is the most common complication of severe intraabdominal infections. In several studies the mortality rate of postoperative peritonitis was still between 60% and 79%. Scoring systems were developed, some of them with the idea to predict mortality in peritonitis. Although the APACHE

Common complications involved in treating pediatric patients with cancer are bacterial, viral and fungal infections of the gastrointestinal tract including esophagitis, gastritis, duodenitis, colitis and hepatobiliar infections. In many cases there are multiple factors that predispose these patients to gastrointestinal infections, such as granulocytopenia, T-cell dysfunction, and mucosal damage. In addition, newer therapies have changed the spectrum of infection that is seen in these patients. The profound T-cell suppression associated with therapies such as stem cell transplantation has led t

Tigecycline is a new antimicrobial agent; it is the first in a new class of antibiotics, the glycylcyclines, with properties conferring the ability to overcome many common resistance mechanisms, thus allowing its use for many serious and life-threatening infections for which the use of other antibiotics is no longer appropriate. It has a wide antibacterial spectrum including most methicillin-resistant Staphylococci, vancomycin-resistant Enterococci, ESBL-producing Gram negative bacteria, and other MDR Gram negative bacteria such as Acinetobacter, and Stenotrophomonas. It has good antibacterial

The exact role the anaerobic enteric flora plays in the pathogenesis and persistence of intraabdominal infections due to perforation of a hollow viscus is still unclear. Four lines of evidence have been evaluated. (1) Careful cultures of intraabdominal infections almost always yield anaerobic bacteria, but only relatively few species are predictably found, compared with the hundreds of species populating the colon. (2) Experimental models have demonstrated that the pathogenetic roles of aerobes and anaerobes differ from one another during the establishment of mixed intraabdominal sepsis. Facul

Since its first description in 1967, the mortality of the adult respiratory distress syndrome (ARDS) has remained unchanged despite the increasing sophistication of supportive techniques. Few patients now die of refractory hypoxemia, the majority succumbing to the multiple systems organ failure syndrome, commonly due to sepsis. Sepsis is both the most common cause of ARDS, usually involving the abdomen, and the most frequent complication, usually affecting the lungs. ARDS is, thus, increasingly seen as the pulmonary component of multiple systems organ failure, triggered by the systemic respons

Intra-abdominal infection is a potentially serious condition that requires early and accurate diagnosis. Point-of-care ultrasound (POCUS) has emerged as a key diagnostic tool due to its safety, availability, low cost, and bedside applicability. It offers several advantages over other imaging modalities such as computed tomography (CT) and magnetic resonance imaging (MRI), although its diagnostic performance strongly depends on the operator's expertise. Ultrasound is particularly useful in diagnosing common conditions such as appendicitis, cholecystitis, cholangitis, liver abscesses, pyelonephr

Pediatric critical care medicine : a journal of the Society of Critical Care Medicine and the World Federation of Pediatric Intensive and Critical Care Societies2005eng
OBJECTIVE: To define intraabdominal infections in infants and children.
DESIGN: Summary of the literature with review and consensus by experts in the field.
RESULTS: Intraabdominal infections are common in infants and children and comprise a broad range of disorders of greatly variable severity. In addition to microbiologically mediated processes, other inflammatory disorders frequently present similar clinical syndromes. More aggressive and effective therapy for prematurity, chronic diseases of childhood, malignancies, immunodeficiencies, and organ failure, including transplantation, is lik

BACKGROUND: Although the proximal role of systemic cytokines in the infectious-inflammatory cascades is well recognized, the magnitude and meaning of its intraperitoneal levels in peritonitis have received little attention. We hypothesized that in peritonitis a significant and clinically relevant cytokine-mediated inflammatory response is compartmentalized in the peritoneal cavity.
METHODS: MEDLINE was used to search the literature for all articles dealing with experimental, primary, and secondary bacterial peritonitis and cytokines.
RESULTS: Bacterial peritonitis is associated with an immen

A number of different organisms can be isolated from intraabdominal infection. The most common are aerobic Gram-negative bacilli. Anaerobes are not unusual. From June 1989 to January 1990, Cefmetazole was administered to 23 patients with intraabdominal infection at Veterans General Hospital-Taipei. There were six patients with spontaneous bacterial peritonitis, five biliary tract infection, five liver abscesses, five with pelvic inflammatory disease, one acute ruptured appendicitis and another intraabdominal abscess. In addition, ten patients had sepsis. Clinical response was satisfactory in 2

Antibiotic prophylaxis and treatment regimens ideally are selected on the basis of efficacy, safety, and cost. This review evaluates current, selected literature on antibiotic prophylaxis for colorectal surgery, presumptive antibiotic administration following penetrating abdominal trauma, and treatment of intraabdominal infections. Single-drug regimens with the newer, broad-spectrum agents are assessed and compared with combination regimens; specific regimens are recommended. Colorectal procedures require an antimicrobial agent with activity against both aerobes and anaerobes. Patients undergo

BACKGROUND: A prospective, randomized, single-blind study evaluated the efficacy and safety of clindamycin plus amikacin versus clindamycin plus aztreonam (Cl-Az) in treating intraabdominal infections in adults.
METHODS: Patients were treated intravenously for 7 to 10 days, clindamycin 900 mg plus amikacin 5.0 mg/kg three times a day or clindamycin 900 mg plus aztreonam 2.0 gm three times a day. All 67 patients enrolled were evaluated for safety and 31 in each group for clinical and microbiologic response. Both groups were similar in initial diagnosis, perforated appendicitis or intraabdomina

A double-blind trial was conducted in 385 patients with suspected bacterial intra-abdominal infections to compare the efficacy and safety of ampicillin-sulbactam with cefoxitin. Patients were randomized to receive either 3 g ampicillin-sulbactam (2 g ampicillin-1 g sulbactam), or 2 g cefoxitin, every 6 hours. To be evaluable, patients had to demonstrate positive culture evidence of peritoneal infection at the time of operation. A total of 197 patients were evaluable for clinical efficacy. The two treatment groups were comparable in demographic features and in the presence of risk factors for i

Bacterial peritonitis, intraabdominal sepsis, and other surgical infections are frequently polymicrobial. Moxalactam, a new beta-lactam antibiotic, has been shown to be active in vitro against most bacterial pathogens commonly isolated from patients with surgical infections. This drug was therefore tested as the sole antimicrobial agent in the treatment of 32 surgical infections (25 cases of intraabdominal sepsis, 6 cases of wound infections, and 1 case of bacteremia). Nearly all (91%) of the infections responded favorably; 66% were cured with moxalactam plus surgery, 16% were cured with moxal

A serious danger to the circulation following the administration of suxamethonium is known to occur in patients reacting with an abnormal release of potassium. So far the following groups of patients have been found to be at an increased risk: burns, multiple trauma, tetanus, neurological disorders, uraemia. As a result of our investigations patients with severe intraabdominal infections are also to be regarded as at risk. Four of ten patients with this diagnosis have shown an increase of serum-potassium of more than 2.5 mequiv/l following suxamethonium injection in spite of a prophylactic sma
