Aneurysms: intracranial aneurysms.

Intracranial aneurysms (IAs) are found in 3.2% of the general population and most commonly are diagnosed in individuals at a mean age of 50 years. Most IAs occur in the internal carotid circulation. When an IA ruptures and causes subarachnoid hemorrhage, patients typically present with severe thunderclap headaches. When aneurysmal rupture is suspected, noncontrast computed tomography scan of the head is indicated. If subarachnoid hemorrhage is confirmed, the patient should be referred to a center that offers neurosurgical and critical care for short- and long-term management. A computed tomogr

Dissecting intracranial aneurysm.

A case of spontaneous dissecting aneurysm of the right internal carotid and middle cerebral arteries is presented in 13-year-old boy. The pathogenetic factors incriminated in previously reported cases are reviewed and the pathological findings are discussed. The abnormalities of the internal elastic lamina as seen in this patient have been observed in numerous cases of intracranial dissecting aneurysms. It is concluded that these defects play an important role in the development of dissection.

Intracranial aneurysms.

PURPOSE OF REVIEW: To review the neuroophthalmic manifestations of cavernous and posterior communicating artery aneurysms as well as the diagnosis and treatment options for patients with these kinds of aneurysms. RECENT FINDINGS: The natural history of cavernous aneurysms has recently been systematically followed. Comparison of coiling and clipping for posterior communicating artery aneurysms has yielded new suggestions on how to best treat these patients. SUMMARY: The review discusses how to follow these cases, diagnose and treat, and assess for complications after treatment decisions have

Intracranial aneurysms.

The modern history of the management of intracranial aneurysm encompasses little more than a quarter century. These are seen in 5% routine autopsies and those under 2 mm exist in 17% normal adult brain vasculature. One third of strokes are SAH and aneurysms account for 70% of these. Aneurysms rupture at a rate of 12/100.000 population/year, mostly on the 5th, and 6th decades. Given the high morbidity and mortality of these (43% from first haemorrhage if untreated), surgeons can attempt to avoid rebleeding for a week or more until surgery becomes safe and expand the ability to deal safely with

Intracranial aneurysms.

A reappraisal of case histories of patients with ruptured intracranial aneurysms emphasized the importance of clinically recognizing severe spasm that contraindicates early angiography and large cerebral haematomas that require immediate evacuation. Observation from the day of haemorrhage is important; most recurrent episodes with cerebral signs in the first 10 days were due to spasm; haemorrhage was more common during the following two weeks. In many attacks the signs were not sufficiently distinctive for diagnosis of spasm or haematoma.

Saccular intracranial neonatal aneurysm.

INTRODUCTION: Neonatal cerebral aneurysms are very rare condition and distinct from those of the adults. CASE REPORT: We reported a 14-day-old male neonate who presented subarachnoid hemorrhage due to a ruptured anterior cerebral artery saccular aneurysm. In addition, we present a review of the relevant literature. CONCLUSION: Intracranial hemorrhage due to cerebral aneurysm rupture in a newborn is an uncommon diagnosis, but it must be unequivocally excluded.

Rare Intracranial Donut Aneurysm.

A 30-year-old man was admitted for sudden loss of consciousness. On angio-CT, a ring-like vascular formation arising from the anterior communicating artery with a low-density center was visualized. This finding was consistent with a giant arterial aneurysm presenting with a central thrombus, also called "donut aneurysm".

Intracranial aneurysm and sildenafil.

Sildenafil is one of the most commonly used drugs for the treatment of erectile dysfunction. To date, we found five reported cases of intracerebral bleeding and two reported cases of subarachnoid hemorrhage related to sildenafil use. We report a 49-year-old hypertensive and diabetic patient who presented with acute pulmonary edema and loss of consciousness following ingestion of 100 mg of sildenafil prior to sexual intercourse. He was not previously aware of the presence of an aneurysm and had no family history of it. Computed tomography of his head revealed a subarachnoid hemorrhage due to ru

[Intracranial aneurysm in Taiwan].

We reviewed the epidemiologic and clinical features of ruptured intracranial aneurysms based on the available data in the literature reported in Taiwan. Among 28,763 stroke patients registered by members of the Neurological Society of Taiwan, 600 patients had verified intracranial aneurysms (2.1%), constituting only 34.7% of 1,730 patients with subarachnoid hemorrhage. The male to female ratio was 0.68.1. The mean age was 54.6 +/ 14.3 years in men, and 56.5 +/ 13.8 years in women. Among the 409 patients whose aneurysmal sites were recorded, the aneurysm occurred at the anterior communicating a

Intracranial ophthalmic artery aneurysm.

A 45-year old black man with a carotid-ophthalmic artery aneurysm developed sudden loss of vision in his left eye, a mid-dilated pupil, and pain on movement of the left eye. A temporal island of vision was present in the left eye and the right visual field was normal. There were no other neurologic abnormalities. The aneurysm spontaneously ruptured, resulting in the death of the patient.

Iatrogenic false aneurysm following repair of intracranial aneurysm.

False aneurysm of the intracranial arteries occurs infrequently as a complication of head injury and rarely as a consequence of other pathology. A case of false aneurysm of the supraclinoid internal carotid artery following minor intraoperative injury during clipping of a basilar-superior cerebellar aneurysm by the pterional approach is described. False aneurysm is a potential cause of recurrent intracranial hemorrhage after successful aneurysm clipping. Arterial bleeding which appears to come from the region of a branch origin should be viewed with concern even if it subsides spontaneously.

Intracranial hemorrhage produced by spontaneous dissecting intracranial aneurysm.

Two patients with intracranial dissecting aneurysms were seen initially because of acute hemorrhage. One patient died, and a false dissecting aneurysm secondary to atherosclerosis was found at autopsy. In the other patient, the appearance of the carotid arteriogram was typical of a dissection. Because of the propensity of these aneurysms to rupture, caution is indicated in the use of anticoagulants or vasopressors in patients with evolving cerebral infarction secondary to intracranial arterial dissection.

Unruptured intracranial aneurysms in the Familial Intracranial Aneurysm and International Study of Unruptured Intracranial Aneurysms cohorts: differences in multiplicity and location.

OBJECT: Familial predisposition is a recognized nonmodifiable risk factor for the formation and rupture of intracranial aneurysms (IAs). However, data regarding the characteristics of familial IAs are limited. The authors sought to describe familial IAs more fully, and to compare their characteristics with a large cohort of nonfamilial IAs. METHODS: The Familial Intracranial Aneurysm (FIA) study is a multicenter international study with the goal of identifying genetic and other risk factors for formation and rupture of IAs in a highly enriched population. The authors compared the FIA study co

Two consecutive ruptured intracranial aneurysm in patient with multiple intracranial aneurysms.

When aneurysmal subarachnoid hemorrhage due to multiple aneurysms is suspected, identifying the rupture site is essential to determine the exact surgical site, but it may not be easy. Even if embolization is adequately performed, complications may remain. Typical complications include rebleeding and hydrocephalus in the early phase and delayed cerebral ischemia in the delayed phase. Herein, we describe a case of rupture of an intracranial aneurysm after performing embolization for a different ruptured intracranial aneurysm in a patient with multiple intracranial aneurysms. Patients with multip

[Intracranial aneurysm and coronary ectasia].

BACKGROUND: Coronary aneurysms are uncommon and can exceptionally be associated with other aneurysmal localizations. CASE REPORT: A 34 year-old man with a history of aneurysm of the coronary arteries revealed by myocardial infarction without a wave six months earlier, presented cerebral hemorrhage secondary to an aneurysmal rupture of the anterior communicating artery. DISCUSSION: Other aneurysmal localizations such as the abdominal artery, pulmonary artery, hepatic artery, popliteal artery, coronary venous system or the left ventricle have been associated with coronary artery aneurysm. Intracranial aneurysm formation following radiotherapy.

Two patients sustained a subarachnoid haemorrhage secondary to radiation-induced vasculopathy and associated aneurysm. A 65-year-old woman was treated by radiotherapy for a stereotactically biopsied left parieto-temporal astrocytoma. Three-and-a-half years later, she suffered a subarachnoid haemorrhage secondary to a giant aneurysm arising from the middle cerebral artery bifurcation. A 44-year-old man sustained a subarachnoid haemorrhage 21 years after receiving whole brain irradiation for a large left parietal angioma. Angiography following his subarachnoid haemorrhage revealed a distal right

[Genetic dissection of intracranial aneurysm].

Subarachnoid hemorrhage (SAH) due to rupture of an intracranial aneurysm (IA) is a devastating condition with high mortality and morbidity. Genetic as well as environment factors play important roles in the pathogenesis of SAH and IAs. We review the present knowledge on the genetic factors responsible for SAH or IAs. Linkage analysis and association study are used for genetic dissection. Genome-wide linkage analyses have specified several genetic loci for IAs and 6 loci (1p34-36, 7q11, 11q24-25, 14q22-31, 19q13, and Xp22) have been replicated in different populations. Cluster headache and intracranial aneurysm.

In the present study we describe the cases of two patients with cluster-like headache related to intracranial carotid artery aneurysm. One of these patients responded to verapamil prescription with headache resolution. In both cases the surgical clipping of the aneurysm resolved the cluster pain. These findings strongly suggest a pathophysiological link between the two conditions. The authors discuss the potential pathophysiological mechanisms underlying cluster-like headache due to intracranial carotid artery aneurysm.
