Non-Atherosclerotic Intracranial Arterial Diseases.

Atherosclerosis is not the only cause of intracranial arterial disease. Arterial dissection, moyamoya disease, vascular inflammatory disease, vasospasm and immunologic disorders are important non-atherosclerotic intracranial arterial diseases. Identification of the correct etiology is important in establishing treatment strategies and assessing prognosis. Careful history taking and appropriate laboratory testing are essential. Although catheter angiography is the most important diagnostic tool to examine various intracranial arterial diseases, other diagnostic modalities such as CT angiography

Clinical interpretation of high-resolution vessel wall MRI of intracranial arterial diseases.

Intracranial arterial pathology has traditionally been evaluated with luminal imaging. Recently, high-resolution vessel wall imaging (HR-VWI) with MRI has facilitated submillimetre evaluation of the arterial walls. This technique can help differentiate various causes of intracranial steno-occlusive disease, identify culprit atherosclerotic plaques with a recent cerebral infarct, locate vessel wall pathology in areas with minimal or no narrowing on luminal imaging, predict aneurysm stability and identify a ruptured aneurysm when multiple aneurysms are present. Interpretation of HR-VWI examinati

Emerging Concept of Intracranial Arterial Diseases: The Role of High Resolution Vessel Wall MRI.

Intracranial arterial disease (ICAD) is a heterogeneous condition characterized by distinct pathologies, including atherosclerosis. Advances in magnetic resonance technology have enabled the visualization of intracranial arteries using high-resolution vessel wall imaging (HR-VWI). This review summarizes the anatomical, embryological, and histological differences between the intracranial and extracranial arteries. Next, we review the heterogeneous pathophysiology of ICAD, including atherosclerosis, moyamoya or RNF213 spectrum disease, intracranial dissection, and vasculitis. We also discuss how

Intracranial arterial aneurysm complicating Behçet's disease.

Behçet's disease, a rare condition in central Europe but more common in Turkey where it was originally described, is characterized clinically by the presence of a diagnostic triad of oral and genital aphthous ulcers, meningitis, and relapsing iridocyclitis. Vascular lesions including arterial and venous occlusions, arterial aneurysms and varices are one of the common complications of Behçet's disease occur most commonly in the abdominal aorta, femoral arteries and pulmonary arteries. There have been only four reports of aneurysms of cerebral arteries in the literature. The authors describe a p

Transcranial Doppler ultrasound for the assessment of intracranial arterial flow velocity--Part 2. Evaluation of intracranial arterial disease.

Transcranial (TC) pulsed Doppler ultrasound was used to evaluate flow velocity within the intracranial basal cerebral arteries in 71 patients with cerebrovascular disease as revealed by arteriography. Abnormalities of the fast-Fourier transformed (FFT) Doppler spectra were used to detect and classify obstructive lesions as well as dilative arteriopathy of the carotid siphon, the middle, anterior, and posterior cerebral arteries, and the basilar artery. Functional stenosis due to large shunting volumes in patients with arteriovenous malformations and in the presence of significant collateraliza

Multiple progressive intracranial arterial occlusions ('moyamoya' disease).

Four adult female patients with moyamoya vessels are described. Hypertension and hyperlipidaemia were discovered in three patients. Vessels similar to moyamoya vessels were seen in the orbital and cortical collateral vessels as well as in the basal ganglia region. Attention should now be paid to the aetiological rather than the descriptive aspects of the disorder. An empirical trial of corticosteroids is worth while. Risk factors such as hypertension, hyperlipidaemia, and smoking should be eliminated.

Cerebral Hemodynamics Underlying Artery-to-Artery Embolism in Symptomatic Intracranial Atherosclerotic Disease.

Artery-to-artery embolism (AAE) is a common stroke mechanism in intracranial atherosclerotic disease (ICAD), associated with a considerable risk of recurrent stroke. We aimed to investigate cerebral hemodynamic features associated with AAE in symptomatic ICAD. Patients with anterior-circulation, symptomatic ICAD confirmed in CT angiography (CTA) were recruited. We classified probable stroke mechanisms as isolated parent artery atherosclerosis occluding penetrating artery, AAE, hypoperfusion, and mixed mechanisms, largely based on infarct topography. CTA-based computational fluid dynamics (CFD)

Role of the ophthalmic artery in the endovascular treatment for intracranial vascular diseases.

The ophthalmic artery (OA) is a crucial artery. Centered at the OA, there are numerous extracranial-intracranial anastomoses. The OA and its collaterals can be involved in some intracranial vascular diseases. So, it is very important to understand its specific anatomy, variation, and role in different neurovascular diseases. The OA has various anomalies both in the origin and collateral circulation. When performing endovascular treatment (EVT), the OA may suffer unexpected embolization through the numerous dangerous anastomoses. In case of a dural arteriovenous fistula or brain arteriovenous m

Associations of retinal microvascular signs and intracranial large artery disease.

BACKGROUND AND PURPOSE: Intracranial large artery disease (ICLAD) is a major cause of ischemic stroke. Retinal microvascular changes are associated with stroke, including small vessel cerebral disease and extracranial carotid disease. We examined the relationship between ICLAD and retinal microvascular changes. METHODS: This is a prospective cohort of 802 acute ischemic stroke patients. Retinal changes were assessed from photographs by graders masked to clinical data. ICLAD was evaluated using prespecified criteria. RESULTS: ICLAD was not associated with ipsilateral retinal arteriolar/venula

The importance of Doppler studies in asymptomatic intracranial and extracranial arterial disease.

Knowledge of intracranial and carotid disease in the symptomatic and especially the asymptomatic high risk population may be useful for evaluating future treatment modalities. A group of 204 symptomatic patients and 105 asymptomatic elderly patients at high risk for stroke were tested by carotid duplex ultrasound and transcranial doppler for the presence of carotid and intracranial stenosis. Quantitative measurements of the stenosis were made directly from the hard copy of the carotid duplex and the transcranial doppler. Of the 204 symptomatic patients 168 (83%) had some degree of stenosis: 84
