Intracranial carotid arteriosclerosis and their prognosis in intracerebral hemorrhage.

BACKGROUND: Intracerebral hemorrhage (ICH) is a manifestations of sporadic cerebral small vessel disease, and the survivors are predisposed to higher long-term risks of vascular events. Intracranial carotid artery calcification (ICAC), a potential marker for arteriosclerosis, is a risk factor for stroke but their roles in ICH is unknown. We aimed to investigate the prevalence and morphological subtypes of ICAC and their associations with long-term vascular events in ICH survivors. MATERIALS AND METHODS: Survivors of spontaneous ICH treated at a single center in Taiwan were included. ICAC was

Intracranial arteriosclerosis is not associated with cerebral amyloid deposition.

BACKGROUND: Intracranial arteriosclerosis and cerebral amyloid beta (Aβ) are both involved in the etiology of Alzheimer's disease (AD) dementia, but the direct link between these two pathologies remains elusive. METHODS: In 633 participants (mean age 69 years, 51% women) from the population-based Rotterdam Study, we quantified cerebral Aβ accumulation on amyloid positron emission tomography (PET). We assessed calcification of the intracranial internal carotid (ICAC) and vertebrobasilar arteries (VBAC) as proxies of arteriosclerosis on non-enhanced computed tomography (CT). Using logistic and

Different roles of arteriosclerosis in the rupture of intracranial dissecting aneurysms.

AIMS: Although intracranial dissecting aneurysm (IDA) is a newly described variant of the brain aneurysms that affects mainly the vertebrobasilar arterial system, its pathogenesis remains obscure. We aimed to clarify the role of arteriosclerosis in the pathogenesis of IDA based on histopathological findings in seven autopsy cases of IDA. METHODS AND RESULTS: All cases exhibited systemic hypertension or left ventricular hypertrophy. Macroscopically, all cases exhibited subarachnoid haemorrhage. Two types of dissection were recognized in the vertebral artery. Six of seven IDA cases showed a wid

CT-derived density of intracranial arteriosclerosis: a population-based cohort study.

BACKGROUND: The CT-derived density of coronary artery calcification is increasingly associated with the risk of ischemic heart disease. Whether this principle also applies to intracranial artery calcifications (IAC) and cerebrovascular disease risk is unknown, primarily due to the lack of population-based estimates of IAC density and its determinants. We investigated these facets in this cohort study. MATERIALS AND METHODS: In 2464 community-living individuals who underwent non-contrast CT, we measured IAC density and assessed its correlation with IAC volume using Spearman's ρ. We described i

Hypertension, intracranial arteriosclerosis, and structural brain changes in patients with TIA or ischemic stroke.

INTRODUCTION: Hypertension is a major risk factor of structural brain changes, including atrophy and cerebral small vessel disease. Intracranial arteriosclerosis could be an underlying mechanism between hypertension and structural brain changes. This study investigated whether intracranial carotid artery calcification (ICAC), as a proxy for intracranial arteriosclerosis, explains the association between hypertension and structural brain changes in patients with TIA or ischemic stroke. PATIENTS AND METHODS: About 968 patients (mean age 62.7 years) with TIA or ischemic stroke from a registry wh

Intracranial Carotid Arteriosclerosis Mediates the Association Between Blood Pressure and Cerebral Small Vessel Disease.

BACKGROUND: Intracranial arteriosclerosis could explain the association between blood pressure (BP) and cerebral small vessel disease (CSVD). Therefore, we tested whether intracranial carotid artery calcification (ICAC) mediates the association between BP and CSVD and determined pathophysiological mechanisms based on ICAC subtypes. METHODS: One thousand four hundred fifty-eight stroke-free participants from the Rotterdam Study (mean age, 68 years; 52% women) underwent nonenhanced computed tomography scans to quantify ICAC volume (mm3) between 2003 and 2015. ICAC was categorized into intimal a

Variations in intracranial arterial anatomy of the circle of Willis and their association with arteriosclerosis in patients with ischemic cerebrovascular disease.

INTRODUCTION: An estimated 20-31% of all people are born with a textbook anatomical configuration of the intracranial arteries comprising the Circle of Willis. Individuals with specific anatomical variants may be at elevated risk of intracranial arteriosclerosis, and possibly its sequelae of stroke and dementia, as the distribution of blood flow and pressure is known to be different in variants with missing arteries or arterial segments. Therefore, we studied the association of anatomical variation of intracranial arteries with arteriosclerosis. METHODS: Between December 2005 and October 2010

Intracranial arterial calcification and cerebrovascular function in the general aging population - A 7T MRI Study.

BACKGROUND: Intracranial arteriosclerosis may impair downstream cerebrovascular function, but the underlying mechanisms remain unclear. Using 7T MRI, we investigate the association between intracranial arteriosclerosis and cerebrovascular function in the middle cerebral artery (MCA) and in downstream small perforating arteries. METHODS: We included 195 Rotterdam Study participants (43.6% female, average age 70.8 years (± 4.52 years)) who previously underwent a non-contrast CT on which we measured intracranial carotid artery calcification (ICAC) and vertebral artery calcification (VAC) as hall

[Multiple and simultaneous intracranical hemorrhages with homocysteinemia].

A decreased intake of brightly colored vegetables results in increased levels of serum homocysteine, and homocysteinemia is associated with the progression of arteriosclerosis. Moreover, homocysteinemia is a known risk factor for cerebral infarction. Here, we report three cases wherein multiple simultaneous intracranial hemorrhages developed in the presence of homocysteinemia. The dietary habits of the patients involved indicated that decreased intake of brightly colored vegetables and the consequent progression of arteriosclerosis caused intracranial hemorrhage. Thus, in daily medical care, t

High Prevalence of Cerebrovascular Calcifications and Clinical Correlates in Indigenous Bolivian Forager-Horticulturalists: A Population-Based Observational Study.

BACKGROUND: Intracranial arteriosclerosis (large- and small-vessel disease) is considered a risk factor for major neurological disorders, such as stroke, cognitive impairment, and dementia. While most studies investigating intracranial arteriosclerosis include individuals from industrialized populations, the prevalence and clinical meaning of intracranial vascular calcifications in populations with a subsistence lifestyle is unknown. METHODS: In this population-based study evaluating data collected between 2017 and 2019 from Tsimane and Moseten people, 2 indigenous populations of forager-hort

Cerebellopontine Angle Tumors Are Associated with a Greater Incidence of Unruptured Intracranial Aneurysms.

OBJECTIVE: We tested the hypothesis that cerebellopontine angle (CPA) tumors are associated with a greater incidence of unruptured intracranial aneurysms (IAs). METHODS: Patients with intracranial tumors (ITs) undergoing computed tomography angiography and magnetic resonance imaging were enrolled in an observational cohort study that prospectively collected age, sex, hypertension, diabetes, cerebral arteriosclerosis, tumor type, tumor location, hydrocephalus, smoking, alcohol intake, CPA tumor size, cerebral aneurysms, and cerebral arteriosclerosis. Predictors of in-stent stenosis and occlusion after endovascular treatment of intracranial vascular disease with the Willis covered stent.

Covered stent placement has emerged as a promising therapeutic option for intracranial vascular lesions. However, in-stent stenosis and occlusion continue to be important concerns with the use of a covered stent, which is more thrombogenic than other types of stents. The purpose of this study was to determine predictors of in-stent stenosis and occlusion for covered stents used in the treatment of intracranial vascular diseases. Clinical, procedural and angiographic data of 46 patients with 49 intracranial vascular lesions treated with the Willis covered stent (Micro-Port, Shanghai, China) bet

[Diffuse intracranial arterectasias: megadolichocarotids, megadolichovertebrals].

Intracranial diffuse arteriectasis may be located to the carotid and vertebral arteries and their branches separately or concomitantly and associated with saccular aneurysms and generalized arteriectasis. The principle clinical aspects of intracranial arteriectasis are cranial nerve palsies, particularly of the cerebellopontine region in association or not with cerebellar disorders and transient or permanent motor deficiencies. Arterial hypertension exists in a high proportion of cases. The some times complex clinical picture may be due to pressure of nervous structures by the ectatic vessels,

Lipoprotein(a) is associated with a larger systemic burden of arterial calcification.

AIMS: Lipoprotein(a) [Lp(a)] is a genetically determined risk factor for cardiovascular disease. However, population-based evidence on the link between Lp(a) and subclinical arteriosclerosis is lacking. We assessed associations of Lp(a) concentrations with arteriosclerosis in multiple arteries. METHODS AND RESULTS: From the population-based Rotterdam study, 2354 participants (mean age: 69.5 years, 52.3% women) underwent non-contrast computed tomography to assess arterial calcification as a hallmark of arteriosclerosis. We quantified the volume of coronary artery calcification (CAC), aortic ar

Pathology of a dissecting intracranial aneurysm.

The pathological findings of six autopsy cases of dissecting intracranial aneurysm are studied. Clinically, all cases exhibited systemic hypertension or left ventricular hypertrophy. Macroscopically, all cases exhibited rupture of the vertebral artery and subarachnoid hemorrhage. Two types of lesion were present. First, all cases showed the formation of a dilatated pseudoaneurysm with widespread disruption of the entire arterial wall, which was composed of thin adventitia. Second, a medial disruption of the arterial wall and subadventitial dissecting hemorrhage, which formed a false lumen and

[Diagnostic clues in pulsatile tinnitus (somatosounds)].

Pulsatile tinnitus is a sound from within the body, mostly of vascular origin, that stimulates the patient's hearing in the same way as an external sound does, generally at the same pace as the pulse. Although not frequent, the diagnosis of its cause is crucial because of its potential severity in some cases. This article describes some of the diagnostic clues for arterial causes (arteriosclerosis, aberrant carotid artery, arteriovenous fistula or malformations, increased vascularization in Paget's disease) and venous causes (benign intracranial hypertension, high jugular bulb). In some cases
